Abstract
本发明涉及造血生长因子,红细胞生成素(EPO)和血小板生成素(TPO),或者其衍生物、类似物或部分,用于促进组织的结构性再生的用途。
Description
本发明涉及特别是在肝脏再生中用于诱导组织的结构性生长的方法,并要求德国专利申请103 61 813.9-41和欧洲专利申请03 029961.4的优先权,就内容而言本发明与这些专利申请相关。
生长因子在个体发育过程中表达,它们能够引发基本的结构性的和与细胞数量相关的构造组织的许多过程。然而,在生长中的或成年的生物体中,在很大程度上失去了构造出或者在组织受损的情况下再生出在结构和功能上完整的组织的能力。据推测,生长因子的表达减少是再生能力降低的原因,这些生长因子在其那方面控制着对于组织构造所需的蛋白质的表达。
但是,已知至少一些器官,它们本身在成年生物体中保留了自我再生的能力,这种能力能够被损伤过程诱导。例如,自从古希腊罗马时期以来就已经知道了肝脏的再生能力。几乎所有的其他器官由其本身均不能相应地弥补结构缺损而重新产生原始的组织。
在成年生物体中,肝脏通常以静止的状态即非增殖性状态存在,在这种状态下器官必须满足各种不同代谢功能的复杂的多样性。然而,在体内,经受细胞量的损失,例如肝脏细胞损害或经外科手术介入,肝脏被刺激而再次生长。
但是,增生性肝脏组织在大多数情况下不以所期望的方式补偿器官的功能性和解剖学上的结构,而是通常导致存余的肝脏组织的增大和肥大,直至弥补原来的肝脏细胞量。生长应答的强度取决于组织损失的程度。在此,肝脏再生的时间进程是呈反比相关的,即如果是小的肝脏细胞损失,则补偿进程就慢,如果是大的肝脏细胞损失,则快得多。
因此,通过细胞增殖本身的器官质量的补偿并没有提供足够的基础来治疗具有显著的器官损害的患者。因此,已知多种不同的用于诱导组织的结构性生长即成形性(formschaffend)生长的方法,但是它们全都仍然不能产生令人满意的结果。但是,这样的细胞的结构性生长恰好对于治疗或生物技术方法具有重要的意义。
在过去,尝试了通过给予生长因子,例如表皮生长因子(EGF)、血管内皮生长因子(VEGF)或肝细胞生长因子(HGF),来诱导细胞生长。但是,这些因子对于在体外增加初级细胞的作用是有限的。于此相反,由于其可能的副作用,例如激活癌基因,它们在体内的使用并不是不成问题的。
另一种方法基于使用复杂的异源组织提取物(例如来自垂体或下丘脑)来诱导例如培养的肝细胞的细胞增加(参见例如US 6,008,047)。但是,在可传播的病毒疾病例如BSE、猪病毒或绵羊病毒的背景下,在实验室活动或临床应用中使用动物或人类的组织提取物是有问题的,并且早就表明了缺乏涉及参与构造复杂的器官结构的过程以及真正相关的因子及其使用和作用潜能的知识。此外,提取物的质量难以确定,因为提取物的质量尤其取决于来源和其培养条件。
然而,对于原代组织培养的常规应用的少量认识本身不能直接地转嫁为组织工程的问题。通常,组织工程理想地是以患者特异的成熟细胞系统为出发点的,所述成熟细胞系统相对于胎儿或胚胎细胞而言已经进一步分化了。此外,组织工程不仅在原位而且在体外均涉及共培养情形,这在常规应用中是不会考虑的。与此相反,很早以前就甚至尝试在肝实质细胞扩张的情形下避免内皮细胞、巨噬细胞和成纤维细胞的共培养,如它们在肝脏中存在的那样,因为它们是不希望的。
从WO 02/092013 A2中获知,给予为了治疗肝脏损害的患者治疗有效量的生长激素(GH),从而促进肝脏的自然再生能力。据其,GH具有增进生长因子Fox M1B在肝细胞中的表达并因而重新引发肝脏生长的作用。
但是,该生长因子对于组织生长具有非常宽的、因而非特异性的作用。因此,通过给予GH还可产生意想不到的副作用或过度反应,例如以肢端肥大症的形式,即带有病理性骨状态的过度骨化。此外,在这期间已知,Fox1M在基底细胞癌中被高度调节。Fox-蛋白在增长、分化和转化中,尤其还在激活所谓的SONIC HEDGEHOG(Shh)信号途径中,对于调节生长基因起着重要的作用。另一方面,它们还参与了人类皮肤中基底细胞癌的活化。Teh等人(Cancer Research 2002,August 15;62(16):4773-80)能够证明,FoxM1在基底细胞癌中的高度调节是重要的起始机制之一,由此SONIC HEDGEHOG信号途径在基底角质化细胞中产生促有丝分裂的作用,由此造成广泛散布的人类癌溃疡的形成。因此,Fox1M激动剂的这种致瘤性潜能以及缺乏特异性和在所有组织中普遍存在阻碍了通过给予GH来促进肝脏再生。
根据本发明,该任务通过使用造血细胞因子以及其衍生物或类似物或部分用于结构或功能性的肝脏再生而得以完成。在一个特别优选的实施方案中,给予红细胞生成素(EPO)或者其衍生物、部分或类似物。但是,对于所涉及的组织的根据本发明的效果还可以通过施用血小板生成素或其部分来实现。
已经令人惊奇地证明,通过施用造血生长因子如EPO和TPO,不仅开始出现细胞的增加,而且开始出现结构性生长。这种生长特别是在先前受创伤的组织上开始的。由此诱导的生长在体内导致真正意义上的组织再生,即不仅存在增生性生长,而且存在定向的、分化的生长以形成复杂的结构。
实质上,根据本发明将造血因子用于组织再生基本上基于EPO的两种迄今为止未知的作用,即一方面刺激不同细胞类型(例如成纤维细胞、平滑肌细胞和血管范围内的内皮细胞,并结合在考虑了细胞外基质(胶原、弹性蛋白、纤连蛋白、内功素(Entaktin))的情况下重新形成完整血管的结构)彼此间和共同地以同步和相互协调的形式进行结构性生长,另一方面完备真正的实质性组织联合。这包括例如形成具有结合的枯否细胞、隐窝细胞(Pit-zellen)、贮脂细胞(Ito-zellen)和内皮细胞(所谓的肝脏的非实质细胞)的肝细胞。因此,除了形成确实的血管树及其相互连接之外,根据本发明还在恢复和复原的意义上诱导了组织再生。
在肝脏中,除了肝细胞以有序的3-D结构进行真正的实质性联合之外,这导致在末梢血流区域内的窦样毛细血管与流入和流出性血管混合。
正如已经说明的,根据本发明的造血生长因子的作用特别是在受创伤的组织和细胞中显现。在此,将术语“创伤”定义为与组织发生(组织形成)相反的过程。因此,创伤是指这样的过程,即这一过程在相关位点处抵抗了个体生物体中的作为组织形成过程的组织发生,或者使得组织发生的成果破灭。作为组织损害的创伤可以由于许多事件而引起,例如通过损伤、炎症或者通过具有自身损害的自身免疫疾病。这些组织损害或组织破坏反过来又引起了大量的反应,例如激活巨噬细胞、肥大细胞和免疫活性细胞,它们分泌趋化性的、血管活性的和促进伤口愈合的因子,并由此调节全身性的和区域选择性的机制。
根据本发明的造血生长因子(特别是EPO)的用途的优点包括可再生所有四种基本组织类型(即结缔组织、肌肉组织、上皮组织和神经组织)的组织。这些组织在个体发生上来说来源于中胚层(结缔组织、肌肉、内皮(视为特殊形式的上皮))、内胚层(衬在胃肠道内壁的上皮)或外胚层(神经组织)。在过去已经证明,EPO受体不仅表达在中胚层以及内胚层来源的细胞上,而且表达在神经元细胞上。
在这些组织中,根据本发明的EPO或TPO的应用导致组织特异性始祖群体(干细胞)的位置固定的募集、细胞的迁移和细胞分化或转分化成实质细胞和结构细胞。在组织形成期间和之前,细胞通过给予EPO而增加。
在根据本发明的方法例如用于肝脏再生时,可以获得先前受损的器官的重新完备以达到完整的实质性组织联合,包括形成具有枯否细胞、隐窝细胞、贮脂细胞和内皮细胞的肝细胞。因此,随着进一步形成血管树,根据本发明获得恢复和复原性的组织再生是可能的。
因此,本发明方法的一个特别的优点在于,不仅刺激了在内皮芽殖(Endothelaussprossung)上再生中的组织的微毛细血管化(Mikokapillarisation),而且促进了壁结构的实质性再生和形成。首先,这导致所期望的结果,即相互协调地三维地生长从而形成有功能的器官。
因此,本发明的用途基于EPO的作用,这种EPO的作用已远远超越了迄今为止已知的EPO作为用于内皮细胞增加的生血管性因子的作用(Journal of Nephrology 2002 15,97-103)。由于微血管结构如毛细血管和窦状隙紧紧由内皮细胞内衬组成,并且不具有自身的壁结构,因此从EPO的生血管性作用出发,迄今为止也只是能够推测EPO是否也能够在血管重新形成和伤口愈合方面具有一定的意义(Journalof Nephrology 2002 15,97-103)。但是,迄今仍未实现对这种推测的证实。
因此,本发明更加优异的是,第一次成功检测了EPO对于血管自身的同步和相互协调的生长(即包括壁结构的形成)和实质性再生的作用。
本发明方法的另一个优点在于,结构性生长非必需地需要预先规定的有机或无机三维结构作为起始点,而是更可能重新产生器官(部分)结构。因此,造血生长因子的施用能够诱导受损组织的明显加快的自身再生,这在本发明的临床治疗应用中具有很大的意义。
在本发明的一个特别有利的实施方案中,使用造血生长因子以诱导具有创伤性地受损的区域的组织的再生。因此,在这些组织区段中,不仅可以随着血管发生的开始通过肉芽组织的形成来推动伤口封闭,而且可以推动由细胞外基质(例如胶原、弹性蛋白、纤连蛋白或内功素)构成的组织特异性三维结构的重新形成。
根据本发明使用造血生长因子。在此,首先涉及血小板生成素、红细胞生成素和生长激素(GH)以及其功能和结构上同源的类似物、衍生物和/或部分。但是,特别地使用EPO或TPO或者其部分、衍生物或类似物。其模拟肽也是适合的(见下文)。此外,G-CSF和GM-CSF也属于造血生长因子。
造血生长因子是左侧的四股螺旋束蛋白,其方向是上-上--下-下,并具有两个突出的环带(Schleifen),该环带将前两个螺旋结构和后两个螺旋结构相互连接在一起(Livnah O.等人,Science 1999,283(5404):987-90;和Ultsch M.H.等人,Blood 1995,86(2):540-7)。每个RBD结构域(受体结合结构域)与EPO具有显著的同源性。血小板生成素、红细胞生成素和生长激素结合MPL-受体复合物。在Youssoufianh等人的出版物(Blood 1993,819 2223-36,Structurefunction and activation of the Erythropoietin receptor)中,描述了生产性的二聚体化。红细胞生成素和血小板生成素,模拟肽(EMP和DMP),以及此外还有非肽性小分子同样是有功能的,如果也在较低的摩尔基础上(出版物:Wrighton NC等人,Small peptides asmimetics of the protein hormone erythropoietin,Science 1996,273(5274)458-64;和Cwirla S.E.等人,Peptide agonist of thethrombopoeitin receptor as potent as the natural cytokine,Science 1997,27653191696-9;和Qureshi S.A.等人,Mimicry ofErythropoietin by non peptid molecules,Proceedings NationalAcademy of Sciences PNAS USA 1999,(9621):12156-61)。EMP 1与受体的结合发生在与生长激素受体结合的热点同源的结合位点处。血小板生成素的结构序列详细描述在EP 1 201 246 A2中。红细胞生成素的结构序列详细描述在欧洲专利申请EP 84 308 654.7中。
在本发明的一个特别有利的实施方案中使用EPO,其在体内刺激红细胞的形成以及红细胞-前体细胞的细胞分裂和分化。EPO可以从尿液中分离或者重组制备。制备重组的人类EPO是WO 86/03520的主题。此外,EPO是EP 0 148 605、EP 0 205 564、EP 0 209 539、EP 0 267678或EP 0 411 678的主题。
但是,还可以使用天然或重组EPO的衍生物。例如,已知一种EPO衍生物(EP 0 640 619 B1),其具有额外的糖基化位点,因而具有含有22个唾液酸残基的较高糖含量。这种修饰的优点在于,随着唾液酸含量的增加,EPO在血浆中的半寿期增加了,因为只有非唾液酸化的EPO能够和参与EPO降解的肝脏半乳糖受体结合。虽然,该EPO衍生物(简称为NESP(Novel Erythropoiesis Stimulating Protein(新型红细胞生成刺激蛋白)或者Darbepoetin))与重组EPO相比较具有在五处位置发生改变的氨基酸序列。但是,其对于刺激红细胞形成的作用基本上与天然或重组的EPO相当(Fisher,J.W.:Erythropoietin:Physiology and Pharmacology Update.Erythropoietin 2003,1-14)。鉴于NESP的结构和制备,完全引用EP 0 640 619 B1。
NESP还可以有利地与聚乙二醇(PEG)化学偶联以进一步延长体内半寿期,而并不会因此造成生物活性的改变。如此修饰的NESP可从WO01/76640中获知,该专利文献完全涉及该EPO衍生物的结构和制备。
从WO 02/49673 A2和WO 01/02017 A2中同样可获知PEG化的EPO衍生物,其除了变长的血浆半寿期之外还表现出更高的临床效果。为此,例如通过定向诱变在EPO的氨基酸序列中引入1-6个附加的糖基化位点。根据本发明也可以使用该衍生物。
除了EPO和具有相应的生物功能的EPO衍生物之外,根据本发明还可以使用其他造血生长因子,例如TPO或其部分。
TPO(也称为c-Mpl-配体、mpl-配体、巨促红细胞生成素(Megapoietin)或巨核细胞生长因子和巨核细胞发育因子)是具有复杂生物作用的造血生长因子,其尤其可调节巨核细胞的发育和增殖。成熟的TPO由具有332个氨基酸的序列组成,其中具有154个氨基酸的N-末端区域(RBD结构域)与EPO有着显著的序列和结构同源性(20%的同一性,以及25%的类似性)。特别地,TPO形成两个高度保守的半胱氨酸桥,这在EPO中的同源位置处也能找得到。
过去已经证明,TPO的N-末端区域负责与细胞因子受体的结合,并由此引起经JAK/STAT途径的被诱导的信号级联反应(Geddis A.E.,Linden H.M.,Kaushansky K:Thrombopoietin:a pan-hematopoieticcytokine.Cytokine & Growth Factor Reviews 13(2002)61-73)。因此,根据本发明还可以只使用TPO的一部分,特别是N-末端的片段。
TPO及其变体的制备和表征例如描述于EP 1 201 246、WO95/21919、WO95/21920和WO95/26746中。出于公开目的,这些文献被完全引用。TPO对于胎儿肝细胞的作用可从E.Schmelzer的试验中获知(E.Schmelzer:Optimierung der Kultur und Charakterisierungembryonaler Hepatozyten der Ratte;Dissertation2002)。
作为TPO的变体,合适的例如有在WO95/21919中描述的TPO衍生物,或者在WO95/21920中描述的等位基因变体或种同源物,或者在WO95/26746和EP 1 201 246中描述的PEG化的TPO,但并不局限于此。在本发明的范围内,PEG化的TPO是指结合在有机聚合物例如聚乙二醇、聚丙二醇或聚氧化烯上的TPO衍生物。其他的TPO变体也可以是这样的TPO衍生物,即其具有低于100%的序列同一性,但具有优选如EP 1 201 246中所描述的TPO的活性。通常,与人TPO(包括其具有TPO活性的片段)相比,TPO衍生物具有至少70%,优选至少75%,特别是至少80%,和尤其至少85%的序列同一性。在本发明的范围内,特别优选的TPO活性是通过TPO或其变体促进巨核细胞或巨核细胞前体增殖、分化和/或成熟为这些细胞的产生血小板的形式。
根据本发明,EPO和TPO不仅可以以其人型的形式进行使用,而且可以以其非人型的形式进行使用。例如,人的TPO与猪的TPO或小鼠的TPO具有超过70%的同源性。
在本发明的一个可选择的实施方案中,不是给予患者造血生长因子本身,而是给予诱导生长因子在受创伤的组织中表达的因子。
例如Naughton BA等人(Age-related variations in hepaticregeneration and erythropoietin production in the rat,Am J Anat.1977 Jul;149(3):431-8)描述了,于新生儿时期和在肾切除术时,红细胞生成素主要在肝和脾中形成,在肝切除术之后的肝EPO形成的现象可以再次增加。这种能力随着年龄的增大而降低。随后,了解到再生中的肝脏作为对于缺氧的应答而产生红细胞生成素(Hepaticregeneration and erythropoietin production in the rat,NaughtonBA,Kaplan SM,Roy M,Burdowski AJ,Gordon AS,Piliero SJ.Science 1977Apr.15;196(4287):301-2)。发现EPO的产生取决于肝脏再生的阶段,其中发现在肝脏生长最强烈的时期中有着最高的EPO浓度。
Dornfest等人(Recovery of an erythropoietin inducing factorfrom the regenerating rat liver,Dornfest BS,Naughton BA,KolksGA,Liu P,Piliero SJ,Gordon AS,Ann Clin Lab Sci.1981 Jan-Feb;11(1):37-46)在1981年描述了,肝脏是形成EPO诱导因子的主要来源。在肾切除术时,在肝脏中的因子的形成增加了。因此,根据本发明的EPO的作用还可以间接地通过给予诱导EPO表达的因子来实现。
同样地,也可以例如通过经中期或长期地给予生长激素(GrowthHormone,GH)而刺激EPO分泌来获得内源性的EPO表达(Sohmiya,M.,Y.Kato.Effect of long-term administration of recombinanthuman growth hormone(rhGH)on the plasma erythropoietin(EPO)and haemoglobin levels in anaemic patients with adult GHdeficieny.Clinical Endocrinology(2001)55,749-754)。

根据本发明给予EPO或TPO,单独地或者在更大的结构缺损时与支架材料一起导致生物学的组织补偿。这些载体材料可以在体外或者说在身体外事先移殖上所需物质,是可生物塑造的(可生物降解的),并且在理想情况下在微结构和大结构以及生物化学方面尽可能地与待补偿的结构类似。生物化学上的近似或同一包括通过胶原和基质蛋白(弹性蛋白、纤连蛋白和在如已知的组织特异性特征中的所有身体基质成分)来重构体内组成。
干细胞可以从不同的、在患者身体中存在的来源获得:骨髓、外周血、脂肪组织、该组织本身、脐带血或脐带组织。可以相应地获得同种异体的干细胞,但是其具有免疫学上的缺点。可以使用胚胎细胞,但是其具有相应的缺点。
在一个特别优选的实施方案中,培养的细胞的结构化的生长过程通过用本发明的生长因子涂覆的基质来诱导。为此,可以将基质依次地用一种或多种所述的生长因子进行处理。
如果涉及生物基质,那么通常为植入物(移植片固定膜、补片(Patch)或导管);移植物(皮肤移植物);用于细胞生长的载体材料,例如所谓的缓慢释放材料(例如基于血纤蛋白和/或聚合物(如多乳酸聚合物(Polylaktid)或聚羟基链烷酸酯)和/或藻酸盐的水凝胶);骨替代材料(例如磷酸三钙);同种异体、自体或异种的、无细胞化或非无细胞化的组织(例如心瓣膜、静脉瓣、动脉瓣、皮肤、血管、主动脉、肌腱、角膜(Comea)、软骨、骨、气管、神经、半月板(Miniskus)、椎间盘、输尿管、尿道或膀胱(参见例如EP 0 989 867或EP 1 172 120));或者基质(例如层粘连蛋白基质、IV型胶原基质和/或基质胶-基质),或者优选饲养层,例如I型胶原饲养层、3T3饲养层和/或MRC-5饲养层,或者胶原网(Collagenflie β)。
有利地,在生物基质上事先移殖组织特异性细胞、前体细胞、骨髓细胞、外周血、脂肪组织和/或纤维组织,例如与来自骨髓的成熟前体细胞一起。事先移殖导致,再生过程在体外就已经开始,因而在将基质植入身体后能够缩短体内的再生时间。
所使用的基质还可以事先进行活化。生物基质或载体结构的活化例如可以借助于等离子体离子化(Plasmaionisation),例如使用过氧化氢,或者借助于激光活化来实现。

本发明的方法特别适合于成熟细胞,即最初分化的细胞,其不再具有胚胎或胎儿的表型。本发明的方法特别适合于人成熟细胞,例如成熟的祖细胞、组织特异性细胞,优选成骨细胞、成纤维细胞、肝细胞和/或平滑肌细胞。
除了停止或减少向培养物中供给所述的生长因子之外,促生长素抑制素和/或TGFβ和/或前列腺素也适合于终止本发明的生长过程。
特别有利的是,在体内局部地使用本发明的方法。为此,可以将生长因子施加到器官(例如肝脏)的切除面上。它们可以局部地或者通过导管局部地或全身地施用。在肝脏切除术的情况下,它们可以在手术之前、期间或之后可选择地或补充地施用。同样也可以将生长因子(例如用于促进软骨再生)直接注射入相关地组织或关节中。因此,生长因子可以通过滑液直接对于新的软骨结构的形成发挥作用。
在一个特殊的实施方案中,除了造血生长因子之外还给予一种或多种下述的生长因子:转化生长因子β(TGFβ)、前列腺素、粒细胞(-巨噬细胞)-刺激因子(G(M)-CSF)、生长激素释放激素(GHRH)、促甲状腺素释放激素(TRH)、促性腺激素释放激素(GnRH)、促肾上腺皮质激素释放激素(CRH)、多巴胺、抗利尿激素(ADH)、催产素、催乳素、促肾上腺皮质激素、β-促细胞激素(β-Celltropin)、促黄体素和/或血管升压素,或者另外还使用一种或多种神经再生因子,优选神经生长因子(NGF),和/或一种或多种血管再生因子,优选血管内皮生长因子(VEGF)和/或血小板衍生生长因子(PDGF)。
造血生长因子在肠胃外作为可注射的微球、可侵蚀的颗粒、聚合化合物(与多肽、聚乙二醇酸一起)、脂质体和小颗粒进行施用。可注射的或可植入的药物递送器具同样也是可以的。这些物质还可以吸入,经皮下、肌肉内注射,或者对于局部施用作为皮肤膏药而给予。
与之相关,可以将膏药与能够达到局部充血并由此增加皮肤渗透性的物质(例如蜂毒)混合。具有或没有直接的偶联或者盐涂层的超离子的(hyperionisch)结构可以更好地转运通过皮肤屏障,和可以与优选正电离结构结合。负电离也是可以的。这些因子可以与杏仁油或霍霍巴油一起使用以改善通过肠区域内或皮肤的粘膜的转运。
可以与聚乙二醇(PEG)结合以克服转运屏障(口、胃、肠中的粘膜;粘膜,角膜)。
已知可以从蛋白质准备混合制剂。悬浮液、胶乳液、固体化合物、脱水的或冻干的粉末是可能的。生长因子可以吸收在颗粒上或包裹入胶囊中。
特别有利的是,干细胞(真正意义上的祖先细胞)与EPO/TPO一起用于组织再生,从而明显地促进从4种基本组织类型募集组织细胞以用于再生过程。EPO/TPO以及其他所述的因子可以与干细胞和例如作为载体基质的血纤蛋白粘结剂混合施用。在需要时,载体基质可以省去,或者用经更强预先结构化的和成形的载体基质替代。这些因子还可以在没有所有生物载体基质的情况下,例如只在含水的悬浮液中,全身性或局部地进行给予。
根据本发明给予EPO通过干细胞的组织特异性特征以及在增加和整合之后的分化而改善了组织再生,并协调了关于基本组织类型的生长。
I.应用领域
1.扁平上皮
根据本发明使用EPO以促进基底膜(上皮下的细胞外支撑层)的形成。EPO有益于乳头的边界区域的形成。
根据本发明使用EPO以促进腺上皮的形成。EPO导致片状的似棱柱形(isoprismatisch)和棱柱形(prismatisch)的上皮(食管扁平上皮、输精管中的高棱柱形(hochprismatisch)的上皮、膀胱的移行上皮)的再生。
通过EPO可以在结构性生长过程中重新形成桥粒,即细胞和其固定结构之间的连接物。还可在例如小肠(附睾、气管)的上皮上形成所谓的微绒毛。
EPO对于单层的扁平上皮,心血管、血管和淋巴管的内皮内衬,和高棱柱形的上皮(胃、小肠和结肠、胆囊、输卵管、子宫)的再生具有直接的作用。根据本发明使用EPO以形成两排的上皮(口腔的唾液腺、鼻泪管、附睾管、输精管)。根据本发明,EPO还用于高棱柱形的上皮(鼻腔、鼻咽腔、喉、气管支气管树、尿道、咽鼓管),进一步用于形成未角质化的和角质化的扁平上皮。EPO的对于实质性再生的促进结构的方面有益于外分泌腺体的形成。属于外分泌腺体的尤其是胰腺、小肠上皮中的腺体(杯状细胞、下颌骨唾液腺、用于产生胰岛素的胰岛细胞)。EPO有益于内分泌腺体的再生。
2.结缔组织
EPO能够恢复与周围的实质腺体结构相协调的结缔组织的基本组分。由此受影响的基本组分是与下列所述的相协调的基本物质(葡糖胺聚糖):所谓的浆细胞、脂肪细胞、血管和围绕其的平滑肌细胞,此外还有结缔组织细胞、成纤维细胞、肥大细胞和胶原纤维以及弹性蛋白纤维和在这之间插入的巨噬细胞(还有在肝脏中的枯否细胞、隐窝细胞、贮脂细胞)和毛细血管内皮细胞。毛细血管只有内皮细胞内衬。
但是,在此,EPO对于所述的微环境具有更广泛的协调内容。微环境通过创伤性损伤而被置于起始状态。如果在创伤性损伤后应当加速起始级联反应,则可以根据本发明治疗性地给予EPO。在此,根据本发明,EPO作为组织再生的催化剂以及全身性和局部的媒介物,有益于由巨噬细胞区域性地引发的免疫调节效果,这种效果在联合了浆细胞的巨噬细胞中包括免疫调节以及白细胞介素和细胞因子的释放。通过本发明的相互作用能力,能够获得在所需位点(创伤、炎症、自身免疫疾病)治疗性地给予EPO/TPO的高的区域选择性。
根据本发明,EPO在治疗性的应用中协调具有全身性的、内源性的应答的区域性免疫刺激。由此,作为治疗效果,EPO在级联反应加速的意义上促进炎症反应的起动。其增强了内源性生长因子的释放,并因而还控制其副作用潜能。因此,可以避免全身性地给予生长因子,和在生理作用情形的范围内避免其在潜在的致瘤性方面不受控制的作用。
根据本发明可以修复结缔组织结构。属于此的有胶原纤维,网状纤维,例如淋巴结,弹性纤维,和形成基本物质(蛋白聚糖、葡糖胺聚糖、透明质酸、硫酸软骨素、硫酸皮肤素、硫酸角质素、硫酸乙酰肝素、肝素)。受调控的结缔组织细胞为成纤维细胞、网状细胞(特别是在淋巴组织和器官中的)、间质细胞、脂肪细胞、单核细胞和巨噬细胞。已知单核细胞能够分化成巨噬细胞。在随后的和必需的组织修复和3-D再生方面,EPO可以促进在炎症中和在感染反应时的巨噬细胞防御。
巨噬细胞的趋化功能由EPO一起进行协调。位置固定的、在结缔组织中存在的单核细胞能够被EPO卷入组织募集之中,由此它们能够在全身和局部可动用的祖先细胞的意义上直接对于位置固定的修复作出贡献。
EPO协调网状内皮系统(retikoenterial System,RES)的功能。其被筹集在网状纤维的网络中,并将肝、脾和骨髓的内皮细胞包括进来。此外,通过EPO可协调淋巴细胞活性,协调浆细胞、巨噬细胞活性,和免疫调节地协调肥大细胞活性。

在组织再生中,出现结缔组织的形成和松弛的结缔组织结构(皮肤的皮下组织,在空心的器官中,少量特异化的结缔组织)的构建。EPO导致结缔组织(无序的和有序的结缔组织,在系带、韧带、筋膜、肌腱中形成并与弹性结构及其三维网状结构(其组织特异性地混合或者还以胶样的形式表现出来)相协调)的形成。
EPO有益于特别可修复网状结缔组织的网状细胞的形成。EPO有益于形成脂肪组织和通过募集而归入其中的祖先细胞,和使用EPO的增殖功能。
通过本发明的作用,能够改善这些结缔组织成分的支撑功能和能量传递。能够改善经胎盘的物质交换,因为结缔组织掌管着身体血管。能够改善储存有脂质、葡糖胺聚糖的结缔组织结构的存储功能。EPO可治疗性地引起水和电解质功能的改善。
通过EPO可导致防护功能的改善,因为导致重新形成了抵抗致病性病原体的机械屏障。在紧接着炎症之后引起修复性的过程,因为修复了组织结构。根据本发明,EPO有益于实质性再生和结构性再生。
这与在形成肉芽组织(如其对于所谓的伤口愈合来说是特征性的)时所见的不同,肉芽组织在伤口愈合的意义上导致替代组织并因而只导致替代功能。在这方面有瘢痕组织和松弛的纤维组织,其能够完成缺损覆盖,但是这种缺损遮盖是与高的功能损失相联的。在此,根据本发明,EPO在修复以前的组织的意义上具有形成结构的过程。但是,在特别大的缺损中,与位置支持材料(Platzhaltermaterialien)的组合有益于和促进了形成结构的过程(在形态上和功能上)。这对于组织过程来说具有重大的意义,因为可以将每种用于整合的支架材料(例如用于血管、心瓣膜、移植物的载体材料)与EPO组合。永久性植入物(不可生物降解的材料)可以进行涂覆以获得更好的组织整合(例如金属和陶瓷的假体)。
3.血管系统和淋巴循环系统
如迄今已知的,EPO导致重新构造血管系统以及还有大血管,不只是毛细血管结构。静脉和动脉(大和小的体循环)也属于大血管的结构。
此外,EPO使得能够在还涉及淋巴循环系统的组织结构以及淋巴管自身的内部进行再生过程。通过EPO-整合的修复不仅涉及内膜、内皮,而且还尤其涉及由结缔组织和肌细胞构成的中膜。此外,还有构造纵向的结缔组织层(Bindegewebslage)的外膜。在构造血管时,重要的是各种不同类型的动脉。因而,存在弹性肌肉构造类型的动脉和通过再生过程一起诱导的不同结缔组织结构(即弹性内膜和弹性外膜),以及构造vasovasalen血管。在此,将脂肪细胞整合入壁层中。在弹性构造类型的动脉的情况下,EPO使得能够在除了具有Vasovasolen的外膜之外还再生出弹性内膜。在横截面为0.1-7mm的肌肉构造类型的动脉的情况下,EPO使得能够在血管中膜中强有力地形成肌群。此外,EPO使得能够形成内皮下的结缔组织状况。能够修复典型的构造形式,例如迄今已知为内皮细胞-成分的内膜,但尤其还有弹性内膜、弹性相结构、肌细胞、中膜、弹性膜和外膜。通过治疗性地给予EPO还能够达到空间上的几何布置,例如在处于特殊的相邻几何布置的伴随性小静脉的范围内形成肌肉小静脉。
这不是具有聚集物形成-推动力的二维结合过程,而是特异的三维结构形成过程。因此,根据本发明,EPO和TPO并不等同于传统生长因子意义上的已知的增殖因子,而是作为“3-D再生因子”而发挥作用。
在具有内膜、中膜和外膜的形成的静脉中,在EPO-刺激的范围内也存在类似的结果。此外,特别是在与具有几何形状的支架材料联合时能够再生出静脉瓣结构,或者还有一些支架,只要其在创伤性损伤和退行性过程之后在更大的血管范围内是可能的。
在更大体积缺损的情况下,或许可以将载体材料一起植入,从而将EPO-作用集中于重塑过程和整合入整个生物体中的过程。但是,在完整的再生过程中,尤其是在实质性结构中,一起形成这些血管。
不存在对于不同血管结构(例如动静脉吻合术)的限制。与治疗性地给予EPO相联系,特别还可以修正由于年龄而引起的血管和退行性结构的改变。在变老的过程中,形成平滑的纵肌群结构。退行性过程表示根据组织结构的构建原则安排的新结构形成活性。这可以作为重新制备的血管而在组织结构中形成,并且首先不发生病理性改变。
根据本发明,炎症反应可以借助于给予EPO而进行调节。借助于给予EPO,在心内膜以及心肌的范围内存在再生过程。心外膜是心包的层(Blatt),其通过给予EPO而导致再生。心包壁层(parietaleBlatt)是可靠的膜,其由处于结缔组织状况的间皮细胞层构成。在心脏的范围内,如果引来结构辅助物(支架)并联合干细胞移殖和EPO-给予,出现了心瓣的再生和特别是然后还有腱索的再生。刺激传导系统的重新形成可以在区域性的破坏之后进行。属于此的还有浦金埃氏纤维,以及还可以被修复的房室结。另外,将淋巴管、神经、心神经丛、冠状动脉-结构相互结合在一起。
瓣膜管和联集管是同样在结构形成过程中一起整合的皱襞(Falten)。在血管系统中,出现成血管细胞的活化,其部分是从外周的,但还有区域性的祖先细胞中募集的。首先,修建了由内皮细胞结构构成的毛细血管。但是,在EPO应用范围内重要的是,可以通过形成顺次连接的肌肉结构和结缔组织结构来起始结构形成。
刺激传导系统对于EPO作用的协调起着重大的作用,因为存在与调控性细胞和结节(Knoten)的相互作用。
淋巴细胞、巨噬细胞、浆细胞、干细胞属于免疫系统的细胞。网状结缔组织的网眼织物结合免疫系统中的免疫机构细胞。通过给予EPO,出现了加快的网状结缔组织、网状细胞和网状纤维的修复。同样也可以重新修复胸腺结构。在那里,在组织学上恢复皮质和髓质结构。当给予EPO时,在组织学上还再生出淋巴结,其能够重新形成中间窦、髓窦-、缘窦-、囊-和结缔组织中隔(Bindegewebssepten),但还有流出性淋巴管、淋巴小结或髓索
组织学构造与随后进行的修复过程相关,和与具有囊-和结缔组织中隔、门结缔组织和流入/流出性淋巴管、淋巴小结、髓索和流出性淋巴管的通常结构的经相互协调的联合相关。
再生原则还可以在脾的范围内导致正常的囊、颗粒结构和门的恢复。此外,还发现在乳索中脾窦板的白髓马耳皮基氏小体的恢复。恢复了脾的血液循环,其中将脾的动脉、脾窦静脉和髓静脉相聚在一起。恢复了白髓以及淋巴鞘的结合。
通过在损伤后给予EPO,出现了基底膜、网状细胞、网状纤维和红细胞的三维相互作用。形成了开放的和封闭的循环系统。在此,与可以从骨髓细胞产生的巨噬细胞的相互作用具有能够起始身体内的内源性级联反应和生长介体(生长激素)的间介作用的意义。
4.皮肤/皮
在皮肤范围内的烧伤、创伤、烫伤、机械性损伤或炎症之后的修复过程是非常多种多样的。通过给予EPO或TPO,可以在有或没有结构性替代结构的情况下获得再生。
特别是在慢性疾病、血流障碍、糖尿病性溃疡、还有免疫性质的现象的情况下,EPO在此可以调控性地参与。在组织学上看到,恢复了正常的表皮、真皮和皮下组织,其完成了相应的具有微结构的血管形成。在手掌和足底上,形成了基底层、粒层、透明层、角质层。在真皮和外侧上形成了真皮乳头(Dermispapillen)。在表皮(内表面)上,重新形成了相应的表皮凹陷。另外,共同促进了附属器官及其从祖先细胞的形成。在迄今的组织工程中的一个问题是,在皮肤范围内结构性修复与皮肤附属器官的形成之间的结合。
通过给予EPO能够在人的黑素细胞的范围内达到相互作用过程。考虑了眼、脑膜和色素沉着的形成之间的生理学反馈过程,因为在此不涉及分离的细胞化合物,而是涉及器官类型的再生产物。
真皮部分的再生过程包括肌肉结构(立毛肌(musculus arectorpili))、触觉结构、毛发的毛根鞘外层和内层以及毛球结构。这或许突入脂肪组织中。在毛发和毛囊中,出现了结缔组织状的毛根鞘,其包含在再生中重要的功能结构。由此形成玻璃样膜、毛根鞘外层、毛根鞘内层和最终的毛发。在甲再生的范围内可以支持和引起类似的构成。在组织结构的内部,一起形成了汗腺。在皮肤结构中,重新形成了毛囊周围的网结构、网络、毛细血管下的动脉网、末端的(termale)静脉网。
5.消化系统
通过给予EPO,修复了粘膜的最内层和所谓的固有层的结缔组织层。最深的层形成了粘膜肌层中的一层平滑肌群。层的构造如下:浆膜细胞、肌层、粘膜下层和粘膜层。这些结构被消化腺穿过,粘膜腺体存在与粘膜层中。另外,再生出了粘膜下层腺体和肠肌丛(Plexusmyintericus)(Auerbach)和粘膜下丛(Meiβner)。在舌的范围内,再生出叶状乳头。在口的范围内,在组织再生的范围内还一起再生出了味蕾。这是由4型基底细胞、1型支持细胞、3型受体细胞、糖蛋白和味孔形成的。浆液粘液的唾液腺由纹状管、闰管、粘液的末管(Endstücken)、基底膜、肌上皮细胞和浆液的末管。当牙周病时,重要的修复过程还可以在牙范围内进行。例如,其涉及联合了牙槽骨和带尖的牙根管的牙周组织的结构。修复过程可以通过额外引入移殖有干细胞的磷酸钙结构或其他矿物质替代材料来完成,它们涂覆有EPO。由EPO促进的再生过程的形成在处于上颌窦、下颌结构的范围内的囊的范围之中,在牙根切除术和于体内原位重新修复这些结构之后进行。
相应的材料和成牙质细胞同样可以在体外通过给予EPO来进行准备。
骨软骨结构例如骨软骨圆柱体(osteochondrale Zylinder)可以在体外进行准备,和在给予EPO的情况下以促进生长再生的方式植入体内。具有新的神经支配的牙根管结构的形成通过递入的经EPO涂覆的导向结构(Leitstrukturen)而得到促进。在此,细胞外基质的分子是有帮助的,其在涂覆过程中还可以作为肽结构而被引来。与牙髓成牙质细胞、中间区牙釉质-牙髓(Zwischenzonenschmelz-Pulpa)一起的牙釉质器官结构可以在损伤和创伤性过程之后的生长过程中得到促进。修复和再生的过程集中在周围的结缔组织的范围内,和牙结构或者颌区域内的植入物的整合。在此,髓-结缔组织和具有向内和向外阻挡牙根管尖的血管的构造对于神经支配起着大作用。因此,再生出牙周组织(牙根膜)。沙比氏纤维穿过牙根膜,并锚定在在牙骨质或牙槽骨中。

咽:

9.肝脏

以通常的方式通过胶原酶消化,从非可移植的器官或肝脏切除物中分离肝细胞(Bader,A.,Rinkes,I.H.B.,Closs,I.E.,Ryan,C.M.,Toner,M.,Cunningham,J.M.,Tompkins,G.R.,Yarmush,M.L.(1992)A stable long-term hepatocyte culture system for studiesof physiologic processes:Cytokine stimulation of the acutephase response in rat and human hepatocytes.Biotechnol Prog.8,219-225)。
将分离的或预培养的细胞置于液氮中。在根据已知的方案将细胞融化(Karim,N.,Allmeling,C.,Hengstler,J.-G.,Haverich,A.,Bader,A.(2000)Diazepam metabolism and albumin secretion ofporcine hepatocytes in collagen-sandwich aftercryopreservation.Biotechnol Letters 22:1647-1652)之后,将肝细胞悬浮液/培养物与100-150IU/kg/KGW的阿法依泊汀(相对于受者)混合。为此,将阿法依泊汀以体积为1-1.5ml的无菌溶液添加到浓度为2-10百万个细胞/ml的10ml肝细胞悬浮液中。
缓慢地于门静脉内注射这种悬浮液(1ml/分钟)。另外,将该悬浮液与1000IE肝素混合以避免血栓形成。
可选择地,细胞/EPO混合物还可以在多个位点,注射在肝纤维囊之下或者直接注射入肝实质中。为此,合适的是将肝细胞的浓度提高2-5倍,并相应地减少注射体积。
将穿刺创道用商业上常用的血纤蛋白粘结剂(Baxter Tissucol)封闭。可选择地,可以使用具有Kollagenflies的填塞物。该填塞物同样也可以与阿法依泊汀溶液混合。要注意,粘附位置上的填塞物仍要保持干燥。
血纤蛋白粘结剂是双组分混合物。通常,一个成分由血纤蛋白原构成,另一个成分由具有Ca++和蛋白酶抑制剂(例如抑酶肽)的活化溶液构成。可以通过以100-150IU/kg/KGW的终浓度添加混合而向该活化溶液中加入阿法依泊汀。
类似地,可以使用干细胞,其来自骨髓、脂肪细胞、特殊实质或源自脐带血的血液(从血沉棕黄层(Buffy Coats)中纯化,CD34阳性细胞)和源自脐带组织的间质细胞。
与在局部缺血性、毒性、感染性或机械性(创伤性)受损的区域内进行细胞移植的同时,可以将细胞引入血纤蛋白粘结剂或类似的血浆中,并在添加阿法依泊汀(100-150IU/kg/KGW)的情况下进行聚合化。
与此同时,可以开始以每次10000IE皮下给予EPO,从而一个星期内总共施用40000IE。
结果是提高了2-3倍的组织再生,其导致结构性的修复。
2.手术后给予
在处于心-胸血管领域、内脏外科学、整形外科学中的扩展的手术之后,还有在扩展的创伤之后,出现了缺损状况,其导致患者的致命的危险。
在此,以100-150IU/kg/KGW的浓度给予阿法依泊汀。
通过给予EPO,导致生长激素内源性地增加2倍,由此加快了手术之后地组织再生。比作为对照而不给予EPO的患者早大约1-2周出现了恢复和复原。
EPO还可以在劈离式肝移植(split liver transplantation)或肝移植之后给予以诱导肝脏再生。
在肝移植之后出现了,移植的组织,或者保留的组织(在活性供体中),并不足够快地和以足够的量作为活性组织以供使用。在这种情况下,可以在手术之前24小时内和正在手术的时候和手术之后间隔24小时内给予100-150IU/kg/KGW的阿法依泊汀。由此,出现了明显加快的肝脏再生,其中特别是在第4-5天可以用超声波诊断出体积增大。
3.在当良性和恶性肿瘤时进行肝脏切除术之后给予
当根据需要进行扩展的肝脏切除术时,获得了加快的再生,因为足够细胞质量的可用性对于患者的存活来说是重要的。在切除术之后,可以在全身性给予时给予100-150IU/kg/KGW的阿法依泊汀,或者在局部施用(在血纤蛋白粘结剂、血浆中)时给予相应量的阿法依泊汀。
当对持续性的肿瘤侵害或者非可重复剖析的(ressezierbar)转移有怀疑时,不仅可以全身性地而且可以根据肿瘤类型和预后局部地,联合EPO施用常见的细胞抑制剂。
在切除术和给予EPO之后,与非治疗组相比,治疗组出现了加快了30%的增大生长。首先,切除面上的肝细胞在结构性生长过程中增强地出现。完全形成了血管树和在其周围存在的组织。在此,在具有血管分布并具有有着非实质细胞(枯否细胞、隐窝细胞、贮脂细胞和内皮细胞)的细胞片(Zellplatten)的小叶中,以通常的相应于正常器官的方式布置肝细胞。
出现了全身性的增大生长。增大生长在达到初始大小时停止。
在手术期间给予EPO的组在手术后1-2天就已经具有高大约0.5g/dl的Hb-值。这被看作是EPO的已知作用的标志。但是,同时出现了肝脏再生。在此,不仅肝细胞增加了,而且所有细胞类型,尤其是作为肝脏的结构骨架的结缔组织结构增加了。
试验的实施:
将28头母猪(重量40.0-62.0kg)根据随机原则分成三组。采用腹部内窥镜检查术的技术进行左侧肝脏的部分切除。
对照组(n=16)不给予EPO。第2组(n=6)在肝脏切除表面上局部给予10,000个单位的EPO和血纤蛋白-密封料(Quixil)的组合。第3组同样进行处理;但是,除了局部EPO治疗之外,还在第0、3、7和11天对猪全身性地给予10,000个单位的EPO。
在第0天从切掉的肝脏块中,在切除术之后24小时从切除表面下1cm的区域中,在切除术之后14天从切除表面和右侧小叶下获取肝脏样品。
为了检测Ki-67抗原、PCNA(PZNA)(增殖细胞核抗原)和凋亡,在用福尔马林固定并包埋在石蜡中的肝脏组织上实施链霉抗生物素-生物素免疫过氧化物酶检测法。

4.优化的体内假体或植入物
植入物可以由可生物降解的材料构成,但也可以由永久材料构成。对此的一个实例是金属体内假体,例如在髋区域内。在紧接着制备了金属的粗制形式之后,通过磨蚀、浸蚀或激光处理进行微结构化。由此,能够产生在从0.1至50-60μm的范围内的开放孔隙率和粗糙度。
随后,这些结构在相纯的β-磷酸三钙的溶液中进行填满,从而获得了均质的表面涂层。然后,理想地,将这些结构经历进一步的烧结过程以加固β-TCP结构。
在矿物质结构中可以加入可溶的盐/糖,或者引入产气剂,以获得进一步的相互连接性的孔隙率。紧接着加固过程之后,将这些结构用本发明的生长因子或者其衍生物、部分或类似物浸渍或涂覆。通过用EPO填充空穴或者使用缓慢释放的物质,可以相应地将贮存库放置在表面上。可选择地,也可以就在进行植入之前,用EPO和其类似物涂覆无菌地从包装中取出的植入物。
由此,出现了改善的与植入物的组织结合过程。骨在大血管上进行了结合,和植入物得到了促进并与骨更持久地整合。
以类似的形式,可以准备在口、颌和面部区域中的植入物(牙植入物)。
可能与在具有干细胞的生物反应器中的细胞移殖相结合。
生物植入物(血管、心瓣膜、皮肤)和膜同样也可以用EPO和/或GH和/或TPO进行涂覆。
5.骨质疏松症的治疗
通过针刺法将用EPO涂覆的三磷酸钙颗粒引入自体血浆溶液中的缺损/变稀的椎体中。在那里,颗粒加快地被包围在内源性的骨中,和以同化效果转变了变性过程。
这种效果可在受到急性侵入威胁的椎体的情况下使用。可以采用与移殖过程的组合,有利地与来自自身骨髓或者说骨膜和血液和/或脂肪组织的干细胞。
6.适应症:软骨再生
软骨细胞是强烈营养作用徐缓的组织。通过区域性创伤和擦伤会出现炎症过程,其能够变成关节炎。通过在关节腔中或者全身性地和/或与软骨细胞的细胞移植物或骨软骨圆柱体相组合而给予EPO,促进了组织再生和结构性的重新构建。可以组合地全身性或皮下给予10000IE/天。
7.适应症:皮肤疾病
愈合不好的伤口在准备好创底之后根据本发明用EPO或TPO(衍生物、类似物、部分)涂覆。为此,优选地进行机械清创术。在血液凝聚胶中引入10000IE的EPO。这一过程可以重复进行直至清洁了伤口基部(Wundgrund)。
这种2-3天之后开始的生长导致肉芽组织加速形成。
8.适应症:炎性肠病
在具有体重减轻和贫血的肠道炎症现象的情况下,已经证明贫血不是由于营养吸收不良的继发现象,而可能是原始EPO缺乏的伴发表现。以10000IE/天的量给予EPO导致肠修复/再生的改善。
9.适应症:神经再生
在割断脊髓之后,EPO引起神经元的结构性生长和轴突的重新伸出。给予维生素C起着促进作用。
EPO可以在局部区域与血纤蛋白/自身血浆和/或身体自身的干细胞(骨髓,血液CD34,来自脂肪细胞,骨膜,脐带)联合给予。
10.适应症:帕金森病/具有已经消退的炎症反应的慢性疾病的实
例
将通过使用可降解颗粒的刺激而受到刺激的自体巨噬细胞定位地整合入变性的区域中。与此同时,将EPO(10000IE)联合自体干细胞(0.3ml)注射入该区域中。与此同时,以2周的时间stemisch给予EPO。在其他慢性疾病的情况下,也可以使用通过巨噬细胞诱导炎症的这种刺激原理,在所述这些疾病中没有急性创伤或急性炎症反应占据主要地位。
11.烧伤之后的伤口愈合
在8位烧伤患者中,当给予EPO时,皮肤移植物的供体位置的痊愈快50%。如果将EPO给予患者,则面部的深二度(2B度)烧伤可以以不形成疤痕的方式痊愈。当不给予EPO时,这种伤口在痊愈的同时会形成疤痕。

1.红细胞生成素(EPO)用于制备促进创伤组织结构性再生的药物的用途,其中所述创伤组织是肝脏组织或皮肤组织。
2.权利要求1的用途,其特征在于,所述药物用于局部施用或全身性施用。
