Abstract

技术领域
本发明涉及生物医药领域,特别是涉及PD-L1作为药物靶点在筛选抑制肌腱粘连药物中的应用。
背景技术
肌腱粘连的形成是一个复杂的病理过程,涉及多种因素,病理机制不清楚。在组织愈合的后期,过度增殖的成纤维细胞可能会失去细胞之间的接触抑制和密度抑制,导致其不受控制的过度增殖,出现肌腱粘连。而目前没有对术后肌腱粘连形成进行很好控制和治疗的药物,这导致肌腱粘连的治疗受到一定的限制,因此,亟需进一步研究肌腱粘连的发病机理找到新的药物靶点,以为肌腱粘连的治疗提供科学的依据和方法。
发明内容
本发明的目的是提供PD-L1作为药物靶点在筛选抑制肌腱粘连药物中的应用,以解决上述现有技术存在的问题,通过敲低PD-L1,或者联合敲低PD-L1和TGF-β1能够重塑肌腱损伤后细胞外基质环境,激活CD8+T细胞免疫优势,为抑制肌腱粘连形成和改善滑动功能提供了一种潜在的治疗方法。
为实现上述目的,本发明提供了如下方案:
本发明提供了PD-L1作为药物靶点或者PD-L1和TGF-β1联合作为药物靶点在筛选抑制肌腱粘连药物中的应用。
本发明还提供了PD-L1抑制剂或者PD-L1和TGF-β1的抑制剂在制备抑制肌腱粘连药物中的应用。
优选的是,所述PD-L1抑制剂包括降低PD-L1表达的调节剂;
所述PD-L1和TGF-β1的抑制剂包括降低PD-L1和TGF-β1表达的调节剂。
优选的是,所述降低PD-L1表达的调节剂包括敲除或者沉默PD-L1的siRNA;
所述降低PD-L1和TGF-β1表达的调节剂包括分别敲除或者沉默PD-L1和TGF-β1的siRNA。

本发明的发明人研究团队发现PD-L1调控免疫逃逸可以作为抑制肌腱粘连的新靶点,本发明首次将免疫调节与肌腱粘连联系在一起,通过实验发现抑制PD-L1后能够促进CD8+T细胞的杀伤能力,减少肌腱粘连形成促进肌腱的功能。同时,本发明还首次将PD-L1siRNA和TGF-β1siRNA联合使用,并在大鼠和鸡损伤肌腱模型上进行应用,通过实验发现两者的协同作用,可以同时靶向TGF-β1/PD-L1重塑肌腱损伤后细胞外基质环境和免疫微环境,进一步抑制肌腱粘连和改善滑行功能,取得了满意治疗疗效,说明PD-L1 siRNA和TGF-β1siRNA联合使用可用于治疗肌腱的术后粘连和滑动功能的改善。本发明为抑制肌腱粘连提供了新的药物靶点,为抑制肌腱粘连形成和改善滑动功能提供了一种潜在的治疗方法。

实施例1siRNAs的设计与合成
1、免疫组化染色法和蛋白印迹法检测正常的和损伤修复后的肌腱组织的差异
1.1免疫组化染色法
从大鼠或鸡体内收集正常的和损伤修复后的肌腱组织,并在室温下在4%多聚甲醛中固定24小时。然后将样品脱水并嵌入石蜡块中。在石蜡切片机上切下5μm厚的组织片并附于载玻片上。封闭后,将载玻片与小鼠抗PD-L1抗体(1:2000,Proteintech)在4℃下孵育过夜。PBS清洗后,将载玻片与二抗在37℃下孵育2小时。使用3,3-二氨基联苯胺(DAB,Sigma,St.Louis,MO,USA)显色,并用苏木精对切片进行复染。最后封片,并用显微镜(LeicaDMR 3000;Leica,Bensheim,Germany)观察。
1.2蛋白质印迹法
用含有蛋白酶抑制剂的RIPA裂解缓冲液(Servicebio,武汉,湖北,中国)处理组织或细胞,收集裂解液并煮沸使其变性。蛋白质样品在10%十二烷基硫酸钠(SDS)-聚丙烯酰胺凝胶上电泳分离,并转移到聚偏氟乙烯(PVDF)膜上进行封闭(5%脱脂乳)1.5小时。然后,将膜与小鼠抗PD-L1抗体(1:2000,Proteintech)和小鼠抗β-actin抗体(1:10000,Proteintech)在4℃孵育过夜。第二天,使用TBST洗涤膜,并与抗小鼠IgG偶联的IRDye800(1:10000,Rockland Gilbertsville,CA,USA)二抗在4℃下进一步孵育过夜。最后,使用奥德赛红外成像系统(LI-COR,Lincoln,NE,USA)对膜进行可视化操作。通过Image J软件定量测定膜上每个带的光密度值,β-actin作为内参。
结果如图1所示,在大鼠肌腱损伤修复后的3周和鸡肌腱损伤修复后的6周,两种肌腱损伤模型联合显示出PD-L1蛋白的高表达。在未损伤的大鼠肌腱及周围组织中,PD-L1呈弱阳性,然而,在损伤后3周,发现PD-L1的强阳性信号(图1中A)。通过蛋白质印迹的半定量分析,与正常组织相比,损伤组织中PD-L1的表达增加了约3倍(图1中B)。PD-L1在鸡屈肌腱损伤模型中的表达与大鼠模型相似,其增加倍数约为2倍(图1中C和D)。
2、siRNAs的设计与合成
基于上述在大鼠和鸡的正常的和损伤的肌腱组织中的PD-L1表达存在显著差异,发明人涉及了该基因的siRNA,具体如下:
根据大鼠TGF-β1(NM_021578.2)、大鼠PD-L1(NM_001191954.2)、鸡TGF-β2(NM_001318456.1)、鸡PD-L1(XM_046935705.1)的基因序列设计了TGF-β-siRNA和PD-L1siRNA,由GenePharma(中国上海)合成,并通过实验证实有效。siRNA寡聚物的序列如表1所示。

实施例2
1、纳米球/siRNAs复合物的制备
(1)PLGA纳米球的制备方法
通过双乳液法制备PLGA纳米球,即将100mg聚乳酸-羟基乙酸共聚物(PLGA)溶解在1mL二氯甲烷中,获得浓度为10%的PLGA溶液,随后制备质量百分比为7%和1%的聚乙烯醇(PVA)水溶液。将浓度为10%的PLGA溶液和7%的PVA水溶液混合并用超声波破碎机超声处理1分钟以制备乳液,然后加入50mL 1% PVA溶液中继续超声处理3分钟并搅拌12小时以去除二氯甲烷,得到PLGA纳米球;最后PLGA纳米球洗涤、重悬,得到PLGA纳米球溶液,在4℃储存备用。将20μL聚乙基亚胺(PEI)水溶液(100mg/mL)添加到1mL PLGA纳米球溶液(20mg/mL)中以形成PEI修饰的纳米球。
(2)纳米球/siRNAs复合物的制备
将siRNA干粉溶解在DEPC水中,浓度为20μM。然后,根据凝胶阻滞实验的结果,将PEI修饰的纳米球和siRNAs溶液以6:1的N/P比(PEI的胺基的摩尔数与RNA的磷酸基的摩尔量的比率)混合。混合后,将溶液在室温下静置20分钟以获得纳米球/siRNAs复合物。
纳米球/siRNAs复合物的制备流程如下图2中A所示。在扫描电子显微镜下观察,PEI纳米球和纳米球/siRNAs复合物的形态没有显著差异,都是球形结构(见图2中B)。
2、透明质酸水凝胶的制备
30mg叠氮修饰的透明质酸溶解于2.4mL PBS中,加入2mg二苯基环辛烯聚乙二醇二苯基环辛烯(DBCO-PEG-DBCO),通过无铜点击反应形成空载水凝胶。若同时加入纳米球/siRNAs复合物(含33μg siRNA)和DBCO-PEG-DBCO,即在无铜点击反应后形成水凝胶-纳米球/siRNAs复合物。
空载水凝胶以及水凝胶-纳米球/siRNAs复合物的制备流程如图3中A所示。在电子显微镜下观察发现:空载水凝胶是透明的,而水凝胶-纳米球/siRNAs复合物是浑浊的;它们都是三维网络结构。在高倍镜下,空载水凝胶表面光滑,而水凝胶-纳米球/siRNAs复合物中,其纳米球均匀分布在水凝胶的表面(由箭头指示)(见图3中B)。
3、构建肌腱损伤的动物模型
动物实验程序经南通大学实验动物中心动物护理委员会批准(编号:S20200325-906)。
3.1大鼠FDL肌腱的损伤与修复
3.1.1大鼠肌腱损伤的动物模型
30只8周龄的Sprague-Dawley(SD)大鼠(60根肌腱)被用于大鼠FDL肌腱损伤模型,并被平均分为六组:手术修复组(Control)、空载水凝胶治疗的手术修复组(Gel)、经水凝胶-纳米球/NC siRNAs复合物治疗的手术修复组(NC,阴性对照)、经水凝胶-纳米球/TGF-β1+PD-L1 siRNAs复合物治疗的外科修复组(T)、经水凝胶-纳米球/PD-L1 siRNAs复合物治疗的外科修复组(P)和经水凝胶-纳米球/TGF-β1+PD-L1 siRNAs复合物治疗的外科修复组(TP)。每组有10个样本,所有样本均用于滑行功能的检测,其中6个样本解剖后进行生物力学测试,4个样本用于组织学的分析。对于手术过程,首先通过腹腔注射2%戊巴比妥钠(3mL/kg)麻醉动物。为防止术中严重出血,将含有1%肾上腺素和50%盐酸利多卡因的生理盐水溶液注入掌部(0.05mL/掌)。在暴露FDL肌腱后,横向切断屈肌腱,使用4-0聚丙烯缝线(HOLYCON,中国江苏南通),采用2股改良的Kessler法和环形修复进行缝合。然后,将水凝胶包裹在修复后的肌腱周围,最后用3-0尼龙缝合线(HOLYCON)缝合皮肤。
3.1.2鸡肌腱损伤的动物模型
36只一年龄的三黄鸡(72根肌腱)用于实验,与上述大鼠分组一样,共分为6组,每组12个样本。所有样本均用于滑动位移和粘连评分的检测,其中9个样本进行了解剖学和生物力学测试,3个样本用于组织学分析。对于手术过程,首先使用2%戊巴比妥钠(3mL/kg)对鸡进行肌肉麻醉,然后在鸡趾的近端指间关节(PIP)和远端指间关节(DIP)之间的足底皮肤上进行Z字形切口。FDP肌腱暴露后用手术刀进行横断,并采用5-0的聚丙烯缝线(Prolene;Ethicon,Somerville,NJ,USA)对切断的屈肌腱进行4股Kessler缝合。肌腱缝合后,水凝胶包裹在修复后的肌腱周围,并用3-0尼龙缝合线(HOLYCON)缝合皮肤。手术后,用胶带将手术后的脚趾固定在半弯曲位置长达3周。治疗6周后,收集肌腱组织和粘连组织。
3.1.3滑动功能的检测
(1)大鼠FDL肌腱模型的滑动功能检测
大鼠在安乐死后,将其后肢在膝盖处分离。沿着胫骨的近端屈肌腱在跗骨隧道附近被分离,然后胫骨被移除。随后在肌腱的近端用两根注射针固定,避免刺穿肌腱。然后,用一圈单股缝合线牵引肌腱的滑动和脚趾的弯曲。简而言之,缝合线的一端穿过肌肉和肌腱的连接处,另一端连接放置砝码的袋子。自由悬挂的砝码从0、5、10、15到20g不等,以加载肌腱。脚趾从保持完全伸展作为测试的起点,然后因为砝码对近段肌腱的拉力,脚趾被允许被动弯曲。通过观察者测量每个载荷下最长脚趾与水平面之间的角度来记录屈曲角度。手机支架固定在爪子的侧面,并在每次改变载荷下拍摄一张照片。
(2)鸡FDP肌腱模型的滑动功能检测
鸡安乐死后,收集修复后的鸡脚趾,使用生物力学测试仪(型号4411;InstronCorp.,Canton,MA,USA)检测肌腱的滑动位移。首先解剖暴露FDP肌腱,随后将修复后的脚趾用克氏针固定在测量板上,并用止血钳夹住肌腱的近端并固定于上夹具。然后测试仪以25mm/min的恒定速度拉动上夹具,直到力达到15N。测试软件(Series IX软件;InstronCorp.)同时测量载荷和滑动位移。荷载-位移曲线由计算机自动生成。
3.1.4肌腱粘连的评估
首先对修复过的肌腱进行纵向剥离,然后可以观察到粘连的形态和外观,如严重程度和范围。最后,通过既定的分级方法对肌腱损伤部位周围形成的粘连进行评估和评分,参考的文献为Jin Bo Tang,Yi Cao,Bei Zhu,et al.Adeno-associated virus-2-mediated bFGF gene transfer to digital flexor tendons significantly increaseshealing strength.an in vivo study.J Bone Joint Surg Am,2008May;90(5):1078-89。
3.1.5生物力学检测
将修复的大鼠或鸡肌腱与周围组织轻轻分离,并将粘连组织完全去除,随后用Instron力学测试仪(型号3365;Instron Corp.)测试肌腱愈合强度。首先肌腱两端分别夹在测试仪的下夹钳和上夹钳中,修复部位保持在两个夹钳的中间。然后,上夹具以25mm/min的恒定速度向上拉动,并在肌腱断裂时停止。
3.1.6免疫荧光检测
用含有0.3%Triton X-100的封闭试剂封闭组织切片1.5小时。然后用PE标记的抗鼠CD8抗体(1:200,Biolegend)在4℃下染色过夜。洗涤后,用DAPI对载玻片进行染色,并在显微镜(Leica)下观察。
3.2统计学分析
数据用平均值±标准差表示。对于来自两组的数据,在检测到正态性后,使用student t检验进行统计分析。对于三组或三组以上的数据,如果方差不均匀,则对数据进行对数转换或进行非参数检验。如果方差满足同质性,则进行单因素方差分析。一旦检验具有统计学意义,就用Fisher LSD检验对任意两组进行比较。在所有测试中,p值小于0.05被认为具有统计学意义。
4、结果与分析
(1)构建的肌腱损伤模型
本发明建立了两种肌腱损伤模型:大鼠FDL肌腱损伤和鸡FDP肌腱损伤,并确保负载不同siRNAs的水凝胶纳米球系统均可以完全覆盖在整个缝合部位(见图4)。
(2)滑动功能和生物力学检测结果
大鼠FDL肌腱损伤修复后3周,收集肌腱和粘连的组织。首先进行屈肌腱的滑动试验,随着砝码重量的增加,TP组的肌腱屈曲角度在六组中最大,其次是T组和P组。对于TP组的肌腱,从5g到20g的砝码重量变化,其屈曲角度的增加显著大于Control组和NC组。除了Gel组的肌腱与NC组的肌腱在10g的载荷下,其屈曲角度有统计学差异,在其他载荷下,Control组、Gel组和NC组的肌腱滑动角度均没有显著差异(见图5中A)。在生物力学测试中,肌腱全部在切断部位断裂,TP组的肌腱极限强度(38.4±8.1N)明显高于Control组(29.2±5.1N,p=0.044)和NC组(24.6±4.2N,p=0.004)。与NC组相比,P组肌腱极限强度显著增加,但与Control组无差异。T组肌腱的极限强度没有显示出任何优势(见图5中B)。
(3)PD-L1 siRNA和TGF-β1siRNA单独与共同使用对促进鸡肌腱修复的影响
鸡FDP肌腱损伤修复后6周,收集肌腱和粘连的组织。首先进行15N滑动位移检测,P和TP组的肌腱在15N下肌腱的滑动位移更大,与Control组和NC组相比有显著差异。尽管T组的肌腱与Control组之间没有统计学差异,但T组的肌腱滑动位移均值有所增加(见图6中A)。与滑动位移的结果一样,P组和TP组肌腱的粘连评分显示出显著优势(见图6中B)。六组肌腱之间的极限强度没有显著差异(见图6中C)。
(4)免疫荧光检测结果
为了探讨CD8+T细胞的杀伤功能是否在体内得到增强,本发明检测了CD8+T淋巴细胞在大鼠和鸡肌腱与粘连过渡区的分布。在大鼠和鸡组织中,Control组、Gel组和NC组的CD8+T细胞数量罕见且分布均匀,在T组的组织中略有增加,而P组和TP组的组织中CD8+T淋巴细胞数量显著增加并呈簇状分布(见图7)。
以上所述的实施例仅是对本发明的优选方式进行描述,并非对本发明的范围进行限定,在不脱离本发明设计精神的前提下,本领域普通技术人员对本发明的技术方案做出的各种变形和改进,均应落入本发明权利要求书确定的保护范围内。
Claims (10)
1.PD-L1作为药物靶点或者PD-L1和TGF-β1联合作为药物靶点在筛选抑制肌腱粘连药物中的应用。
2.PD-L1抑制剂或者PD-L1和TGF-β1的抑制剂在制备抑制肌腱粘连药物中的应用。
3.如权利要求2所述的应用,其特征在于,所述PD-L1抑制剂包括降低PD-L1表达的调节剂;
所述PD-L1和TGF-β1的抑制剂包括降低PD-L1和TGF-β1表达的调节剂。
4.如权利要求3所述的应用,其特征在于,所述降低PD-L1表达的调节剂包括敲除或者沉默PD-L1的siRNA;
所述降低PD-L1和TGF-β1表达的调节剂包括分别敲除或者沉默PD-L1和TGF-β1的siRNA。
5.如权利要求4所述的应用,其特征在于,所述敲除或者沉默PD-L1的siRNA的核苷酸序列如SEQ ID NO:1或SEQ ID NO:3所示序列中的任意一条;
敲除或者沉默TGF-β1的siRNA的核苷酸序列如SEQ ID NO:2或SEQ ID NO:4所示序列中的任意一条。
6.一种抑制肌腱粘连的药物,其特征在于,其包括PD-L1 siRNA,或者PD-L1 siRNA和TGF-β1siRNA的组合;其中,所述PD-L1 siRNA的核苷酸序列如SEQ ID NO:1或SEQ ID NO:3所示序列中的任意一条;所述TGF-β1siRNA的核苷酸序列如SEQ ID NO:2或SEQ ID NO:4所示序列中的任意一条。
7.如权利要求6所述的抑制肌腱粘连的药物,其特征在于,所述药物包括PD-L1 siRNA或者PD-L1 siRNA和TGF-β1siRNA的组合,以及负载PD-L1 siRNA或者PD-L1 siRNA和TGF-β1siRNA的组合的纳米载体。
8.如权利要求7所述的抑制肌腱粘连的药物,其特征在于,所述负载PD-L1 siRNA或者PD-L1 siRNA和TGF-β1siRNA的组合的纳米载体的制备方法包括以下步骤:
采用双乳液法制备聚乳酸-羟基乙酸共聚物纳米球,之后用聚乙基亚胺修饰所述聚乳酸-羟基乙酸共聚物纳米球,制得聚乙基亚胺修饰的纳米球;
将PD-L1 siRNA或者PD-L1 siRNA和TGF-β1siRNA的组合与所述聚乙基亚胺修饰的纳米球混合均匀,制得纳米球/siRNAs复合物;
将叠氮修饰的透明质酸、所述纳米球/siRNAs复合物和二苯基环辛烯聚乙二醇二苯基环辛烯混合,通过无铜点击反应,制得水凝胶-纳米球/siRNAs复合物,即为负载PD-L1siRNA或者PD-L1 siRNA和TGF-β1siRNA的组合的纳米载体。
9.如权利要求8所述的抑制肌腱粘连的药物,其特征在于,采用双乳液法制备聚乳酸-羟基乙酸共聚物纳米球的方法包括以下步骤:
将聚乳酸-羟基乙酸共聚物的溶液与质量百分比为5%-10%的聚乙烯醇溶液混合超声,制成乳液后,再与质量百分比为1%-4%的聚乙烯醇溶液混合超声,制得所述聚乳酸-羟基乙酸共聚物纳米球;其中,所述聚乳酸-羟基乙酸共聚物的溶液质量百分比为8%-15%。
10.如权利要求8所述的抑制肌腱粘连的药物,其特征在于,所述PD-L1 siRNA或者PD-L1siRNA和TGF-β1siRNA的组合与所述聚乙基亚胺修饰的纳米球的P/N比为1:(5-10),所述P/N比为siRNA的磷酸基的摩尔数与聚乙基亚胺的胺基的摩尔数的比率;
所述叠氮修饰透明质酸、所述纳米球/siRNAs复合物和所述二苯基环辛烯聚乙二醇二苯基环辛烯的质量比为1mg:0.1mg:0.2mg;其中,所述纳米球/siRNAs复合物中纳米球和siRNAs的质量比为1mg:10μg。
