Abstract

技术领域
本发明涉及药物技术领域,具体地说,是氢化镁在制备防治急性肾损伤药物中的应用。
背景技术
急性肾损伤(Acute Kidney Injury,AKI)是指肾小球滤过率在短期内骤降,导致血浆中肌酐与尿素等含氮废物潴留的一种综合征。急性肾损伤是临床常见危重症之一,随着对其认识的深入以及危重症救治手段的快速发展,急性肾损伤相关的死亡风险显著下降,然而存活患者的肾脏远期预后却不容乐观。目前全球急性肾损伤发病率为20-200人/百万人口,住院率达7%-18%,病死率呈明显上升趋势,长期随访研究显示,8%-16%的急性肾损伤患者病情会进展为慢性肾脏病乃至终末期肾脏病。急性肾损伤临床治疗仍是以肾脏替代治疗为主,在治疗的同时等待肾脏自我修复,这也是导致急性肾损伤病死率久居不下的重要原因,寻找急性肾损伤治疗新策略是亟待解决的问题。AKI的致病机制较为复杂,目前认为主要与肾小管细胞的损伤、炎症反应、氧化应激、肾血流动力学障碍及线粒体功能障碍有关。
氢化镁(Magnesium Hydride,MgH2)是一种新型高效的镁基储氢材料,氢化镁水解产物主要包括氢气和氢氧化镁,具有释放氢气量大,持续时间长,水解产物副作用小等优点。
中国专利文献CN114948989A公开了氢化镁在制备防治肾结晶结石病的药物中的应用,所述的药物是以氢化镁作为唯一活性成份,或者包含氢化镁的药物组合物。然而目前尚未有证据表明氢化镁可以治疗急性肾损伤保护肾功能。
发明内容
本发明的目的在于提供氢化镁新的医药用途,具体是在制备治疗急性肾损伤、保护肾功能药物中的应用。

具体实施方式
下面结合实施例对本发明提供的具体实施方式作详细说明。
实施例1:氢化镁改善对乙酰氨基酚诱导急性肾损伤小鼠肾组织损害,且具有剂量效应
利用对乙酰氨基酚(APAP)腹腔注射(500mg/kg,腹腔注射3天)的方式构建C57BL/6小鼠的急性肾损伤模型(AKI组),LMH组和HMH组分别提前3天使用50、100mg/kg剂量的MgH2灌胃治疗。APAP-AKI组小鼠血清SCr、BUN水平较对照组明显升高,MgH2给药后,血肌酐和尿素氮水平均显著降低,且呈剂量依赖性(图1A,B)。对照组肾脏HE染色结果显示,肾小管上皮细胞形态正常,AKI组肾小管扩张,上皮细胞刷状缘脱落,部分小管上皮细胞出现变性、坏死等损伤,MgH2可以改善APAP诱导的肾小管损伤(图1D)。蛋白免疫印迹法分析各组肾小管损伤标志物KIM-1和NGAL的蛋白表达水平,与对照组相比,AKI组中KIM-1和NGAL的表达增强,而低剂量和高剂量MgH2给药组可下调KIM-1和NGAL蛋白的表达水平(图1C)。
实施例2:氢化镁改善对乙酰氨基酚诱导急性肾损伤小鼠肾组织炎症水平
在炎症指标方面,与对照组相比,AKI组小鼠肾脏中TNF-α和IL-1β的mRNA表达上调,而MgH2给药后逆转了这一变化(图2A,B)。同样,蛋白免疫印迹法显示,低剂量和高剂量MgH2给药均能降低APAP诱导的急性肾损伤小鼠肾脏中TNF-α和IL-1β的蛋白表达水平(图2C)。观察各组小鼠肾组织F4/80细胞的表达情况,结果显示,相比较对照组,AKI组表达F4/80阳性细胞增多,LMH和HMH组表达F4/80阳性细胞有所减少(图2D)。
实施例3:氢化镁可减轻对乙酰氨基酚共孵育的HK-2细胞毒性、氧化应激、线粒体功能障碍
使用CCK-8检测试剂盒检测对乙酰氨基酚刺激和氢化镁干预对HK-2细胞增殖的影响,结果显示,APAP(5mM)刺激HK-2细胞后,细胞活性有所下降,MgH2(0.2和0.4mM)对APAP刺激HK-2细胞活力下降有保护作用(图3A)。与对照组相比,AKI组HK-2细胞内ROS水平明显升高,予MgH2干预后,ROS水平显著下调(图3B)。使用JC-1荧光探针检测线粒体的膜电位,当荧光颜色为红色,说明线粒体膜电位较高,当荧光颜色为绿色,说明线粒体膜电位较低,通过荧光颜色的转变来检测线粒体膜电位的变化。结果显示:AKI组线粒体膜电位明显下降,反映线粒体功能受损,给予氢化镁干预后,线粒体膜电位有所提高(图3C)。
实施例4:氢化镁可抑制对乙酰氨基酚刺激的HK-2细胞TXNIP/NLRP3/NF-κB信号通路活化
对乙酰水杨酸可增加HK-2细胞中TXNIP/NLRP3/NF-κB通路相关分子蛋白(TXNIP、NLRP3、P-p65和p65)的表达,而MgH2干预后可降低上述分子蛋白的表达(图4A)。免疫荧光结果同样显示,乙酰水杨酸共孵育的HK-2细胞,TXNIP的表达是增加的,而LMH和HMH干预后的HK-2细胞,TXNIP的表达是回调的。推测氢化镁可通过抑制TXNIP/NLRP3/NF-kB炎症小体活化,减少炎症介质释放,从而降低炎症反应。
实施例5:氢化镁可改善对乙酰氨基酚共孵育HK-2细胞内活性氧水平
用DHE ROS探针法检测HK-2细胞内活性氧水平(ROS),结果提示:与对照组相比,APAP共孵育的HK-2细胞红色荧光强度及面积明显增加,红色荧光强度与细胞内活性氧水平成正比,予氢化镁干预后,HK-2细胞内红色荧光强度和面积有所下降。结果提示:氢化镁可改善对乙酰氨基酚共孵育的HK-2细胞内活性氧水平(图5)。
实施例6:氢化镁可抑制对乙酰氨基酚诱导的急性肾损伤小鼠肾组织TXNIP/NLRP3/NF-κB信号通路活化
蛋白免疫印迹结果显示,与对照组相比,AKI组TXNIP和NLRP3、P-p65、p65蛋白表达水平上调,低剂量和高剂量MgH2干预组,上述分子的表达有所回调(图6A)。免疫组化同样显示,MgH2干预可以降低AKI肾组织TXNIP的表达(图6B),推测氢化镁主要通过抑制TXNIP的水平,从而抑制NLRP3炎症小体活化,减少炎症介质释放,降低炎症反应。
以上已对本发明创造的较佳实施例进行了具体说明,但本发明创造并不限于所述实施例,熟悉本领域的技术人员在不违背本发明创造精神的前提下还可做出种种的等同的变型或替换,这些等同的变型或替换均包含在本申请权利要求所限定的范围内。

1.氢化镁在制备防治急性肾损伤药物中的应用。
2.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,所述的氢化镁在制备改善急性肾损伤并保护肾功能药物中的应用。
3.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,所述的急性肾损伤为对乙酰氨基酚诱导的急性肾损伤。
4.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,氢化镁在制备改善急性肾损伤肾组织损害的药物中的应用。
5.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,氢化镁在制备改善急性肾损伤肾组织炎症水平的药物中的应用。
6.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,氢化镁在制备减轻对乙酰氨基酚诱导的急性肾损伤细胞毒性、氧化应激、线粒体功能障碍的药物中的应用。
7.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,氢化镁在制备改善对乙酰氨基酚诱导的急性肾损伤细胞内活性氧水平的药物中的应用。
8.根据权利要求1所述的氢化镁在制备防治急性肾损伤药物中的应用,其特征在于,氢化镁通过抑制TXNIP/NLRP3/NF-kB炎症小体活化,减少炎症介质释放,从而降低炎症反应。
9.一种防治急性肾损伤的药物,其特征在于,其活性成分为氢化镁。
10.根据权利要求9所述的药物,其特征在于,所述的药物为口服制剂。
