V současné době jsou opěrky hlavy všech vozidel konstruovány tak, aby ochránily hlavu v případě centrického nárazu, tj. v podélné ose vozidla a to jak v případě nárazu vozidla do překážky před sebou, tak i v případě nárazu jiného vozidla zezadu. Taková opěrka je známa např. ze spisu CZ 1998-2331 Al. V případě nárazu je do opěrky přes tyče přiveden tlakový vzduch a objem i tvar opěrky se zvětší. Hlava se nejprve pohybuje šikmo stranou a posléze, kdy se již nachází mimo podélnou osu sedadla a tím i opěrky hlavy, se vrací, avšak již na jiné místo, než je nainstalovaná opěrka. V důsledku toho dochází často ke zraněním, která jsou mnohdy odůvodňovaná špatným nastavením opěrek.
