Abstract
Die Erfindung bezieht sich auf die Befestigung von Kunststoffimplantaten (Pseudophakos) in den hinteren Kapselsack der Linse zur optischen Korrektur des Auges nach Entfernung der getruebten Augenlinse bei grauem Star. Ziel der Erfindung ist die Befestigung der kuenstlichen Augenlinse so, dass eine mechanische Belastung des Gewebes der Regenbogenhaut weitestgehend vermieden wird, ein geringer operativer Aufwand zur Befestigung der Linse notwendig ist und ein Verschieben der kuenstlichen Augenlinse nach dem operativen Eingriff ausgeschlossen ist. Erfindungsgemaess resultiert die Halterung fuer eine kuenstliche Augenlinse aus einem Kunststoffimplantat, bestehend aus einer Plankonvexoptik mit einem Durchmesser von etwa 5,5 bis 6,0 mm, die zentral in einem Tragrand eingebettet ist, auf dem sich in regelmaessigen Abstaenden kleine Loecher befinden, durch die ein nichttoxischer, resorbierbarer Gewebekleber aufgetragen wird, der das Implantat auf dem hinteren Kapselsack des Auges fixiert.The invention relates to the attachment of plastic implants (Pseudophakos) in the posterior capsular bag of the lens for optical correction of the eye after removal of the dull eye lens in cataracts. Bekannt sind künstliche Augenlinsen, die einen Linsenkörper und je eine auf diesem an diametral gegenüberliegenden Seiten festgelegtes Befestigungs- und Stützelement zur Unterbringung der Linse am Linsenbeutel des Auges eines Patienten einschließt, wobei das Befestigungselement ein Winkelstück aus Draht ist, das auf der Seitenfläche der Linse befestigt ist, dessen Spitze auf einer Linie liegt, die durch den Mittelpunkt des Linsenkreises und die Mitte des Stützelementes geht. (DE-OS 2945349). Nachteilig ist, daß die Stützelemente in der Peripherie des Linsenbeutels hinter der Regenbogenhaut nicht kontrolliert werden können. Auch Verletzungen des Linsenbeutels durch die Stützelemente sind nicht festzustellen. Weiterhin ist eine Linse zur Implantation in die Linsenkapsel des Auges bekannt, an deren Linsenumfang zwei oder mehrere weiche und elastische Abstandshalter in Form von Ringen, Scheiben, T-Stegen und dergleichen ausgebildet sind. (DE-OS 2717706) ein Nachteil dieser Linsenform und ihrem Befestigungssystem ist der kleine optische Durchmesser, da durch den Halteapparat Platz benötigt wird und der äquatoriale Durchmesser des Linsenkapselsack der menschlichen Linse nur 9mm beträgt. Nach DE-OS 3326341 ist ein Befestigungssystem für Intraocular-Linsen beschrieben, das durch einen elastischen Halterahmen einteilig oder nicht einteilig mit dem optischen Bereich durch eine Vielzahl nachgiebiger Fasern oder Stege gekennzeichnet ist. Erfindungsgemäß wird die Aufgabe dadurch gelöst, daß ein Kunststoffimplantat aus einer Plankonvexoptik besteht, deren Durchmesser ca. 5,5 bis 6,0 mm beträgt, die zentral in einen Tragrand eingebettet und aus einem Stück gefertigt ist. Auf dem Tragrand befinden sich in gleichmäßigem Abstand kleine Löcher, durch die ein Kleber aufgetragen wird, mit dessen Hilfe das Implantat auf dem hinteren Kapselsack des Auges fixiert wird. Es wird ein nichttoxischer Gewebekleber verwendet, wie beispielsweise Cyanoacrylat, der sich nach 6 bis 8 Wochen abgebaut hat. Bereits nach 2 bis 3 Tagen nach Befestigung des Implantates bildet sich ein Fibringerüst aus, das nach Resorption des Klebers das Implantat festhält. Die Plankonvexoptik mit dem Tragrand besteht zweckmäßigerweise aus Polymethylmethacrylat. Es können auch andere Kunststoffe, wie Polyurethanelastomer, Siliconelastomer, Hydrogelpolymerisat, Collagenverbindung, organische Gelverbindung, synthetische Gelverbindung und dergleichen Verwendung finden. Durch die sekundäre Klebung kann die Linse im Pupillarbereich gut zentriert werden.
