Abstract
Description
The invention relates to a system for treating vascular adhesions, in particular for removing varicose veins, with the features of the preamble of claim 1.
From DE 197 54 779 a vein stripper is known which has a hollow cylinder which is arranged coaxially to a tubular or rod-shaped stem, the stem of an ela tical and dimensionally stable material. One end face of the hollow cylinder is as Cutting edge formed and may be ground in a wave shape if necessary. To one To remove the vein, the free end of the vein to be removed is inserted into the hollow cylinder and kept under constant tension. Then the rotary movements of the Hollow cylinder performed about its longitudinal axis and at the same time the hollow cylinder ahead exaggerated so that the vein is separated from the surrounding tissue. In a handle on Also attached to the handle is a motor that houses the handle with the hollow cylinder rotated. A control unit communicates the desired speed of rotation, Spin and angle of rotation.
The disadvantage here is that it requires a high level of manual dexterity, at the same time as the rotational movements of the hollow cylinder so forward by a metered use of force to ensure that the vein is separated from the tissue in the desired manner.
It is therefore an object of the invention to provide a system for treating vascular adhesions to create, which in particular facilitates the removal of varicose veins.
The object of the invention is achieved by a system for treating vascular adhesions gene solved according to claim 1. Advantageous embodiments of the invention are the subject of Dependent claims.
According to the invention, an electric drive is provided, which drives the treatment instrument in a reciprocating motion along the longitudinal axis of the probe offset, the electrical The drive can be switched on and the amplitude of the back and forth movement in steps or is continuously adjustable.
Preferably, the probe is made of an electrically insulating material or is with a electrically insulating material coated, the treatment instrument on its Au has a heatable ring made of electrically conductive material. That ring is subjected to high-frequency current in the practical embodiment in order to for example, side branches of a vein, which are separated by means of the treatment instrument have been to coagulate immediately after separation, so that subsequent bleeding, which ha matoms and thus a long healing phase, cannot arise in the first place.
According to a further preferred embodiment, the electric motor and the electri cal drive arranged in the handpiece. The electric motor is more preferably a Mi. kromotor. It can also take care of the stroke movement of the probe, but it is expensive gere devices certainly useful to provide another micromotor, so not only the amplitude, but also the frequency of the stroke movement is adjustable.
A special coupling can be provided on the handpiece, which connects the probe directly or individually takes up directly, and which can be released from the handpiece.
To simplify the condition, a foot control can be connected to the control unit may have a first pedal for actuating the electric motor, further a second Pedal as a double-function pedal for adjusting the angle of rotation of the Son's rotation de on the one hand and for coagulating the tissue on the other hand, with the control device loading operating time of the double-function pedal and the angle of rotation of the rotary movement gradually increased when the actuation duration exceeds a predetermined first value, continued operation of the dual function pedal resetting the Angle of rotation results in a minimum value, and tissue that is in contact with the ring is coagulated when the actuation duration a predetermined, second value, which in the is generally much less than the first value.
The foot control unit also advantageously has a third pedal for setting the rotary speed digkeit of the treatment instrument, wherein the control unit, the duration of actuation third pedal is detected and the rotational speed increases gradually when the actuation time falls below a predetermined switching time and the speed of rotation continuously reduced if the actuation duration exceeds the specified switching time.
It is further advantageous if a light source is as close as possible to the surgical site is brought up and arranged for example on the probe. Can be used as a light source serve a fiber optic cable. It is also possible to arrange a micro camera on the probe, whose images are transferred to a monitor.
Im folgenden wird die Erfindung anhand der beigefügten Zeichnungen erläutert, in denenThe invention is explained below with reference to the accompanying drawings, in which

Fig. 4 shows the handle of the vein stripper according to the invention with a connecting line and inserted stem.
Fig. 1 shows a probe 10 , which is releasably placed in a connector 12 to enable easy sterilization. The probe 10 consists of a sterilizable and elec trically insulating material. In the present embodiment, the probe 10 is equipped as a Ve stripper, but it is also possible to use probes of a different design in the connection piece 12 if the treatment purpose requires this. According to FIG. 1, the probe 10 has an elongated stem 14 , which ends at the angled region 16 on the outer circumference of a hollow cylinder 18 . The hollow cylinder 18 is provided at its distal end facing away from the connector 12 in the usual manner with a cutting edge and is arranged coaxially with the elongated stem 14 . The hollow cylinder 18 has on its outer surface a ring 19 made of electrically conductive material, to which a through the stem 14 and the connector 12 extending electrical line, not shown in the drawing, is placed in a connector 13 , which the proximal Forms end of the connector, opens. With the help of a neutral electrode applied to a patient, a circuit is formed so that severed side branches of veins can be coagulated via the ring 19 . In order to facilitate the localization, for example, of side branches of a vein, a light source 15 , for example an optical fiber line, is arranged at the transition point from the elongated stem 14 to the angled section 16 . The supply for the fiber optic line is also led through the stem 14 and the connector 12 to the connector 13 . On the connector 12 , a selector switch 11 for the stroke of the back and forth movement to be explained later is also arranged.
Fig. 2 shows an embodiment of a handpiece 20 which is used in the system of the vorlie invention. In the handle 22 , an electric motor, for example a micromotor, for the rotary movement, the probe is housed, further an electric drive for the lifting movement or the reciprocating movement of the probe, both not Darge, housed. The electric motor and the further electric drive can also be combined. The desired movement is given to a coupling piece 21 , on which the connector 12 of the probe 10 ( Fig. 1) is placed. Matching Ge counterparts for the connector 13 ( Fig. 1) are also provided. The entire electrical lines are discharged in a common connecting line 23 and connected to the control unit of the system according to the invention.
Fig. 3 zeigt das Steuergerät 30 in schematisierter perspektivischer Ansicht. Fig. 3 shows the controller 30 in a schematic perspective view. The connection cable 23 of the handpiece 20 ( Fig. 2) is inserted into the socket 31 . Another socket 32 is provided for a connecting cable for a foot pedal, which will be described later in connection with FIG. 4. The control device is put into operation by actuating an on / off switch 34 . The conditions for the rotary movement of the probe ( Fig. 1) can be set via control panels 35 , 36 and 37 . As a rule, there will be default values that can be increased or decreased using pushbuttons. In this case, the rotational speed is from the control panel 35 is increased or decreased from the control panel 36, the spin is increased or decreased, while with the control box 37 to influence the angle of rotation, which is the oscillation amplitude, is possible. The radio frequency mode is set via the control panel 38 , ie the power supply for the ring 19 of the probe 10 ( FIG. 1). Current, voltage or frequency can be set, for example, as parameters for the coagulation. On the back of the Steuergerä tes is, indicated by the reference number 39 , the connection for the Neutralelektro de, which, as already mentioned above, is necessary for the coagulation in high frequency mode.
Fig. 4 shows a foot control 40 with which the functions of the probe can be called up. A first pedal 42 is used to start the motor housed in the handle 20 ( Fig. 2), and as long as the first pedal 42 is pressed, the motor runs with the parameters entered in the Steuerge ( Fig. 13). A second pedal 41 has a double function. By pressing the pedal 41 for a relatively long time, for example for more than 4 seconds, the angle of rotation of the device is slowly increased in small steps. When the largest possible angle has been reached, the system will reset to the smallest possible angle and the drive will start again. The high-frequency mode is activated or deactivated by briefly tapping the pedal 41 over a period of less than one second. A third pedal 43 in the form of a push button is used to regulate the speed of rotation, and it is increased by briefly tapping the speed of rotation in small increments, a long press on the pedal 43 will reduce the speed in small increments.
Damit wird bei der Operation ein an die Gegebenheiten anpaßbarer Vorschub der Sonde und damit des Behandlungsinstrumentes möglich, so daß der Chirurg von komplizierten Manipulationen befreit istThe lifting movement of the probe is not part of the electronic control in this embodiment and is also not operable via the foot pedal. Rather, this is set mechanically via the selector switch 11 already mentioned. With a simple configuration, four different settings for the stroke are possible. This makes it possible to adapt the probe and thus the treatment instrument to the conditions during the operation, so that the surgeon is freed from complicated manipulations
The obvious in the above description, in the drawing and in the claims Features of the invention can be used both individually and in any combination the realization of the invention may be essential.
