A chronic patient management monitoring system, comprising: the system comprises a client mobile terminal, a terminal processor and a doctor health management terminal, wherein the client mobile terminal is connected with the terminal processor through a mobile network, and the terminal processor is connected with the doctor health management terminal through the mobile network or a local area network;
the client mobile terminal is set as wearable equipment, and specifically comprises: the device comprises an information acquisition module, a data receiving and displaying module, a motion monitoring module and a position indicating module;
wherein:
the information acquisition module is used for acquiring health data information of a patient wearing the client mobile terminal equipment, wherein the health data information comprises blood pressure information, heart rate information and blood oxygen content information of the patient;
the data receiving and displaying module is used for receiving the information sent by the terminal processor or the doctor health management terminal and displaying the information through the wearing equipment;
a motion monitoring module for monitoring a motion state of a patient;
the position index module is used for identifying the coordinate information of the current wearable equipment and sending the coordinate information to the terminal processor;
the terminal processor is used for analyzing the health data, constructing a health database and supervising the health state of the patient, and specifically comprises the following steps: the system comprises a health data analysis module, a health state evaluation module, a health adjustment analysis module, an emergency response module, a coordinate sensing and positioning module, a file database and a health knowledge database;
wherein:
the health data analysis module is used for receiving the transmission data of the client mobile terminal, and analyzing the current body state of the patient according to the hospital health archive and the health management based on the body health characteristic data and the motion state data of the patient;
the health state evaluation module is used for analyzing and evaluating the health state of the patient based on the data of the health data analysis module, presetting a health state evaluation threshold value, and dividing 1-10 levels of health states according to the health state evaluation threshold value;
a health adjustment analysis module for analyzing the current patient health adjustment conditions including meal schedule, exercise schedule and work and rest schedule based on the health status analysis of the health status evaluation module and based on the medical health database and the historical case data;
the emergency response module is used for making emergency response to the emergent state of the patient, and the response content comprises early warning medical staff and hospital emergency medical procedures;
the coordinate sensing and positioning module is connected with the position indicating module of the client mobile terminal and used for receiving the position information sent by the position indicating module and positioning the specific position information of the current wearing equipment;
the archive database is used for storing hospital medical record archives;
the health knowledge database is used for storing health management knowledge, including health management and recovery adjustment knowledge of chronic diseases;
the doctor health management end is used for carrying out personalized independent analysis and query on patient monitoring data by a doctor, and specifically comprises the following steps: the system comprises a file management and calling module, a return visit module and an early warning receiving module;

the file management and calling module is used for calling patient file information and patient historical body state monitoring information by medical staff and looking up health state adjustment conditions formulated by the health adjustment analysis module;
the return visit module is used for setting return visit information by medical staff, including setting up the modification of a patient treatment scheme and issuing medical suggestions, and also including sending contact information including telephone contact, mail contact or visit contact;
the early warning receiving module, the early warning receiving module and the emergency response module are used for receiving the early warning information of the emergency response module.
