技术领域
本发明涉及用于增加甲氧西林耐药性金黄色葡萄球菌(MRSA)对抗菌药物的药物敏感性的组合物、用于治疗或预防MRSA感染的组合物以及用于降低MRSA毒力的组合物。
背景技术
近年来,随着甲氧西林耐药性金黄色葡萄球菌(MRSA)的感染病例数量不仅在日本而且在全世界范围内迅速增加,MRSA已被认为是临床上有问题的严重感染的致病菌,并且已对其治疗剂进行了研究。
噬菌体是感染细菌、以细菌为宿主感染细菌、然后在细菌体内生长的病毒的统称。生长产生的子噬菌体通过细菌的细胞壁排出细菌细胞外,细菌经历溶菌而死亡。另外,噬菌体的优点在于,例如,它们不影响作为其宿主的病原菌之外的其他细菌菌群,并且它们易于生长并且可以以低成本制备大量噬菌体。
鉴于这些优点,已经尝试使用噬菌体作为细菌的杀菌剂等,并且已经报道了一些利用噬菌体对金黄色葡萄球菌(S.aureus)进行杀菌的研究结果。
在专利文献1中,本发明人发现并报道了具有引起金黄色葡萄球菌溶菌特性的噬菌体,其是通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体。
此外,非专利文献1提出了与单独施用抗菌药物相比,施用噬菌体PYOSa后施用抗菌药物对于金黄色葡萄球菌更有效的可能性。

技术问题
虽然已经尝试寻找MRSA感染的治疗方法,但MRSA感染的多样化正在进展:例如,社区获得性MRSA感染变得更加频繁;与牲畜相关的MRSA已成为世界性的问题。在这种情况下,更加强烈地需要适合MRSA感染个体病例的治疗策略,而针对MRSA的新方法仍有待开发。

为了实现该目的,根据本发明的第一方面的用于增加甲氧西林耐药性金黄色葡萄球菌(methicillin-resistant Staphylococcus aureus,MRSA)对抗菌药物的药物敏感性的组合物包含:
至少一种噬菌体,所述噬菌体选自由通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体组成的组。
例如,通过使MRSA获得对噬菌体的耐药性来增加药物敏感性。
例如,抗菌药物为β-内酰胺类抗菌药物。
根据本发明的第二方面的用于治疗或预防MRSA感染的组合物包含:

具体实施方式
(1.用于增加MRSA对抗菌药物的药物敏感性的组合物)
本发明的组合物是用于增加甲氧西林耐药性金黄色葡萄球菌(MRSA)对抗菌药物的药物敏感性的组合物。该组合包含:至少一种噬菌体,所述噬菌体选自由通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体组成的组。
用作本发明的组合物的活性成分的噬菌体之一是于2008年12月25日以保藏编号NITE BP-693保藏于NITE,国家高级工业科学技术学院,国际专利生物保藏中心的噬菌体(2-5-8,上总镰足,木更津市,千叶县2920818,日本)。本发明人将该噬菌体命名为“ΦSA012”。
用作本发明的组合物的活性成分的另一噬菌体是于2008年12月25日以保藏编号NITE BP-694保藏于NITE,国家高级工业科学技术学院,国际专利生物保藏中心(2-5-8,上总镰足,木更津市,千叶县2920818,日本)。本发明人将该噬菌体命名为“ΦSA039”。
通过透射电子显微镜观察可知,ΦSA012和ΦSA039均具有与T4噬菌体几乎相同的尺寸,并且比T4噬菌体稍长。ΦSA012和ΦSA039均具有可收缩的鞘,因此推断属于肌病毒科。对于本发明的噬菌体的其他结构和特征,参见日本专利公开第2011-50373号中的描述。
根据本发明的噬菌体可以通过向保藏噬菌体的保藏单位提交提交请求来获得。可选择地,噬菌体可以通过用常规方法如日本专利公开第2011-50373号中描述的噬菌斑测定/分离方法的分离/纯化而从生活污水流或污水处理设施收集的废水等获得,或者从生物样品(例如从感染金黄色葡萄球菌的动物的血液、鼻腔、咽、皮肤或肠道)获得。根据本发明的噬菌体可以利用常见的噬菌体培养方法如日本专利公开第2011-50373号中描述的平板裂解液方法来培养。
本发明的组合物至少需要含有上述任一种噬菌体。例如,可以使用通过上述培养方法得到的噬菌体溶液、通过离心等从培养残余物中分离出噬菌体溶液获得的产物。
本发明的组合物中含有的噬菌体的详细情况如上所述。本发明的组合物中所含的噬菌体可以是ΦSA012和ΦSA039中的任一株,也可以是ΦSA012和ΦSA039的噬菌体混合溶液(噬菌体混合物)。还可以包含另外的噬菌体。另外的噬菌体的实例包括但不限于:针对不同于根据本发明的MRSA的细菌(例如金黄色葡萄球菌)的噬菌体,并且另外的噬菌体可以是温和噬菌体,而不限于溶菌噬菌体。
本领域技术人员可以根据使用形式适当调整本发明组合物中噬菌体的浓度,而不受限制;例如,作为液体剂型使用时的浓度优选为1×103至1×1013PFU/mL(PFU:噬菌斑形成单位),更优选为1×105至1×1011PFU/mL,甚至更优选为1×108至1×1010PFU/mL。
本文中,确定对抗菌药物的药物敏感性是否增加的方法例如如下:(i)应用圆盘法(药物敏感测试)确定MRSA对抗菌药物的敏感性的抑菌圈直径和确定MRSA在施加根据本发明的噬菌体的情况下对相同抗菌药物的敏感性的抑菌圈直径,若后者直径大于前者直径,则可确定对抗菌药物的药物敏感性增加;(ii)应用MIC法(药物敏感测试)确定MRSA对抗菌药物的敏感性的MIC(最低抑制浓度)值和确定MRSA在施加根据本发明的噬菌体的情况下对同一抗菌药物的敏感性的MIC值,如果后者MIC值小于前者MIC值,则可确定对该抗菌药物的药物敏感性增加。
例如,可以通过使MRSA获得对根据本发明的噬菌体的耐药性来增加MRSA对抗菌药物的药物敏感性。本文中,“获得耐药性”是指MRSA变得对根据本发明的噬菌体具有耐药性,从而使噬菌体对MRSA无效(例如,防止MRSA的生长被噬菌体抑制)或使噬菌体对MRSA的效力降低(例如,噬菌体对MRSA的生长抑制程度降低)。是否已获得对噬菌体的耐药性的确定例如如下:通过随后描述的EOP(噬菌斑形成率)或比浊法测定MRSA在施加根据本发明的噬菌体的情况下的生长活性,可以通过与施加噬菌体之前的生长活性进行比较来确定是否“获得了耐药性”。可选择地,例如,如果遗传分析检测到mgrA、femA或两者的突变,则可以确定“获得了耐药性”。
抗菌药物的实例包括但不限于:β-内酰胺类抗菌药物、氨基糖苷类抗菌药物、林可霉素类抗菌药物、磷霉素类抗菌药物、四环素类抗菌药物、氯霉素类抗菌药物、大环内酯类抗菌药物、酮内酯类抗菌药物、多肽类抗菌药物、糖肽类抗菌药、链霉素类抗菌药、喹诺酮类抗菌药和噁唑烷酮类。
抗菌药物优选为β-内酰胺类抗菌药物。β-内酰胺类抗菌药物的实例可包括:青霉素类抗菌药物(青霉素G、氨苄西林、巴氨西林、仑氨西林、环己西林、阿莫西林、匹美西林、阿扑西林、氯唑西林、哌拉西林、甲氧西林);联合青霉素类抗菌药物(氨苄西林、氯唑西林);含β-内酰胺酶抑制剂的青霉素类抗菌药物(氨苄西林、舒巴坦、克拉维酸、阿莫西林、哌拉西林、他唑巴坦);头孢烯类抗菌药物(头孢唑啉、头孢噻吩、头孢氨苄、头孢曲嗪、头孢沙定、头孢克洛、头孢羟氨苄、头孢替安、头孢美唑、氟氧头孢、头孢米诺、头孢拉宗、头孢呋辛、头孢呋辛酯、头孢地尼、头孢托仑、匹酯、头孢特仑、头孢泊肟、头孢泊肟酯、头孢卡品、头孢噻肟、头孢曲松、头孢哌酮、头孢甲肟、头孢他啶、头孢布坦、头孢克肟、头孢地嗪、拉氧头孢、头孢唑肟、头孢匹罗、头孢唑兰、头孢吡肟);含β-内酰胺酶抑制剂的头孢烯类抗菌药物(头孢哌酮、舒巴坦);碳青霉烯类抗菌药物(亚胺培南、西司他丁、帕尼培南、倍他米隆、美罗培南、比阿培南、多利培南、泰比培南);单环β内酰胺类抗菌药物(氨曲南、舒巴坦、他唑巴坦、卡芦莫南);和青霉烯类抗菌药物(法罗培南)。
例如,将本发明的组合物施用于受MRSA感染的受试者,例如哺乳动物如啮齿类动物(包括小鼠、大鼠、仓鼠和豚鼠),灵长类动物(包括人类、黑猩猩和恒河猴),牲畜(包括猪、牛、山羊、马、绵羊和禽类)以及宠物动物(包括狗和猫)。人类是优选的受试者。
例如,本发明的组合物可以按照常规方法配制成如片剂、颗粒剂、散剂、胶囊剂、糖浆剂和注射剂等剂型,可以使用适当的药物递送系统(DDS)。此外,还可以添加常规药品中使用的稀释剂、粘合剂、润滑剂、着色剂、崩解剂、增稠剂、防腐剂、稳定剂、pH调节剂等。组合物的剂量可以根据受试个体的年龄、体重、适应症等来适当设定。
(2.用于治疗或预防MRSA感染的组合物)
本发明提供的用于治疗或预防MRSA感染的组合物包含:至少一种噬菌体,该噬菌体选自由通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体组成的组;和抗菌药物。
本发明的组合物通过由根据本发明的噬菌体(如上所述)增加MRSA对抗菌药物的药物敏感性,从而引发抗菌药物治疗或预防MRSA感染的效果。
本文中,“MRSA感染”是指由MRSA引起的感染,例如呼吸道感染、菌血症、感染性心内膜炎、皮肤/软组织感染、腹内感染、骨/关节感染、中枢神经系统感染、尿路感染和小儿感染。本发明的组合物可用于治疗或预防任何那些MRSA感染,以及用于预防术后感染。
本文中,“治疗MRSA感染”涵盖用于各种目的的医学上可接受的治疗性干预,包括延迟或停止MRSA感染的进展、改善或暂时改善MRSA感染、对MRSA进行灭菌、去定植、抑制生长等,以及预防MRSA感染的复发。本文中,“预防MRSA感染”涵盖用于各种目的的医学上可接受的预防性干预,包括预防MRSA感染的发作或患上MRSA感染以及预防被MRSA感染。
例如,抗菌药物预防或治疗MRSA感染的效果可以通过使MRSA获得对作为本发明组合物的活性成分的噬菌体的耐药性,从而增加MRSA对抗菌药物的药物敏感性来引发。如何确定是否“获得了耐药性”如上所述。
抗菌药物如上所述,并且可以是例如β-内酰胺类抗菌药物。β-内酰胺类抗菌药物的详细情况如上所述。

本发明的组合物可以进一步含有另外的药物,或者与其组合使用。组合使用时,可以同时施用,也可以分时施用。“另外的药物”不受限制,只要另外的药物不抵消根据本发明的噬菌体增加对抗菌药物的药物敏感性的效果即可,并且可以根据使用方式从常规用于不同目的的药物中适当选择使用,例如抗炎剂、免疫抑制剂、感染预防剂、感染治疗剂、杀虫剂、驱虫剂、抗真菌剂和抗病毒剂。
(3.用于降低MRSA毒力的组合物)
本发明提供的用于降低MRSA毒力的组合物包含:至少一种噬菌体,该噬菌体选自由通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体组成的组。
本发明的组合物用于通过根据本发明的噬菌体降低MRSA的毒力(如上所述)。
本文中,“降低MRSA的毒力”是指降低MRSA感染另一种生物体以在宿主中引起感染的特性和能力。对于毒力的降低,例如,如果在施加根据本发明的噬菌体的情况下MRSA中与MRSA毒力相关的基因(例如EsaA、EsaB、EsaC、EssA、EssB、EssC、EsxA、EsxB、RNAIII、LukGH亚基H、LukGH亚基G、耐热核酸酶、葡萄球菌超抗原样蛋白11(SSL11)、葡萄球菌激酶、O型葡萄球菌肠毒素,各自在随后的实施例中描述)的表达水平低于未施加噬菌体的情况下MRSA中的表达水平,即可确定“MRSA的毒力已降低”。
例如,可以通过使MRSA获得对噬菌体的耐药性来降低毒力。对噬菌体的耐药性的获得可以通过上述方法来确认。
例如,将本发明的组合物施用于,例如哺乳动物,如啮齿类动物(包括小鼠、大鼠、仓鼠和豚鼠),灵长类动物(包括人类、黑猩猩和恒河猴),牲畜(包括猪、牛、山羊、马、绵羊和禽类)以及宠物动物(包括狗和猫)。人类是优选的受试者。
例如,本发明的组合物可以按照常规方法配制成如片剂、颗粒剂、散剂、胶囊剂、糖浆剂和注射剂等剂型,可以使用适当的药物递送系统(DDS)。此外,还可以添加常规药品中使用的稀释剂、粘合剂、润滑剂、着色剂、崩解剂、增稠剂、防腐剂、稳定剂、pH调节剂等。组合物的剂量可以根据受试个体的年龄、体重、适应症等来适当设定。
(4.简要概述)
如上文所解释的,本发明可以提供包含根据本发明的噬菌体并增加MRSA对抗菌药物的药物敏感性的组合物。用于MRSA的本发明的组合物针对以前的药物无法发挥灭菌、去定植、抑制生长等作用的使用增加了对抗菌药物的药物敏感性,从而使抗菌药物发挥其对MRSA的灭菌、去定植、抑制生长等作用。
本发明提供的用于治疗或预防MRSA感染的组合物可以通过由根据本发明的噬菌体增加MRSA对抗菌药物的药物敏感性,来引发抗菌药物治疗或预防MRSA感染的效果。即使对于以前的药物无效的MRSA感染,也可以通过由根据本发明的噬菌体增加对抗菌药物的药物敏感性,来引发有效的治疗或预防效果。
本发明提供的用于降低MRSA毒力的组合物可以通过根据本发明的噬菌体降低MRSA本身的毒力,最终可以预期对MRSA感染的治疗或预防效果。
应当注意的是,本发明还涵盖增加MRSA对抗菌药物的药物敏感性的方法、治疗或预防MRSA感染的方法、以及降低MRSA的毒力的方法,并且这些方法中的每一种都可以包括使MRSA获得对根据本发明的噬菌体耐药的步骤。
实施例
下面,将参考实施例具体描述本发明。然而,本发明不限于这些实施例。
本发明人先前已成功分离出具有引起金黄色葡萄球菌溶菌特性的噬菌体,其是通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体(ΦSA012和ΦSA039)。
在本实施例中,在如下所示的测试中,评估了ΦSA012增加MRSA对抗菌药物的药物敏感性的能力以及与毒性MRSA相关的基因的差异表达。
(实施例1)
已知获得噬菌体耐药性的细菌由于涉及其基因突变(权衡)的耐药性获得而导致其原始功能发生改变(丧失)。因此,研究了噬菌体耐药性(ΦSA012耐药性)MRSA的基因突变。
如下获得ΦSA012耐药性MRSA(ΦSA012mts)。在4mL LB液体培养基中,混合40μLMRSA2007-13菌珠(GCA_018409145.1)菌液和40μLΦSA012(109PFU/mL),并将所得物在37℃下振荡培养6天。此后,通过离心收集细菌细胞,用1mL PBS洗涤,并在LB琼脂培养基上划线。37℃下培养过夜后,收集培养基上形成的单菌落,添加到3mL LB-肉汤中,37℃下振荡培养过夜进行克隆。对克隆的菌珠通过进行软琼脂LB培养基点样测试和读板仪时间比浊法检查,将未发现ΦSA012溶菌活性的菌珠用作突变菌珠。
以下示出了突变体的突变分析方法。对突变体SAm1-201进行基因组提取,并使用下一代测序仪进行基因组分析。SAm1-201在LB-肉汤中预培养,并使用GenElut(R)细菌基因组DNA试剂盒(Sigma-Aldrich Co.,LLC)进行基因组提取(按照产品说明书进行)。基因组分析外包给Bioengineering Lab Co.,Ltd.,并采用新一代测序仪DNBSEQ-G400以2×200bp进行测序分析。使用Trimmomatic(版本0.39)去除接头序列和低质量读段,然后使用CLCGenomics Workbench 20进行参考序列映射和突变检测。结果,mgrA和femA的突变被检测为单核苷酸多态性,因此对两个基因的突变进行菌落PCR,其中待研究的突变体数量进一步增加,然后通过Sanger测序确定多个突变体的mgrA和femA的核苷酸序列。将突变体的菌液分别在LB琼脂培养基上划线,并在37℃下培养过夜,收集单菌落,进行PCR。PCR反应中使用的引物如下所示。

使用FastGene Gel/PCR提取试剂盒(NIPPON Genetics Co.,Ltd.)纯化所得的PCR产物(按照产品说明书进行)。Sanger测序外包给Hokkaido System Science Co.,Ltd。
获得的突变体的信息如下所示。所有突变体均发现mgrA突变(22/22:100%),并且发现约70%的突变体进一步具有femA突变(16/22:73%)(表1)。已知mgrA可调节大约多达350个基因表达。femA是一种合成构成细菌外壁的肽聚糖的酶,其突变预计会很大程度上改变肽聚糖的结构。表1中突变体名称后的三位数中,数字100至199表示仅mgrA发生突变。数字200至299表示mgrA和femA同时发生突变。

随后,检查噬菌体对上述获得的ΦSA012耐药性MRSA(突变体:SAm1-201、SAm1-106)的感染性。
测试方法如下所示。向3mL的LB顶级琼脂(LB-Top Agar)中添加100μL预培养的测试菌液(SAm1-201、SAm1-106),将所得物接种于LB-琼脂上。向其上逐滴添加1×102pfu/mL至1×108pfu/mL的系列稀释噬菌体溶液(ΦSA012)和SM缓冲液(阴性对照)各4μL,并将所得物在37℃下培养过夜。第二天,对突变菌珠(SAm1-201、SAm1-106)和参考菌珠(野生型菌珠:MRSA2007-13菌珠)中由噬菌体的溶菌活性所形成的噬菌斑进行计数,并基于以下计算公式对EOP(涂布效率:噬菌斑形成率)进行计算。

图1显示了结果。对于野生型菌珠(图1中的“wt”),通过添加ΦSA012形成噬菌斑,确认了被ΦSA012感染。相比之下,对于突变菌珠(SAm1-201、SAm1-106),没有观察到ΦSA012的噬菌斑形成能力,这证明ΦSA012没有感染突变菌珠(图1(a))。图1(b)显示了野生型菌珠(图1(b)中的“wt”)和突变菌珠(SAm1-201、SAm1-106)的溶菌状态的照片。对SAm1-201完全未发现溶菌,而对SAm1-106则在108pfu/mL(或107pfu/mL)下观察到由于溶菌达到称为混浊的水平而出现的噬菌斑,但未发现清晰的噬菌斑(图1(b))。因此,对于任何突变菌珠均未发现ΦSA012的溶菌作用,可以肯定地确认突变菌珠没有被ΦSA012感染。
随后,检查噬菌体(ΦSA012)对上述获得的ΦSA012耐药性MRSA(突变体:SAm1-201、SAm1-106)的生长抑制效果。
测试方法如下所示。该测试通过比浊法进行。比浊法可以实时测量噬菌体的感染性和细菌生长之间的关系。下面对比浊法的具体方法进行说明。使用96孔板,用时间比浊法分别测量野生型菌珠(MRSA2007-13菌珠)和突变体(SAm1-201、SAm1-106)在不存在ΦSA012(对照)或存在ΦSA012的情况下的生长活性。将预培养过夜的菌液(野生型菌珠(MRSA2007-13菌珠)或突变体(SAm1-201、SAm1-106))用LB-肉汤稀释至达到OD600≈1.0,进一步稀释10倍。向每个孔中添加90μL菌液和10μLΦSA012(1×108pfu/mL)。将板设置在SunriseRainbow Thermo RC(Tecan Japan Co.,Ltd.)中,在37℃下振荡培养24小时,同时每15分钟测量590nm波长处的吸光度值,并评价生长活性。
图2至图4显示了结果。在图2至图4的每个图中,纵坐标表示浊度,横坐标表示时间(h),灰色图形线和黑色图形线分别表示在没有噬菌体的情况下和有噬菌体的情况下的细菌生长。野生型菌珠(MRSA2007-13菌珠)立即被ΦSA012溶菌,表明有细菌生长的浊度没有增加;因此,确认了噬菌体抑制细菌生长(图2)。另一方面,对于mgrA中有一个突变的突变体(SAm1-106),ΦSA012对细菌生长的抑制程度减弱,并且早期发现浊度增加(图3)。此外,mgrA和femA两者中均有突变的突变体(SAm1-201)表现出与野生型菌珠相似的生长,且没有噬菌体抑制细菌生长,因此确认了该细菌获得了对噬菌体的耐药性(图4)。由此,准确地确认了对于突变体,噬菌体(ΦSA012)的细菌生长抑制效果没有引发,特别是,揭示了mgrA和femA两者中均有突变的突变株获得了,更高的MRSA的噬菌体耐药性。
(实施例2)
获得噬菌体耐药性的MRSA预计会通过权衡改变的药物敏感性。因此,对伴随噬菌体耐药性的获得的药物敏感性变化进行了检查。
通过药物敏感性测试(圆片法)测量野生型菌珠(MRSA2007-13菌珠)和突变体(SAm1-201)对β-内酰胺类抗菌药物的药物敏感性。将对测试方法进行解释。将野生型菌珠(MRSA2007-13菌珠)或突变体(SAm1-201)在LB-琼脂上划线,并在37℃下培养过夜。第二天,收集单菌落,添加至5mL胰蛋白酶大豆肉汤中,并在37℃下振荡培养过夜。培养后,将菌液调至McFarland浊度0.5号,用无菌棉签均匀涂抹在Mueller-Hinton II琼脂培养基上。将敏感性圆片置于琼脂培养基上后,在37℃下培养24小时,并测量每个圆片周围形成的抑制区的直径,单位为毫米。

接下来,在药物敏感性测试(MIC法)中测量了野生型菌珠(MRSA2007-13菌珠)和mgrA和femA两者中均有突变的突变菌珠(SAm1-201、SAm1-206、SAm1-210)对抗菌药物的药物敏感性。将对测试方法进行解释。将预培养过夜的菌液(野生型菌珠或突变体)用无菌生理盐水稀释至达到OD600≈0.26,将25μL菌液添加到12mL的Mueller-Hinton液体培养基(Eiken Chemical Co.,Ltd.)中。向干板(Eiken Chemical Co.,Ltd.)的各孔中添加100μL菌液,培养20小时。培养后,根据干板测定标准测定MIC。

表3显示了结果。在MIC方法中,药物所需量越少(MIC值越低),则认为药物敏感性越高。具有mgrA突变和femA突变两者的突变体表现出的对抗菌药物的药物敏感性是野生型菌珠(表3中的“WT”)的4至1000倍或更高。特别是,如CEZ(头孢唑啉)的“WT:>128”与“SAm1-201:0.125”的数据所证明,发现突变菌珠对抗菌药物的药物敏感性显著高于野生型菌珠的药物敏感性。虽然之前有报道称甲氧西林敏感金黄色葡萄球菌中的femA突变增加了其对苯唑西林的敏感性(Proc Natl Acad Sci U S A.2021Mar 9;118(10):e2008007118.doi:10.1073/pnas.2008007118.),但本研究结果首次报道了MRSA对抗菌药物的敏感性显著增强。

因此,证明了通过利用噬菌体(ΦSA012)使MRSA获得噬菌体耐药性,来增加MRSA对包括β-内酰胺类药物在内的抗菌药物的药物敏感性。
(实施例3)
现在已经揭示了噬菌体耐药性的获得会导致对抗菌药物的敏感性的改变;接下来,从测试开始就将噬菌体和抗菌药物(苯唑西林)两者都添加到MRSA的野生型菌珠中,以检查生长抑制效果。
测试方法如下所示。该测试通过比浊法进行。将预培养过夜的菌液(野生型菌珠(MRSA2007-13菌珠))用LB-肉汤稀释至达到OD600≈1.0,进一步稀释10倍(≈1.0×108cfu/mL)。各组如下建立。
-对照:细菌90μL+SM缓冲液10μL
-phi:细菌90μL+ΦSA012 10μL(最终浓度≈1.0×107pfu/mL(MOI≈0.1))
-Abx:细菌99μL+苯唑西林钠1mL(最终浓度:16μg/mL)
-phi+Abx:细菌89μL+噬菌体10μL(最终浓度≈1.0×107pfu/m(MOI≈0.1))+苯唑西林钠1μL(最终浓度:16μg/mL)
将板设置在Sunrise Rainbow Thermo RC(Tecan Japan Co.,Ltd.)中,在37℃下振荡培养90小时,同时每15分钟测量590nm波长处的吸光度值。
图5显示了结果。单独使用抗菌药物(Abx:灰色实线)没有抑制细菌生长。单独的噬菌体(ΦSA012)(phi:灰色虚线)在一定程度上成功地抑制了细菌的生长,但在培养开始40小时后发现细菌再次生长。对于添加噬菌体(ΦSA012)和苯唑西林的组(phi+Abx:黑色虚线),在开始培养后90小时内细菌生长被成功抑制,并且在90小时期间没有观察到细菌再次生长。
这些结果证明,由噬菌体(ΦSA012)和抗菌药物形成的混合物增加了MRSA对抗菌药物的敏感性,成功地确保了对MRSA生长的抑制。
(实施例4)
考虑到噬菌体耐药性(ΦSA012耐药性)MRSA菌珠在mgrA中含有100%突变,通过RNA-seq分析检查了mgrA突变引起的差异基因表达。
RNA提取和RNA-seq分析的方法如下所示。将野生型菌珠(MRSA2007-13菌珠)和噬菌体诱导的突变体(SAm1-201)在LB-肉汤中培养直至达到OD600≈1.0。将1mL各菌液离心形成沉淀物后,将沉淀物悬浮于1mL TRIzol试剂(Thermo Fisher Scientific)中,向其中添加500μL氧化锆珠,利用粉碎机破碎细菌细胞。粉碎后,离心收集上清液,用氯仿和乙醇提取RNA级分。文库制备和RNA-seq分析外包给Rhelixa,Inc。文库使用Ribo-Zero Plus rRNA消耗试剂盒和适用于Illumina的NEBNext Ultra Directional RNA文库制备试剂盒来产生,并使用Illumina NovaSeq 6000以150bp×2进行测序分析。使用Trimmomatic(版本0.39)去除接头序列和低质量读段,然后使用HISAT2(版本2.1.0)进行参考序列映射。使用featureCounts(版本2.0.1)对每个基因进行读段计数,然后使用TCC-GUI进行TMM标准化,并检测差异表达的基因。
先前的报告称mgrA充当转录调节因子来调节约350个基因,包括涉及毒力的基因和涉及药物外排泵的基因。因此,对因获得噬菌体耐药性而导致的mgrA突变所引起的差异基因表达进行了检查。
图6显示了结果。结果显示,ΦSA012耐药性MRSA(SAm1-201)中涉及毒力的15个基因的表达水平低至野生型菌珠(MRSA2007-13菌珠)中表达水平的约1/2至1/10。下面将描述这些基因。本结果表明,获得噬菌体耐药性的MRSA的毒力降低。也就是说,证明了通过利用噬菌体(ΦSA012)使MRSA获得噬菌体耐药性,降低了MRSA的毒力。
<EsaA、EsaB、EsaC、EssA、EssB、EssC、EsxA、EsxB>
T7SS由四种膜蛋白(EsaA、EssA、EssB、EssC)、两种细胞内蛋白(EsaB、EsaG)、五种分泌底物(EsxA、EsxB、EsxC、EsxD、EsaD)和与分泌底物相互作用的EsaE构成。T7SS在确保金黄色葡萄球菌的毒力方面发挥着重要作用。报道了T7SS的全部或特定因子(EsxA、EssB、EssC、EsxC、EsxB、EsaB、EsaD、EsaE)缺失会导致小鼠感染模型中毒力降低。
<RNA III>
细菌感知同一物种的种群密度并响应于种群密度而控制物质产生的机制称为群体感应(quorum sensing)。金黄色葡萄球菌的一些毒力因子通过被称为附属基因调节因子(Agr)调节系统的群体感应进行调节。通过群体感应激活Agr系统引起RNA III的转录,并且该RNA III直接或间接调节毒力因子的转录和表达。
<LukGH亚基H,LukGH亚基G>
LukGH,也称为LukAB,是杀白细胞素之一。杀白细胞素是在白细胞(如中性粒细胞)中形成孔洞以破坏白细胞的毒素成分。
<耐热核酸酶>
耐热核酸酶(nuc)是细胞外分泌的,能够降解存在于细胞外的DNA。特别是,已知中性粒细胞破坏自身以对抗金黄色葡萄球菌,并通过释放其自身的DNA来诱捕细菌细胞(NET:中性粒细胞胞外诱捕器)。耐热核酸酶作用于NET,帮助其逃离诱捕器。
<葡萄球菌超抗原样蛋白11(SSL11)>
SSL是一种蛋白质,其结构与金黄色葡萄球菌的超抗原相似。SSL11通过阻止中性粒细胞被激活并滚动到血管上来抑制免疫力。此外,SSL11已知抑制细胞粘附和运动。
<葡萄球菌激酶>
已知葡萄球菌激酶(sak)与纤溶酶原和由中性粒细胞分泌的α-防御素结合。与sak结合后,纤溶酶原变成纤溶酶,被激活。纤溶酶作为广谱蛋白酶发挥作用,有助于金黄色葡萄球菌侵入周围组织。抗菌肽α-防御素在与sak结合时作用失活,使金黄色葡萄球菌存活。已知sak的基因表达受到作为群体感应的Agr系统的正向调节。
<O型葡萄球菌肠毒素>
SEO是由金黄色葡萄球菌分泌的肠毒素之一,已知接种后会引起呕吐。此外,已知SEO可通过炎症小体途径诱导中性粒细胞分泌IL-1β。
通过与上述相同的RNA-seq分析来检查作为与药物外排泵相关的基因的NorB和Tet的差异表达。NorB和Tet是涉及药物外排泵的基因,也是多重耐药性的致病基因。这些基因表达的减少会抑制细菌中抗菌药物的外排,从而增加对抗菌药物的敏感性。
图7显示了结果。结果表明,ΦSA012耐药性MRSA(SAm1-201)中涉及药物外排泵的NorB和Tet的表达水平低至野生型菌珠(MRSA2007-13WT)中的1/5或更低。因此,涉及药物外排泵的基因表达的减少预期是通过噬菌体(ΦSA012)增加MRSA对抗菌药物的药物敏感性的作用机制之一。特别提出的是NorB表达的减少可能增加对新的喹诺酮类抗菌药物(如ERFX)的敏感性,而Tet表达的减少可能增加对四环素类抗菌药物(如OTC)的敏感性。简而言之,证明了通过利用噬菌体(ΦSA012)使MRSA获得噬菌体耐药性,增加了MRSA对抗菌药物的药物敏感性。
(实施例5)
对MRSA2007-13 SAm1-201菌珠(其是具有mgrA突变的噬菌体耐药性细菌)进行RNA-seq分析的结果发现,各种毒力基因的表达水平降低(实施例4),因此,通过使用MRSA感染模型小鼠来评估噬菌体耐药性细菌的毒力。
(用小鼠腹膜炎模型评价毒力)
将预培养过夜的野生型菌珠(MRSA2007-13菌珠)和噬菌体诱导的突变体(噬菌体耐药性细菌MRSA2007-13 SAm1-201)的菌液分别添加到新鲜的LB肉汤中,并在37℃下进行振荡培养直至达到OD600≈1.0。将菌液离心(8,000×g,4℃,5分钟),并将沉淀物各自悬浮在PBS中以达到1.0×1010CFU/mL。对8周龄雌性BALB/c小鼠(Sankyo Labo ServiceCorporation,INC.,Tokyo,Japan)腹腔内施用菌液各100μL(1.0×109CFU/头),施用后48小时内每12小时检查一次它们的存活率。本动物实验是在酪农学园大学动物实验委员会的批准下进行的(批准号:VH21A13)。对每个由八只小鼠(n=8)组成的组进行对数秩检验,其中p<0.05被认为是差异显著。
(用小鼠皮肤脓肿模型评价毒力)
将预培养过夜的野生型菌珠(MRSA2007-13菌珠)和噬菌体诱导的突变体(噬菌体耐药性细菌MRSA2007-13 SAm1-201)的菌液分别添加到新鲜的LB肉汤中,并在37℃下进行振荡培养直至达到OD600≈1.0。将菌液离心(8,000×g,4℃,5分钟),并将沉淀物各自悬浮在PBS中以达到2.0×109CFU/mL。向8周龄雌性BALB/c小鼠(Sankyo Labo ServiceCorporation,INC.,东京,日本)腹腔内施用三麻醉混合物(0.75mg/kg美托咪定、4mg/kg咪达唑仑、5mg/kg布托啡诺),麻醉下用理发器对每侧从胁腹到股骨的部分剃毛,并用70%酒精消毒皮肤表面。然后,将菌液各50μL分别皮下施用在每侧从胁腹到股骨周围的部位(1.0×108CFU/部位),然后用0.75mg/kg阿替美唑使小鼠苏醒。施用后,每天一次测量脓肿大小(长轴(mm)×短轴(mm)=mm2)。施用后第7天,通过麻醉处理将小鼠安乐死,测量脓肿大小并对病变部位拍照。为了测定病变部位的细菌计数,收集每个病变部位并精细切成片,向其中添加200μL PBS和Φ5mm氧化锆球,将组织粉碎以产生裂解物,并通过平板稀释测定细菌计数。本动物实验是在酪农学园大学动物实验委员会的批准下进行的(批准号:VH21A13)。
图8(a)显示用小鼠腹膜炎模型评价毒力的结果。腹腔内施用野生型菌株(MRSA2007-13菌株)的小鼠均在24小时内死亡(图8(a):黑线)。另一方面,施用噬菌体诱导的突变体(SAm1-201)的小鼠的48小时存活率为50%(图8(a):灰线)。这些结果证明,获得噬菌体耐药性的MRSA毒力较低,小鼠的存活率显著更高。
图8(b)至图8(e)显示了用小鼠皮肤脓肿模型评价毒力的结果。在施用野生型菌珠(MRSA2007-13菌珠)的三只小鼠中,一只在施用后2天死于败血症样症状,另一只在整个观察期间出现左右病变部位连合;因此,对这两只小时无法测量脓肿大小。在施用野生型菌珠(MRSA2007-13菌珠)的小鼠中发现了广泛的脓肿(图8(b)),相比之下,施用噬菌体诱导的突变体(SAm1-201)的小鼠中脓肿的范围是有限的并且脓肿的尺寸显著更小(图8(c))。施用后7天脓肿大小的变化证明,施用噬菌体诱导的突变体(SAm1-201)的小鼠(图8(d):虚线)的脓肿大小表现出比施用野生型菌珠(MRSA2007-13)的小鼠(图8(d):实线)所表现出的显著更小(图8(d))。施用7天后,切除病变部位的组织,测定组织中的细菌计数发现,施用噬菌体诱导的突变体(SAm1-201)的小鼠中的细菌计数显著小于施用野生型菌珠(MRSA2007-13菌珠)的小鼠中的细菌计数,前者检测到的细菌数量约为后者的1/50(图8(e))。
因此,在小鼠腹膜炎模型和小鼠皮肤脓肿模型中,揭示了噬菌体诱导的突变体(SAml-201)对小鼠的毒力低于野生型菌珠(MRSA2007-13菌珠)。简而言之,证明了通过利用噬菌体(ΦSA012)使MRSA获得噬菌体耐药性,降低了MRSA的毒力。

Claims (8)
1.一种用于增加甲氧西林耐药性金黄色葡萄球菌(MRSA)对抗菌药物的药物敏感性的组合物,所述组合物包含:至少一种噬菌体,所述噬菌体选自通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体。
2.根据权利要求1所述的组合物,其中,通过使MRSA获得对所述噬菌体的耐药性来增加所述药物敏感性。
3.根据权利要求1或2所述的组合物,其中,所述抗菌药物为β-内酰胺类抗菌药物。
4.一种用于治疗或预防MRSA感染的组合物,所述组合物包含:至少一种噬菌体,所述噬菌体选自通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体;和抗菌药物。
5.根据权利要求4所述的组合物,其中,使MRSA获得对所述噬菌体的耐药性。
6.根据权利要求4或5所述的组合物,其中,所述抗菌药物为β-内酰胺类抗菌药物。
7.一种用于降低MRSA的毒力的组合物,所述组合物包含:至少一种噬菌体,所述噬菌体选自通过保藏编号NITE BP-693鉴别的噬菌体和通过保藏编号NITE BP-694鉴别的噬菌体。
8.根据权利要求7所述的组合物,其中,通过使MRSA获得对所述噬菌体的耐药性来降低所述毒力。
