Abstract
本发明涉及一种诊断系统性红斑狼疮生物标志物及其应用,所述生物标志物为BND细胞在生物样品中的比例。相对于现有技术,本发明具有简单易实施、省时间、高效的特点;该生物标志物在疾病早期即可进行有效诊断,诊断效果具有高敏感性和特异性。
Description
技术领域
本发明属于疾病诊断与治疗领域,特别涉及一种诊断系统性红斑狼疮生物标志物及其应用。
背景技术
系统性红斑狼疮(Systemic lupus erythematosus,SLE)是一种高度异质性、慢性系统性自身免疫性疾病,其特征是循环自身抗体和影响多个组织和器官的炎症反应。临床表现的异质性、不可预测的病程和发作,使得SLE的临床诊断和治疗极具挑战。目前SLE诊断主要依据抗核抗体(ANA)、抗双链DNA(ds-DNA)抗体和抗Smith抗体等检测指标。但SLE初期为临床前无症状期,表现为所有自身免疫性疾病常见的自身抗体阳性,疾病进展后才表现为狼疮特异性自身抗体阳性,并伴有严重的组织损伤。因此,SLE的早期识别及诊断极具挑战。
近年来研究发现,SLE产生自身反应性抗体的浆细胞来源于激活的无能自身反应性B细胞。在B细胞发育过程中,B细胞受体通过V(D)J基因随机重组而产生的。据估计,50-70%自发产生的B细胞可能产生识别自身抗原的抗体,因此,去除自身反应性B细胞克隆以避免自身反应对人体的免疫平衡至关重要。中枢免疫耐受通过受体编辑或克隆清除约50%的自身反应性B细胞。其余进入循环的自身反应性B细胞被外周耐受性通过诱导无能抑制。无能的B细胞降低了细胞表面的B细胞受体密度,并减弱了B细胞受体激活后的信号,从而限制了细胞分化、增殖和自身反应性抗体的产生。自身反应性无能B细胞已被鉴定为IgMlow/-成熟初始B细胞(CD19+CD27-IgD+),称为BND细胞。在疾病状态下,BND细胞逃脱了外周免疫耐受并成分化为自身抗体分泌细胞。BND细胞在人体内的表面标记是低至阴性的IgM表达的初始B细胞(CD19+CD27+IgD+IgMlo/-)。在病理条件下,BND细胞的无能状态可以被逆转,并有助于自身免疫。
目前,临床对SLE的诊断依赖于自身抗体等指标检测,但是部分患者在早期常出现自身抗体阴性等情况。此外,目前C-反应蛋白、血沉等生化指标作为主要评估SLE疾病活动性的主要指标,易受感染等状况的影响,存在特异性差等局限性。因此,对于SLE的疾病诊断和病情评估,仍需可靠性生物学标志物,来实现早期诊断和病情活动度评估。
发明内容
本发明所要解决的技术问题是提供一种协助系统性红斑狼疮诊断和病情评估的生物标志物及其应用,以克服目前生化指标易受感染等状况的影响,存在特异性差等的局限性。
本发明提供了一种系统性红斑狼疮生物标志物的检测及评估方法,所述生物标志物为BND细胞在生物样品中的比例,用于SLE的疾病诊断、疾病活动度评估和药物疗效判断。
所述BND细胞亚群表面分子标志物包括CD19、CD27、IgD和IgM。
所述比例通过流式细胞术检测。
所述比例包括BND细胞占B细胞的比例和BND细胞占初始B细胞的比例。
所述生物样品为外周血、血清和/或外周血单个核细胞。
所述生物标志物在SLE患者体内相对正常组下调,其含量与SLEDAI评分和抗ds-DNA抗体滴度呈显著负相关。根据其含量比照阈值可判断SLE的发生与否,活动度的高低,从而用于SLE疾病的体外辅助诊断、用药指导、疗效监测及预后观察。
本发明还提供了一种生物标志物在制备诊断系统性红斑狼疮试剂中的应用。
有益效果
本发明针对系统性红斑狼疮的诊断,提供了一个新的细胞亚群作为诊断疾病的生物标记物,其比例可以协助疾病诊断,评估疾病的活动性以及治疗效果。相对于现有技术,本发明具有简单易实施、省时间、高效的特点;该生物标志物在疾病早期即可进行有效诊断,诊断效果具有高敏感性和特异性。

下面结合具体实施例,进一步阐述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。此外应理解,在阅读了本发明讲授的内容之后,本领域技术人员可以对本发明作各种改动或修改,这些等价形式同样落于本申请所附权利要求书所限定的范围。
实施例1
(一)实验方案
(1)研究对象:通过招募志愿者,收集对照组46例,在上海长征医院住院及门诊系统纳入SLE患者40例,其中低活动度(5-9分)17例,中活动度(10-14分)7例,高活动度(≥15)16例。收集入组人员外周血样本。研究经上海长征医院伦理委员会批准,并获得之情同意。所有纳入人群的临床特征见表1。
(2)病例组纳入标准:①年龄在18-70岁之间;②参照2019 EULAR/ACR最新系统性红斑狼疮(SLE)分类标准;③自愿签署知情同意书。
(3)病例组排除标准:①近3个月有感染病史;②妊娠和哺乳期女性;③明确肿瘤病史;④合并呼衰、心衰、急性心脑血管事件、肝肾损伤、精神疾病的SLE患者。
(4)样本收集:本研究中,收集纳入所有人群外周血5ml,置于EDTA抗凝血采集管中,用密度梯度离心法分离外周血的单个核细胞。
(5)流式细胞术分析:
BND细胞表面标志物用于流式细胞术检测的抗体:抗CD19-APC抗体,抗IgD-PE抗体,抗CD27-APC-cy7抗体和抗IgM-BV421抗体。
BND细胞流式染色:取1×106外周血单个核细胞,100μL PBS重悬,加入FC受体阻断剂1μL,4℃孵育30min。加入5mL磷酸盐缓冲液(PBS)重悬细胞,300g离心5min。弃上清,100μLPBS重悬,加入不同荧光标记抗CD19、抗IgD、抗CD27、抗IgM抗体以及活细胞染料。4℃孵育30min。加入5mL PBS重悬细胞,300g离心5min。弃上清,300μL PBS重悬细胞后,使用LSR-fortessa流式细胞仪进行检测,用FlowJo软件进行分析。
(6)统计分析:使用GraphPad Prism 7.0软件进行统计学分析,呈正态分布且方差齐性的计量资料以x±s表示,两组间比较采用t检验,多组间比较采用ANOVA检验,组间进一步两两比较采用Dunnt’t检验;偏态分布计量资料以中位数(下四分位数,上四分位数)表示,两组间比较采用Mann-Whitney U检验,多组间比较采用Kruskal-Wallis检验,组间进一步两两比较采用Dunn′s多重检验。相关性分析采用Spearman相关分析。检验水准(α)为0.05。
(二)数据分析结果
(1)受试者基本情况
SLE组共有40例,其中男性4例,女性36例,男女之比1∶9。平均年龄36.2岁,平均病程3.52年,抗ds-DNA抗体阳性率30%,抗smith抗体阳性率30%,平均血沉41.23mm/h,平均SLE疾病活动度指数(SLEDAI)为12.13。其他检测结果详见表1。

(2)BND细胞亚群鉴定
在CD19+的B细胞中,可根据CD27和IgD分为CD27-IgD+初始B细胞、CD27+IgD+非类别转换型记忆B细胞、CD27-IgD-双阴性B细胞、CD27+IgD-类别转换型记忆B细胞。初始B细胞再根据IgM的表达分为IgM+初始B细胞和IgM-的BND细胞(图1)。
(3)SLE患者和对照组B细胞亚群比例差异
总共收集40名SLE患者和46名健康对照进行B细胞亚群分析,性别和年龄在两组间无统计学差异(表1)。流式数据分析结果显示(表2,图2):与正常对照相比,SLE患者外周血B细胞占总淋巴细胞比例、初始B细胞占B细胞比例、记忆B细胞占B细胞比例、双阴性B细胞占B细胞比例无统计学差异(p>0.05),而BND细胞占B细胞比例、BND细胞占初始B细胞比例均有统计学差异(p<0.0001),SLE患者的BND细胞比例较低。

(4)SLE患者BND细胞比例和临床指标的相关性
为了研究SLE患者外周血BND细胞比例与疾病活动度的相关性,采用了Spearman相关分析检验了SLE患者外周血BND细胞占初始B细胞比例与SLEDAI、抗ds-DNA抗体滴度的相关性。还比较了BND细胞占初始B细胞比例在治疗前后变化。结果如图3显示,BND比例与SLEDAI呈显著负相关(p=0.011,r=-0.4),与抗ds-DNA抗体滴度呈显著负相关(p=0.011,r=-0.4)。SLEDAI、抗ds-DNA抗体滴度这两个指标是临床上常用的用来评估疾病活动度和严重程度的重要指标。这表明外周血BND细胞比例具有成为SLE疾病活动度评价指标的可能性。
此外对SLE患者在同次住院期的入院检查及出院检查时分别抽取外周静脉血,检测BND细胞比例,这13名患者住院期间均接受激素治疗(2-4mg/kg/天)且治疗后病情均有所好转。结果显示,BND细胞比例在激素治疗前后均有统计学差异,激素治疗能提高BND细胞比例。这说明,BND细胞比例能很好的反应患者的激素治疗效果。
(三)结论
综上所述,本发明证明了BND细胞比例可作为SLE的生物标志物,该标志物在SLE患者体内降低,并且与SLEDAI、抗ds-DNA抗体滴度呈显著相关性。并且在疾病治疗缓解时升高,可指征SLE疾病的发病和进展。在炎症环境中BND细胞比例的下调提示外周免疫耐受机制破坏。因为BND细胞存在高度的自身反应性,可以转化为自身抗体分泌细胞,影响SLE的疾病发生和发展,所以BND细胞可作为SLE的直接标志物。由于SLE在疾病早期出现BND细胞比例下降,因此该标志物用于SLE的体外辅助鉴别诊断、病程判断或用药指导,具有相对现有临床标志物更高的敏感性和特异性。

2.根据权利要求1所述的生物标志物,其特征在于:所述BND细胞亚群表面分子标志物包括CD19、CD27、IgD和IgM。
3.根据权利要求1所述的生物标志物,其特征在于:所述比例通过流式细胞术检测。
4.根据权利要求1所述的生物标志物,其特征在于:所述比例包括BND细胞占B细胞的比例和BND细胞占初始B细胞的比例。
5.根据权利要求1所述的生物标志物,其特征在于:所述生物样品为外周血、血清和/或外周血单个核细胞。
6.根据权利要求1所述的生物标志物,其特征在于:所述生物标志物在SLE患者体内相对正常组下调,其含量与SLEDAI评分和抗ds-DNA抗体滴度呈显著负相关。
