背景技术
冠状病毒(冠状病毒科(Coronaviridae))长期以来一直被认为是人类普通感冒和呼吸道感染以及动物多种呼吸道疾病的致病物之一。然而,直到21世纪,冠状病毒变体才作为大流行病原体出现。2002年,SARS-CoV病毒出现,并在短时间内展现出现代毒株的高传染性特征。MERS冠状病毒在阿拉伯半岛的出现遵循类似的轨迹。
自从SARS(严重急性呼吸综合征)冠状病毒2(SARS-CoV-2)爆发并引起冠状病毒病(COVID-19)以来,这种高传染性病毒在世界范围内迅速传播,导致全球大流行。虽然大约15%-20%的检测病例是无症状的,但大多数受影响的患者有轻度症状,包括发热(83%-98%的有症状病例)、咳嗽(59%-82%的有症状病例)、呼吸短促(19%-55%的有症状病例)和肌肉疼痛(11%-44%的有症状病例)。然而,在一些患者中,该疾病将发展到更严重的状态,这将在感染发生后8天左右发生。主要症状从呼吸困难到呼吸窘迫,3%-29%的患者需要入住重症监护室(ICU)。然后病程可能导致急性呼吸窘迫综合征(ARDS)(17%-29%的患者住院)、严重脓毒症伴休克,并且在一些情况下导致在一周内出现多器官功能障碍。最后,据估计,被感染患者的全球死亡率为大约5%-7%。

此外,与其他冠状病毒不同,该变体保留了其毒力,尽管作为RNA病毒,但预计它会以相对高的频率发生突变。另外,缺乏该疾病的动物模型阻碍了疫苗开发者证明疗效的能力,并且快速传播要求在没有大量动物数据的情况下采用捷径来进行人体试验。
因此,除少数国家/地区外,所有被感染国家/地区都进入了广泛的封锁状态,限制社会集会,限制旅行,并迫使大多数企业将活动限制到最低限度(在大多数情况下关闭)。这样的措施引发了“停工经济学”,造成全球经济衰退,给全球人口带来了破坏性后果。
在过去的几个月里,政府、非政府组织(NGO)和私营公司重新集中精力寻找针对COVID-19的治疗方法。最常见的方法可以分为三大类:i)被动免疫/中和抗体、ii)疫苗和iii)药物重新定位(drug repurposing)。
关于上述方法,一种产品(小分子药物瑞德西韦(Remdesivir))已被批准用于治疗COVID-19的重症病例。然而,最近公布的临床数据并没有显示出与安慰剂相比有很大的益处。2020年5月12日,在clinicaltrials.gov中注册了1368项针对COVID-19的临床试验。
发明内容
为了克服目前SARS-CoV-2和其他冠状病毒引起/相关的呼吸道疾病或病症的爆发所带来的挑战,本发明人认为,预防或治疗人群的一般冠状病毒相关严重呼吸综合征疾病、特别是COVID-19要取得成功的关键将是提供一种产品,该产品将:(i)使免疫系统能够(a)分泌抗SARS-CoV-2的抗体,(b)刺激预先存在的SARS-CoV-2交叉反应T细胞,并且(3)使用具有微生物交叉反应抗原(mCRAG)的细菌菌株产生新的SARS-CoV-2交叉反应T细胞,并且/或者(ii)激活免疫系统以在感染SARS-CoV-2之前刺激抗病毒免疫。本发明诸位发明人认为,这种方法(即使用mCRAG刺激T细胞)可以单独使用和/或与其他研究组目前正在考虑的其他方法(如疫苗)一起使用。
因此,本发明的目的是提供用于通过刺激T细胞预防或治疗与冠状病毒相关的疾病和/或症状的方法和包含mCRAG的组合物。特别地,本发明的目的是提供用于预防或治疗有需要的受试者的与SARS-CoV-2病毒相关的疾病和/或症状的mCRAG。

黏膜IgA对微生物群具有广泛的交叉反应性,有助于维持微生物群与宿主的稳态。不受理论的约束,通过用与冠状病毒(包括SARS-CoV-2,表面多肽(主要是S蛋白))具有同源表位的mCRAG刺激T细胞,有可能诱导交叉反应IgA抗体,该抗体可以通过在黏膜表面与SARS-CoV-2或其他冠状病毒病毒粒子发生交叉反应而降低感染风险。黏膜IgA抗体还经由在上皮细胞中表达的聚合Ig受体将病毒颗粒从上皮细胞的宿主侧转运到微生物群侧。
为了产生抗原特异性反应和新抗体产生,mCRAG应在例如肠道/黏膜中诱导T细胞依赖性反应。如果以粉末形式食用,这些抗体也可能在呼吸道的黏膜中产生。
抗原特异性T细胞依赖性反应在次级淋巴组织(淋巴结/派伊尔斑)中由以下细胞驱动:(i)摄入外源抗原并在其表面上的MHC-II/MHC-I分子上展示交叉反应肽的抗原呈递细胞,(ii)识别肽:MHC-II复合物的CD4 T细胞,(iii)由CD4 T细胞活化的抗原特异性CD8 T细胞,以及(iv)由结合病原体并在MHC-II中展示肽的B细胞受体活化的B细胞,以及(v)由来自同一外源抗原的肽通过抗原呈递细胞活化的CD4 T细胞。
T细胞依赖性IgA的产生由TGF-β(IgA类别转换)驱动,并且B细胞的扩增/分化由CD4 T细胞分泌的IL-5、IL-6、IL-10和IL-21驱动。这基本上是一种致耐受性Treg/屏障,保护由分泌IL-6、IL-10、IL-23、TGF-β的树突细胞诱导的Th17对微生物群的反应。
mCRAG对T细胞的刺激诱导肠道中的IgA反应。已经表明,一些浆B细胞(Ab产生细胞)会移动到其他黏膜部位(约10%),例如呼吸道(约1-2%),以添加到现有的浆细胞/IgA池中。

mCRAG刺激T细胞也可以影响现有的浆细胞活化和IgA池。如果存在针对SARS-CoV-2的交叉反应IgA,则mCRAG可以通过不依赖T细胞的机制来支持总IgA产生。共生微生物群/益生菌在肠中诱导总IgA产生(不依赖抗原),并且也可能对肠外IgA产生具有影响。例如,短链脂肪酸和来自上皮细胞的TGF-β(IgA类别转换)、IL-6、视黄酸、BAFF和APRIL的诱导会驱动IgA产生。不受理论的约束,据信衍生自这样的微生物群/益生菌的mCRAG也会诱导类似的总IgA和肠外IgA产生。
例如,20多年来,已经在临床研究中研究了针对感冒和流感的益生菌补充剂。临床研究的几项荟萃分析表明,益生菌通常可能对降低呼吸道感染的风险和持续时间有效。然而,益生菌对免疫刺激和对呼吸道感染的有益效果之间存在菌株特异性差异。对乳酸双歧杆菌(B.lactis)Bl-04、嗜酸乳杆菌(L.acidophilus)NCFM和乳酸双歧杆菌Bi-07的研究以及文献中的其他研究表明,益生菌通过“训练”先天免疫反应发挥作用,即通过在病毒感染之前启动免疫系统发挥作用。研究表明,在将病毒载量或感染风险降低大约20%之前,干扰素和先天免疫细胞因子的表达增加。

已经表明益生菌如嗜酸乳杆菌(Lactobacillus acidophilus)NCFM可以诱导与抗病毒免疫如干扰素β相关的特定途径并且增加检测病毒RNA的受体(TLR3)的表达。此外,NCFM降低了3-5岁儿童的感冒症状的发生率。它还在体外驱动IL-12产生。不受理论的约束,据信通过选择衍生自驱动/刺激抗病毒反应的一种或多种细菌菌株或一种或多种优化益生菌的聚生体的mCRAG,可以降低SARS-CoV-2感染和严重COVID-19疾病的风险,并且缩短疾病的持续时间和病程,同时可能减少总症状负荷。

实例
实例1:富集和评价共生微生物与致病性抗原之间具有交叉反应的T细胞库
在本实例中,实验测定与已知病毒病原体具有高氨基酸序列同源性的微生物肽的交叉反应潜力。设计了体外T细胞回忆方案以识别这样的相互作用。

一旦获得对微生物抗原有反应的T细胞亚群,将进行体外扩增阶段,通过细胞因子补充支持此阶段。这些细胞因子可包括(IL-2、IL-7、IL-15、IL-4)。通过抗体缀合的聚苯乙烯微球进行的T细胞受体(TCR)复合物和共刺激受体(例如CD28和CD137)进一步拮抗用于模拟由抗原呈递细胞提供的刺激信号。T细胞培养方法用于维持T细胞的体外增殖,并持续14天或直至获得足够的细胞以在实验的第二阶段中作为测定的种子,此时去除刺激性微球和细胞因子以使富集的T细胞系恢复至静息状态。
将静息T细胞系与初始微生物抗原刺激中使用的来自相同供体样品的自体PBMC细胞以1:10的比例重新组合,以提供再刺激测定中T细胞活化所需的抗原呈递功能。保留的自体PBMC还用于测量对在再刺激测定中应用的肽抗原的基线初始反应。
表1和表2中列出的以下肽用于测试如第1阶段描述的方法中建立的T细胞系中的交叉反应性,并包括以下内容。
SARS-CoV-2肽:对应于IEDB数据库中整理的已知免疫原性T细胞表位的合成肽,依靠经验证明其可在先前感染病原体(在此情况下是SARS-CoV-2)的人类中诱导T细胞反应。
CRAg(mCRAG)肽:微生物衍生的与SARS-CoV-2具有同源性的氨基酸序列,其保持与SARS-CoV-2肽(与其同源)相当的HLA结合。
CRAg对照肽:与SARS-CoV-2非同源但衍生自与CRAg肽相同的微生物蛋白质的肽,具有与SARS-CoV-2肽和匹配的CRAg肽相似的HLA等位基因的理想HLA结合特性。

确定交叉反应性的测定使用基于流式细胞术的测定进行,这些测定可测量活化信号,包括细胞因子的细胞内蓄积、活化诱导的T细胞活化标记物的表面水平或对肽刺激作出反应的增殖。
实例2:对益生菌衍生的CRAG肽有反应的T细胞扩增增加健康人供体PBMC中SARS-
CoV-2反应性TCR克隆型的百分比
为了证明CRAG肽能够交叉活化对SARS-Cov-2肽抗原有反应的T细胞受体(TCR),使用来自健康供体的冻存PBMC制剂进行了2步离体测定。在含有合并肽的培养基中培养PBMC,其中肽的氨基酸序列源自益生菌生物体所表达的蛋白质。在一个肽池中,益生菌序列与SARS-CoV-2抗原具有高度的氨基酸序列同源性,已知这些抗原会在从康复期Covid-19患者中采集的T细胞中引发回忆反应。第二个池由一组来自相同益生菌蛋白质但与SARS-CoV-2没有同源序列同一性的匹配肽组成。此外,还对每个配对的CRAG和对照肽进行了序列基序的筛选,将预测这些序列基序能够以与亲本SARS-CoV-2抗原相似的亲和力与相同的HLA等位基因结合。这是为了最大程度增加每个肽池中相同组的肽可被个体供体抗原呈递细胞识别的概率,尽管供体群体中的HLA等位基因遗传学具有高变异性。因此,将来自各供体的CRAG和匹配的对照肽池在单独的孔中孵育,孵育期2周,以支持各池中TCR特异性克隆型的增殖。在方案的最后48小时内,去除支持抗原诱导的活化和增殖的细胞因子和抗原补充剂,以使T细胞恢复至静息状态。
在方案的第二阶段,用多种肽池和对照处理对CRAG和对照肽刺激供体部分中的T细胞比例进行再刺激,持续16小时,并通过流式细胞术分析CD4+T细胞的细胞内IFN-γ产生。用CRAG、对照和SARS-CoV-2肽池对各离体扩增部分进行再刺激。将来自相同供体的CRAG和对照部分作为成对样品处理,以比较再刺激时IFN-γ阳性CD4细胞%的差异。
如所预期的,CRAG部分比来自相同供体的对照部分含有更多的CRAG反应性CD4 T细胞,相反,对照部分比CRAG部分含有更多的对照肽反应性CD4 T细胞。如图1所示,对SARS-CoV-2肽再刺激作出反应时,在CRAG部分中的IFN-γ百分比较高,这表明CRAG反应性克隆型可能有助于对SARS-CoV-2肽作出反应而产生的信号。此结果表明,在分子水平上,可在益生菌蛋白质中包含的氨基酸片段中发现的序列同源性程度足以诱导已知参与SARS-CoV-2免疫反应的T细胞的活化。
Claims (5)
1.一种用于刺激T细胞的方法,所述方法包括使所述T细胞与一种或多种包含SEQ IDNO.1-71的多肽接触,其中刺激所述T细胞导致以下中的一种或多种:i)T细胞增殖增加;ii)细胞因子分泌;以及iii)一种或多种表面表达的活化标记物上调。
2.根据权利要求1所述的方法,其中所述细胞因子是选自由IFNγ、TNFα、IL2、IL17、IL22、IL4和IL5组成的组的一种或多种细胞因子。
3.根据权利要求1或权利要求2所述的方法,其中所述表面表达的活化标记物是选自由CD69、CD134、CD137、CD154和CD25组成的组的一种或多种标记物。
4.根据权利要求1-3中任一项所述的方法,其中所述T细胞是细胞毒性T细胞、辅助性T细胞和/或γδT细胞。
5.根据权利要求1-4中任一项所述的方法,其中所述一种或多种多肽在细胞中表达。
