背景技术
最新的进展表明,基因疗法在治疗人类遗传障碍方面具有很大的前景[1]。已经描述了超过6,000种遗传疾病[2],随着基因组测序技术的进一步披露,遗传疾病的数量仍在增加[3]。近年来,更安全的递送载体的发现和新的基因编辑技术的揭示,显著增强了用于治疗这些遗传疾病的工具箱,采用该工具箱时缺陷基因可以通过治疗性载体携带的外源性DNA进行补充,或通过基因编辑在原位被永久纠正[4]。与这个有前途的前景相反,临床上可治疗的遗传障碍的列表增长缓慢[5]。这种情况的一个主要限制因素是可向人转化的动物模型缺失,这阻碍了基因疗法的治疗效果的评价[6]。
结晶样视网膜变性(Bietti Crystalline Dystrophy,BCD,MIM 210370)是一种常染色体隐性遗传疾病,最早由意大利眼科医生G.B.Bietti在1937年描述,因在患者眼底观察到其明显的黄白色结晶样沉积物而得名[7]。在欧洲,BCD占所有非综合征性视网膜色素变性(RP)的约3%,并且占非综合征性常染色体隐性RP的约10%[8]。BCD还显现在东亚更常见,尤其是在中国,基因突变频率估计为1/20,000[9]。
在2000年,鉴定出BCD的遗传基础与4q35染色体相关[8]。CYP4V2是细胞色素P450超家族的成员,在2004年被鉴定为BCD的致病基因[10]。除了视网膜和角膜中的结晶样沉积物外,BCD患者的血清中还发现了脂肪酸浓度的改变,这意味着脂质代谢的失调[11,12]。更重要的是,BCD患者不可避免地在20-40岁的年龄之间出现视力丧失和夜盲症,并且最终在50-60岁的年龄之间发展为法定盲[11]。不幸的是,在首次发现BCD后80年,仍然没有针对这种严重的致盲疾病的治疗方法。

发明内容
诸位发明人开发了包含CYP4V2的密码子优化编码序列和某些基因表达调控序列组合的rAAV载体,与先前公布的形式[13]相比,所述载体实现了显著增强的表达水平(约26.1倍),从而完成了本发明。

本公开文本提供了包含编码CYP4V2蛋白(特别是人CYP4V2)的核苷酸序列的分离的核酸序列。“分离的核酸”意指从多核苷酸的全部或部分中除去的DNA或RNA(该分离的多核苷酸在自然界中存在于该多核苷酸中)或与在自然界中不与之连接的多核苷酸连接的DNA或RNA。“包含”特异性核苷酸序列的分离的核酸分子除指定的序列之外还可以包含可操作地连接的调控序列,所述调控序列控制所列举的核酸序列的编码区的表达。由于密码子简并性,本领域技术人员可以理解,特定的氨基酸序列可以由不同的核苷酸序列编码。
本公开文本的编码CYP4V2的核苷酸序列已经经密码子优化和筛选,导致与未经密码子优化的野生型编码序列相比,其表达水平增强。
例如,经蛋白质印迹分析,在HEK293细胞和ARPE-19细胞中,与野生型编码序列相比,CYP4V2的经密码子优化的编码序列可以达到约2倍、约3倍、约4倍、约5倍、约10倍、约15倍、或约20倍的CYP4V2蛋白表达水平。
优选地,CYP4V2的经密码子优化的编码序列具有减少的CpG数和/或减少的CpG岛数。例如,经密码子优化的编码序列包含小于100个CpG、小于70个CpG、小于20个CpG、小于15个CpG、小于10个CpG、小于5个CpG或0个CpG。例如,经密码子优化的编码序列不具有CpG岛。“CpG岛”代表长度为至少200bp、GC百分比含量大于50%并且观察到的CpG与预期的CpG的比率大于60%的DNA区域。
例如,与CYP4V2的野生型编码序列(如SEQ ID NO:1中所示)相比,经密码子优化的编码序列具有低于80%的序列同一性百分比。两个序列的同一性百分比可以通过本领域已知的程序来计算。由于不同的比对参数和同一性的不同的定义,计算出的两个序列的同一性百分比可以因所使用的计算程序而异。在本公开文本中,使用blastn以获得两个核苷酸序列的同一性百分比,并且具体地选择“高度相似序列(megablast)”模式。
经密码子优化的编码序列可以选自如SEQ ID NO:2-17中所示的核苷酸序列。优选地,经密码子优化的编码序列是SEQ ID NO:8、9、15、16和17中的任一个。更优选地,经密码子优化的编码序列是SEQ ID NO:16。
核酸分子中包含的调控序列可以选自启动子、增强子、多腺苷酸化序列和翻译终止信号中的一个或多个。本公开文本的调控序列的某些组合可以在提高编码序列的表达效率方面达到出乎意料的效果。
本公开文本的启动子可以是组成型启动子、组织特异性启动子或细胞类型特异性启动子。例如,启动子可以是RPE细胞特异性启动子。启动子可以是CYP4V2的天然启动子。在更优选的实施方案中,启动子是具有如SEQ ID NO.35所示的序列的CAG启动子。CAG启动子是强组成型启动子,其在哺乳动物表达载体中驱动高水平的基因表达。CAG启动子由巨细胞病毒(CMV)早期增强子元件、挑选自鸡β-肌动蛋白基因的第一个外显子和第一个内含子的启动子、以及兔β-珠蛋白基因的剪接受体组成。
科扎克共有序列(科扎克序列)以发现它的科学家命名,是大多数真核生物mRNA转录物中的核酸基序,天然地充当蛋白质翻译起始位点[14]。科扎克序列确保蛋白质正确翻译并且增强蛋白质表达。
此外,本公开文本的核酸可以进一步包含插入在启动子与编码序列之间的内含子。如本领域已知的,一些内含子可以增强真核生物中基因的表达。本公开文本的内含子可以是CYP4V2基因中天然存在的内含子的一部分。
本公开文本的多腺苷酸化序列可以是bGH多聚A、SPA或SV40多聚A[15],优选SV40多聚A。任何一种多聚A都可以与土拨鼠肝炎病毒转录后调控元件(WPRE)[16]组合,WPRE是DNA序列,当转录时产生增强表达的三级结构,并且分别配置为WPRE-bGH多聚A、WPRE-SPA或WPRE-SV40多聚A。
在本公开文本的优选的实施方案中,核酸序列包含编码以下的核苷酸序列:CAG启动子、科扎克序列、CYP4V2基因的经密码子优化的编码序列和WPRE-SV40多聚A。例如,分离的核酸序列包含SEQ ID NO:18-34中任一个的核苷酸序列。在更优选的实施方案中,所述分离的核酸序列包含SEQ ID NO:25、26、32、33和34中任一个的核苷酸序列。在最优选的实施方案中,所述分离的核酸序列包含SEQ ID NO:33的核苷酸序列。
rAAV载体和病毒颗粒
本公开文本的核酸分子可以被构建到重组AAV载体中,以获得用于递送至治疗受试者的rAAV颗粒。
除了如上所述的插入的核苷酸序列外,rAAV载体也是单链形式。rAAV载体通常由在插入的核苷酸序列两端的两个反向末端重复(ITR)序列构成。本公开文本的ITR可以是源自AAV血清型的ITR。当提及AAV ITR的血清型时,短语“源自”意指ITR可以是某种血清型的ITR或由其来源的具有一个或多个修饰的变体。在本公开文本的优选的实施方案中,rAAV载体包含两个源自AAV2的ITR[17]。例如,rAAV载体包含两个AAV2 ITR,或包含一个野生型AAV2 ITR以及一个缺乏区域C或区域C’的AAV2 ITR变体。例如,野生型AAV2ITR位于插入的核苷酸序列的5’处的位置,而AAV2 ITR变体位于插入的核苷酸序列的3’处的位置;反之亦然。
rAAV基因组被包装到AAV衣壳中。衣壳可以源自本领域已知的或将来表征的任何AAV血清型。衣壳和ITR可以源自AAV的相同血清型或源自AAV的不同血清型。优选地,衣壳适合于眼部递送,例如视网膜下、玻璃体内或眼内递送。在具体的实施方案中,AAV载体包含AAV1、AAV2、AAV4、AAV5、AAV7、AAV8、AAV9、AAVrh10、AAV2.7m8或AAVAnc80L65血清型或其变体的衣壳。
在本公开文本的优选的实施方案中,rAAV由AAV血清型8衣壳[18]构成。有证据表明,ssAAV8比ssAAV2和ssAAV5更有效地转导光感受器和视网膜色素上皮细胞(RPE)。在衣壳蛋白的指导下,病毒转导靶细胞,然后转移的含有CYP4V2基因和调控元件的基因组释放至靶RPE中。在治疗受试者中,释放的rAAV基因组保持稳定并且独立于宿主基因组,以稳定地产生功能性CYP4V2蛋白。
药物组合物
术语“药物组合物”是指适合于递送至受试者的组合物。本公开文本的药物组合物包括分离的核酸、本公开文本的rAAV载体或病毒颗粒以及药学上可接受的赋形剂。常规的药学上可接受的赋形剂是本领域已知的,并且可以是固体或液体赋形剂。
治疗用途
本公开文本的rAAV、病毒颗粒或组合物可以用于治疗或预防与CYP4V2突变有关的BCD。此外,本公开文本的rAAV、病毒颗粒或组合物也可以有效治疗或预防与视网膜色素上皮(RPE)萎缩相关的其他病症或疾病,如眼底营养不良。
术语“治疗”(“treat”、“treating”或“treatment”)”包括治愈或至少减轻BCD或与RPE萎缩相关的病症或疾病的症状。
施用
当应用于受试者(例如动物,包括人)、或应用于细胞、组织、器官或生物流体时,如本文使用的术语“施用”(“administration”和“administering”)意指使外源药物、治疗剂、诊断剂或组合物与该受试者、细胞、组织、器官或生物流体接触。术语“施用”还包括由试剂、诊断剂、结合化合物或由另一细胞在体外和离体处理例如细胞。
在本公开文本中,本申请的病毒颗粒或药物组合物优选地通过眼内递送例如视网膜下注射来递送。
实施例
实施例1.启动子的选择
诸位发明人首先制备了表达CYP4V2的AAV载体,该载体由两个ITR、CMV启动子、野生型CYP4V2 CDS和SV40多聚A构成[13]。由于报道了CMV启动子的沉默风险[19],CMV启动子后来被CAG启动子替代,产生了新载体,命名为“CAG-hCyp4v2(BCD1)”,该新载体示出了显著提高的CYP4V2蛋白表达(图1,下小图)。
实施例2.密码子优化增加Cyp4v2表达
为了获得可以提供更高的CYP4V2蛋白表达水平的转基因构建体,对CYP4V2的编码序列进行密码子优化,以提高密码子在人细胞中的使用频率。此外,减少了经修饰的编码序列的GC含量和CpG岛,以将TLR9介导的体内免疫应答的可能性减至最小。基于以上标准,设计、合成16个不同的编码序列(SEQ ID NO:2-17),并且克隆至CAG启动子中以再获得16个转基因构建体(BCD2-BCD17)。下表1示出了SEQ ID NO:2-17的CpG数、CpG岛数以及它们与野生型序列的序列同源性。

实施例3.HEK293和ARPE-19细胞的转染以及表达水平的检测
将HEK293和ARPE-19细胞维持在DMEM+10%FBS中,并且通过TrypLE每3天传代。转染前一天,将HEK293细胞以1×105个细胞/cm2的密度接种于24孔板,同时将ARPE-19细胞以7×104个细胞/cm2的密度接种于24孔板。使用Lipofectamine 3000转染试剂(Invitrogen,L3000008)按照用户指南转染质粒。转染后72小时,将细胞收集在具有蛋白酶抑制剂混合物(Roche,04693159001)的RIPA裂解缓冲液(Beyotime,P0013C)和SDS-PAGE上样缓冲液(Cowin Bio,CW0027)中,在95℃下变性15min,在12,000rpm下离心10min。使上清液在4%-10%SDS-PAGE凝胶(CowinBio,CW0022M)中分离,并且印迹于0.45μm NC转移膜(Merck,HATF00010)上。
分别通过针对人CYP4V2(Sigma,HPA029122)和GAPDH(Abcam,ab8245)的抗体检测CYP4V2和管家基因GAPDH的蛋白表达水平。计算条带的灰度值,并且相对于BCD1归一化。
重复实验的结果显示,在16个测试的经密码子优化的载体中,转基因构建体BCD8、BCD9、BCD15、BCD16、BCD17表达更高水平的CYP4V2蛋白,如在HEK293细胞(图3)和ARPE-19细胞(图4)中评价的。因此,使用BCD1作为对照,选择转基因构建体BCD8、BCD9、BCD15、BCD16和BCD17用于进一步评价。蛋白质印迹(WB)结果在图5和图6的上小图中示出。对所有条带的灰度值进行计算、分析并且相对于BCD1归一化(图5和图6,下小图)。
基于蛋白质印迹结果,确定BCD8在HEK293细胞中展现出最高的CYP4V2蛋白表达水平(图5),并且BCD16在ARPE-19细胞中展现出最高的CYP4V2蛋白表达水平(图6)。
随后,BCD1、BCD8和BCD16被分别包装至到复制缺陷型AAV8中。使用AAV8-BCD1、AAV8-BCD8和AAV8-BCD16病毒颗粒,在2mM羟基脲的存在下,以MOI=5×105转导ARPE-19。
AAV转导实验显示,AAV8-BCD16具有最高的表达水平(图7)。

3.根据权利要求2所述的分离的核酸分子,其中所述核苷酸序列是SEQ ID NO:8或SEQID NO:16。
4.根据权利要求1至3中任一项所述的分离的核酸分子,所述分离的核酸分子进一步包含与编码CYP4V2的核苷酸序列的5’可操作地连接的启动子。
5.根据权利要求4所述的分离的核酸分子,其中所述启动子是CAG启动子。
6.根据权利要求1至5中任一项所述的分离的核酸分子,所述分离的核酸分子进一步包含在编码CYP4V2的核苷酸序列的3’处的多腺苷酸化序列。
7.根据权利要求6所述的分离的核酸分子,其中所述多腺苷酸化序列是牛生长激素(bGH)多聚A、合成的多聚A(SPA)或猿猴病毒40(SV40)多聚A,更优选SV40多聚A。
8.根据权利要求1至7中任一项所述的分离的核酸分子,所述分离的核酸分子进一步包含土拨鼠肝炎病毒转录后调控元件(WPRE)。
9.根据权利要求1至8中任一项所述的分离的核酸分子,所述分离的核酸分子包含(a)或(b)的任一个
(a)SEQ ID NO:18-34中任一个的核苷酸序列,或
(b)与SEQ ID NO:18-34中的任一个具有至少85%同源性、至少90%同源性、至少95%同源性、至少96%同源性、至少97%同源性、至少98%同源性、至少99%同源性的核苷酸序列。
10.一种重组AAV载体,所述重组AAV载体包含根据权利要求1至9中任一项所述的核酸分子。
11.根据权利要求10所述的重组AAV载体,所述重组AAV载体包含一个或两个ITR。
12.根据权利要求11所述的重组AAV载体,所述重组AAV载体包含两个AAV2 ITR。
13.一种病毒颗粒,所述病毒颗粒包含包装至AAV衣壳中的根据权利要求10至12中任一项所述的重组AAV载体。
14.根据权利要求13所述的病毒颗粒,其中所述AAV衣壳是AAV8衣壳。
15.一种药物组合物,所述药物组合物包含根据权利要求13或权利要求14所述的病毒颗粒和药学上可接受的赋形剂。
16.根据权利要求10至12中任一项所述的rAAV载体在制造用于治疗或预防结晶样视网膜变性(BCD)的药剂中的用途。
