发明背景
很多年来,能用于向人类癌症特异性送递化疗药物的抗体一直是特异性导向的药物治疗领域的科学家们的一个目标。这个目标的实现最终可以给癌症化学疗法带来魔力弹这个概念。朝着这个目标的显著进展随着1975年Kohler和Milstein的杂交瘤技术的出现,以及接之而来的制备单克隆抗体(mAbs)的能力而发展。在过去25年间,已经制备出抗癌细胞上过量表达的很多抗原靶的mAbs。针对药物、毒素、放射性核素或其他治疗剂本身,或者它们的偶联物,已经临床前,并且随后临床上试验了很多mAbs。一般情况下,mAbs本身,经常称作“裸mAbs”,对于患有实体肿瘤的患者以长期存活标准看还没有成功,虽然最近用抗乳腺和结肠癌的mAb治疗已经看到存活优势(分别抗HER2-neu和17-1A的mAbs)。对于血液恶性肿瘤,使用裸mAbs,特别是抗B-细胞淋巴瘤(抗B-细胞表面上CD20和CD22的mAbs)实现更大程度成功。

在为研究人癌症而建立的临床前动物异种移植模型中,描述过能竞争性消退甚至治愈动物肿瘤的很多mAb偶联物。然而,在很多这样的动物异种移植模型中,肿瘤摄入mAb偶联物经常是每克组织10-50%注射剂量,而在临床情形下,每克组织肿瘤摄入0.1-0.0001%注射剂量更正常。意料之中的是用更具有毒性的放射性核素和毒素制备的mAb偶联物临床上一般比用标准化疗药物制备的相应的mAb-药物偶联物远远要好。然而,放射性核素mAb偶联物由于与肿瘤定位活性相比大大过量的循环的衰变的放射性的存在经常能产生大的毒性。毒素-mAb偶联物有着双重缺点:大的非靶组织损伤和大的对一般使用的植物或细菌蛋白质的免疫反应性。而现在能制备人或人源化形式的(互补决定区移植的)mAbs,任何偶联物的毒性部分的去免疫作用可能显著阻碍进步。
虽然目前可看到临床研究中没有必然的效力,但是mAb-药物偶联物仍然具有令人感兴趣的理论优点。药物本身结构是完全确定的,不以异构体存在,并且通常在远离mAbs的抗原结合区的特定位点的非常确定的偶联化学,可以与mAb蛋白连接。与涉及Abs和毒素的化学偶联物相比,MAb-药物偶联物制备更具有可重复性,因此更有商业发展前途并且容易获得管理批准。因为这样的原因,尽管碰到挫折,但是对mAb药物偶联物的兴趣持续不断。但是,在最近的一些情况下,临床前结果已经相当有希望。在偶联化学中继续精益求精,最新考虑了去除或减少mAb的免疫原性质的能力,考虑到有用的mAb-药物偶联物对于临床癌症治疗的难以捉摸的前景。
对mAb-药物偶联物的相关早期工作发现,在使用化学键的体外和体内临床前试验期间经常导致药物效力的损失。因此,很多年以前就认识到,理想地,一旦通过mAb成分被靶细胞内化,为了成为有用的治疗,药物需要以它原来的形式释放。然后上世纪八十年代和九十年代早期焦点大都集中在药物和mAb之间化学接头的性质上。尤其是,根据肿瘤中的pH经常低于正常生理pH的发现,开发了使用温和的酸可裂解的接头制备的偶联物(美国专利Nos.4,542,225;4,569,789;4,618,492;和4,952,394)。这项研究在Trail等的划时代文章中达到顶峰(Science 261:212-215(1993)),这篇文章证明,临床前研究中,用合适的接头制备的mAb-阿霉素(DOX)偶联体能用来治愈携带各种人肿瘤异种移植物的小鼠。使用在被导向的肿瘤细胞上有非常大量的受体的抗体(称作BR96)实现这个有希望的结果,mAb-药物偶联物被高度取代(每个mAb单位6-8个DOX残基),以反复为基础,以大剂量施用偶联物。
在临床情形中,肿瘤对mAbs的摄入非常低,因为这个变量在某种程度上是寻址的更有毒性的药物,需要实现期望的治疗效果。在几种不同的mAb-药物偶联物的开发中使用更有毒性的药物(美国专利Nos.5,208,020;5,416,064;5,877,296;和6,015,562)。这些努力使用药物,例如maytansinoids和calicheamicin的衍生物,它们本质上太有毒性不能在标准化疗中使用。与mAb的偶联使得相对于单独化疗可见到的经常是非特异性的细胞和蛋白质结合,相对更多的药物被导向于肿瘤。由于肿瘤靶上通常可见的低水平抗原结合位点,象这些的药物的敏锐毒性可能会克服临床上见到的肿瘤-定向的mAb的低水平。在临床前研究中,在比先前用标准药物例如DOX,用mAb-药物偶联物见到的相比低得多的mAb-药物偶联物剂量,见到携带人肿瘤异种移植物的小鼠的治愈(Liu等,Proc.Natl.Acad.Sci.USA93:8616-8623(1996)和Hinman等,Cancer Res.53:3336-3342(1993))。在maytansinoid-mAb偶联物情况下(Liu),治疗所需要的偶联物的量比以前用DOX偶联物(Trail,上文)需要的少50倍以上。
在这些偶联物的开发中,认为药物和mAb之间的接头对于体外和体内保留好的抗肿瘤活性是决定性的。用分子内可裂解部分(腙)和药物和mAb之间的易发生还原的键(二硫键)制备所述偶联物。体内血清条件下腙键显然是稳定的,发现二硫键对于实际使用通常不足够稳定。在calicheamicins情况下,以位阻大的(成对二甲基)二硫键置换标准二硫键而制备偶联物,或者在maytansinoids情况下用甲基二硫键置换标准二硫键而制备偶联物。进行这项工作时,对较新的蒽环霉素-取代的mAb偶联物继续分离工作。在较新的DOX偶联mAbs情况下,发现通过仅插入腙作为可裂解单元,并且通过硫醚键而不是二硫键连接DOX和mAb,获得卓越结果(美国专利No.5,708,146)。当以相同的方式连接时,还使用能使每个Mab取代位点DOX单元数目加倍的支化接头,获得新的DOX-Mab偶联物效力增加近一个数量级(King等,Bioconjugate Chem.10:279-288,(1999))。
发明概述
本发明涉及蒽环霉素药物的新的内化抗体偶联物。具体实施方案举例来说是阿霉素(DOX),表柔比星,吗啉代阿霉素(吗啉代-DOX),氰基吗啉代-阿霉素(氰基吗啉代-DOX),和2-吡咯啉基(pyrrolino)-阿霉素(2-PDOX)的偶联物。2-PDOX是特别有毒的,其结构中插入烯胺,其不仅作为嵌入剂和拓扑异构酶起作用,而且还作为具有增加的毒性的烷化剂。和DOX一样,2-PDOX具有相对好的水溶性,这意味着其能以多取代量偶联mAbs而不沉淀mAb。下面详细描述的药物始终以每mAb分子8个药物部分的平均水平(典型地测得7-9)被取代。但是药物的数目也可以是每mAb分子6-10个分子之间的范围。

导论
化疗药物,例如上面讨论的那些,能通过几种方法与抗体偶联,形成mAb-药物偶联物。例如,化疗药物可以在还原mAb链间二硫键之后与mAb,或其片段连接。这种方法产生每个抗体分子平均8-10个(取决于IgG类型)游离巯基,并且以在还原反应中使用的硫醇的有限水平以可重复方式这样做。这种化疗药物的连接方法是有利的,由于下面的原因:首先,在亲水性赖氨酸残基上没有暴露的mAb或其片段上内部或半内部位点中放置连接的化疗药物。由于mAb的放置了化疗药物的更多的疏水性区域而使它们保持更稳定。第二,这样的位点不改变mAb或其片段的总电荷。第三,当使用mAb裸形式时,内部硫醇的放置较不可能干扰特别重要的ADCC和补体作用。因此,选择不干扰的连接位点,使得ADCC和补体固定,能与mAbs,或mAb片段互补,起药物送递载体作用。第四,与暴露的赖氨酸基团上放置多个化疗药物分子相比,内部硫醇位置的放置较不可能导致对化疗药物的免疫反应。在一些实施方案中,与偶联前抗体电荷相比,Ab-药物偶联物中抗体的总电荷不改变。这是因为在偶联反应中没有利用赖氨酸残基,因此,没有游离的带正电荷的氨基被修饰形成例如中性酰胺键。
抗体
如这里描述的,抗体指全长(即,天然存在的或者正常免疫球蛋白基因片段重组方法形成的)免疫球蛋白分子(例如,IgG抗体)或免疫球蛋白分子的免疫活性(即,特异性结合)部分,象抗体片段。
抗体片段是抗体的一部分,例如F(ab′)2,F(ab)2,Fab′,Fab,Fv,scFv(单链Fv)等。不考虑结构,抗体片段结合完整抗体识别的相同的抗原,因此,它是抗体的一部分的抗原结合片段。
术语“抗体片段”还包括通过与特异抗原结合形成复合体的象抗体一样作用的任何合成的或基因工程蛋白质。例如,抗体片段包括可变区构成的分离的片段,例如由重链和轻链可变区构成的“Fy”片段,其中重链和轻链可变区通过肽接头连接的重组单链多肽分子(“scFv蛋白质”),和由模拟超变区的氨基酸残基构成的最小识别单位。可以以不同方法构建Fv片段,得到多价和/或多特异性结合形式。多价结合形式与抗特异性表位的一个以上结合位点反应,而多特异形式与一个以上表位(抗原的或甚至抗特异性抗原和不同抗原)结合。

在优选实施方案中,在人疾病的治疗中使用的抗体是人或人源化(CDR-移植到人构架中)抗体形式;但是能使用鼠,嵌合和灵长动物化抗体形式。对于兽医应用,相同物种IgG可能是最有效的载体,但是交叉物种IgGs也是有用的,例如对狗使用鼠抗体(例如,L243抗-HLA-DR mAb用于治疗犬淋巴瘤)。最优选使用相同物种免疫球蛋白(IgG)分子作为送递试剂,使免疫反应最小化。当考虑反复治疗时,这是特别重要的。对于人,人IgG抗体或人源化IgG抗体较不可能产生患者的抗-IgG免疫应答。导向于内化抗原,在与靶细胞结合之后,象hLL1和hLL2这样的抗体快速内化,这意味着偶联的化疗药物快速内化于细胞中。
免疫调节剂,例如细胞因子,也能偶联于单克隆抗体-蒽环霉素药物,或者能施用与本发明的优选实施方案的嵌合,人源化或人单克隆抗体-蒽环霉素药物偶联物不偶联的免疫调节剂。可以在施用本发明的优选实施方案的单克隆抗体-蒽环霉素药物偶联物之前,同时或之后施用免疫调节剂。免疫调节剂还可以和由与不同抗原结合的一个或多个抗体构成的杂合抗体偶联。这样的抗原也可以是免疫调节剂。例如,在施用抗体组合的同时,之前或之后,CD40或其他免疫调节剂可以和抗-CSAp或抗-CSAp/非-CSAp抗体联合给药。单克隆抗体-蒽环霉素偶联物可以与抗CD40联合,或者与抗CD40偶联成融合蛋白来使用。

偶联物的抗体部分可以是鼠,嵌合,灵长动物化的,人源化,或人抗体部分。抗体可以是完整免疫球蛋白或者其抗原-结合片段,例如IgG或者其片段。优选地,抗体针对B-细胞,例如选自CD19,CD20,CD21,CD22,CD23,CD30,CD37,CD40,CD52,CD74,CD80,和HLA-DR的抗原。抗体,其抗原-结合片段或融合蛋白优选选自LL1,LL2,L243,C2B8,A20,MN-3,M195,MN-14,抗-AFP,Mu-9,PAM-4,RS7,RS11和17-1A。更优选地,抗体是LL1,LL2,L243,C2B8,或hA20。另外,抗体通过与抗体上还原的二硫键连接的接头与药物连接,其可以是抗体上的链间二硫键。
偶联物的蒽环霉素药物部分选自柔红霉素,阿霉素,表柔比星,2-吡咯啉基阿霉素,吗啉代-阿霉素,和氰基吗啉代-阿霉素。此外,蒽环霉素药物可通过13-酮基部分与抗体连接。优选地,每分子抗体有6-10个分子的蒽环霉素药物。另外,抗体-蒽环霉素偶联物内化到靶细胞中,然后抗原在细胞表面上再表达。
本发明涉及这里描述的偶联物的制备方法,其中接头首先与蒽环霉素药物偶联,从而产生蒽环霉素药物-接头偶联物,并且其中蒽环霉素药物-接头偶联物接着偶联硫醇还原的单克隆抗体或抗体片段。与硫醇还原的单克隆抗体或抗体片段偶联之前可以将蒽环霉素药物-接头偶联物纯化,但是不必须这样做。因此,优选地,在与硫醇还原的单克隆抗体或抗体片段偶联之前不需要纯化蒽环霉素药物-接头偶联物。偶联物的制备方法应该如此,使得蒽环霉素药物上第二反应官能团不被破坏。另外,偶联物的制备方法不应该破坏蒽环霉素药物上的烷基化基团。偶联物中的蒽环霉素药物优选是2-吡咯啉基-阿霉素,吗啉代-阿霉素或氰基吗啉代-阿霉素。
为了与抗体偶联,分别将化疗药物分子活化,使得它们包含在中性pH下对硫醇反应特异性的游离马来酰亚胺基团。当化疗药物带有反应性酮时,利用可商购的接头4-(N-马来酰亚胺基甲基)环己烷-1-羧酰肼(M2C2H;Pierce Chemical Co.,Rockford,IL)[MolecularBiosciences,Inc.,Boulder,CO也提供其三氟乙酸盐]能将酮转化为腙,如下文方案I所示。
在方案I中,药物是化疗药物,优选蒽环霉素药物,并且R基团是氢原子或任选地被羟基(-OH)取代的C1-C6烷基。
方案1
不受理论约束,认为接头M2C2H在本发明优选实施方案中是特别有用的基团,有两个原因。第一,认为接头中的环己基稳定腙官能团。重要的是使用的腙键对于血清条件基本上是稳定的,并且环己基邻近形成的腙,导致与更标准的直链烷基相比更稳定的腙。第二,一旦化疗药物-mAb偶联物内化到细胞中,则酮与这种羧酰肼反应产生的腙被裂解。
对于还原的mAb上可获得的巯基稍微过量(1-5倍摩尔)的马来酰亚胺-取代的化疗药物在水溶液中与还原的mAb混合。在中性,接近中性或低于中性pH下进行反应,优选大约pH4至大约pH7。让成分进行大约5至大约30分钟短时间的反应。然而,本领域技术人员明白,就反应时间和pH来说可以优化反应条件。下面方案所示的化疗药物-mAb偶联物(其中n是1-10的整数,优选1-8),然后通过大小排阻色谱法和疏水性相互作用色谱柱从化疗药物和其他缓冲成分分离。在优选的实施方案中,药物是蒽环霉素并且n是6-10的整数。
化疗药物-mAb偶联物
上面的条件对于2-PDOX是最优的。反应条件要最优,因为这保证只有mAb的自由产生的硫醇基团与马来酰亚胺活化的药物反应,而反应条件对2-PDOX的烯胺不起作用。令人惊奇的是硫醇-马来酰亚胺偶联能在可烷基化的基团的存在下进行,例如烯胺基团。

DOX
阿霉素和表柔比星都在癌症治疗中广泛使用。在本发明另一个优选的实施方案中,化疗药物包括高毒性2-PDOX的类似物,即,吗啉代-和氰基吗啉代-阿霉素(分别是吗啉代-DOX和氰基吗啉代DOX)。在另一个优选的实施方案中,化疗药物包括柔红霉素。
本领域技术人员认识到本发明优选实施方案的蒽环霉素药物包含多个反应性基团,可以称作第二反应官能团,在药物与接头偶联之前和/或药物-接头偶联物与mAb偶联之前,要求用现有技术中公知的保护基团将其保护;保护作用是必须的,这样不会破坏反应基团的完整性。参见Greene和Wuts,Protective Groups in Organic Synthesis(John Wiley & Sons第二版1991。反应基团包括蒽环霉素药物中的蒽醌核心中的羰基;某些条件下可以与亲核试剂反应的基团。其他反应基团包括位于蒽环霉素药物分子中的各种醇基;某些条件下可以与亲电试剂反应的基团。最后,其他反应基团包括DOX中存在的胺基和2-PDOX中存在的烯胺;这两种都与亲电试剂反应。在带有烷基化基团(例如,2-PDOX的烯胺)的蒽环霉素药物情况下,需要控制反应条件,使得烷基化基团的完整性不被破坏。

氰基吗啉代-DOX
蒽环霉素类这类药物中,为了制备更大或更小毒性的免疫偶联物,以它们不同的毒性为基础,可以互换毒性1-10,000倍范围不同(3-4个数量级)的各个药物。蒽环霉素类能通过几种机理对靶细胞施加毒性作用,包括抑制DNA拓扑异构酶2(top 2),插入到DNA中,氧化还原反应和结合某些细胞内或膜蛋白。另外,可以设计具有另外的细胞杀伤机理的类似物,例如被烷基化的可能性。举例的类似物是带有烷基化部分的蒽环霉素类,如2-PDOX类似物的情况。在这个例子中,烷基化部分是烯胺基团。在2-PDOX类似物中,吡咯啉环中的烯胺在生理条件下对于亲核试剂是高反应性的。
药物组合物和给药方法
本发明的一些实施方案涉及含有本发明的mAb-药物偶联物和药学可接受载体或赋形剂的药物组合物。所谓“药学可接受载体”意指但不限于无毒固体,半固体或液体填料,稀释剂,胶囊化材料或者本领域技术人员公知的配制辅助剂。稀释剂,例如多元醇,聚乙二醇和葡聚糖,可以用来提高偶联物的生物半寿期。
本发明还涉及治疗哺乳动物疾病的方法,包括施用这里描述的抗体和蒽环霉素药物的偶联物。本发明方法还包括在其他标准治疗法之前、同时或之后施用这里描述的抗体-蒽环霉素偶联物,其中所述标准治疗法选自放射疗法,手术和化学疗法。
本发明意在包括治疗哺乳动物疾病的方法,包括施用导向于相同病细胞上不同抗原或相同抗原的不同表位的抗体和蒽环霉素药物偶联物中的两种或多种。另外,本发明意在包括治疗哺乳动物疾病的方法,包括在以第二种抗体为基础的治疗之前、同时或之后施用抗体和蒽环霉素药物的偶联物,使得与偶联物中的抗体相比,第二抗体为基础的治疗中第二抗体导向于病变细胞上的不同抗原或相同抗原的不同表位。
在一些实施方案中,单独的mAb-药物偶联物或者含有本发明的mAb-药物偶联物和药学可接受载体或赋形剂的药物组合物可以在对受试者治疗的方法中使用,包括对受试者施用治疗有效量的本发明的mAb-药物偶联物。
在优选的实施方案中,受试者是哺乳动物。例举的哺乳动物包括人,猪,绵羊,山羊,马,小鼠,狗,猫,牛等。可以用本发明的mAb-药物偶联物治疗的疾病包括癌症,例如皮肤癌,头颈癌,肺癌,乳腺癌,前列腺癌,卵巢癌,子宫内膜癌,宫颈癌,结肠癌,直肠癌,膀胱癌,脑癌,胃癌,胰腺癌,淋巴系统癌。通过施用治疗有效量的本发明的mAb-药物偶联物可以治疗患有B-或T-细胞癌,非何杰金氏淋巴瘤,何杰金氏病,淋巴细胞或髓细胞白血病,多发性骨髓瘤,肉瘤和黑素瘤的患者。
本发明的mAb-药物偶联物可以静脉内,腹膜内,动脉内,鞘内,膀胱内或肿瘤内给药。偶联物可以以反复和/或周期为基础,以药团剂或输注给与。根据药物的剂量和就副作用而言的偶联物的耐受性,输注可以重复一次或多次,由负责的主治医师决定。本领域技术人员明白能凭经验确定本发明的mAb-药物偶联物的有效量。对需要治疗癌症的受试者施用和一种或几种药学可接受赋形剂组合的药物组合物药物。要理解,当对人患者给药时,在合理医学判定范围内由负责的主治医师决定本发明的药剂或组合物的总的日用量。对于特定患者的特定治疗有效剂量水平取决于多个因素:要实现的细胞反应的类型和程度;使用的特异性mAb-药物偶联物或组合物的活性;使用的特异性mAb-药物偶联物或组合物;患者的年龄,体重,一般健康状况,性别和饮食;给药时间,给药途径,和药剂排泄速度;治疗持续时间;与特定药剂联合或同时使用的药物等医药领域公知的因素。例如,本领域公知药物起始剂量是比实现期望的治疗效果所需要的剂量更低的水平,并且逐渐增加剂量,直到实现期望的效果。
在本发明的优选的实施方案中,在其他标准治疗法包括放疗、手术或化疗之前、同时或之后施用抗体-蒽环霉素偶联物。
在另一个优选的实施方案中,施用抗体和蒽环霉素药物的一种或多种偶联物,偶联物导向于病变细胞上相同抗原或相同抗原上的不同表位。在另一个优选的实施方案中,在另一种以抗体为基础的治疗之前、同时或之后给与抗体和蒽环霉素药物的偶联物。这种附加的以抗体为基础的治疗可以包括施用两种或多种以抗体为基础的治疗,包括裸治疗,其中单独给与抗体或者与另一种治疗药剂联合施用,该药剂是与抗体偶联的或没有偶联的。偶联可以使用本文公开的接头或其他类型的接头。当给与两种以抗体为基础的治疗时,这些治疗无论施用哪一种抗体,第二种抗体导向于相同病变细胞上不同的抗原或相同抗原上的不同表位。第二种抗体也与另一种(不同)药物或者与治疗性同位素偶联,这样提供以抗体为基础的治疗。也理解这种治疗能联合,在施用增强抗肿瘤效果或预防或减轻治疗偶联物的脊髓抑制效果的细胞因子之前,同时或之后施用。

试剂盒
本发明的优选实施方案还涉及在合适的容器中含有单克隆抗体和蒽环霉素药物的偶联物的试剂盒。偶联物优选包括含有酰肼和马来酰亚胺的接头。在无菌容器中提供液体,冷冻或冻干形式的单克隆抗体-蒽环霉素药物偶联物。在对需要治疗的患者施用之前能将单克隆抗体-蒽环霉素药物偶联物稀释或重新配制。
在另一个实施方案中,其中通过接头连接蒽环霉素药物和抗体的蒽环霉素药物和抗体偶联物含有酰肼和马来酰亚胺,并且其中至少一种免疫调节剂进一步与抗体偶联。然后根据这里描述的,对需要治疗的患者施用单独的偶联物或者与其它治疗联合施用。

实施例
概述
根据最初Nagy等的描述(Proc.Natl.Acad.Sci,U.S.A.93:2464-2469(1996))为基础,利用改进的方法制备2-吡咯啉基-阿霉素。利用公开的方法(Acton等,J.Med.Chem.27:638-645(1984))从阿霉素合成吗啉代-DOX和氰基吗啉代-DOX。
实施例1:2-PDOX的合成
2-吡咯啉基-阿霉素(2-PDOX)的合成:4-碘代丁醛:2-(3-氯丙基)-1,3-二-氧戊环(1.3mL;10mM)溶解于含有30克碘化钠(200毫摩尔;20-倍过量)的200毫升丙酮中。将溶液回流24小时后蒸发至干。在下面的反应中使用粗混合物。阿霉素盐酸盐(550毫克,946微摩尔)溶解于6.5毫升的DMF中,并且加入3.86克(19.48毫摩尔,20-倍过量)的4-碘代丁醛,接着加入500微升N,N-二异丙基乙胺(DIPEA)。五分钟之后,通过在Waters NovaPakC-18柱子上用梯度洗脱通过反相HPLC将材料纯化。梯度是:90∶10洗脱剂A至70∶30洗脱剂B,每分钟75毫升,经40分钟,其中洗脱剂A是0.1%三氟乙酸(TFA)并且洗脱剂B是含有0.1% TFA的90%乙腈。电子喷射质谱证实产物的身份,M+H+=596。
实施例2:2-PDOX与抗-CD22抗体人源化LL2(hLL2)的偶联
a)2-PDOX的活化:2-PDOX(5.95毫克;1×10-5摩尔)与一摩尔当量的商业上可购得的接头4-(N-马来酰亚胺基甲基)环己烷-1-羧酰肼(M2C2H;Pierce Chemical Co.,Rockford,IL)(2.88毫克;1×10-5摩尔)在0.5毫升二甲亚砜(DMSO)中混合。反应混合物在50-60℃下在减压下被加热30分钟。通过制备RP-HPLC纯化期望的产物,使用下面成分构成的梯度:0.3%乙酸铵和90%乙腈,pH 4.4中0.3%乙酸铵,从大多数没有反应的2-PDOX(洗脱,早大约0.5分钟)和从没有反应的M2C2H(更早洗脱)分离期望的产物。通过参考相对于乙腈/乙酸铵缓冲液中2-PDOX的标准溶液的样品(496nm)的UV吸收水平估计回收的量。如果不是立即使用,将马来酰亚胺活化的2-PDOX冷冻并且冻干。当需要与抗体进一步反应时,采用最小量的DMSO。

实施例3:2-PDOX与抗-CD74抗体人源化LL1(hLL1)的偶联

c)活化的DOX与还原的hLL1的偶联:用来自上面a)的马来酰亚胺基活化的DOX处理来自上面b)的硫醇还原的抗体,含水/DMSO混合物中DMSO的最终浓度是15%。4℃下反应15分钟之后,通过用0.2M乙酸铵,pH 4.4中平衡过的G-50-80旋转柱洗脱,接着通过在相同缓冲液中平衡过的SM-2 Bio-Beads的柱子渗滤,获得没有未反应马来酰亚胺基-DOX的期望的产物。通过在280和496nm下通过UV扫描分析产物,从而估计DOX与mAb的摩尔比。通过MALDI-TOF质谱分析测得DOX与Mab之绝对比。UV和MS分析都证明在这组反应条件下获得每摩尔hLL1抗体7-8单位的DOX的取代比例。通过大小排阻HPLC(GF-250柱,Bio-Rad,Hercules CA),流速1毫升/分钟,0.2M乙酸盐缓冲液,pH 5.0,使用设置在496nm的UV检测器,基本上一个检测峰在hLL1抗体的滞留时间附近洗脱出来。痕迹(见图1;496nm下的UV检测器,设置检测DOX)表明阿霉素-LL1偶联物基本上是9分钟左右滞留时间的单一峰,没有聚集的蛋白质物质或游离DOX(滞留时间14分钟左右)。这表明产物中存在极少或没有游离药物。DOX-hLL1偶联物样品等份分为单级分,典型是0.1-1.0mg,冷冻备用,或者将它们冻干。根据需要,将它们解冻并且重新配制用于进一步实验。
实施例5:阿霉素与hLL1的偶联和dox-hLL1偶联物的配制
a)阿霉素与SMCC酰肼反应
在13毫升1∶2甲醇∶乙醇(无水)中混合90mg的阿霉素(1.56×10-4mol)和60.23mg的SMCC酰肼,并且加入10.4微升三氟乙酸。室温下避光将混合物搅拌4小时。然后将反应溶液通过0.22微米注射器过滤器过滤到100mL圆底烧瓶中。加入75微升二异丙基乙胺并且在300℃下在旋转蒸发器上蒸发溶剂。残余物用4×40mL乙腈接着用1×40mL二乙醚研制,并且在高真空下在旋转蒸发器上干燥成粉末。将粉末再溶解于5mL无水甲醇,再如上所述蒸发至干,然后在-200℃下贮存直到需要。
b)用二硫苏糖醇还原hLL1-IgG
在20mL圆底烧瓶中混合8.4mL的hLL1-IgG(10.3mg/mL,5.78×10-7mol),160微升0.1M磷酸钠缓冲液,pH 7.5,500微升的0.2M EDTA,pH 7.0,和290微升的去离子水。通过在真空和氩气氛之间将溶液循环六次而将混合物去氧。通过通入氩气10分钟而将新制备的40mM二硫苏糖醇(DTT)水溶液(0.015g于2.4mL水中,2.3×10-5摩尔;对于IgG 40-倍摩尔过量)去氧,并且向去氧hLL1抗体溶液加入640微升的这种DTT水溶液。得到的混合物在37℃下温育1小时。通过用去氧10mM PBS/100mM L-组氨酸,pH 7.4缓冲液渗滤(一个30K过滤器,氩气下,4℃下),纯化还原的抗体。持续加入缓冲液,直到总的滤液体积是300mL。还原的hLL1溶液(hLL1-SH)的体积减小到10mL。
c)阿霉素-SMCC与hLL1-SH的偶联和偶联物的纯化
活化的阿霉素(1.9mL,2.09×10-5摩尔,对于IgG 36-倍过量)溶解于二甲亚砜(DMSO)溶液,并且室温下氩气下缓慢加入hLL1-SH抗体溶液(40mL)。DMSO的最终浓度是5%。温和搅拌下让反应在4℃下进行40分钟。将反应混合物加载于BioBeadTM(Bio-Rad,RichmondCA)柱(1.5cm直径×34cm高,用10mM PBS/100mM L-组氨酸,pH 7.4缓冲液平衡过),并且以2mL/min洗脱。在Amicon过滤装置中浓缩产物偶联物并且在冻干之前配制用于通过0.22微米注射过滤器过滤。
d)偶联物配制和冻干
向40mL的上面的hLL1-dox溶液加入8mL的0.5M甘露糖醇水溶液和0.48mL的1%吐温20,得到最终浓度1.64mg/mL hLL1-dox,82.5mM甘露糖醇,和0.01%吐温20。以1mg和10mg dox-hLL1数量(分别3和10mL小瓶)冻干样品,在干冰上冷冻,并且在真空下冻干48小时。真空封闭小瓶,在-20℃避光密闭贮存备用。
实施例6:抗体的吗啉代-DOX和氰基吗啉代-DOX偶联物的制备
利用Acton等的方法(J.Med.Chem.27:-638-645(1984)),通过用2,2′-氧-双[乙醛]对阿霉素还原性烷基化来制备吗啉代-DOX和氰基吗啉代-DOX。
这些DOX类似物以对于DOX和2-PDOX类似物如上所述相同的方式与M2C2H偶联。氰基吗啉代-DOX在室温下在无水甲醇(代替DMSO)中用10%摩尔过量的酰肼偶联过夜。去除溶剂,接着通过急骤层析法提供腙。电喷射质谱分析:M+H m/e 872,M+Na 894;M-H 870。以类似方式,使用1.5当量试剂,采用SMCC-酰肼在无水甲醇中反应4小时将吗啉代-DOX衍生化为它的腙,并且通过急骤层析纯化产物。电喷射质谱分析:M+H m/e 847,M-H m/e 845,M+Cl m/e 881。
如上面实施例2-4中所述,将抗体的链间二硫键还原为游离硫醇,产生二硫化物-还原的mAbs,利用实施例2,3,和4的c)部分中描述的相同的方法制备偶联物。制备下面的吗啉代-DOX和氰基吗啉代-DOX的mAb偶联物:

实施例7:蒽环霉素-抗体偶联物的体外效力
从美国典型培养物保藏中心(ATCC,Rockville,MD)获得RajiB-淋巴瘤细胞,并且在加有谷氨酰胺,丙酮酸盐,青霉素和链霉素(LifeTechnologies,Grand Island,NY)的含有12.5%胎牛血清(Hyclone,Logan,UT)的RPMI 1640培养基中培养。简要地说,在24-孔板的孔中,3.75×105个细胞与指定浓度的药物-mAb偶联物在1.5mL组织培养基中温育2天。然后将细胞转移到含有20毫升培养基的T25烧瓶中,并且温育最多21天,或者直到细胞扩增16-倍。在第0天,第2天,然后每3-5天使用台盼蓝进行活细胞计数。从没有处理的细胞的生长速度计算加倍时间,从处理的细胞扩增16-倍需要的时间计算存活比例,假定处理不影响加倍时间。能容易检测单个的保留的活细胞。在1微克/毫升药物-mAb偶联物浓度下,DOX-LL1偶联物表现出与DOX-MN-14偶联物相比存活细胞比例的三个数量级的差异。见图2。
实施例8:用蒽环霉素-抗体偶联物治疗带肿瘤动物
a)对实体肿瘤异种移植模型的治疗
用DU145人前列腺癌细胞对无胸腺裸鼠组皮下注射。大约两周之后,当动物体内长出可触摸的前列腺肿瘤异种移植物时,用单剂量的药物-抗体偶联物2-PDOX-RS7治疗一半动物,另一半保持未处理(对照)。图3说明未处理小鼠中肿瘤异种移植物生长与用2-PDOX-RS7治疗的小鼠异种移植物生长。证明用药物-抗体偶联物治疗动物,在延迟异种移植物生长方面的治疗性效果。
b)动物模型中全身癌的治疗
NCr-SCID小鼠,每组10个动物,通过尾静脉注射对每个动物静脉内注射2.5×106人Burkitt′s B-细胞淋巴瘤细胞系Raji的悬浮液。五天之后,动物未接受处理或者用单剂量的350微克DOX-LL1或150微克2-PDOX-LL1治疗。图4给出试验结果。没有接受处理的动物由于全身癌症而瘫痪并且在注射Raji细胞之后23天左右死亡。用LL1抗体的DOX-和2-PDOX-偶联物治疗的动物存活延长的时间,相当于对于2-PDOX-LL1-治疗的动物寿命延长四倍左右,相对于DOX-LL1治疗的动物寿命延长甚至更多。
c)动物模型中全身癌的治疗
NCr-SCID小鼠,每组10个动物,通过尾静脉注射对每个动物静脉内注射2.5×106人Burkitt′s B-细胞淋巴瘤细胞系Raji的悬浮液。五天之后,动物未接受处理或者用单剂量的150微克2-PDOX-LL1或150微克2-PDOX-MN-14(非特异性对照抗体偶联物)治疗。图5给出试验结果。没有接受处理的动物由于全身癌症而瘫痪并且在注射Raji细胞之后23天左右死亡。用2-PDOX-MN-14偶联物处理的动物也是如此。用2-PDOX-LL1抗体偶联物治疗的动物存活延长的时间。
