Abstract
Die Erfindung betrifft eine Anordnung zur Stimulation der Muskelkontraktion im Gesichtsbereich. Sie findet Anwendung zur Behandlung von Zahnstellungs- und Biszanomalien. Weiterhin findet die Erfindung Anwendung um kosmetische Effekte, insbesondere eine Gesichtsformgestaltung zu erreichen. Durch eine Anordnung zur Elektrostimulation der mimischen- und/oder Kaumuskulatur in Verbindung mit einem kieferorthopaedischen Geraet werden Gebiszanomalien beseitigt und/oder die AEsthetik des Gesichtes verbessert. Hautelektroden sind in Abhaengigkeit vom angestrebten Behandlungsziel jeweils symmetrisch zur median-saggital-Ebene so angeordnet, dasz erste Hautelektroden im Ursprung und mindestens zweite Hautelektroden im Ansatz des gleichen Gesichtsmuskels und vorteilhafterweise dritte und weitere Hautelektroden im Ursprung und/oder Ansatz weiterer Gesichtsmuskeln angeordnet sind. Dabei ist die örtliche Muskeltätigkeit die entscheidende Quelle von Umbaureizen auf die zu bewegenden Zähne, Kieferknochen und Gelenke. Neben der Extraktionstherapie gibt es zur Zeit noch zwei auf die Zahnerhaltung orientierte Behandlungskonzepte: Bei der aktiven Therapie wird zuerst durch ein durch Schrauben und Federn aktivierbares kieferorthopädisches Gerät die Form des orofazialien Systems in der Hoffnung verändert, daß sich die Funktion der veränderten Form anpaßt; dagegen verändert die tunktionelle Therapie zuerst den Funktionsablauf, um danach zu einer Formveränderung zu kommen. Das Prinzip der Funktionskieferorthopädie besteht darin, daß durch das kieferorthopädische Gerät eine Störung der neuromuskulären Ruheschwebe bewirkt wird, die den Muskeltonus aktiviert. Die auf diese Weise erzeugten Kraftimpulse, sie betragen nach SANDER und SCriMUTH (Sander, F. E.; Schmuth, G. P. F.: Der Einfluß verschiedener Bißsperren auf die Muskeltätigkeit bei Aktivatorträgern; Fortschr. Kieferorthop. 1(! [1979] S. 107-116) etwa 500-700 Impulse pro Nacht, sind in der Rigel zuwenig und zu klein, um zeitlich annähernd gleiche Behandlungsresultate wie bei der aktiven Therapie zu erzielen. Um diesen Behandlungseffekt zu erzielen, ist grundsätzlich Mundschluß erforderlich, der aber nicht immer, und insbesondere nicht in der Schlafphase durch Erschlaffung der Schließmuskulatur gegeben ist. Sehr häufig, und am Beginn d6r Behandlung fast ausschließlich, wird das kieferorthopädische Gerät kurz nach dem Einschlafen aus dem Munde verloren. Somit ist die Fffektivität e'er Behandlung hauptsächlich abhängig vom Tragemodus am Tage. Die tagsüber im wachen Zustand durchgeführte Behandlung ist für den Patienten relativ unangenehm. Erfahrungsgemäß wird deshalb das Gerät tagsüber am unzuverlässigsten getragen. Gemäß Fig. 1 wird eine zur Median-saggital-Ebene paarweise symmetrische Anordnung von Hautelektroden 1,2,3 mit einer speziellen Kopf-Kinn-Kappe 4 für die Aktivierung der Kaumuskulatur gezeigt. Vor dem Schlafengehen wird die Kopf-Kinn-Kappe 4 mittels Klettverschlüssen drucklos zugebunden, und anschließend ein Muskelstimulator in die Brusttasche der Nachtbekleidung gesteckt und die Verbindung mittels eines elektrischen Kabels S zur Kopf-Kinn-Kappe 4 hergestellt. Die Hautelektroden 1,2,3 befinden sich jeweils im Ansatz- und Ursprungsgebiet des musculus masseter und des musculus temporalis auf der rechten wie auf der linken Gesichtshälfte und werden durch den Muskelstimulator mittels elektrischer Impulse zu rhytmischen Kontraktionen mit Mundschluß und Schlußbiß veranlaßt.According to FIG. 1, a pairwise symmetrical arrangement of skin electrodes 1, 2, 3 with a special head-and-chin cap 4 for the activation of the masticatory muscles is shown in the median sagittal plane. Durch die Stimulation der Muskelkontraktion werden die in saggitaler Richtung wirkenden Kaukräfte der Zähne in eine transversale Richtung umgelenkt und infolgedessen in die beabsichtigte Richtung dauerhaft reguliert. Nach dem Einsetzen eines kieferorthopädischen Gerätes 9 wird das für den jeweiligen angestrebten Behandlungszweck vorprogrammierte Muskelstimulationsgerät eingeschaltet. Für kieferorthopädische Behandlungszwecke wird die Impulsfrequenz auf 1 ...2 Impulse pro Sekunde eingestellt. Das ergibt bei 7 Stunden Schlaf pro Nacht zirka 25000... 50000 Kaubewegungen. Die kieferorthopädischen Geräte ohne Elektrostimulation müßten dafür zirka 35...70 Nächte getragen werden, um eine vergleichbare Anzahl der gebißformverändernden Impulse zu erreichen. Vorteilhafterweise wird die Muskelkontraktionskraft auf 2...3N begrenzt, um Schädigungen durch zu große Kräfte zu vermeiden. Diese Kraft und die eingestellte Impulsfolge verhindern zuverlässig ein Verlieren des Gerätes während des Schlafes. Diese Methode ist besonders effektiv bei notorischen Mundatmern wo es nur beim Schlucken zum Mundschluß kommt, der infolge des Schlafes deutlich gegenüber der am Tage reduziert ist. Die große Anzahl der Impulse reicht für eine zuverlässige zahnregulierende Wirkung aus, so daß besonders vorteilhaft das Gerät am Tage nur noch 1... 2 Stunden und gegen Ende der Behandlung nur nachts getragen werden muß.2, the masticatory muscles G are contracted by electrical impulses via the head and chin cap with the integrated skin electrodes 1, 2, 3.
