人类乳头瘤病毒16型(HPV16)是宫颈癌主要的致癌因素。现有的诊疗技术,包括HPV病毒检测、病理活检、早期筛查,以及治疗手段如手术、放疗、化疗等,存在显著的局限性。对于复发或晚期宫颈癌,现有手段效果有限,治疗过程可能带来显著的身体负担,并可能对患者的生活质量和生育能力造成严重影响。虽然有HPV感染的防护疫苗及早期筛查手段如HPV DNA检测、液基薄层细胞学检查和Pap smear涂片检查,但它们无法对所有类型的HPV产生保护作用,且不能完全防止疾病的发生,尤其在已经感染HPV的女性中。这些筛查手段在敏感性和特异性上存在局限,无法在早期准确识别高危患者或准确评估疾病的进展,容易导致过度诊断或漏诊,增加了患者的压力和治疗成本。
宫颈癌的临床治疗现状仍然堪忧。一方面,传统的治疗手段,如手术治疗、放射治疗和化学药物治疗,无法完全控制和消除宫颈癌的发生和转移。另一方面,针对宫颈癌的靶向药物研发仍处于初级阶段,市场上的靶向药物稀缺。现有的靶点研究主要集中在表皮生长因子受体(EGFR)、血管内皮生长因子(VEGF)、mTOR、致癌蛋白E6/E7以及免疫检查点PD-1/PD-L1、CTLA-4等。此外,一些科研人员在宫颈癌组织中发现了多种基因的异常表达,这些基因可能驱动肿瘤的发生和发展,对这些异常表达的基因进行深入研究,有望找到抗肿瘤治疗的有效靶点。寻找一种具有持久疗效,靶点多样,同时价格相对低廉的新型药物成为了当前的重要研究方向。宫颈病变相关研究应更深入探索其他可能的治疗靶点,并优化当前的治疗策略,以提供更有效和更可承受的治疗选项。
现有的技术主要侧重于病毒本身的检测和处理,忽视了病毒感染后宿主细胞的变化,如自噬这一细胞现象。这些细胞内部的变化和自噬现象,对病毒的感染和疾病的发展有重要影响,但尚未得到深入的研究。囊泡相关膜蛋白8(VAMP8),又称endobrevin,是SNARE(可溶性NSF附着蛋白受体)家族的成员。该蛋白家族参与囊泡运输,促进运输囊泡与其靶膜的融合。SNARE蛋白,包括VAMP8,以SNARE基序为特征,该基序是蛋白质-蛋白质相互作用所必需的60-70个氨基酸长区域。VAMP8位于2号染色体上,长度约为10千碱基,由8个外显子组成。该基因编码的蛋白是一种ⅳ型膜蛋白,已知主要参与转运囊泡到其靶区室的融合。

通过深入理解VAMP8在HPV16感染相关宫颈疾病中的作用,找到一种新的、更精确的宫颈癌预防、诊断和治疗方法,以提高患者的生存质量和预后。
1、VAMP8在HPV16感染及宫颈疾病中的双重作用:明确揭示了囊泡相关膜蛋白8(VAMP8)在HPV16感染及宫颈疾病进程中的核心作用。在HPV16感染早期,VAMP8的高表达通过促进细胞自噬抑制细胞增殖、迁移和侵袭,维持细胞稳态,阻止疾病的进展;而在宫颈癌阶段,VAMP8高表达能促进肿瘤细胞的增殖、迁移和侵袭。这表明了VAMP8在宫颈疾病发展中的双重角色。
2、基于VAMP8的宫颈疾病早期诊断与个性化治疗:提出了新的诊断和治疗策略。通过检测VAMP8的表达,能更精确地评估和预测HPV16相关宫颈疾病的发展,实现早期诊断和个性化治疗。针对VAMP8的调控,旨在控制HPV16感染初期的细胞增殖、迁移和侵袭,同时抑制癌症阶段的细胞增殖、迁移和侵袭,为患者提供更精准和个性化的治疗手段,降低副作用和复发率。
3、VAMP8作为宫颈癌的预后标志物:根据TCGA数据库的生物信息学分析,发现高VAMP8表达与宫颈癌的预后不良和免疫浸润程度密切相关,因此,VAMP8可以作为宫颈癌的预后标志物,帮助医生评估患者的治疗反应和预后。
4、深化对HPV16宫颈疾病机制的理解与治疗优化:研究结果有助于更深入理解HPV16感染引发宫颈疾病的分子机制,为疾病预防和治疗提供了新的理论依据。相较于现有技术,本发明具有更高的敏感性和特异性,对提高HPV16感染相关宫颈疾病的筛查和治疗效果有着重要作用。
总结来说,本发明通过研究VAMP8在HPV16感染和宫颈疾病进程中的表达和功能,为HPV16相关宫颈疾病的预防和治疗提供了新的潜在标志物和治疗靶点,具有重要的临床应用价值。相较于现有技术,本发明能够更为精准地针对HPV16感染和诱发的宫颈疾病进行治疗,减少副作用,提高治疗效果。

具体实施方式
下面结合实施例和附图对本发明做更进一步地解释。下列实施例仅用于说明本发明,但并不用来限定本发明的实施范围。
实施例1
囊泡相关膜蛋白8(VAMP8)是一个涉及到胞内囊泡运输和融合过程的膜结合蛋白,对免疫应答、神经递质释放、病原体入侵等生理和病理过程起到关键作用。VAMP8与宫颈疾病之间的潜在关联尚不像其基本功能和相互作用那样得到充分研究。本研究主要关注了VAMP8在HPV16感染相关宫颈疾病中的影响。
1.VAMP8作为诊断标志物
1.1.样本收集
①从医院收集HPV16阳性和阴性的宫颈组织样本。在同意书的基础上,对所有参与者进行隐私保护。
②分别将阳性和阴性样本分为两组,并确保每组样本数量相等,以保证统计学有效性。
③对样本进行临床分类:正常、低级别宫颈鳞状上皮内病变(LSIL)、高级别宫颈鳞状上皮内病变(HSIL)和宫颈癌。
1.2.蛋白质组学分析
①样本收集与处理:对收集到的组织样本进行机械碎化和化学破碎,释放组织中的蛋白质。
机械碎化可采用以下方式之一:(1)摩擦器/组织研磨器:通过高速旋转的刀片来研磨组织,将其物理碎化为粉末或液态。(2)超声波:通过超声波的高频震动来破坏细胞结构,使蛋白质释放出来。常用的设备是超声波细胞破碎机。(3)震荡球研磨:利用钢珠的高速震荡来破坏细胞壁和细胞膜,常用于植物和固态组织的蛋白质提取。(4)冷冻研磨:通过将组织在液氮中迅速冷冻,然后进行物理研磨,使细胞破裂并释放蛋白质。
化学破碎可采用以下方式之一:(1)利用洗涤剂:例如Triton X-100、NP-40等,这些洗涤剂可以破坏细胞膜和细胞器膜,使蛋白质释放出来。(2)使用溶解剂:例如尿素、硫酸铵等,它们可以破坏蛋白质的结构并使其溶解。(3)用酸或碱处理:通过改变pH值来破坏细胞结构并释放蛋白质。
为确保获得高质量的蛋白质样本,机械碎化和化学破碎的过程往往需要在冷冻或低温条件下进行,以防止蛋白质降解或失活。而在机械碎化过程中,还常常需要加入蛋白酶抑制剂和其他化学试剂,以确保蛋白质的稳定性和完整性。
②蛋白质提取与纯化:使用适当的蛋白质提取缓冲液提取组织样本中的蛋白质。使用蛋白质纯化套件对提取的蛋白质进行纯化。
蛋白质提取缓冲液是一种专用于溶解并提取组织或细胞中蛋白质的特定配方液体。这种液体通常包含某些离子,如Tris、EDTA和NaCl,以及可以破坏蛋白质-蛋白质或蛋白质-脂质之间相互作用的表面活性剂如Triton X-100或SDS。此外,为了保护蛋白质不被降解,提取缓冲液中通常还包含蛋白酶抑制剂。
蛋白提取过程:a.将组织样本放入均质器,加入适量的蛋白质提取缓冲液;b.用均质器将组织物理打碎,使蛋白质释放到缓冲液中;c.将均质物离心,将上层清液转移至新的离心管;d.清液中即包含了所提取的蛋白质。
蛋白纯化过程:a.结合:蛋白质溶液首先与特定的亲和柱进行接触,使目标蛋白质结合到柱材上;b.洗涤:使用缓冲液冲洗柱子,以去除非特异性结合的蛋白质和杂质;c.洗脱:使用特定的洗脱缓冲液从柱材上洗脱目标蛋白质;d.浓缩:如果需要,可以使用超滤等方法对洗脱得到的蛋白质溶液进行浓缩;e.进一步纯化:如凝胶渗透层析、离子交换层析等以进一步提高蛋白质的纯度。
③蛋白质质谱分析:将纯化的蛋白质通过液相色谱与质谱LC-MS/MS联用技术进行分析。根据质谱图鉴定VAMP8蛋白,并量化其丰度。
a.液相色谱LC-MS条件:
柱:C18反相柱(例如,2.1mmx150mm,1.9μm颗粒大小)。
溶剂A:0.1%甲酸水溶液。
溶剂B:0.1%甲酸乙腈溶液。
梯度:从5%B到40%B,40分钟;从40%B到80%B,10分钟。
流速:0.3mL/分钟。
注射体积:5μL。
b.质谱MS条件:
质谱仪:Q-Exactive HF,或同类仪器。
电喷雾源:阳离子模式。
范围:m/z375-1600。
分辨率:30000。
自动增益控制(AGC):3e6。
最大喷雾时间:50ms。
二级质谱碎片方式:高碰撞能散裂(HCD)。
c.数据分析:
使用软件,如MaxQuant、Proteome Discoverer或Mascot,分析原始数据,鉴定和定量肽段和蛋白质。
对于VAMP8蛋白的鉴定,应当有至少两个独特的、高置信度的肽段匹配。
量化采用标准化的肽段内标策略或标签自由定量方法如MaxLFQ进行。首先,选择特定的代表性VAMP8肽段,并通过峰面积进行定量。利用已知浓度的内标或相对于其他参照蛋白质的峰面积进行相对或绝对量化。
1.3.定量PCR(qPCR)分析
①提取RNA:利用RNA提取试剂盒(TRIzol,TaKaRa公司,RT reagent Kit)从样本中提取总RNA。
②逆转录:利用反转录试剂盒(诺唯赞,ChamQ SYBR qPCR Master Mix)将RNA转化为cDNA。
③定量PCR:设计针对VAMP8基因的特异性引物(VAMP8:正向5'-tgtgcggaacctgcaaagt-3';反向5'-cttctgcgatgtcgtcttgaa-3'),利用SYBR Green或Taqman探针进行qPCR反应。同时,选取一种或多种内参基因进行标准化(ACTB/β-actin:正向5'-catgtacgttgctatccaggc-3';反向5'-ctccttaatgtcacgcacgat-3')。
④数据分析:利用qPCR仪器的数据分析软件,比如2^-ΔΔCT法,进行数据分析。
1.4.数据分析和结果解释
①对蛋白质组学、qPCR的结果进行统计分析,确定VAMP8在不同类别样本中的表达差异。
②结合临床数据,分析VAMP8表达水平与宫颈疾病严重程度的关联。
③讨论VAMP8是否可作为诊断HPV16感染以及评估宫颈疾病严重程度的潜在标志物。
2.VAMP8作为预后标志物
2.1.数据获取和预处理
①使用癌症基因组图谱(TCGA)数据库中宫颈癌的基因表达数据集,包括临床信息数据和基因表达量数据。
临床信息数据:如病人的年龄、性别、诊断日期、生存时间、临床分期、病理类型、病理分期、治疗方式等。
基因表达量数据:显示每个基因在样本中的表达量或活性。
②对这些数据进行质量控制和预处理,包括数据清洗、格式化、标准化、批次效应修正和数据整合,以提高后续分析的准确性。
数据清洗:首先,需要去除不完整或明显错误的记录。例如,可能会排除具有大量缺失值的样本或基因。同时,对于那些与其他数据明显不一致的记录也需要进行修正或删除。
格式化:为确保所有数据都采用相同的格式和标准,可能需要转换数据格式。例如,将所有的基因表达数据转换为同一标准化形式,或将所有日期转换为同一格式。
标准化:由于数据可能来自不同的实验室或平台,因此其范围和分布可能会有所不同。标准化过程旨在使所有数据具有相同的比例和分布。例如,对基因表达数据进行Z分数转换或最大-最小标准化。
批次效应修正:在集成多批次数据时,可能会观察到由于技术原因导致的系统性差异。常用的方法,如ComBat,可以用来纠正这些批次效应,以确保不同批次之间的数据是可比的。
数据整合:如果同时使用多种类型的数据(例如mRNA表达和miRNA表达),可能需要将它们整合到一致的格式中,以便进行整体分析。
2.2.差异表达分析
①使用R语言中的limma包或DESeq2包对VAMP8的表达量进行差异表达分析,比较宫颈癌组和正常组之间的差异。
②将结果进行多重假设检验校正,以减少假阳性的出现。
2.3.ROC曲线分析
①ROC曲线(Receiver Operating Characteristic Curve)是反映灵敏性和特异性的综合指标,常用于评估诊断试验的准确性。给定不同的分类阈值,ROC曲线描绘了真正例率(TPR,灵敏性)与假正例率(FPR,1-特异性)之间的关系。曲线下面积(AUC,Area UnderCurve)为ROC曲线与横轴之间的面积,可以量化分类器的总体性能。AUC值为0.5表示随机分类器,值为1表示完美分类器。
②具体分析方法:对于二分类问题,确定正例和负例的标签;为每个样本计算正例的概率;按照概率值对样本进行排序;逐步改变分类阈值,计算TPR和FPR;在坐标图上绘制ROC曲线,横轴为FPR,纵轴为TPR;计算AUC值以量化分类器的性能。
2.4.生存分析
①根据VAMP8的表达量,将宫颈癌病例分为高表达组和低表达组。使用VAMP8表达量的中位数作为阈值,高于或等于该阈值的被视为高表达组,而低于该阈值的被视为低表达组。
②使用生存分析(如Kaplan-Meier法)来研究VAMP8表达量与宫颈癌患者的总体生存率(OS)和无疾病生存率(DFS)的关系。总体生存率(OS):是指从随机分组或治疗开始到任何原因导致的死亡为止的时间。这是衡量患者生存状态的一种直接指标,不考虑死亡的原因。无疾病生存率(DFS):是指从随机分组或治疗开始到疾病复发或进展或由疾病导致的死亡为止的时间。DFS重点关注患者是否处于无疾病状态,即未出现疾病的复发或进展。OS重视的是死亡事件,而DFS重视的是疾病复发、进展或由疾病导致的死亡事件。
③使用Cox比例风险模型进一步验证VAMP8表达量与宫颈癌预后的独立关联。
2.5.临床相关性分析
①数据准备:收集患者的临床数据和生物标志物或模型预测值。
②不同临床指标选择适当的统计测试:依据数据的分布和类型,如皮尔逊相关性、斯皮尔曼秩相关性或卡方检验。
③计算统计显著性:评估所观察到的关系是否不大可能是由随机因素导致的。
④效应量评估:衡量生物标志物或模型预测与临床结果之间的关系强度,如相关系数的大小。
2.6.免疫浸润程度分析
TIMER(Tumor IMmune Estimation Resource)是一种计算免疫细胞浸润水平的分析工具。该工具主要用于分析肿瘤组织中免疫细胞的渗透水平,通过估计肿瘤微环境中各种免疫细胞的丰度来揭示肿瘤和免疫系统之间的相互作用。TIMER采用了基于大规模肿瘤基因表达谱数据的统计分析方法,使用包括六种主要免疫细胞类型(B细胞、CD4+T细胞、CD8+T细胞、中性粒细胞、巨噬细胞和树突细胞)的基因表达特征。通过这些数据,可以推断这些细胞在肿瘤样本中的丰度。利用TIME对VAMP8表达量和免疫细胞浸润的相关性进行分析,从而了解VAMP8在宫颈癌的免疫微环境中的作用。
2.7.免疫组织化学实验
①样本准备:选择与生物信息学分析中使用的样本相对应的组织切片,确保它们已经经过固定和包埋处理。
②抗体选择:购买或制备针对VAMP8的特异性抗体,同时选择适当的二抗,这是与荧光标记或酶联标记结合的抗体。
③免疫染色:对组织切片进行脱酸、过氧化物酶封锁、抗体孵育、漂洗、二抗孵育、染色(如DAB)等步骤。
④结果观察:使用光学显微镜或荧光显微镜观察组织中VAMP8的表达情况。
2.8.数据可视化
所有的结果都需要清晰、准确地展现。使用R或Python中的相关包(如ggplot2,Matplotlib等)来生成高质量的图表,包括差异表达图,ROC诊断效能曲线,生存曲线,免疫细胞浸润图。
在进行这些分析时,将严格控制错误发现率,使用适当的多重检验校正方法,如Bonferroni或Benjamini-Hochberg方法。同时,也将关注并处理可能出现的偏见和混淆因素,如批次效应、年龄、性别等,通过设置归一化、批次效应校正、多变量回归分析或其它统计方法进行校正和消除。
3.VAMP8对宫颈细胞功能的影响
3.1.细胞培养及处理
①使用含有10%胎牛血清和1%抗生素(青霉素/链霉素,1/1)的Dulbecco's改良鹰氏培养基(DMEM),即完全培养基,在37℃,5%CO2恒温恒湿培养箱中培养HPV16阳性的宫颈细胞系Ect1/E6E7(4.中为宫颈癌细胞系HeLa、SiHa、C-33A)。
②每日通过倒置显微镜观察细胞形态和生长状态,细胞覆盖达到70%~80%时,使用无菌PBS溶液洗涤后用0.25%胰酶-EDTA消化剥离细胞。
③采用离心法(1000rpm,3分钟)收集细胞(即离心去除胰酶)后复悬于新鲜的完全培养基中,然后接种至新的培养板中以备用。
3.2.慢病毒制备与转染
①使用慢病毒包装系统制备VAMP8基因过表达或者敲低的慢病毒。首先,用钙磷共沉淀法将VAMP8过表达的慢病毒载体或VAMP8敲低的慢病毒shRNA载体转染到293T细胞中,48-72小时后,收集悬浮液并通过0.45μm滤膜进行过滤,然后用超速离心机将病毒沉淀。VAMP8过表达的慢病毒载体为CMV-MCS-3Flag-Ubi-ZSGreen-IRES-Puromycin,选用的基因CDS区序列为:atggaggaagccagtgaaggtggaggaaatgatcgtgtgcggaacctgcaaagtgaggtggagggagttaagaatattatgacccagaatgtggagcggatcctggcccggggggaaaacttggaacatctccgcaacaagacagaggatctggaagccacatctgagcacttcaagacgacatcgcagaaggtggctcgaaaattctggtggaagaacgtgaagatgattgtccttatctgcgtgattgtttttatcatcatcctcttcattgtgctctttgccactggtgccttctcttaa。VAMP8敲低的慢病毒shRNA载体为U6-MCS-CMV-zsGreen-PGK-Puromycin,选用的干扰片段为sh-5’-gaaatgatcgtgtgcggaacc-3’。
②对细胞生长至60%-70%的HPV16阳性的宫颈细胞(4.中为宫颈癌细胞),按MOI值=10进行慢病毒转染,并加入6-8μg/mL的Polybrene增强转染效率。转染24小时后,更换新鲜的完全培养基。
筛选并构建稳定细胞系:
③转染72小时后,使用含有2μg/mL嘌呤霉素的完全培养基对细胞进行筛选,通常持续筛选7-10天,筛选出稳定表达VAMP8的细胞系。
3.3.VAMP8的mRNA与蛋白表达检测
①检测mRNA表达,用TRIzol提取总RNA,然后通过RT-PCR合成cDNA,最后用qPCR检测VAMP8 mRNA的表达。具体步骤同1.3.。
②检测蛋白表达,用RIPA溶解液提取细胞蛋白,通过BCA法测定蛋白浓度后,用SDS-PAGE电泳分离蛋白,然后进行Western Blot,检测VAMP8蛋白的表达水平。
a.蛋白质提取和SDS-PAGE电泳:
蛋白质提取:从HPV16阳性和阴性宫颈组织样本中提取总蛋白质。通常使用RIPA缓冲液或其它蛋白提取缓冲液进行组织的裂解和蛋白质的提取。裂解后,可进行离心以去除细胞残渣,上清液即为总蛋白样本。
蛋白浓度测定:通过BCA法测定蛋白浓度。
SDS-PAGE电泳:将提取的蛋白质样本与SDS载样缓冲液混合,沸腾5分钟后加载到聚丙烯酰胺凝胶中进行电泳。使用恒电流模式,初级电泳通常在80-120V进行,直至样品进入分离胶,然后增加电压至120-200V直至电泳结束。
b.转膜和免疫印迹:
转膜:SDS-PAGE电泳后,将凝胶放入转膜装置中。与之配套的是PVDF膜,预先用甲醇活化后,再用转膜缓冲液湿润。使用电流将蛋白质从凝胶转移到PVDF膜上。
免疫印迹:PVDF膜转膜后,先在5%脱脂奶粉或BSA的封闭液中孵育,以阻止非特异性结合。然后使用稀释的特异性抗VAMP8抗体(针对VAMP8蛋白的特定区域制备的抗体,用以特异性地检测和识别VAMP8蛋白)孵育,随后清洗并使用与原始抗体种属相匹配的二抗孵育。
c.信号检测和定量:
信号检测:如果使用荧光二次抗体,可以直接使用生物成像系统进行信号检测。如果使用化学发光二次抗体,需要添加ECL(enhanced chemiluminescence)显示剂,并在特定条件下进行暴露以检测化学发光信号。
图像获取和蛋白质定量:用生物成像系统检测和获取免疫印迹膜上的信号。可以是数字化的或者用胶片捕获的,检测荧光或化学发光信号,并提供蛋白质的相对定量。
3.4.细胞功能学的实验验证
①细胞计数试剂盒-8(CCK-8)实验:采用CCK-8试剂盒测定细胞增殖速率。将接受不同处理或转染的细胞以等量接种在96孔板中(于完全培养基中),在37℃和5%CO2条件下培养。培养24、48、72小时后,更换为含10%CCK-8的新鲜的完全培养基,并继续培养2小时。在450nm处使用酶标仪测量光密度(OD)值。
②划痕愈合实验:为确保实验的一致性和可重复性,以0.5cm的间隔在6孔板的底部划出水平线条,每个孔应至少有四条线。将细胞在6孔板中以10^6的密度种植,旨在次日达到每个孔板底部覆盖单层细胞的目标。利用200μL移液管的尖端轻轻划过细胞层,制造一条初始宽度一致的直线划痕。然后,使用PBS清洗,然后添加无血清培养基(即含有1%抗生素(青霉素/链霉素,1/1)的Dulbecco's改良鹰氏培养基(DMEM))进行继续培养。实验开始(0小时)和24小时后,使用倒置显微镜进行拍照。通过水平线定位,以保证观察和测量同一位置的细胞划痕宽度的变化。
③Transwell细胞迁移和侵袭实验:首先将含有20%血清的培养基(即含有20%胎牛血清和1%抗生素(青霉素/链霉素,1/1)的Dulbecco's改良鹰氏培养基(DMEM))添加至Transwell实验装置的下室。将各组细胞以同样浓度重悬于无血清培养基(即含有1%抗生素(青霉素/链霉素,1/1)的Dulbecco's改良鹰氏培养基(DMEM)),并接种于Transwell的上室。在37℃和5% CO2环境下孵育24小时后,使用湿棉签轻轻移除残留在上室的细胞。在侵袭实验中,Transwell上室涂覆Matrigel基质胶(与无血清培养基的稀释比为1:2),而在迁移实验中则无需涂覆基质胶。随后,将已迁移或侵袭至下室并黏附的细胞以4%多聚甲醛固定1小时,用结晶紫染色15分钟后,随机选择不同视野进行显微镜下拍照。
④流式细胞仪进行细胞周期分析:
预处理:收集待测细胞,然后进行离心处理以去除培养基。
固定:冷冻细胞后,用70%冰冷乙醇缓慢加入到细胞中进行固定,置于4℃存储过夜。
染色:去除冰冷乙醇,并用PBS洗涤细胞至少两次。之后,向细胞中加入适量的PI(Propidium Iodide)染色液,并在暗处孵化15-30分钟。
流式检测:利用流式细胞仪进行细胞检测,收集数据后,用专业软件进行细胞周期分析。
⑤利用Annexin V/PI双染法检测细胞凋亡情况:
预处理:收集待测细胞并进行离心处理以去除培养基。
染色:根据试剂盒说明,准备Annexin V和PI染色液。先加入Annexin V染料在暗处孵化15分钟,随后加入PI染料,并在暗处孵化5分钟。
流式检测:利用流式细胞仪进行细胞检测。Annexin V附着于外向的磷脂酰丝氨酸,而PI则染色死亡细胞中的DNA。收集数据后,用专业软件进行凋亡细胞的鉴定与分析。
注意:④、⑤两个实验均需在暗处处理PI,因为PI对光敏感。且实验中需避免泡沫的产生,以保证流式细胞仪检测的准确性。实验前后务必确保设备、工具、试剂的无菌性。
3.5.透射电子显微镜(TEM)观察自噬小体形态与数量变化
①样本准备:选取待测细胞并在适当的刺激或处理后培养。使用固定液(例如,2.5%戊二醛)固定细胞。细胞离心并以2%的醋酸钌处理,增强电子密度。将细胞包埋于树脂(如EPON 812)中。制备超薄切片(约60-90纳米厚)。
②透射电子显微镜操作:将超薄切片放置在铜网上。若需要,可对切片进行对比染色,如铅柠檬酸或铀醋酸染色。在透射电子显微镜下进行观察,通常在80-100kV下操作。
③图像获取与分析:捕捉不同区域的高清晰度图像。使用专业软件进行图像分析,标记和计数自噬小体。对自噬小体的形态进行描述,如完整性、大小、内容物等。
④统计分析:对多个观察视场下的自噬小体数量进行统计。采用适当的统计方法对比不同实验组之间的差异。
4.VAMP8在宫颈癌细胞中的作用
细胞实验部分技术方案同“3.”。
4.1.裸鼠体内成瘤实验:
①在满足伦理条件下,将裸鼠麻醉后,皮下注射稳定转染调控VAMP8表达的宫颈癌细胞SiHa及转染模型中的对照细胞株,每只鼠注射约100万个细胞,以构建裸鼠宫颈癌移植瘤模型。
②每2天观察并记录裸鼠的体重,同时用游标卡尺测量肿瘤最长轴的长度和最宽轴的宽度,根据公式V=0.5×长×宽^2计算肿瘤体积。
③通过分离肿瘤组织,利用IHC和qPCR实验手段检测肿瘤组织中VAMP8的表达水平,观察其对肿瘤生长的影响。
④在实验结束后,执行严格的裸鼠解剖和实验后处理程序,以满足伦理和科研规定。对解剖出的肿瘤进行测量、记录,进一步对其进行病理组织切片和HE染色,以观察肿瘤的微观病理变化。
裸鼠解剖与后处理:在确保鼠类处于麻醉状态后,进行严格的裸鼠解剖。依据相关伦理和科研指导原则,进行后续处理,以确保对鼠类的人道对待。
肿瘤取样与测量:仔细解剖出肿瘤组织,并放置在无菌条件下。使用测量工具如卡尺测量肿瘤的长宽尺寸,并进行记录。
病理组织切片与染色:将解剖出的肿瘤组织固定于10%中性缓冲甲醛中。组织固定后,采用脱水、透明、浸蜡等步骤,为切片做预处理。使用组织切片机,制作4-5微米厚度的组织切片,并置于玻片上。对组织切片进行苏木精-伊红(HE)染色:首先使用苏木精染核为蓝色,然后使用伊红染细胞浆为红色。染色完成后,使用显微镜观察并记录肿瘤组织的微观病理变化。
实施例2
统计显著性是一个用于评估观测结果与零假设之间是否存在显著差异的工具。p值是一个用于表示观测数据在零假设下出现的概率的指标。在以下技术成果的图中,“ns”代表结果不存在统计显著性,不同级别的统计显著性通过特定的标记来表示:
"*":表示p值小于0.05,暗示在5%的显著性水平上,观测结果与零假设存在显著差异。
"**":表示p值小于0.01,暗示在1%的显著性水平上,观测结果与零假设存在高度显著的差异。
"***":表示p值小于0.001,暗示在0.1%的显著性水平上,观测结果与零假设存在极度显著的差异。
HPV16病毒感染相关宫颈疾病的病程发展通常包括三个阶段:
低级别宫颈鳞状上皮内病变(LSIL):LSIL通常表示轻度的细胞改变。这些改变经常是由人类乳头瘤病毒(HPV)感染引起的。大多数LSIL会在数月或数年内自然消退,只有很少数会进展为HSIL。
高级别宫颈鳞状上皮内病变(HSIL):HSIL表示更为严重的细胞变异。这些变异有更高的风险进展为宫颈癌,尤其如果不接受适当的治疗或长时间的监测。
宫颈癌:如果HSIL没有得到适当的诊断和治疗,一部分病例可能会进展为宫颈癌。宫颈癌是指癌细胞侵犯了宫颈的更深层组织,已经不仅仅局限于上皮层。
LSIL和HSIL是宫颈细胞的两个不同程度的异常增生,而宫颈癌是这种异常增生没有得到控制、进一步进展的结果。HPV感染是导致这三种情况的主要原因,但并不是所有的HPV感染都会导致癌症。
如图1A,蛋白质组学分析揭示HPV16阳性宫颈组织中的VAMP8表达显著高于HPV16阴性组织。在宫颈疾病的进展过程中,宫颈LSIL(低级别宫颈鳞状上皮内病变)的VAMP8表达高于HSIL(高级别宫颈鳞状上皮内病变)和宫颈癌,但均显著高于正常HPV16阴性宫颈组织。如图1B,不同宫颈相关细胞系的qPCR实验结果表明,除C-33A宫颈癌细胞系几乎未检测到VAMP8基因外,HPV16阳性细胞(Ect1/E6E7)的VAMP8基因表达显著高于HPV16阴性的宫颈原代细胞;宫颈癌细胞系(HeLa和SiHa)的VAMP8表达低于HPV16阳性细胞(代表宫颈病变的进展期)但显著高于HPV16阴性宫颈细胞。细胞qPCR实验结果趋势与蛋白质组学结果趋势相同。
使用蛋白质组学分析、qPCR技术,发现HPV16阳性宫颈组织中的VAMP8表达显著高于HPV16阴性组织。特别在低级别宫颈鳞状上皮内病变(LSIL)中,VAMP8的表达量显著增加,大于高级别宫颈鳞状上皮内病变(HSIL)和宫颈癌中VAMP8的表达量,且均显著大于正常的HPV16阴性宫颈组织。VAMP8可作为诊断HPV16感染以及评估宫颈疾病严重程度的潜在标志物。即在宫颈组织中检测到VAMP8显著高表达为HPV16感染阳性,而在同一HPV16阳性患者定期检测的结果中观察到VAMP8表达下降,则提示宫颈疾病发生恶性进展。考虑到VAMP8可能不是HPV16感染和宫颈疾病进展的唯一标志物,结合其他已知的生物标志物可能会增加诊断的准确性。
如图2所示,在术后宫颈标本中通过对切片的免疫组织化学染色进一步验证了HPV16阳性的宫颈组织的VAMP8的相对高表达。
如图3-图5所示,对TCGA数据库的生物信息学分析结果表明宫颈癌中存在VAMP8基因的高表达并具有诊断效能,且与宫颈癌的不良预后密切相关。图3A-C证明了在TCGA大样本数据库中,相对于正常组织,VAMP8基因在宫颈癌中相对高表达;图3D的ROC曲线表明VAMP8可以作为宫颈癌灵敏的诊断标志物;图3E-G的生存分析得出VAMP8高表达的患者具有更差的预后情况。如图4所示,在TCGA中的宫颈癌临床相关性分析显示VAMP8基因的高表达与病例的T分期、N分期、M分期、临床分期、治疗疗效、病理类型、病理分级和更年期状态相关。其中VAMP8的表达与T分期、N分期、M分期、临床分期、治疗疗效的进展程度负相关;与病理分级、更年期状态的进展程度正相关;在病理类型中,宫颈鳞状细胞癌中的VAMP8表达量高于腺癌和腺鳞癌。如图5所示,在TCGA中对VAMP8进行免疫浸润分析,结果显示VAMP8与多种免疫细胞浸润显著相关,如DC、aDC、Tcm和NK细胞。
根据TCGA数据库的生物信息学分析,发现高VAMP8表达与宫颈癌的预后不良和免疫浸润程度密切相关,因此,VAMP8可以作为宫颈癌的预后标志物。
在4种宫颈相关细胞系(Ect1/E6E7、Hela、SiHa、C-33A)中分别构建VAMP8基因的敲低与过表达细胞系,具体分组名称为:shNC——敲低对照细胞株;shVAMP8——敲低VAMP8基因的细胞株;OE_NC——过表达对照细胞株;OE_VAMP8——过表达VAMP8基因的细胞株。通过4组细胞株两两分别对照的细胞功能学实验,验证调控VAMP8基因对宫颈细胞的作用。图6到图10的体外实验结果表明在HPV16阳性的Ect1/E6E7中VAMP8的相对高表达导致细胞自噬通量增加,细胞增殖、迁移、侵袭能力下降,凋亡率和细胞周期的间期比率(G0/G1和S期的占比)增加,即在HPV16感染初期的宫颈细胞中VAMP8的高表达促进了细胞自噬、但抑制HPV16阳性的宫颈细胞生长。而在宫颈癌细胞系Hela、SiHa、C-33A中,VAMP8的相对高表达同样增强了自噬,但细胞的增殖、迁移和侵袭被促进,凋亡率减少和分裂期(G2/M)比率增加,即在宫颈癌细胞中,VAMP8的高表达促进了细胞自噬、并促进肿瘤细胞生长。无论是HPV16阳性的细胞还是宫颈癌细胞,VAMP8的高表达都与增强的自噬有关。这意味着VAMP8在调节宫颈细胞的自噬中起到了关键作用。VAMP8在HPV16感染初期的宫颈细胞和宫颈癌细胞中具有不同的功能。在HPV16感染初期的宫颈细胞中,VAMP8能起到保护作用,帮助细胞应对HPV感染并抑制恶性行为。然而,在宫颈癌细胞中,VAMP8可支持肿瘤的生长和扩散。这些结果提示VAMP8在宫颈细胞的生物学功能中扮演着复杂的角色。
图11展示的裸鼠体内成瘤实验的肿瘤肉眼观、体积和质量进一步验证了高表达的VAMP8促进宫颈癌的生长和转移。图12在透射电子显微镜下观察不同组宫颈癌细胞成瘤组织的自噬体表明了VAMP8在体内的促自噬效应。
通过慢病毒转染构建VAMP8基因稳定过表达或敲低的宫颈细胞系。在HPV16阳性的Ect1/E6E7细胞中,VAMP8的相对高表达导致自噬通量增加,细胞增殖、迁移、侵袭减少,细胞凋亡增加,G0/G1和S期细胞比例增加。在宫颈癌细胞系(Hela、SiHa、C-33A)中,VAMP8的相对高表达反而促进了细胞增殖、迁移和侵袭,减少了细胞凋亡,增加了G2/M期细胞比例。体内实验进一步证实,高表达的VAMP8会促进宫颈癌的生长和转移。
综上所述,本发明为深入理解HPV16感染和宫颈癌进展的分子机制提供了新的视角,同时揭示了VAMP8作为诊断标志物、预后标志物和治疗目标的潜力。VAMP8的高表达与HPV16感染和宫颈癌的发展有关,因此VAMP8可作为一种潜在的诊断工具,帮助医生在早期阶段识别出可能患有宫颈癌的患者。此外,VAMP8的表达水平也可作为预后标志物,帮助医生评估患者的治疗反应和预后。最后,由于VAMP8在宫颈癌发展中的重要作用,针对VAMP8的治疗将有助于抑制宫颈癌的发展,因此提供了一种新的宫颈癌治疗策略。这为开发新的诊断方法和治疗手段,以及改善宫颈癌患者的生存率和生活质量提供了可能。
