本发明属于生物医学技术领域,具体涉及一种用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒。
背景技术
银屑病是一种与免疫和炎症相关的慢性疾病,主要病理特征为表皮细胞过度增殖、炎症细胞浸润和真皮组织血管增生,发病率较高且呈逐年上升的趋势。银屑病一直是皮肤科领域研究的热点和难点,其致病机制至今尚未明了,目前研究认为T细胞介导的细胞免疫在其中扮演着重要角色。银屑病在病理上表现为与自身免疫系统和炎症反应相关,某些机体的免疫细胞和炎症因子的变化是该病发病、加重和复发的重要原因。
银屑病复发以免疫T细胞反应为主要表现,其特征是在停止治疗后,先前的炎症部位复发。银屑病的特异性部位记忆与CD8+CD103+组织常驻记忆细胞(Tissue ResidentMemory T cells,Trm)有关,表皮中的Trm细胞曾经被认为只提供对病原体的“前线”保护和癌症发展过程中的免疫监测,近些年研究Trm细胞明显参与了银屑病发病机制中部位特异性T细胞驱动的疾病记忆,表皮Trm细胞在银屑病病变的患者和介导有害的促炎反应中被保留下来,并在刺激下产生白细胞介素IL-17A和IL-22细胞因子,这两种细胞因子都是关键的银屑病发病因子。
目前,糖皮质激素、角质剥脱剂、生物制剂、免疫抑制剂等是临床上银屑病的常见治疗用药。临床研究发现,大部分银屑病患者在治疗初期均能取得良好的治疗效果,但随着用药时间的延长,疾病大部分出现了复发或加重的现象,无论停药还是持续使用也无法改善。目前国内外研究中对此现象仍无突破性进展,银屑病复发成为难以攻克的重大医学问题。因此,减少银屑病复发是临床中亟待解决的重要问题。
发明内容
本发明的其中一个目的在于提供一种用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,通过采用NF-κB抑制剂治疗联合调节性T淋巴细胞(Tregs)过继回输治疗的方式,治疗和/或辅助治疗T细胞介导的免疫性疾病,如银屑病复发等。
根据本发明的一个方面,提供了一种用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其包括:独立存在的NF-κB抑制剂和调节性T淋巴细胞。
本发明中,调节性T淋巴细胞可以为从离体外周血中分离的调节性T淋巴细胞或者人工培养的调节性T淋巴细胞;其中,人工培养的调节性T淋巴细胞,可以是细胞库保存的细胞,也可以是市售的细胞。
在一些实施方式中,用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,可以包括独立存在的NF-κB抑制剂以及用于从离体外周血中分离调节性T淋巴细胞的试剂和/或试剂盒。
在一些实施方式中,NF-κB抑制剂可以选自NIK SMI1、Bortezomib、BardoxoloneMethyl、NF-κB-IN-1、QNZ(EVP4593)中的至少一种。
在一些实施方式中,NF-κB抑制剂为NIK SMI1。
在一些实施方式中,药盒中还含有调节性T淋巴细胞培养基和/或调节性T淋巴细胞保存液。
在一些实施方式中,调节性T淋巴细胞培养基可以为含10%FBS的1640培养基。
在一些实施方式中,调节性T淋巴细胞为CD4+CD25+T细胞。
在一些实施方式中,T细胞介导的免疫性疾病包括银屑病、银屑病复发、特异性皮炎、湿疹、类风湿性关节炎、Ⅰ型糖尿病、甲状腺炎和胃炎等。
在一些实施方式中,本发明提供的治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒可用于治疗银屑病复发,减轻银屑病复发的症状。
用于治疗银屑病复发时,本发明提供的治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒中,NIK SMI1的治疗有效量为(2.198~21.98)mg/kg;调节性T淋巴细胞的治疗有效量为(1×106~5×106)个/kg;给药方式可以是静脉给药。
本发明提供的治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,可显著下调IL-6、TNF-α、IL-17A等细胞因子的表达水平、显著降低体内Th17细胞、组织常驻记忆T细胞的数量,因此,还能够应用于制备至少具有以下(1)~(5)任意一项效果的药物:
(1)降低IL-6表达水平;
(2)降低TNF-α表达水平;
(3)降低IL-17A表达水平;
(4)降低Th17细胞水平;
(5)降低组织常驻记忆T细胞水平。
与现有技术相比,本发明的有益效果在于:
(1)本发明公开了利用NF-κB抑制剂联合调节性T淋巴细胞治疗在治疗T细胞介导的免疫性疾病如银屑病复发中的应用,提供了一种可用于治疗和/或辅助治疗T细胞介导的免疫性疾病,尤其是银屑病复发的药盒。
(2)本发明公开了NIK SMI1联合调节性T淋巴细胞治疗在治疗银屑病复发中存在协同作用,能有效抑制自身免疫反应,减轻病状,有利于提高病患的治疗效果及生活质量。
(3)动物实验结果表明,本发明提供的药盒,可以显著改善银屑病皮损处的症状并降低PASI评分,减轻银屑病皮损组织中炎症细胞的浸润,降低IL-6、TNF-α、IL-17A炎症因子表达,下调体内Th17细胞(CD4+IL-17A+)的水平及组织常驻T记忆细胞(CD8+CD103+T细胞)水平,从而起到治疗T细胞介导的免疫性疾病如银屑病复发的作用。

具体实施方式
下面结合具体实施例和附图对本发明作进一步详细的说明。
实施例1、用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒包括独立存在的NIK SMI1和调节性T淋巴细胞(CD4+CD25+T细胞)。
调节性T淋巴细胞(CD4+CD25+T细胞)可以通过常规的提取分离方法从离体外周血中分离得到,给药时,将其与1640完全培养基(含10%FBS)混匀,配制成含(1×106~1×107)个/mL调节性T淋巴细胞的调节性T淋巴细胞悬液,静脉注射给药。
用于治疗银屑病复发患者时,可以将NIK SMI1溶解于生理盐水中静脉注射给药;给药剂量一般是2.198mg/kg~21.98mg/kg,给药周期是2天一次,共3次。调节性T淋巴细胞与1640完全培养基(含10%FBS)混匀,静脉注射给药;给药剂量一般是1×106个/kg~5×106个/kg,给药周期是2天一次,共3次。

1、制备及分离纯化C57BL/6小鼠CD4+CD25+T细胞

小鼠颈椎脱臼处死后,于75%酒精中浸泡5min;在无菌条件下取小鼠脾脏,置于培养皿中,倒入少量1640培养基,然后在40μm无菌滤网中研磨,收集细胞悬液于15mL离心管中,300g离心5min,弃上清后加入3mL红细胞裂解液冰上裂解5min,400g离心5min,PBS洗涤2次,300g离心5min,PBS重悬细胞后,即可计数。
(2)孵育Anti-CD4和Anti-CD25抗体
按照每106个细胞加0.1μg抗体将适量的Anti-CD4和Anti-CD25抗体加入步骤(1)制备好的脾细胞悬液中,室温下避光孵育30min;PBS洗1次后,用含2%FBS的PBS重悬细胞至3~4mL,备用。
(3)流式细胞仪分离纯化CD4+CD25+T淋巴细胞
使用BD FACSAria III流式细胞仪按标准操作流程将上述步骤(2)制备好的细胞悬液进行分选,细胞接收管提前加入2mL 1640完全培养基(含10%FBS)作为缓冲液,收集CD4阳性和CD25阳性群的细胞;将收集好的细胞放入37℃培养箱中保存备用,待所有细胞收集完成后统一处理。
(4)将收集的CD4+CD25+T淋巴细胞计数后,300g离心5min,用1640完全培养基重悬至2×106cell/mL,然后加入浓度为2.5μg/mL的抗小鼠CD3e和CD28抗体以及rmIL-2(10ng/mL)、rh TGF-β1(5ng/mL),混匀后加到6孔板中;置于37℃,含5%CO2的培养箱中培养。
2、银屑病复发小鼠模型的建立
健康Balb/c雄性小鼠,先脱去小鼠背毛。脱毛处均外涂5%咪喹莫特软膏7天,1次/d,初次银屑病模型成功后,再过14天(免疫耐受期),即从第21天开始,在背部脱毛区继续涂5%的咪喹莫特,连续7天(第21天-第28天,复发期),再次出现鳞屑增多,皮肤增厚明显,红斑明显等现象,形成银屑病复发病状,从而得到银屑病复发小鼠模型。
3、动物分组及给药剂量
将已成模的小鼠随机分为模型对照组(模型组)、NF-κB抑制剂NIK SMI1组(NIKSMI1组)、调节性T淋巴细胞过继回输组(Tregs回输组)、NF-κB抑制剂(NIK SMI1)联合调节性T淋巴细胞(Tregs)过继回输治疗组(NIK SMI1+Tregs回输组),每组6只;另取6只健康Balb/c雄性小鼠,设为正常对照组(正常组)。
调节性T淋巴细胞治疗用量(尾静脉注射)为Tregs细胞1×106个/只·day,NIKSMI1用量(尾静脉注射)为20mg/kg·day,NF-κB抑制剂(NIK SMI1)联合调节性T淋巴细胞(Tregs)过继回输治疗为(尾静脉注射Tregs细胞1×106个/只+尾静脉注射NIK SMI1 20mg/kg)·day,给药时间为第1、4、7天,共给药3次。正常组、模型组均等量注射生理盐水。
4、实验方法及结果分析
(1)银屑病复发小鼠皮损处的表观症状及PASI评分
用不同的药物处理小鼠,每日通过采取数码照相拍照来记录小鼠皮损处的表观症状情况,7天后,对小鼠皮损处的表观症状情况进行评分(红斑、鳞屑、浸润增厚水平)(由轻到重评为:0-4的积分),将三者积分相加得到总积分。PASI评分标准如下:0,无;1,轻度;2,中度;3,重度;4,极重度。
结果如图1-2所示,从图1-2可以看出:各组小鼠给予不同治疗7天后,模型组小鼠中,皮损处背部皮肤明显增厚,且出现红斑、鳞屑等症状,与人类银屑病样皮损类似。与模型组相比,NIK SMI1组、调节性T淋巴细胞过继回输治疗组、NIK SMI1联合调节性T淋巴细胞治疗组小鼠背部皮损处的皮肤增厚程度减轻,红斑的颜色变浅,且皮损伤的鳞屑变薄,PASI评分下降,其中NIK SMI1联合调节性T淋巴细胞治疗组小鼠对症状的改善效果更为显著,与NIK SMI1组、调节性T淋巴细胞过继回输治疗组相比,差异具有显著性(P<0.01),说明NIKSMI1联合调节性T淋巴细胞治疗可以显著改善银屑病复发小鼠背部皮损表观症状,两者具有协同作用。
(2)银屑病复发小鼠皮损组织中炎症细胞的浸润情况
用不同的药物处理小鼠,7天后,取小鼠皮损处组织进行HE染色检测。
结果如图3所示,从图3可以看出:各组小鼠给予不同治疗7天后,与模型组相比,NIK SMI1组、调节性T淋巴细胞过继回输治疗组、NIK SMI1联合调节性T淋巴细胞治疗组小鼠皮损区域中,角化不全的细胞显著降低,表皮棘细胞层及表皮层厚度均显著下降,其中NIK SMI1联合调节性T淋巴细胞治疗组小鼠皮损中角化不全的细胞、表皮棘细胞层及表皮层厚度减少症状更为显著。
(3)银屑病复发小鼠皮损组织中炎症因子表达情况
用不同的药物处理小鼠,7天后,取各组小鼠的银屑病皮损处组织进行Elisa检测。
检测结果如图4-6所示:与模型组相比,NIK SMI1组、调节性T淋巴细胞过继回输治疗组、NIK SMI1联合调节性T淋巴细胞治疗组小鼠背部皮损处IL-6、TNF-α、IL-17A水平表达均显著下降,其中NIK SMI1联合调节性T淋巴细胞治疗组小鼠对IL-6、TNF-α、IL-17A水平表达下调的效果更为显著,与NIK SMI1组、调节性T淋巴细胞过继回输治疗组相比,差异具有显著性(P<0.01),说明NIK SMI1联合调节性T淋巴细胞治疗可以显著降低银屑病复发小鼠背部皮损的炎症因子水平,NIK SMI1联合调节性T淋巴细胞治疗具有协同作用。
(4)银屑病复发小鼠体内Th17细胞(CD4+IL-17A+)水平
用不同的药物处理小鼠,7天后,利用流式细胞仪定量分析小鼠脾脏中的Th17细胞水平。
结果如图7-8所示,从图7-8中可以看出:与模型组相比,NIK SMI1组、调节性T淋巴细胞过继回输治疗组、NIK SMI1联合调节性T淋巴细胞治疗组小鼠脾脏中的Th17细胞水平均显著下降,其中NIK SMI1联合调节性T淋巴细胞治疗对银屑病复发小鼠Th17细胞水平下调的效果更为显著,与NIK SMI1组、调节性T淋巴细胞过继回输治疗组相比,差异具有显著性(P<0.05或P<0.01),说明NIK SMI1联合调节性T淋巴细胞治疗具有协同作用。
(5)银屑病复发小鼠皮损处组织常驻T记忆细胞(CD8+CD103+T细胞)水平
用不同的药物处理小鼠,7天后,利用流式细胞仪定量分析小鼠皮损处组织常驻T记忆细胞(CD8+CD103+T细胞)水平。
结果如图9-10所示,从图9-10中可以看出:与模型组相比,NIK SMI1组、调节性T淋巴细胞过继回输治疗组、NIK SMI1联合调节性T淋巴细胞治疗组小鼠皮损处组织常驻T记忆细胞(CD8+CD103+T细胞)水平均显著下降,其中NIK SMI1联合调节性T淋巴细胞治疗对银屑病复发小鼠Th17细胞水平下调的效果更为显著,与NIK SMI1组、调节性T淋巴细胞过继回输治疗组相比,差异具有显著性(P<0.05或P<0.01),说明NIK SMI1联合调节性T淋巴细胞治疗具有协同作用。
5、实验结论
NIK SMI1联合调节性T淋巴细胞过继回输治疗可显著改善银屑病复发小鼠皮损处的症状并降低PASI评分,减轻银屑病皮损组织中炎症细胞的浸润,降低银屑病皮损处组织中炎症因子表达,下调体内Th17细胞(CD4+IL-17A+)的水平及组织常驻T记忆细胞(CD8+CD103+T细胞)水平,通过调节自身免疫进而对银屑病复发起治疗作用。
以上所述的仅是本发明的一些实施方式。对于本领域的普通技术人员来说,在不脱离本发明创造构思的前提下,还可以做出若干变形和改进,这些都属于本发明的保护范围。
Claims (9)
1.用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,包括:独立存在的NF-κB抑制剂和调节性T淋巴细胞。
2.根据权利要求1所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述NF-κB抑制剂选自NIK SMI1、Bortezomib、Bardoxolone Methyl、NF-κB-IN-1、QNZ(EVP4593)中的至少一种。
3.根据权利要求2所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述NF-κB抑制剂为NIK SMI1。
4.根据权利要求1所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述调节性T淋巴细胞为CD4+CD25+T细胞。
5.根据权利要求1~4任一项所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述T细胞介导的免疫性疾病包括银屑病、银屑病复发、特异性皮炎、湿疹、类风湿性关节炎、Ⅰ型糖尿病、甲状腺炎和胃炎。
6.根据权利要求5所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述T细胞介导的免疫性疾病为银屑病复发。
7.根据权利要求6所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述NIK SMI1的治疗有效量为(2.198~21.98)mg/kg。
8.根据权利要求6所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒,其特征在于,所述调节性T淋巴细胞的治疗有效量为(1×106~5×106)个/kg。
9.根据权利要求1~8任一项所述的用于治疗和/或辅助治疗T细胞介导的免疫性疾病的药盒在制备至少具有以下(1)~(5)中任意一项效果的药物中的应用:
(1)降低IL-6表达水平;
(2)降低TNF-α表达水平;
(3)降低IL-17A表达水平;
(4)降低Th17细胞水平;
(5)降低组织常驻记忆T细胞水平。
