本发明属于基因检测技术领域,公开了一种NF1基因检测试剂及检测方法。所述NF1基因检测试剂包括六对引物组。本发明采用长片段扩增技术,使用6对引物组对NF1基因的mRNA进行扩增,PCR产物包含NF1全部编码区(约9K),产物间彼此包含300‑500bp左右的重叠区域,对全部PCR产物混合进行NGS测序。其优点,一是工作量降低;二是引物数量少,从而减少引物区域变异造成的扩增失败或单染色体扩增的假阴性。三是,PCR产物相对较长,可以检测到较长的剪切变异,同时避免高度同源序列的干扰。四是使用NGS检测的测序深度高,对于高于5%的SNV/Indel嵌合,以及剪切变异,有更高的检测敏感性。

技术领域
本发明属于基因检测技术领域,具体涉及一种NF1基因检测试剂及检测方法。
背景技术
神经纤维蛋白(Homo sapiens neurofibromin 1-NF1)的功能异常会导致一种常染色体显性疾病—1型神经纤维瘤病(NF1;OMIM 613113),发病率高达1:2500~3000,不受种族的影响(Huson等人,1989;Gutmann等人,1997;Lammert等人,2005;Evans等人,2010)。NF1特点是多发性咖啡斑(cafe-au-lai macule,CALM)和皮肤神经纤维瘤(Ferner等人,2007)。患者符合以下两个或两个以上特征,则可在临床上诊断为NF1(修订后的诊断标准,2021):六个或六个以上的牛奶咖啡斑(青春期前大直径>5 mm或成人>15 mm);两个或多个任何类型的神经纤维瘤或一个丛状神经纤维瘤;腋窝或腹股沟区域有雀斑;视神经胶质瘤;两个或多个虹膜-利希结节(虹膜错构瘤);骨损伤(蝶骨发育不良或长骨皮质变薄,伴有或不伴有假血栓形成);一级亲属中有确诊为NF1;以及NF1基因在白细胞中具有50%的变异。
人的NF1基因位于17q11.2中。它在基因组上跨越约300kb(GCA_000001405.29)长度,包含57个组成外显子和3个可变剪切体,编码11至13kb的mRNA(Seq NM_000267)(Cawthon等人,1990;Viskochil等人,1990年;Wallace等人,1990,Viskochill等人,1998年)。无论是在基础科学还是在临床诊断中,NF1的基因检测都意义重大。但是,在整个人类基因组中,NF1存在有多个同源伪基因样序列(Purandare等人,1995年;Luijten等人,2000年;Yu等人,2005年;Messiaen等人,2008年),同时,NF1基因DNA序列长,突变类型多,有相当部分的异常选择性剪接并且未发现变异热点,这些特点使NF1基因变异的检测面临挑战(Griffiths等人,2007年;Messiaen等人,2008年)。因此,NF1的基因检测需要能区分同源序列并尽可能覆盖多种变异。
到目前为止,临床已经应用多种检测技术进行NF1的变异检测。变性高效液相色谱(dHPLC)和gDNA/cDNA sanger法测序用于鉴定点变异;荧光原位杂交(FISH)、定量聚合酶链反应(qPCR)和多重连接依赖性探针扩增(MLPA)用于检测基因内以及整个基因拷贝数变异。两种或多种技术的组合是目前NF1变异检测的策略(Griffiths等人,2006;Valero等人,2011;Ko等人,2013;Sabbagh等人,2013年;Maruoka等人,2014年;Zhu等人,2016年;Tspi等人,2018年;Milla等人,2018)。然而,传统的检测策略,复杂度高,周期长,敏感性差,在临床上,患者需要更简单快速且全面的NF1基因检测方案。
专利CN 116083560 A公开了一种用于着床前胚胎NF1基因检测的试剂盒,该试剂盒包括设计的n个引物对中的一个或更多个引物对,所述n为大于2的自然数,其中第2对引物的上游引物位于第1对引物的下游引物的上游或与之重叠,依此类推,第n对引物的上游引物位于第n-1对引物的下游引物的下游或与之重叠,即所述n个引物对扩增的序列涵盖了NF1基因所有外显子的完整CDS区,且n个引物对的每个引物对的上游引物和下游引物分别设计在所述目标基因的不同外显子区域,从而实现了从mRNA水平进行基因序列的扩增,避免假基因和内含子的干扰,覆盖全部的CDS区。但该专利采用的是Sanger一代测序技术,其引物对扩增的序列只涵盖NF1基因所有外显子的完整CDS区,只能检测外显子区域的点突变,无法检测因内含子变异导致的异常选择性剪接;同时需明确致病位点,再进行检测,无法对未明确致病位点的NF1患者进行临床诊断。
发明内容
针对以上现有技术存在的缺点和不足之处,本发明的首要目的在于提供一种NF1基因检测试剂。
本发明的另一目的在于提供一种NF1基因检测方法。
本发明的检测试剂及检测方法通过多对引物对患者的NF1 cDNA全长进行扩增,随后进行二代测序,并对测序数据进行点变异和选择性剪接分析,不仅可对未明确致病位点的NF1患者进行临床诊断,还可检测因内含子变异导致的异常选择性剪接,可以准确、快速地检测出NF1基因多种变异,具有灵敏、可行、快速和相对经济的优点。并可进一步使用外显子捕获技术对NF1 cDNA进行NGS测序二次验证,可提高检测准确性。

进一步地,所述检测方法还包括对步骤(1)的双链cDNA打断建库,对NF1外显子区域进行探针杂交捕获洗脱,二代测序验证是否存在NF1基因变异。
进一步地,所述NF1基因检测方法还包括生信分析与报告出具,所述生信分析与报告出具流程如下:
(1)使用NextGENe软件,对LR-PCR扩增系统的PCR测序数据以及Exome-RNA测序系统的外显子组测序数据进行数据转化、参考序列比对、SNV/Indel变异识别和输出等,根据测序深度、read balance、MAF(Mutant Allele Frequency)、是否为Exon区域变异等数值进行SNV/Indel的筛选,并比较PCR测序数据和外显子组测序数据在检测编码区SNV/Indel方面的异同;对识别出的SNV/Indel进行变异注释,根据人群频率、保守性分析、功能预测、公开数据(ClinVar、HGMD、PubMed等)、家系分析等参数,结合患者的临床症状,进行SNV/Indel的致病性评级;
(2)根据步骤(1)识别出的SNV/Indel数据,通过MAF值判断这些SNV/Indel是纯合状态还是杂合状态,如存在杂合位点,则判断NF1不存在单等位基因表达(Mono allelicexpression);如果存在罕见的纯合SNV/Indel或多个纯合SNV/Indel位点,则需考虑NF1基因可能存在单等位基因表达(Mono allelic expression,MAE);
(3)使用NextGENe软件,对LR-PCR扩增系统的PCR测序数据以及Exome-RNA测序系统的外显子组测序数据进行数据转化、参考序列比对、结构变异的识别和输出等,根据测序深度、read balance、MAF等数值进行结构变异的识别,并比较PCR测序数据和外显子组测序数据在检测结构变异的异同;对识别的结构变异与正常对照样本进行比较,过滤掉正常的选择性剪接位点,并对罕见的选择性剪接进行注释;
(4)根据步骤(2)和(3)的分析数据,结合患者的临床信息,出具NF1 RNAseq检测分析报告。报告的主要内容包括患者基本临床信息、检测方法、SNV/Indel检测结果、单等位基因检测结果和检测结论等,并对阳性变异进行致病性的评级。
与现有技术相比,本发明的有益效果是:
(1)国际上对于NF1 mRNA的检测,是对NF1 cDNA分子进行短片段扩增(400-500bp),后续进行Sanger法测序。这种方法的缺点一是工作量大,需要超过20对引物。二是由于引物数量多,会增加引物区域的变异导致的单染色体扩增而出现假阴性,降低了检测敏感性。三是由于产物片段短,而无法检测跨越产物的剪切变异。四是Sanger法测序,对于低于30%的SNV/Indel嵌合体以及剪切变异敏感性低。基于上述问题,本发明采用长片段扩增技术(LR-PCR),使用6对引物对NF1基因进行扩增,PCR产物包含NF1全部编码区(约9K),产物间彼此包含300-500bp左右的重叠区域,对全部PCR产物混合进行NGS测序。其优点,一是工作量降低;二是引物数量少,从而减少引物区域变异造成的扩增失败或单染色体扩增的假阴性。三是,PCR产物相对较长,可以检测到较长的剪切变异,同时避免高度同源序列的干扰。四是使用NGS检测的测序深度高,对于高于5%的SNV/Indel嵌合,以及剪切变异,有更高的检测敏感性。
(2)本发明通过多对引物对患者的NF1 cDNA全长进行扩增,随后进行二代测序,并对测序数据进行点变异和选择性剪接分析,不仅可对未明确致病位点的NF1患者进行临床诊断,还可检测因内含子变异导致的异常选择性剪接。
(3)本发明检测方法通过进一步采用Exome RNAseq(外显子区域捕获测序)对双链cDNA进行打断建库,随后进行临床外显子CDS区域的捕获测序和数据分析,是对LR-PCRRNAseq(RNA长片段PCR扩增测序)的一种补充。使用捕获测序技术,其优点,一是可以避免PCR引物区域变异导致的假阴性,二是可以检测NF1外显子全长区域发生的剪切变异。同时,Exome RNAseq的结果也是对LR-PCR RNAseq的结果的二次验证。若两者结果一致,则不需要再进行Sanger法验证。
(4)本发明所有检测技术均基于NGS测序平台,无需其它检测方法如HPLC,MLPA,以及Sanger法测序等,方便快捷且成本低;本发明通过对NF1基因长片段RNA分子的检测,可以排除同源基因的干扰,准确度高;本发明可检测出NF1基因多种变异类型,包括点变异,剪切变异以及单等位基因表达,敏感性高。

具体实施方式
下面结合实施例及附图对本发明作进一步详细的描述,但本发明的实施方式不限于此。
实施例1
本实施例为LR-PCR引物的构建,在NCBI上查找NF1 mRNA序列信息(NM_000267),截取编码区上下游300-500bp进行引物序列设计。
首先,设计了三种方案,方案一是只用一对引物,进行NF1编码区全长约9kb的扩增。方案二是用三对引物,每对引物的PCR产物约4kb,涵盖NF1编码区全长。方案三是使用6对引物,每对引物的PCR产物约1-2kb,涵盖NF1编码区全长。三种方案引物序列信息如下表1所示。

采用上述三种引物设计方案进行PCR扩增反应,相应PCR扩增反应体系及反应条件分别如下表2和表3所示。

其中,方案一的引物分别以60℃,64℃,68℃为退火温度进行扩增;方案二的引物以64℃为退火温度进行扩增;方案三的引物以61℃为退火温度进行扩增。
方案一、方案二和方案三的引物设计方案进行PCR扩增反应的扩增结果分别如图1、图2和图3所示。实验结果表明,方案一与方案二的扩增效果较差(图1与图2),方案三的扩增效果较好,扩增效率高,特异性好(图3),可进入下一步PCR体系与反应条件的优化。
通过对不同引物浓度、双链cDNA模板不同用量的研究,确立了最佳反应体系;通过对不同退火温度、时间、循环数的研究,确立了最佳反应程序。最优选反应体系及反应程序见表2表3。
实施例2
本实施例采用实施例1确定的检测试剂及检测条件对NF1基因患者进行检测:
(1)采血
医护人员用无破损,无泄露的RNA采血管采集送检者外周血(每管2-3ml)。采集完外周血后,立即轻轻颠倒混匀采血管8-10次。采血管避免剧烈振荡,以防血液样品溶血和有形成分破坏。将RNA采血管立即送检(采样及运输总时长不超过24h)。运输条件:2-8℃运输(外包装用气泡膜缓冲,防止运输碰撞破裂)。
(2)RNA提取
纯化保存于血液RNA稳定剂的血液样品时,请先将样品室温放置或37℃水浴使其升至室温。然后6600rpm(~4000×g)离心10min,用移液器吸弃上清液,取沉淀进行以下操作。加入细胞裂解液,55℃孵育10min,其间颠倒混匀2-3次,溶液应变清亮。经过离心柱进行RNA的纯化与洗脱,洗脱缓冲液体积不应少于30μl,RNA溶液于-70℃保存(参考RNApreppure 血液总RNA提取试剂盒说明书,DP433,天根生化科技(北京) 有限公司)。
(3)RNA反转录
将提取的RNA用随机引物进行第一链及第二链的合成,得到反应产物双链cDNA,经质检合格后放于-20℃保存(具体过程参考dscDNA Synthesis Module,NR21-CS7,南京诺唯赞生物科技股份有限公司)。
(4)LR-PCR RNAseq检测
引物序列采用实施例1的方案三,反应体系及反应条件分别如表2和表3,对反转录获取的双链cDNA进行LR-PCR RNAseq检测。
(5)Exome RNAseq检测
对质检合格的双链cDNA进行建库。对构建好的适用于MGI高通量测序的DNA文库,首先使用探针(MGIEasy外显子组捕获V4探针试剂套装(16 RXN))进行杂交,探针为标记有生物素的单链DNA,用于捕获与其互补的靶序列;其后,待杂交孵育完成后,加入链霉素标记的磁珠,利用链霉素与生物素之间强大的非共价结合作用,将靶序列捕获出来;最后通过洗脱反应去除非靶区域序列及其它杂质,得到含靶区域捕获产物的终文库。对终文库进行上机二代测序。
以下为具体临床应用结果:
1、患者周XX(5岁,女性)临床诊断为:1)肺部感染;2)蛋白质-能量营养不良:患者有反复“腹泻”史; 3)支原体感染;4)神经纤维瘤病:患儿生长发育落后,皮肤可见散在牛奶咖啡斑,直径5mm以上大于6个;头部核磁共振结果:第四脑室周围实质可见多发小斑片T1WI等信号,T2WI稍高信号影,FLAIR呈高信号,提示神经纤维瘤病I型可能,右侧中耳乳突炎,腺样体肥大。其家族成员中未存在I型神经纤维瘤病患者。
患者LR-PCR RNAseq和Exome RNAseq的检测结果分别如图4和图5所示。
患者Exome RNAseq和LR-PCR RNAseq的检测结果均显示,患者在NF1基因的chr17:29562711-29562712区域存在杂合性缺失AA,该变异c.3791_3792delAA(p.E1264Gfs*9)会导致Exon 28(NM_000257.3)出现移码变异,并导致NF1的提前终止,从而影响NF1蛋白的正常功能。
根据美国医学遗传学与基因组学学会(The American College of MedicalGenetics and Genomics, ACMG)制定的变异解读指南,对该变异进行变异分类可知患者该变异属于:
变异分类:1类-致病突变
对患者进行其它点突变、结构变异、选择性剪接分析和单等位基因表达分析,均未发现其他罕见变异。
检测结论:
患者Exome RNAseq和LR-PCR RNAseq的检测数据均发现患者NF1基因chr17:29562711-29562712区域存在杂合性缺失AA,且LR-PCR RNAseq数据可排除同源干扰,可明确变异的发生。
2、患者陈XX(9岁,男性)临床诊断:1)发现身材矮小8余年,查体发现全身皮肤可见大小不等的牛奶咖啡斑;2)双侧基底节区、丘脑、齿状核、海马及左侧颞下窝异常信号;3)骶前异常结节灶。其家族成员中父母均体健,无家族病史。
患者LR-PCR RNAseq和Exome RNAseq的检测数据分析分别如图6和图7所示。结果显示,在Exon31上有9%的序列存在整个外显子的缺失(测序深度:90X)。同时,患者的LR-PCRRNAseq数据也显示,其在Exon31上有9%的序列存在整个外显子的缺失(测序深度:32000X)。对该变异区域使用sashimi plot进行可视化,结果如图8所示。发现Exome RNAseq和LR-PCRRNAseq数据均显示,相比于对照样本,患者部分转录本的Exon30/31会跳过Exon32,直接连接到Exon33上。
使用hg19做参考序列,分析Exome RNAseq数据的内含子序列,结果如图9所示,发现患者在chr17:29585520位置上存在 G>C的错义突变,该碱基为Exon 32的最后一个碱基,其突变可能影响该exon的正常剪接,导致了该exon剪接的跳跃。
检测结论:
患者的NF1基因在chr17:29585520位点存在杂合性变异c.4269G>C,该变异可能会导致Exon 32出现外显子跳跃,从而使患者出现神经纤维瘤相关的疾病症状。
上述实施例为本发明较佳的实施方式,但本发明的实施方式并不受上述实施例的限制,其它的任何未背离本发明的精神实质与原理下所作的改变、修饰、替代、组合、简化,均应为等效的置换方式,都包含在本发明的保护范围之内。

1.一种用于二代高通量测序中采用长片段PCR引物组扩增NF1基因的检测试剂,其特征在于,包括如下六对引物对:

4.根据权利要求2所述的检测试剂,其特征在于,所述DNA聚合酶为Prime STAR GXLDNA polymerase。
5.根据权利要求1所述的检测试剂,其特征在于,所述检测试剂具体成分组成如下:
5× Prime STAR GXL buffer5μL;
2.5μM dNTP 2 μL;
10µM 混合引物组 0.875μL;
dscDNA 1μL;
Prime STAR GXL DNA polymerase 1μL;
ddH2O 15.125μL。
