Description
The invention relates to a process for the preparation of an oral dosage form of alkylsulfonic acid esters of ethinyl estradiol, preferably of ethinyl estradiol sulfonate. This method is characterized by the use of special pharmaceutical excipients to a preparation with particularly high bioavailability, so that it is possible to achieve drug levels that are not achieved by known means.
Gegenwärtig sind zahlreiche Methoden bekannt, wie durch entsprechende galenischeIn the production of solid oral pharmaceutical preparations the 100 % biological availability is of crucial importance. In general, the active ingredients must be brought into such an application form that they have a certain ΐ / irkungsgeschwindigkeit, duration and intensity with the most complete absorption in the gastrointestinal tract. At present, numerous methods are known, such as by appropriate galenic
Bearbeitung bzw. B. beim Testosteronundekanoat (H.-J. Horst, W. J. Holtje, M. Dennis, A. Coert, J. Geelen und K. D. Voigt: Klin. Wschr. £4, 875 (1976)) und beim Östradioldekanoat (P. M. Kicovic, M. Luisi, F. Franchi und E. Alicicco: Clin. Endocrinol. 7, 73 (1977)). Oils are used as vehicles of steroid derivatives of the carboxylic acid ester type, encapsulated in gelatin, especially with the aim of oral administration, to circumvent the portal vein system and thereby achieve increased lymphatic absorption, such. For example, testosterone undecanoate (H.J. Horst, WJ Holtje, M. Dennis, A. Coert, J. Geelen and KD Voigt: Klin. Wschr. £ 4, 875 (1976)) and estradiol decanoate (PM Kicovic, M Luisi, F. Franchi and E. Alicicco: Clin. Endocrinol., 7, 73 (1977)). The aim of these methods is to use the lymphatic transport to bypass the direct beberpassage in order to withdraw some of the Steroidkarbonsäureester hepatic hydrolysis. It further follows a metabolic inactivation of the steroid.
31 533; G. Bruni, F. Galetti und A. Ercoli: European J. Steroids 1_, 29 (1966)).The effect of improving the absorption and thus oral effectiveness of administration in oily solution instead of tablets and aqueous suspension has been described for the group of Δ 3 -3 -etetosteroid ethers ITfat.-iir. 31,533; G. Bruni, F. Galetti and A. Ercoli: European J. Steroids 1_, 29 (1966)).
Sog. exp. Biol. Med. Ij^, 183 (1966)).Clay to the phenolic steroids ethinyl estradiol and quinestrol it is known that the application in oily solution in comparison to aqueous suspensions neither leads to higher plasma levels nor to higher biological activities (BG Steinetz, A. MeIi, VL Beach and T. Giannina: Proc. Sog. exp. Biol. Med., 183 (1966)).
Ethinyl estradiol sulfonate belongs to the group of alkyl sulfonic acid esters of ethinyl estradiol. This group is characterized by its biological activity through use in the following indications: contraception, inhibition of xactivity, prostate cancer, treatment in orchidectomized men, treatment of breast cancer and post-menopausal symptoms and brightening therapy in cancer track search. All indication areas require a constant absorption of Ethinylöstradiolsulfonat in order to achieve a pharmacologically active ingredient level. This is of particular importance in this oubstance group because it, administered orally, causes a long-term effect and therefore the application intervals repeated several days and thus the one-time reduced absorption leads to a pharmacologically ineffective plasma levels over several days. Although a tablet or dragee of an ethynyl estradiol alkylsulfonate, especially ethinyl estradiol sulfonate, may contain the correct dose, there is a possibility that only a portion of the dose will be absorbed in the gastrointestinal tract. The dissolution rate of the tablet or dragee is known to cause the absorption.
It is also known that the particle size is important for the extent and speed of absorption. The absorption is decisively influenced by the intestinal transit time. It has been found that with rapid intestinal transit a portion of the Alkylsulfonsäureester of Äthinylöstradiols, especially the Äthinylöstradiolsulfonats that is absorbed by the body, for example, when applied in crystalline form in hard gelatin capsules, to about 60 σ β> the dose decreases. In contrast, absorption rates of almost $ 100 > can be achieved with longer intestinal transit time.
The disadvantages of the application of Alkylsulfonsäureestern of Ethinylöstradiols as a crystalline substance in tablets or dragees, which have been explained using the example of Äthinylöstradiolsulfonats lead to a reduction in the quality of the drug in the form of tablets and dragees and result from the described uneven bioavailability.
The aim of the invention is to increase the bioavailability of orally administrable depot estrogens of the alkyl sulfonic acid ester type of ethinyl estradiol and to find a process for the preparation of a suitable oral dosage form. A further aim was that a consistently high proportion of the dose of the Alkylsulfonsäureester of Ethinylöstradiols is absorbed by the organism by the orally administrable dosage form and thus possible production-related fluctuations of the tablet and dragee decay times are excluded.
The object of the invention is furthermore to find a method of the pharmaceutical formulation for the oral administration of alkyl sulfonic acid esters of ethinyl estradiol, which
Time and alimentary factors dependent absorption fluctuations largely precludes without leading to absorption delays.
A process for the preparation of orally administrable alkyl sulfonic acid of the ethinyl estradiol with particularly high bioavailability has been found, which ensures a high constant bioavailability of these esters in single oral doses. It was surprisingly found that z. B. Athinylöstradiolsulfonat in contrast to ethinylestradiol and cyclocyclopentyl ether of Ethinylöstradiols, quinestrol, when administered in oily solution to significantly higher plasma levels compared to aqueous suspensions leads. The plasma levels after oral administration in the vehicle according to the invention were even higher than those achieved after intravenous administration of ethinyl estradiol sulfonate in propylene glycol (Table 1).
Derartige Lösungsmittelsysteme bestehen im allgemeinen aus Ölen pflanzlicherThe alkyl sulfonic acid esters of ethinyl estradiol are long-acting estrogens which are administered orally, either alone or in combination with other sex steroids. Surprisingly, it has been found that the effect of absorption improvement, which could already be observed by dissolving the alkyl sulfensäureester of Äthinylöstradiols in oil, was further improved by the addition of a cosolvent, it was only possible to dissolve the esters in such a concentration, that soft gelatin capsules of small size could be produced. The lipophilic solvent system is characterized in that it is compatible with gelatin and capsules of soft gelatin can not dissolve or penetrate. Such solvent systems generally consist of vegetable oils
Origin, which are liquid at room temperature, such. Peanut oil, sunflower oil, olive oil, sesame oil, soybean oil, cattle claw oil or the like. Peanut oil is preferred as a vegetable oil, but other oily solvents such as synthetic mono-, di- or tri-glycerides or esters of aliphatic carboxylic acid esters having 12 to 25 carbon atoms and their mixtures of vegetable and animal oils can be used.
These oils can be treated with relatively small amounts of a cosolvent, such as benzyl alcohol or the like. be added to increase the solubility of the Alkylsulfonsäureester of Äthinylöstradiols in oil and thus their concentration in oily solvents. Thus, it becomes possible to include even high doses of these esters in relatively small capsules. The addition of cosolvents can be dispensed with if esters of aliphatic carboxylic acids having 12 to 25 carbon atoms are used, which have a high dissolving property for the alkyl sulphate esters of ethinyl estradiol.
Such capsules can achieve an increase in the usability properties with regard to strength, storage temperature and packaging by additional coating or curing.
The gelatin-encapsulated solutions of ethinyl estradiol alkylsulfonic acid esters have the advantages of a dosage form which is in solution, but as convenient as tablets or dragees, without the problems of uniformity and rate of dissolution found with tablets or dragees.
Uicht bekannt ist bisher jedoch, daß sich durch das erfindungsgemäße VehikelIt should be noted that oils are used as solvents for drugs for the preparation of capsule-formulated drug solutions. It is not known hitherto, however, that the vehicle according to the invention provides
Dies führt wiederum zu einer Wirkungsverstärkung und -Verlängerung.and thus the dosage form according to the invention can achieve a substantial increase in the bioavailability of alkyl sulfonic acid esters of ethinyl estradiol. Here, a bioavailability of almost $ 100 is achieved and a plasma peak-1 is achieved which is above that which results from almost complete absorption of alkylsulfonic acid ether of ethinyl estradiol, especially of the smallest particle size ethinyl estradiol sulfonate ( $ 70) C [5 / iMi] is caused. This in turn leads to an effect enhancement and extension.
Another feature of the invention is that with this dosage form enteral absorption differences - caused by gastrointestinal passage time and alimentary pactors - are largely eliminated. This is due to the preferred absorption of the alkyl sulfonic acid esters of ethinyl estradiol in the formulation of the invention in the upper small intestine sections. The invention makes it possible to reduce the therapeutically necessary dose while maintaining the therapeutic effect of Äthinylöstradiol Alkylsulfonsäureester by at least 40 0 Jo .
According to the invention it is possible, for. B. to reduce the content of a single dose of Athinylöstradiolsulfonat for a weekly contraceptive administered (DEPOSISTON as a so-called weekly pill) so that the weekly recorded Ethinylöstradiolmenge of 1.2 / umol, the known v / ei se is considered to be optimal, with 1.5 / umol is exceeded only slightly.
Die Erfindung betrifft ein Verfahren zur Herstellung der oralen Arzneiform der Alkylsulfonsäureester des Äthinylöstradiols, das durch folgenden Ablauf gekennzeichnet isttThe invention relates to a process for the preparation of the oral dosage form of the Alkylsulfonsäureester of Äthinylöstradiols, which is characterized by the following sequence
Der Alkylsulfonsäureester des Äthinylöstradiols wirdThe first stage concerns the evil in the oily solvent system. The Alkylsulfonsäureester of Äthinylöstradiols is
In this case, temperatures above 100 0 C should be avoided to prevent decomposition of the ester. After that, the oil becomes. at room temperature or elevated temperature with constant stirring until the ester is bad. Another possibility is to introduce the drug, which should be in finely chopped, preferably micronized form, into the heated mixture of cosolvent and oil. The temperatures should not be more than 100 0 C to prevent decomposition of the ester.
In the method described, instead of the system of co-solvent and oil of vegetable or animal origin, a mixture of aliphatic carboxylic acid esters of 12 to 25 carbon atoms may be used together with an oil. All of the listed options have the goal that the drug is present in such a final concentration that it does not crystallize out of the oily solvent system at normal room temperature.
The further encapsulation is carried out according to the known encapsulation method, wherein usually the drug solution is enveloped by a liquid gelatin and thereby forms a capsule.
Table 1 shows the plasma level profile of the total radioactivity in rats after intravenous administration and after oral administration of radiolabelled Xthinylöstradiolsulfonat as a crystalline substance in aqueous methyl cellulose suspension (90 io of particles <O0 / um) and in oily solution. The dose was 1 mg / kg body weight, the concentration of ethinyl estradiol sulfonate was 0.2 mg / ml for the methyl cellulose suspension and also 0.2 mg / ml in the vehicle according to the invention. The vehicle according to the invention consisted of peanut oil 3 i ° benzyl alcohol.
It can be seen without further explanation that a considerably higher bioavailability was achieved by the application of ethinyl estradiol sulfonate in the vehicle according to the invention in comparison to the crystalline substance in methyl cellulose suspension and for intravenous administration as an ethanol / ethanol solution.
While significant reductions in the level of total radioactivity plasma levels could already be detected at the aforementioned low dose, with absorption decreasing as the dose increases, it has been shown that the effect of absorption enhancement by application in the vehicle of the invention was more pronounced at higher doses , Thus, in rats 24 hours after oral administration of Athinylöstradiolsulfonat in a dosage of 25 mg / kg as a crystalline substance in methyl cellulose suspension and in the vehicle according to the invention a plasma level of non-metabolized Athinylöstradiolsulfonat found that in the inventive vehicle with 15,23 / ug / 10 ml plasma was 10 times as high as that after application of the crystalline substance in aqueous methyl cellulose suspension with 1.45 / μg / 10 ml plasma.
From these results it can be seen that, especially at higher doses, the improvement of the bioavailability is noticeable by the invention.
Table 2 shows the results of an experiment in which the bioavailability of crystalline Athinylöstradiolsulfonat on! Lactose in hard gelatin capsules (80 jo of the particles <^ 10 / um) compared to the human dosage form according to the invention was examined by renal excretion.
Das Verhältnis der 10tägigen renalen Exkretion bei der Zubereitung - jeweils an der gleichen Probandin untersucht - beweist, daß eine wesentlich höhere renale Exkretion nach ApplikationThe ratio of the 10-day renal excretion in the preparation - examined in each case on the same subject - proves that a significantly higher renal excretion after administration
(Tabelle 1 und 2).of ethinyl estradiol sulfonate in the dosage form according to the invention. This shows in this Pail that the invention makes it possible to increase the biological availability by 4-0 $ over the crystalline substance. Furthermore, it can be seen from the table that the crystalline substance is subject to strong absorption fluctuations, which do not occur in the oily solution. (Table 1 and 2).

In the heated to 100 C mixture of these fatty acid esters with benzyl alcohol Äthinylöstradiolsulfonat is dissolved with stirring, so that a concentration of 8 mg / ml of solvent system is formed. After cooling, the solution is encapsulated in a known manner in soft gelatin.
