Abstract

本实用新型公开了一种供眼科青光眼滤过手术时植入巩膜瓣下的人工小梁支架,其整体呈襻状,由左臂1、右臂2、左手3、右手4和连接臂5组成,两臂环抱,左、右手之间有一个缺口。应用该人工小梁支架后能明显改善并加强滤过性手术的引流效果,防止术后纤维化,提高手术成功率。

Description

人工小梁支架
本实用新型涉及一种可供眼科青光眼滤过手术时植入巩膜瓣下的人工小梁支架。
小梁切除术是目前治疗青光眼最常用的一种滤过性手术,其目的是建立一条新的房水排出通道,降低眼内压。但手术后常因纤维化使滤过口被堵塞,眼压再度升高,导致手术失败。因此眼科界一直在探索新的手术方法,寻求抗纤维化措施。如《国外医学》眼科分册1999年第三期第150-160页中所述的各种改良的小梁切除术,眼前房内植入带导管的眼压调节阀或所谓的人工小梁,巩膜瓣下植入硅胶,胶原,透明质酸钠生物胶膜等。又如术中或术后应用一些抗纤维化药物,丝裂霉素C,5-氟脲嘧啶等。但上述方法均未能更好地阻止晚期纤维化问题,或可出现滤泡破裂、低眼压等新的并发症。
本实用新型的目的是提供一种能改善并加强滤过性手术的引流效果,防止术后纤维化,提高手术成功率的人工小梁支架。
为实现上述目的,本实用新型人工小梁支架呈襻状,由左、右两个臂,左、右两只手和连接臂组成。两臂环抱,左、右手之间有一个缺口。
上述人工小梁支架的周界大小可以为3~4mm,各个组成部分的襻丝的直径可以为0.5mm,从而最适合于植入巩膜瓣下。
上述人工小梁支架可以用聚甲基丙烯酸甲酯制成,从而保证了该人工小梁支架的组织相容性。
在一个优选实施方式中,连接臂向双手问缺口区弯曲,人工小梁支架呈W形。
在另一个优选实施方式中,连接臂不向双手间缺口区弯曲,在左、右臂的纵向中间部位各有一个向内的突起,人工小粱支架呈V形。
采用本实用新型的人工小梁支架,在手术过程中,按特定的手术方法制成巩膜瓣后,将上述人工小粱支架植入巩膜瓣下,使它与巩膜床之间形成新的间隙,称为人工小梁。重建的人工小梁可使房水循环恢复正常的生理压力梯度,既可防止术后眼压过低的并发症,又可长期保持滤过口通畅,防止眼压升高、复发,同时由于维持了晶状体的正常房水营养环境,可降低术后白内障的发生率。
以下结合附图对本实用新型进行具体描述:
图1是本实用新型人工小梁支架的第一种优选实施方式的正面图。
图2是本实用新型人工小梁支架的第二种优选实施方式的正面图。
图3是本实用新型人工小梁支架第一种优选实施方式植入巩膜瓣下的示意图。
图1显示了本实用新型人工小梁支架的一种实施方式,其左臂1、右臂2和连接臂5,构成人工小梁支架的主体,呈W形。左手3、右手4和连接臂5之间有一个缺口,该缺口恰可使切除的小梁区被架空。
图2显示了本实用新型人工小梁支架的另一种实施方式,其左臂6、右臂7和连接臂10构成人工小梁支架的主体,由于其连接臂10的方向背向左手8和右手9之间的缺口,故此种人工小梁支架呈V形。左手8和右手9之间有一个缺口,该缺口恰可使切除的小梁区被架空。在左、右臂的纵向中间部位各有一个向内的突起,分别称为左肘11和右肘12。左肘11和右肘12之间也有一个缺口区,可作为房水引流的通路。
图3为本实用新型中W形人工小梁支架在小梁切除术中植入巩膜瓣下的示意图。制成并掀起巩膜瓣13后,做小梁切除14和虹膜根切15,将人工小梁支架16植入,使其左、右手之间的缺口对准小梁切除区,这样就形成了第一人工小梁间隙17和第二人工小梁间隙18,供房水引流之用。
本实用新型不局限于上述实施方式,不论在其形状或结构上作任何变化,凡是利用弯曲的襻状结构植入眼球壁内的人工小梁支架均落在本实用新型保护范围之内。

1.一种用于眼科青光眼滤过手术的人工小梁支架,其特征是:该人工小梁支架呈襻状,由左、右两个臂,左、右两只手和连接臂组成,两臂环抱,左、右手之间有一个缺口。
2.根据权利要求1所述的人工小梁支架,其特征是:连接臂可向双手问缺口区弯曲,人工小梁支架呈W形。
3.根据权利要求1所述的人工小梁支榘,其特征是:连接臂可不向双手间缺口区弯曲,在左、右臂的纵向中间部位各有一个向内的突起,人工小梁支架呈V形。
4.根据权利要求1所述的人工小梁支架,其特征是:该人工小梁支架的周界大小可以为3-4mm,各个组成部分的襻丝的直径可以为O.5mm。
