Abstract
Description
The oral cavity with an inner surface of about 100 cm 2 represents a particularly diverse function for the human organism. Although it serves only for food intake, but thus represents an organ that must be on the one hand against the external environment, but on the other hand belongs to the internal organ. This is reflected in the different epidermal areas. On the side lies the cheek mucosa (called the buccal application side), above is the palate (palatal application side) and below the space under the tongue (called sublingual application side). Furthermore, the tongue is located in the oral cavity (no application page so far, important part of the linguistic articulation). In addition, the mouth contains the important for the digestion chewing tools (teeth), which are anchored in the gum (also referred to as gingival application side). Exogenously supplied foods contain on the one hand high microbial loads as well as ideal nutrient conditions for these microorganisms with regard to food sources, humidity and temperature. Strongly microbially contaminated oral cavities are characterized by painful inflammation, missing or rotting teeth, bad breath odor and impaired food intake. To remedy the industry offers numerous care products, in addition to instruments such as toothbrushes, dental floss, oral irrigators, etc. also from aids such as toothpastes, mouthwashes, chewing gum and systemic (eg antibiotic) medicines. All aids can be used either by the patient himself or by professionals such as dentists.
The All named application pages in the mouth are all characterized by the presence of water, so in the case of application A patch needs adhesion in the state of wetness / humidity come about. In this case one speaks also of Mucoadhäsion, because the adhesion of the plaster is made with the mucous membrane, which is mucus is lined. Here, the mucoadhesion as a special kind of bioadhesion considers, as any adhesion with at least one active phase is called bioadhesion. The fat The non-keratinized buccal mucosa is about 500 to 800 microns, while the mucous of keratinized, so-called hard palate and gums about 100 to 200 microns are thick. The permeability the oral mucous membranes is higher than the epidermis of the skin, but lower than that of the gastrointestinal tract. The special form of sublingual administration is not intended here because it's already well studied, but only for a few Applications is suitable. The small thickness of the mucosa and to allow good blood circulation rapid absorption and good bioavailability. It looks different gingival and buccal application. In gingival application applies poor permeability, moderate circulation, but good residence time for a plaster. With buccal application, there is slightly higher permeability and good Circulation with good residence time. Both application pages own for the use of patches that have a local effect in the oral cavity should. This is e.g. required for diseases in the oral cavity. To counting the application of disinfectants, antibiotics antifungals, antivirals, corticoids etc., partly also in the form of plant extracts, which in infections by bacteria, fungi and viruses such as oral thrush, herpes, Aphthous ulcers or after tooth extractions. Furthermore, there is a need the prevention of lesions in lesions, ulcers, e.g. heaped up immunosuppressed persons occur. Another need are Local anesthetics for painful inflammatory processes or denture pressure points. It can but also flavors, natural substances or salts are released, the bad breath improve or the formation of plaque as a precursor of gingivitis and periodontitis reduce. There are other body parts with very similar Requirements such as those of the oral cavity, there is talk of a body orifice (Orificium, Atrium) such as the nose, vagina and anus as well as the surrounding Skin like lips, genitals etc .. exists at all these gates a connection of the inside and outside of the organism with the Environment in the form of a mass transfer, hence these body parts have a mucosal lining of the epidermis and the need for oral therapy with mucoadhesives Systems.
Common to all mucoadhesive systems, such as the atrium patch cited herein, is that they contain adjuvants on the side attached to the mucus which are capable of adhesion in this environment. Materials of this type include, inter alia, the publication JD Smart et al., J. Pharm. Pharmacol. 1984, 36: 295-299. They are all viscosity-increasing polymers, which may be of natural origin, such as gelatin, pectin, tragacanth, gum arabic, chitosan, hyaluronic acid, alginates, agar etc., including semi-synthetic cellulosic derivatives, especially celluslose ethers and fully synthetic polymers such as polyacrylic acid derivatives in various degrees of crosslinking or polyvinyl alcohols, pyrrolidones. The choice of these substances must be chosen from various points of view. On the one hand, the materials used to make a plaster have to be able to form a film. Furthermore, the microbiological purity and mucosal compatibility must be ensured and compatibility with the active ingredients and excipients used. When administering a mucoadhesive system, the other side must be replaced by a nichtmucoadhesive layer are covered to prevent sticking (inflation) of the body opening. There are several possibilities to design the atrium patch: a local system delivers the active substance from the mucoadhesive layer directly to the overglued mucosa or a local system delivers the active substance from the non-mucoadhesive layer directly to the oral cavity. A systemic system, on the other hand, delivers the active substance from the mucoadhesive layer directly to the pasted mucous membrane in order to enter the bloodstream and develop a central effect there. In the following, focus will be more on local systems rather than limited.

When water-insoluble nonionic cellulose ether is ethylcellulose (Hercules, 48-49.5% ethoxyl content, degree of substitution 2.41-2.51, low viscosity) and as water-soluble nonionic cellulose ether is hydroxypropyl cellulose (Synthapharm, Pharmacoat 606, MG 11000), ie a comparable derivative as in the adhesive layer, used.
When water-insoluble anionic colloid Carbopol 971 is used (Goodrich; Carbomer 971 CP), as a water-soluble anionic colloid is the salt of arabic acid (gum arabic, Fa. Caelo) used. Surprisingly It was found that the best connection between both layers takes place by the Ahesivschicht for the mucosa, the water-insoluble anionic Colloid with the water-soluble nonionic Cellulose ethers and the backing layer the water-soluble anionic colloid with the water-insoluble cellulose ether. It is one skilled in the art that anionic and nonionic But not anionic and cationic. Not evident for the expert, however, was that just the ratios of anionic and nonionic Hydrocolloids in certain indirectly proportional proportions must be included in both layers for optimal adhesion to reach mucosal epidermal layers of body orifices.
Als Wirkstoffe können verschiedene Mengen, entweder gelöst oder als Suspension eingesetzt werden, z.B. aber nicht ausschließlich, Antiseptika wie Chlorhexidin, Hexetidin, Cetylpyridinium etc. oder Antimykotika wie Miconazol, Clotrimazol, Econazol, Nystatin, Tolnaftat, Sulbentin, Itraconazol etc., oder Viustatika wie Amantadin, Aciclovir, Zidovudin etc., oder Antibiotika wie Tetracyclin, Metronidazol, Clindamycin etc., oder Lokalanästhetika wie Tetracain, Benzocain, Lidocain etc., oder Corticoide wie Triamcinolon, Hydrocortison etc., oder Salze des Fluorids, Kaliums, Zinks etc. oder Vitamine wie Ascorbinsäure, Tocopherol, Vitamin K, β-Carotin, Coenzym Q10 etc. When Active ingredients can different amounts, either dissolved or used as a suspension be, e.g. but not exclusively, antiseptics such as chlorhexidine, Hexetidine, cetylpyridinium etc. or antifungals such as miconazole, Clotrimazole, econazole, nystatin, tolnaftate, sulbentin, itraconazole etc., or Viustatika such as amantadine, acyclovir, zidovudine, etc., or antibiotics such as tetracycline, metronidazole, clindamycin, etc., or local anesthetics such as tetracaine, benzocaine, lidocaine, etc., or corticosteroids such as triamcinolone, Hydrocortisone, etc., or salts of fluoride, potassium, zinc, etc. or vitamins like ascorbic acid, Tocopherol, vitamin K, beta-carotene, Coenzyme Q10 etc.
Es können auch wässrige oder alkoholische Extrakte von Pflanzen wie Echinacea, Sambucus, Matricaria, Thymus, Camiphora, Salix, Camelia, Salvia, Melissa, Myrtillum, Allum und weitere eingesetzt werden. It can also watery or alcoholic extracts of plants such as Echinacea, Sambucus, Matricaria, Thymus, Camiphora, Salix, Camelia, Salvia, Melissa, Myrtillum, Allum and others are used.
The Production must be such that there is an inseparable connection between Adhäsiv- and active layer takes place. Because in the two layers no binder are included and the lamination technology conventional Matrix plaster is to be applied and thus no compression pressure is available Surprisingly, this compound could only with the coating of the layers one above the other by overlaying be achieved. Due to the special circumstances of the used and chosen Hydrocolloids is the use of equal phases (both aqueous or both Orgish) no longer necessary. Nevertheless, they mix both layers do not bind yet and are in the dried Condition not adhesive, so no siliconized protective films during the production is still required in the packaging as in otherwise usual Matrix patches.
An ethanolic adhesive layer is first coated, and this on polyester films, glass or other smooth surfaces can be done. After drying, the active ingredient layer is overcoated in an ethanolic suspension. After final drying, the system can be easily peeled off the support base (polyester, glass, etc.). By punching the separation into individual doses of 0.5 to 2.5 cm 2 , preferably in 0.8-1.25 cm 2 and the packaging in air and opaque, familiar to those skilled Peelpackungen. The special feature of this invention is that the compound of the two layers is always particularly good when the ratio of the hydrocolloids used is such that nonionic polymers such as hydroxypropylmethyl and ethylcellulose ethers and anionic polymers such as carbomer (polyacrylic acid as polyanion) and gum arabic (Salt of arabic acid) in the ratio 1: 1 to 1: 2, preferably 1: 1.2. On the other hand, it is important that the ratio of nonionic hydrocolloid to polyanionic hydrocolloid in the adhesive layer is 1: 2 to 1: 5, preferably 1: 3, but vice versa, namely, 2: 1 to 5: 1 in the back layer, but preferably 3: 1.
Agar-Agar, Alginsäure, Carboymethylcellulose, Carrageen, Gelatine Typ B, Karaya-Gummi, Pektin, Tragant, Polymethylvinylethermaleinanhydrid (Gantrez) etc.The Hydrocolloids mentioned are merely exemplary embodiments. The expert of course knows also nonionic polymers other than those mentioned here, such as cellulose ethers, e.g. Guar gum, locust bean gum, polyvinylpyrrolidone, starch, etc. or other anionic polymers than the carbomers mentioned here and gum arabic such as e.g. Agar-agar, alginic acid, carboymethylcellulose, Carrageenan, type B gelatin, karaya gum, pectin, tragacanth, polymethyl vinyl ether maleic anhydride (Gantrez) etc.

To prepare 3500 cm 2 of a patch laminate according to the invention on a laboratory scale, 4.2 g of Pharmacoat 606 and 2.8 g of glycerol anhydricum are dissolved in 280 g of ethanol 96% for the adhesive layer, then homogeneously suspended therein 0.42 g of titanium dioxide. On a stirrer with the setting 300 rpm, 150 ° C, 14 g of Carbomer 971 are sprinkled onto the ethanolic solution, the system is sealed and then homogenized at room temperature for 60 min in a high-speed mixer. Subsequent slow stirring removes the incorporated air. There are 1.4 g strawberry flavor (eg Dragoco) and 238.185 g of an aqueous plant extract of dry extract of Sambuccus nigra, Centella asiatica, Echinacea purpurea (9: 0.8: 0.2 wt, content dry residue 5.3%) was added, stirred and receives a homogeneous, bubble- and cluster pen-free brown gel, which is spread on polyester (Hoechst, Hostaphan RN 100) with a wet film thickness of 2000 microns. The drying takes place with increasing temperature from 40 ° C to 60 ° C over 30 min. For the backing layer, 18 g of ethylcellulose are sprinkled to 153 g of 96% ethanol and stirred at 700 rpm until everything is dissolved. 0.36 g of dye E172 red are dissolved in 2 ml of water. 0.45 g of titanium dioxide are ground in 9 g of castor oil and 0.04 g added with stirring. Finally, 6.75 g of gum arabic are added and all stirred at 300 rpm, until a homogeneous, bright red, cloudy, thin liquid suspension is formed. This is coated on the adhesive layer with a wet layer thickness of 1000 microns and also dried with increasing temperature of 40 ° C to 60 ° C for 30 min. By punching result mucoadhesive patches of size 1.25 cm 2 elliptical, containing 4.5 mg of dry extract and are used to treat gingivitis and periodontitis. The ratio of nonionic to anionic polymers in the cited case is 1: 1.2; the ratio within the adhesive layer is 1: 3.3; the backing layer is 2.7: 1.

The according to example 2 prepared but drug-free, patch was in an in-house Wear test on the properties of the application, the period of liability and general convenience. All participants had voluntarily agreed. At the time of testing none of the participants of both sexes had a herpes labialis lesion, only one participant of eight is herpes patient. The evaluation took place after issuing a test patch using a questionnaire of the type "multiple choice ". The test took place within a week. 77% of the participants found the application easy. The application provides that the fat-free lips with the tongue moistened and the plaster with the white side on the wet lip for 30 Seconds pressed until a firm adhesion has taken place. 77% of the participants felt a foreign body sensation on the Lip. 62.5% of the participants assessed the adhesion of the patch as good and found the patch taste neutral. The wearing time the patch averaged 5.6 hours; the minimum time was 1.3 hours, the maximum time 19 hours. In any case, was the plaster actively removed by the subject, it never fell by chance. 50% of the participants welcomed it when the plaster even thinner and or more flexible and color neutral. These demands are through a thinner coating the backsheet as well as by change the dyes easily possible.
