Description

Traditional Chinese medicine composition for postoperative gastric cancer and application thereof
Technical Field
The invention relates to the field of traditional Chinese medicine. More particularly, relates to a traditional Chinese medicine composition used after gastric cancer operation and application thereof.
Background
Gastric cancer is a common digestive tract malignant tumor in China, the number of new gastric cancer cases is about 108.9 ten thousand in 2020 worldwide, china accounts for about 43.9%, the coarse incidence rate of the gastric cancer in 2019 in China is increased from 30.5/10 ten thousand in 2000 to 43.1/10 ten thousand, and the health of Chinese residents is seriously threatened. Surgical resection is the preferred treatment method for gastric cancer, and except 30 to 40 percent of patients with advanced gastric cancer have no surgical chance, the rest patients can be treated by surgery. Although the recurrence rate after the early-stage gastric cancer operation is relatively low, the postoperative recurrence rate of the early-stage gastric cancer patient who receives endoscopic submucosal dissection also reaches about 12.56 percent; and the postoperative recurrence rate of the advanced gastric cancer patients is high and reaches 50-80%, the recurrence appears in 3 years after the operation, the median recurrence time is about 16.8 months, and the survival rate is less than 30% in 5 years after the operation. On the other hand, the changes of the esophageal-gastric-intestinal anatomical structure after the gastric cancer operation lead to the occurrence of a plurality of postoperative complications, the gastric hypomotility occurs, or symptoms such as hypodynamia, nausea, vomiting, acid regurgitation, heartburn, abdominal pain, constipation, abdominal distension, insomnia, short breath and the like are generated due to the postoperative recovery failure and the complication of stomatitis, intestinal obstruction and the like. The occurrence of postoperative complications brings uncomfortable symptoms to the patient after the gastric cancer operation, and the life quality of the patient is seriously affected. At present, the prevention and treatment of postoperative recurrence and metastasis of western medicine are mainly assisted chemoradiotherapy, but chemotherapy aggravates uncomfortable symptoms of patients, influences the life quality of the patients and ensures that the curative effect is not satisfied.
With the continuous and deep research of the traditional Chinese medicine for preventing and treating the gastric cancer, the characteristics and the advantages of the traditional Chinese medicine are widely concerned, a large number of clinical research observations show that the traditional Chinese medicine has advantages in preventing and treating postoperative recurrence and metastasis of the gastric cancer, improving discomfort symptoms, improving the quality of life and the like, and the comprehensive treatment participated in the traditional Chinese medicine is the advantages and the important characteristics of the traditional Chinese medicine for treating the gastric cancer.
Traditional Chinese medicine focuses on holism concept, and the whole state of the organism is corrected by adjusting yin-yang balance, so that the disease resistance of the organism is enhanced, the traditional Chinese medicine compound has multiple target points and the effect of overall regulation in treatment, and has great potential and needs to be further developed. In recent years, most of the traditional Chinese medicine researches on tumors focus on screening medicines with effective components, and development and innovation on traditional Chinese medicine compound effect researches are not deep enough. The traditional Chinese medicine differentiation treatment is guided by the overall view, the core pathogenesis causes the internal environment imbalance of the patient, the pathological process is further caused to cause relapse and transfer, uncomfortable symptoms appear, and the life quality and the long-term prognosis of the patient are influenced. Therefore, the development of a traditional Chinese medicine composition which can effectively prevent and treat postoperative recurrence and metastasis of gastric cancer and can improve the relevant symptoms of gastric cancer patients is needed.

2. The research standard is as follows:
EORTC-QLQ-STO22 and a postoperative gastrotomy syndrome evaluation scale PGSAS-45 are adopted to evaluate the clinical curative effect and the quality of life, and the results are compared and analyzed. Wherein the amount of EORTC-QLQ-STO22 comprises two contents of 5 symptom fields (dysphagia, pain, reflux symptom, limited eating, anxiety) and 4 single symptom items (dry mouth, taste change, body image, alopecia), and each score in each field is 100 points.
The PGSAS-45 post-gastrectomy syndrome assessment Scale includes 8 entries from Short Form Health Surgery (SF-8), 15 entries from the Gastrointestinal Symptom assessment Scale (GSRS), and another 22 entries for 47 surgeons to review questionnaires from 1777 patients to screen out the entry that best responds to the patient's post-gastrectomy symptoms, which can objectively and comprehensively assess the patient's complications, work and life status, and quality of life. PGSAS-45 includes 8 entries for assessing health-related quality of life, 8 entries for eating, 1 entry for work status, 3 entries for reflecting post-operative satisfaction of daily living, 25 entries for post-operative symptoms. Of these, 25 symptom-related items reflect seven aspects of symptoms, including: gastroesophageal reflux symptoms, abdominal pain, eating related disturbances, dyspepsia, diarrhea, constipation, and dumping syndrome. Wherein the items 1-8 and 38-45 are scored by a Likert 5 subscale, and the items 9-28 and 30-32 are scored by a Likert 7 subscale. Can be summarized into 10 main observation indicators: a quality of life subscale, a gastroesophageal reflux symptom subscale, an abdominal pain symptom subscale, a feeding-related discomfort symptom subscale, an dyspepsia symptom subscale, a diarrhea symptom subscale, a constipation symptom subscale, a dumping syndrome subscale, a feeding quality subscale, an under-living subscale.
3. The research results are as follows:
the evaluation results of EORTC-QLQ-STO22 and PGSAS-45 after treatment show (see tables 2-3), the traditional Chinese medicine composition is used as the main formula, and the differences before and after treatment (P is less than 0.05) of patients after treatment in the aspects of dysphagia, pain, limited eating, dry mouth, quality of life, gastroesophageal reflux, abdominal pain, eating-related discomfort, dyspepsia, dumping syndrome, unsatisfied life and the like are significant, so that the traditional Chinese medicine composition has statistical significance, and can effectively improve the common symptoms after gastric cancer operation and improve the quality of life.
