背景技术
在大多数癌症类型中,癌症发病率和癌症死亡率都存在着性别差异,其中某些癌症通常具有性别特异性(例如,女性为卵巢癌,男性为前列腺癌)。而在非生殖系统癌症中,男性通常表现出比女性更高的发病率,且死亡率比女性近两倍,如唇癌、喉癌、食道癌和膀胱癌等。除了癌症发病率和死亡率不同外,男性和女性对治疗的反应也不同。例如女性肺癌患者的手术生存率高于男性。此外,在非小细胞肺癌(NSCLC)、成胶质细胞瘤(GBM)和霍奇金淋巴瘤(HL)中,化疗为女性提供了优于男性的生存优势。同时,性别间药物代谢的差异经常导致女性患者出现较高的全身毒性反应,并相应影响治疗效果。因此,了解癌症治疗响应中的性别差异对于优化以在男性和女性患者中获得相似的最佳结果的治疗方法至关重要。

本发明利用机器学习模型从3500种药物中筛选获得了8种药物,包括拉帕替尼、帕苯达唑、布雷非德菌素A、OSI-027、5-氟尿嘧啶、SB-216763、IB-MECA和坦度替尼。相对其他药物而言,这8种药物对癌症患者尤其是食管鳞癌患者有性别差异反应,对雄性(男性)癌症患者具有更好的药物敏感性,能用于对雄性癌症患者的联合治疗。

为使本发明实施例的目的、技术方案和优点更加清楚,下面将对本发明实施例中的技术方案进行清楚、完整地描述。实施例中未注明具体条件者,按照常规条件或制造商建议的条件进行。所用试剂或仪器未注明生产厂商者,均为可以通过市售购买获得的常规产品。
了解男性和女性之间不同癌症易感性的潜在机制,对于实现有效的食管鳞癌抗癌治疗至关重要。前期研究显示ESCC的性别差异与雄激素受体(AR)和雌激素受体β(Erβ)等性激素受体相关。然而,这些研究仅部分解释了男性ESCC病发病率普遍较高的原因,尚未关注ESCC治疗响应中的性别差异。因此,需要做出更多努力来阐明ESCC的性别差异,特别是在治疗响应中的性别差异。
本发明利用机器学习模型从3500种药物中筛选获得了8种药物,包括拉帕替尼、帕苯达唑、布雷非德菌素A、OSI-027、5-氟尿嘧啶、SB-216763、IB-MECA和坦度替尼。相对其他药物而言,这8种药物均对癌症患者尤其是食管鳞癌患者有性别差异反应,对雄性(男性)癌症患者具有更好的药物敏感性,能应用于对雄性ESCC患者的联合治疗。在其他男性治疗响应差的癌症中,如肺鳞癌、食管腺癌、皮肤黑色素瘤、胃腺、管状胃腺癌、膀胱尿路上皮癌、胃粘液腺癌、结肠和直肠粘液腺癌、肺腺癌、弥漫性胃腺癌、结肠腺癌、直肠腺癌和甲状腺乳头状癌等等,这8种药物也有可能具有相同或相似的技术效果。

在一些实施例中,所述目标药物或药物组合物可以是适用于口服的形式,例如片剂、糖锭剂、锭剂、水或油混悬液、可分散粉末或颗粒、乳液、硬或软胶囊,或糖浆剂或酏剂。可按照本领域任何已知制备药用或药物组合物的方法制备口服药物。此类口服药物可含有以下任意一种或多种成分:甜味剂、矫味剂、着色剂和防腐剂,以提供悦目和可口的药用制剂。
所述片剂可含有上述活性成分(拉帕替尼、帕苯达唑、布雷非德菌素A、OSI-027、5-氟尿嘧啶、SB-216763、IB-MECA和坦度替尼中的任意一种或多种的组合)和用于混合的适宜制备片剂的无毒的可药用的赋形剂。这些赋形剂可以是惰性赋形剂,造粒剂、崩解剂,粘合剂和润滑剂中的任意一种或多种。上述片剂可以不包衣或可通过掩盖药物的味道或在胃肠道中延迟崩解和吸收,因而在较长时间内提供缓释作用的已知技术的包衣。
水混悬液含有上述活性成分和用于混合的适宜制备水混悬液的赋形剂。此类赋形剂是悬浮剂,分散剂或湿润剂。
油混悬液可通过使活性成分悬浮于植物油,或矿物油配制而成。油悬浮液可含有增稠剂。
在一些实施例中,所述目标药物或药物组合物也可以是水包油乳剂的形式。油相可以是植物油,或矿物油或其混合物。适宜的乳化剂可以是天然产生的磷脂,乳剂也可以含有缓和剂、甜味剂、矫味剂、防腐剂、着色剂和抗氧剂中的任意一种或多种。
在一些实施例中,所述药物或药物组合物也可以是无菌注射水溶液形式。可以使用的可接受的溶媒或溶剂有水、林格氏液和等渗氯化钠溶液。无菌注射制剂可以是上述活性成分溶于油相的无菌注射水包油微乳,可通过局部大量注射。可将注射液或微乳注入患者的血流中;或者,按可保持本发明活性成分恒定循环浓度的方式给予溶液和微乳。为保持这种恒定浓度,可使用连续静脉内递药装置。
在一些实施例中,所述药物或药物组合物也可以是用于肌内和皮下给药的无菌注射水或油混悬液的形式。可按已知技术,用适宜的分散剂、湿润剂或悬浮剂配制该混悬液。此外,可用无菌固定油作为溶剂或悬浮介质。为此目的,可使用任何调和固定油。此外,脂肪酸也可以制备注射剂。
另一方面,本发明实施例提供了目标药物或药物组合物在制备用于抑制雄性来源的癌症细胞系的产品中的应用,所述目标药物或药物组合物的活性成分包括:拉帕替尼、帕苯达唑、布雷非德菌素A、OSI-027、5-氟尿嘧啶、SB-216763、IB-MECA和坦度替尼中的任意一种或多种的组合,所述癌症包括:食管鳞癌、肺鳞癌、食管腺癌、皮肤黑色素瘤、胃腺、管状胃腺癌、膀胱尿路上皮癌、胃粘液腺癌、结肠和直肠粘液腺癌、肺腺癌、弥漫性胃腺癌、结肠腺癌、直肠腺癌和甲状腺乳头状癌中的任意一种或几种的组合。

以下结合实施例对本发明的特征和性能作进一步的详细描述。
实施例1
1、差异表达分析
采集的数据包括663例ESCC病患者的肿瘤和配对正常组织中的突变信息(表1)。

其中,年龄≥60岁的患者388例。其中,男性256例,女性132例。此外,还有275名患者的年龄低于60岁。其中,男性182例,女性93例。但是其中只有155例有基因表达信息。
使用DESeq2软件包鉴定肿瘤和正常样本之间以及性别之间的差异表达基因。本实施例只使用仅包括p值低于0.05且log2倍数高于1的基因。在验证RNA-seq中,也使用DESeq2包鉴定了正常和治疗后的差异表达基因。热图是由ComplexHeatmap软件包生成的(图1中a,b)。性别间的差异基因表达分析按年龄组显示。与BL60年龄组相比,OV60年龄组(年龄超过60岁)的基因表达在性别之间以及肿瘤与正常组织之间的差异更大。在OV60年龄组中,基因之间的分离以及正常组织与肿瘤之间的分离是清楚的。无论是正常标本还是肿瘤标本,两种性别的表达模式都不同。
OV60组通路富集分析结果显示,性别差异显著的基因主要与G2/MDNA损伤检查点及同源性导向修复相关。在促成这些通路的所有基因中,BRIP1、H2BC11、H2BC13、H2BC15、H4C11和RFC4在大多数通路中是共有的(图1中c)。在两个年龄组中,G2/M通路表现出性别差异。此外,MTORC1是60岁以上患者的另一种性别差异通路,而MYC1和MYC2通路在60岁以下患者中具有性别差异活性。此外,MYC1和MYC2在女性和男性患者中也是年龄相关通路。有趣的是,G2/M通路仅在女性患者中呈年龄相关性(图1中d)。
2、使用的药物敏感性数据库情况
1)Genomics of Drug Sensitivity in Cancer(GDSC)
GDSC是Wellcome资助的Wellcome Sanger Institute(英国)癌症基因组项目与Massachusetts General Hospital Cancer Center(美国)分子治疗中心合作的一部分。GDSC效应可用于指导肿瘤药物的优化临床应用,对肿瘤新药开发的设计、成本和最终成功具有重要影响。在此,本实施例的分析包括805个细胞系的药物敏感性数据,并从GDC2中筛选出198种化合物。
2)The Cancer Therapeutic Response Portal(CTRP)
癌症治疗学响应门户网站(CTRP)由Board研究所治疗学中心的研究人员开发,部分由NCI癌症目标发现和开发网络赞助。CTRP将癌细胞系的遗传特征与小分子敏感性联系起来,加速了与患者匹配的癌症疗法的发现。这里,本实施例的分析包括829个癌细胞系对来自CTRP2的545种化合物的药物敏感性数据。
3)The Connective Map(Cmap)
CMap由NIH LINCs(集成细胞特征库)项目资助。CMap文库包含在多种细胞类型中筛选的约5000种小分子化合物和约3000种遗传试剂的1.5M以上mRNA表达谱。并且CMap提供一系列的web应用程序可供研究人员访问和操作数据。
3、药物重定位分析
OncoPredict是一种R软件包,用于根据细胞系筛选数据预测体内或癌症患者的药物反应和生物标志物。本实施例分别以GDSC2和CTRP2作为训练数据建立2个模型。通过这两种模型预测药物敏感性,并将前3000名的具有性别差异的表达数据作为输入数据。在两种性别之间进行t检验。CMap的Query web应用程序用于查找引起相似或相反表达的化合物。不幸的是,三个数据库预测的性别差异药物并不重叠,因为药物敏感性数据涵盖不同领域,且纳入的药物数量不同。药物重定位的工作流程见图2。
因此,根据药物出版情况、t检验中的p值以及来自CMap的药物评分对药物进行了筛选。根据3个数据库的筛选结果,筛选出8种药物:拉帕替尼、帕苯达唑、布雷非德菌素A、OSI-027、5-氟尿嘧啶、SB-216763、IB-MECA和坦度替尼,见表2。

本发明从转录组学、基因组学和表观基因组学水平证明了G2/M检查点通路对性别差异的贡献。通过评估三个数据库The Connective Map(CMap),Genomics of DrugSensitivity in Cancer(GDSC),和The Cancer Therapeutic Response Portal(CTRP)中体外药物筛选数据的总计约3500种药物,并利用机器学习模型,预测了老年女性和男性的药物反应。通过比较预测的两性之间的药物反应,选择了以上种对ESCC患者有性别差异的药物,为ESCC患者的个体化精准治疗提供了方向,有利于延长男性ESCC患者的生存时间。
