Abstract
一种眼内的植物,它包括一片镜,和一条镜片定位腿,它们由镜片处伸展开来,使植入物可以平稳地安放在眼睛的小室内。至少有一条腿的自由端是附有碰锁装置,可以和附在镜片边沿的配合碰锁部分锁在一起。这附在镜片上的配合装置,处在外围部分的圆周线上,当扣接上腿的末端时,腿会环绕外围部分的边沿拢曲,使到植入物的侧面大小,并不大于原有镜片及其边沿部分,因此由眼睛切口放入植入物方便了。
Description
移植入眼内的眼球内镜片是公知技艺。植入此种镜片需要先在眼球割一切口,用作移去天然晶状体及置入植入物;这植物是由一种有生物惰性的透明物质,例如是聚甲基丙烯酸酯所组成。这切口所需要的长度是决定于被植入镜片的整体宽度。减小植入体的宽度就可以缩短切口的长度,而正需要这样,因为切口越小,病人在手术后的康复就越容易,康复时间也就越短。
虽然一般认为最少需要有三条定位腿才能稳当地把植入物定位及定中,但很多此类植入物只应用两条有弹性的定位腿,因为有两条腿的植入物比有三条腿的植入物,较容易穿过较窄的切口。
这种装置还有另外一个难题,就是它们虽然只有两条腿,但要把此装置植入眼睛内仍然困难,因为进入切口的第一只脚必须脱开眼球组织,使之可以更深植入镜片及让第二只脚进入切口,並安置在眼球的正确空隙内。
眼球内植入体是由适合这目的塑料做成,例如使用甲基丙烯酸甲酯,包括一中心部位的镜片及绕此镜片部位的边缘镜片支撑部份,及至少由一根由边缘镜片支撑部份挠性伸进有弹性定位腿。在不受限制的条件下,这有弹性定位腿是径向分开伸展,及至少绕过挠曲一个边缘部位,末端是自由的。这自由末端附有碰锁装置,而镜体的外围部份也附有相配合的碰锁装置,采用可松开的碰锁设计接受这定位腿的自由末端。这镜体上相配合碰锁是安装在圆周的位置上,而定位腿的大小是当此腿向内伸展时,自由端可以接上碰锁,此时腿部是沿着外围部份的外沿安放,使整件植入物的宽度,在植入眼睛时,不会大于镜片本身的宽度。
本发明的一个较佳实例是,镜体的外围部份,有直立侧向支撑装置协助腿定位,使腿挠性绕着镜体以便接触镜体自由端时,定位腿由这侧向支撑装置所支持,因此这装置控制了腿的内向挠曲量並把这挠曲分散到大部份腿上並防止在腿的根部过份产生挠曲。
只要具有定位脚连接镜体的装置,就可以选用有任何定位脚数目的植入物,因为此时这些脚的存在並不增加植入物的整体宽度。
图1是俯视图,表示可植入眼内的镜片,附着的有弹性定位腿是在伸展开状态,也就是制成品成形。
图2是图1植入物的侧面图。
图3是图1植入物的透视图。
图4是图1植入物的俯视图,富有弹性的腿碰锁在镜体上,是可以马上植入眼睛内的状态。
图5是图4的装置的侧面图。
图6是图4的装置在植入眼睛及松开定位腿后的形状。
图7是一俯视图,表示一拥有本发明特点的装置的变体,各条腿是在松开了的状态。
图8是图7装置的另一视图,各个有弹性的腿已经锁在镜体上,可以植入眼内。
图1-5所示的绘图,是本发明的第一个设计,由植入体10所组成,它包括镜体,中央部份12和外围支撑部份13,以及一对有弹性的定位脚16。
这镜片中央部份12的横切部份可以是任何形状,这样便可得到所希望的镜片效果。这边沿部份14绕镜片12的整个外围伸延开,並具有一直立伸展的筋18,它沿边沿部份伸展。
每一条定位腿16都是边沿的必要部分,它沿径向空间绕镜体伸延,末端是自由端20,自由端20在侧向的增大部份22,有一穿通小孔24。腿16有一贴近部位,它是由根部(边沿部份的交接点)起逐渐收缩到图1的“A”点位置,由“A”点起到自由末端有一段增大部份,宽度大致一致。这样形状的定位腿使安放在眼球小室内部份厚度均匀,但仍可使弯曲应力分散到腿的各处,因此减低了定位腿在根部26破裂的可能性。
腿部的曲率半径,由根部到放大部位,是逐渐加大的,而邻近放大部位的部位29是向内弯曲的,这样做的目的后面将会叙述。
筋18上有一对贯穿的,在半径方向延伸切缝30,每条切缝30都是恰好处于接收定位腿的自由末端位置,放大部位22可以安放在筋内,同时相邻定位腿部份也可以安放在切缝内(参看图4)。
本发明的一个较好实例是,当自由端20被嵌入切缝的时候,定位腿的其馀部份将绕着边沿部份的外缘而弯曲,并位于这外缘的同圆心的圆弧内(如图4所示),这也表示出植入眼睛内的植入体的形状,而保持在这位置装置下面会有介绍。
如图4表示,定位腿锁在边沿部份植入体,宽度不会大于单个的边沿部份,这样在应用植入物时就仅需要最短的外科切口。
外科医生使用的植入物如图1所示情况根部无约束及无应力;在準备植入时,腿16向内弯曲,邻近放大部份的腿部位29,会放进切缝30内。挠曲的定位腿的回弹力,会把放大部位朝着分切缝相反的筋骨拉紧,使两个定位腿始终处在挠曲的状态。当植入物经眼睛切口被植进眼球后,可以用一适当仪器把放大端22由切缝30处提拉开,使它们可以向外回弹到包着眼睛小室内。
应该清楚,有些外科医生宁愿在植入手术前,接牢两条定位腿中的一条。
图1至图5实例,筋18由切缝30分成两部份,每个部份有侧面32。在其接近切缝的渐渐沿着半径方向向内靠,并每一侧面32是如此安排,以便使定位腿向内弯曲而连接筋,压向表面32,因此减少了在镜体的连接点处施加在腿的根部上的栈曲力大小。侧面32和腿部的嵌接,同时控制腿末端挠曲曲程度,並在植先眼入前及处理过程中支撑以防止定位腿的损害。
很多种植入物,定位腿相对镜片水平面,都是稍为向后倾斜,使镜片相对某些类型的眼球结构,可以处于理想的位置。此时,腿16必须稍为向前挠曲,使定位腿可以绕筋骨弯曲,並碰锁入切缝30内。因此,支持定位腿使它们不致向下挠曲,刚好位于筋外的支撑部份14,这样可防此定位腿由切缝脱开。
请参看图7及图8,它们表示出具有典型特征的本发明植入物变异。在这个实例中植入物40包括一镜体,它是由中心镜片部份42和一边沿部份44以及三条有弹性的腿46组成,这些腿绕镜体的边缘,分别在不同位置上伸展出去。这边沿部份44有3个筋节,在圆周外围每节上各有一横向切缝,可安放定位腿的自由端,使定位放大部份能保留在筋内,如前面有关图1至图5的实例。
当三条腿46朝内弯曲绕过边沿部份的外缘,並把腿的末端安放在切缝50上时,放大部份被保留在筋里面,植入物的整体宽度将不会超过镜体本身的宽度,所以具有三条腿的镜体与只有两条腿的镜体相比並不要求更长的外科切口。
不管上述那个实例中,筋骨(18或48)都是处在植入体的前方,所以这些定位腿都附在植入物的前方,这样不仅植入物放进眼内后容易从切缝松出定位腿,也可保证不致把植入物反向放入眼内,也就是说,不会如以前曾发生过的,把后镜片表面放在朝外方向。
虽然所列举的本发明的实例每条腿附着在镜体上方法,大家应该明白,在某些情况下,只能接上一条腿。例如像图1至图5使用植入物时,外科医生可以发现手术前只能把一条腿附接在镜体上。因此,和图7图8实例相类似地,有必要规定一条或两条腿附着在镜体上。
把腿附着在镜体的装置,前面已有介绍,包括腿上的一放大了的末端,及一位于筋骨的边沿切缝,它可接受腿的末端。应该清楚到是,可以有其他附着腿部的装置,但它们仍然属于本发明的范围。
在这里所介绍的本发明的实例,植入物是由单独一种物质所造,如有必要,这些弹性定位腿可以另外制造,及用任何其它方法附着在镜体上。
Claims (13)
1、一种眼睛内的植入物,包括一个镜体,和具有一镜体的定位装置(至少有一条弹性定位腿),其特征在于定位腿在伸开时。它是由连接镜体的根部伸展出去,尾部是一自由的末端,此自由末端附有一挂耳向着背离末端的方向,而且镜体有碰锁装置,可用来接受碰锁啮合松开时的挂耳,这碰锁装置是园周方向安放,以使在碰锁装置扣接着挂耳,腿的其馀都份位于镜体的边沿。
2、如权利要求1的植入物,特征在于在其上有三条有弹性的腿,基本上是以同样的相隔距离绕在镜体的边沿,镜体上用于挂锁及至少上述一条腿的终端可自由松开的碰锁装置,上述碰锁装置是位于边沿部份,並扣住腿的差不多所有部分,並于和镜片同一中心的圆内,该圆的直径不会大于镜体的直径。
3、如权项1的植入物,这里的腿有一横向放大部份,形成挂耳,并此镜体在外围部份有一直立筋,筋骨上有一横向切缝,可接受那邻近腿的放大部份,放大部份夹持在可启开的扣锁啮合装置筋骨内。
4、如权项1植入物,此时腿是与镜体为一整体部,腿是由起始部份根部起逐渐变窄,中间部份较窄,在中间部份后面的末端部份的宽度差不多完全不变,同时由末端动作所产生的应力会分散到起始部份的各个部位。
5、如权项3植入体,其中上述腿处在受约束位置,与横穿过镜片的水平面(和镜片轴心成垂直角度),形成一微小的倾斜,而邻近切缝的筋骨的内表面和此水平面所形成的角度,是使这内表面和腿的挂耳部份互相平行。
6、如权项3植入体,其中上述的筋骨是位于边沿部份,使到腿向内挠曲和筋啮合时,压向筋的外表面达到控制腿的弯曲。
7、一眼睛内的植入物,它是由具有一中心镜片部份和一边缘镜片支持部份镜体和此镜体的夹持置置所组成。此夹持装置包括至少一条有弹性的腿,它在没有约束状态通常是由边缘支撑部份切向地伸展开,螺旋地伸至一向内弯曲的末端部份,又有一碰锁挂耳附在上面,及在镜体上有接受此碰锁挂耳的装置,使这向弯曲的末端部份得以嵌接。
8、如权利要求7植入物,这里的碰锁装置包括一筋骨和一通常横向伸展穿过此筋骨的切缝,这切缝的尺寸及位置是当腿部挠曲绕过边沿部份时,刚好能够接纳这向内弯曲末端,及保留定位腿在挠曲状态;当腿部与筋骨嵌接时,差不多整条腿都是位于边沿部内的圆周线之内。
9、如权利要求8植入物,这里筋骨部份的位置是在边沿部份上,当腿向内挠曲往碰锁位置时,腿是压向筋骨的外向表面。
10、一件眼球内的植入物,包括镜体和镜体保留装置,这装置包含至少一条有弹性的定位腿,它是没有受到约束,并由连接镜体的根部,伸延到一离开镜体的末端部份,这末端部份是自由的,它和镜体相互配合,使这末端部份,既可被锁固,也可松离。
11、如权利要求10植入物,这里的配合装置包括一位于定位腿自由末端的部份,和镜体的一部份,当这腿的自由末端连接镜体这部份时,腿会绕镜体的边缘部份挠曲。
12、一件眼睛内的植入物,包括一镜体和一镜体保留装置,此装置包含一条定位腿,它是处于没有约束状态,由连接镜体的根部,伸延到一离开镜体的末端部份,这末端部份是自由的,它和镜体相互配合,使腿可以环绕镜体挠曲,而这末端部份是附接着镜体,及附接着腿的根部和自由末端之间的镜体边缘装置,当这腿环绕镜体挠曲使末端部份可以接上镜体时,这腿会压向腿的支持装置,由此控制了腿的挠曲程度,及把弯曲应力分散到大部份腿上以防止腿在根部过份挠曲。
13、如权利要求12植入物,腿支撑装置,包括绕过镜体的一部份圆周线上边缘壁。
