Abstract

背景技术
一直以来感染都是全人类健康的最严重威胁。机体免疫功能是抵抗各种外来病原菌入侵的最重要防线。在疾病状态下,患者可能出现免疫功能低下。同时,各种免疫干预治疗方案显著影响机体免疫力,从而导致条件性致病菌感染的风险明显增高。及时准确评估患者的免疫力水平对指导临床医生制定或调整治疗方案至关重要,尤其对于需要长期使用免疫抑制剂方案治疗的患者更是性命攸关的重大问题。长期以来,临床医生习惯于使用血常规、免疫球蛋白(IgG)水平等检验手段来判断患者的免疫力水平。虽然这些传统的生化检测项目能在一定程度上筛选出一部分存在明显免疫力低下的患者(如明显白细胞下降、低IgG血症等),但是绝大多数患者在发生免疫功能受损早期甚至较长一段时间内都不会发生白细胞或IgG水平下降。这是因为免疫细胞在发生凋亡(一种程序性死亡过程)等病理性损伤前已经出现明显的细胞生物学活性异常。此时,虽然免疫细胞(白细胞等)的数量还在正常范围内,但其清除外来致病菌的保护功能已经显著受损,而血常规等检测结果并不能反映出机体免疫力低下的实际情况。因此,传统的生化检测手段早已无法满足现代临床医学的发展要求。
最新的免疫学研究成果揭示了人体免疫系统是一个由免疫器官、免疫细胞和免疫分子组成的复杂而精密的网络。外周免疫细胞由多种细胞群组成,根据细胞成分和功能不同可进一步分出多种不同类型。利用细胞表面标志物分类是目前免疫细胞最主要的鉴别方法之一。例如,通过细胞表面是否表达CD3、CD4、CD8等分子,可以从外周白细胞中分别检测出CD3+CD4+CD8-和CD3+CD4-CD8+两种不同亚型的T细胞。实现这一分选目标的里程碑技术就是流式细胞术,其原理是通过荧光标记的抗体与细胞表达的靶蛋白结合,通过计算机检测细胞所产生的荧光强度得出靶蛋白的表达水平。而根据靶蛋白在不同细胞中的表达水平不同,可将一大群细胞分为若干个特性相近(靶蛋白表达量相似)的细胞亚群。目前的流式细胞术已经能够进行多通道、多参数的快速定量分析,即可以同时从一个细胞中测得多个靶蛋白的表达情况,进而将一大类细胞细分为具有不同功能的多个亚群。随着技术水平的不断发展,流式细胞仪已经成为我国大多数市级医院检验科的常规仪器设备。目前,流式细胞仪的一个重要临床应用即通过分析不同免疫细胞亚群的比例变化评估患者的免疫力水平。例如,CD4+T细胞是细胞免疫中最关键的效应细胞,在抵抗条件性致病菌(病毒、结核、真菌等)感染发挥重要作用,临床医生通过流式细胞仪检测外周血CD4+T细胞数量或CD4/CD8比值变化可有效的评估患者免疫功能水平。该方法较传统生化检测具有更高的临床指导价值。近年来,随着多参数流式细胞术推广,临床医生可通过一次性检测外周免疫细胞的多个表面标志物和细胞因子,从而系统评价免疫细胞的数量、比例以及生物学功能,对患者疾病状态、感染风险以及药物疗效等做出系统性判断。
多参数流式细胞术是在生物医学基础研究领域不断发展并在临床工作中得到初步验证的新技术,但是即使对于临床应用最广泛的CD3+CD4+T细胞分型的流式细胞学检测,仍然存在一些亟待改进的不足之处。首先,临床目前常用的CD3+CD4+T细胞流式细胞学分型检测指标局限于细胞表面标志物(CD3,CD4,CD8,CD25等),仅能反应不同亚群的T细胞数量或比例的变化,不能直接体现CD3+CD4+T细胞生物学功能的变化,无法帮助临床医生对患者的免疫力做出及时的判断;其次,CD3+CD4+T细胞表达的细胞因子种类繁多(白介素家族、干扰素家族等),且某一因子在不同细胞中的生物学功能存在重叠性和多向性,临床往往需要同时测定CD4+T细胞的多个表面标志物和细胞因子水平,并进行复杂的联合分析才能获得较为可靠的分析结果,这将导致整个评价过程消耗大量的抗体试剂和血液标本量,同时结果分析所需的人力物力要求高且耗时长;再次,使用流式细胞仪同时检测CD4+T细胞的多种(>4种)靶蛋白不可避免的存在不同通道间的荧光干扰和各种抗体间非特异性结合的影响,检验的指标越多,对流式细胞学检测的灵敏度和特异性干扰越大,限制其在临床的推广开展;最后,目前临床流式细胞学检测方法严重落后于基础研究领域,缺乏简易可靠的特异性评价CD3+CD4+T细胞病生理学功能的新型生物标志物和检验方法。
目前的科学研究成果一致证实:高表达FOXP3的CD3+CD4+T细胞发挥重要的抑制炎症作用,属于调节型免疫细胞;与之对应,发挥促炎作用的CD3+CD4+T细胞几乎不表达FOXP3的活性,属于效应型免疫细胞。同时,最新的研究发现,mTOR信号通路是决定免疫细胞生物学活性的枢纽,尤其在调控CD3+CD4+T细胞的功能表型转换(效应型或调节型)过程中发挥关键作用;其中,PS6蛋白是决定mTOR通路活性的最关键效应分子。发明人团队对以上机制进行了长期、深入的探讨,并通过细胞、动物及临床研究揭示了mTOR通路调控免疫细胞功能的分子机制。在此基础上,发明人通过流式细胞学技术实现了对人外周CD3+CD4+T细胞PS6和FOXP3表达水平的分层分析。该技术通过FOXP3的表达水平鉴别不同CD3+CD4+T细胞亚群(效应型或调节型);通过PS6的表达水平鉴别CD3+CD4+T细胞的功能活性(静止期或活跃期);通过联合分析两者的表达关系准确判断外周CD3+CD4+T细胞的数量、比例以及功能活性。为此,发明人通过大量临床实例证实了一种高效、准确且相对快捷的免疫细胞临床分型检验方法,即通过流式细胞学技术分析外周CD3+CD4+T细胞的FOXP3和PS6表达水平,根据二维等高线图或散点图不同象限及象限组合的结果对免疫细胞进行生物学功能的分型评价。
发明内容
有鉴于上述临床亟待解决的实际问题,本发明提供免疫细胞标志物及其在制备评价机体免疫功能的产品中的应用。
本发明提供以下技术方案:
标志物,包括代表mTOR通路活性的关键分子PS6蛋白以及代CD3+CD4+T细胞免疫调节功能的核因子FOXP3蛋白。
在一个具体实施例中,本发明通过对FOXP3和PS6表达水平的分层检测,揭示了GC治疗过程中效应型和调节型CD3+CD4+T细胞的数量、比例及生物学功能变化,更为精准的反映CD3+CD4+T细胞介导的免疫反应在GC治疗过程中的变化情况。本发明以为FOXP3蛋白和PS6蛋白作为新的标志物,可从多维度关联分析不同CD3+CD4+T细胞亚群的功能变化,具有高灵敏度、高特异性的优势,并能够从病生理学机制出发评价疾病和药物对免疫细胞的影响,从而填补目前已有的临床检验技术的不足,具有重大的技术创新性和临床应用价值。
本发明还提供了所述的标志物在制备如下任意一种产品中的应用:
(1)用于免疫细胞分型的产品;
(2)用于评估患者或正常人的机体免疫功能的产品;
(3)用于评价药物治疗效果或疾病预后的产品。
本发明中,所述患者包括未接受药物治疗的患者、正在接受药物治疗的患者以及药物治疗后的患者。
本发明中,所述免疫细胞为CD3+CD4+T细胞,根据所述标志物将CD3+CD4+T细胞分为八个亚群:
Q1-活跃期效应型细胞:PS6蛋白上调,FOXP3蛋白下调;
Q2-活跃期调节型细胞:PS6蛋白上调,FOXP3蛋白上调;
Q3-静止期调节型细胞:PS6蛋白下调,FOXP3蛋白上调;
Q4-静止期效应型细胞:PS6蛋白上调,FOXP3蛋白下调;
Q1+Q2-活跃期细胞:PS6蛋白表达上调;
Q2+Q3-调节型细胞:FOXP3蛋白表达上调;
Q3+Q4-静止期细胞:PS6蛋白表达下调;
Q4+Q1-效应型细胞:FOXP3蛋白表达下调。
本研究发现在CD3+CD4+T细胞中,功能活跃的细胞高表达PS6蛋白(Q1+Q2),功能静止的细胞低表达PS6蛋白(Q3+Q4);发挥调节功能(抑制炎症反应)的细胞高表达FOXP3(Q2+Q3),发挥效应功能(促进炎症反应)的细胞低表达FOXP3(Q4+Q1)。通常情况下,外周CD3+CD4+T细胞在没有受到抗原刺激时处于功能静止状态,表现为PS6蛋白低表达,即静止期调节型(Q3)和静止期效应型细胞(Q4)。当发生感染时,外源性微生物的抗原刺激诱导静止期的细胞向活跃期效应型细胞(Q1)转化促进炎症反应清除病原体;同时,机体为控制炎症反应强度避免过度的组织损伤,部分静止期的细胞开始转变为活跃的调节型细胞(Q2)。因此,感染情况下活跃期CD3+CD4+T细胞(Q1+Q2)比例显著增加。当机体免疫细胞清除致病菌后,抗原刺激消失,活化的细胞又重新转化为静止期细胞(Q3+Q4)。因此,CD3+CD4+T细胞的功能稳态是机体在发挥正常免疫功能抵抗感染且同时避免过度炎症损伤的关键因素。在疾病或药物作用的情况下,CD3+CD4+T细胞的功能稳态受到破坏,导致机体免疫力下降。通过对以上八个细胞亚群的分析检测,可以系统地、有效地判断机体在正常或疾病状态下的细胞免疫功能,并作为药物治疗效果和疾病预后的判断指标,有效性和灵敏度较高。
本发明中,所述机体免疫功能的评价指标包括:效应型CD3+CD4+T细胞和调节型CD3+CD4+T细胞的数量、比例及其生物学活性;所述疾病包括可能影响免疫系统的各种原发或继发性疾病,包括但不限于感染、自身免疫性疾病、肿瘤、代谢性疾病、血液系统疾病、中毒等;所述药物包括可能影响免疫系统的各类化学药物、生物类制剂以及中成药等。
本发明还提供一种用于免疫细胞亚群分型、评估患者或正常人的机体免疫功能或评价药物治疗效果或疾病预后的产品,包括检测本发明所述标志物的工具或试剂。
本发明还提供一种评价机体免疫功能或CD3+CD4+T细胞生物学功能的方法,包括:利用多维度流式细胞学方法联合分析免疫细胞表达PS6蛋白及FOXP3蛋白的水平,根据两种蛋白的表达水平评价CD3+CD4+T细胞的生物学功能,判定机体的免疫功能。
本发明还提供一种一种免疫细胞分型的方法,通过流式细胞学技术检测待测样本外周CD3+CD4+T细胞的FOXP3和PS6表达水平,根据二维等高线图或散点图不同象限及象限组合的结果,对CD3+CD4+T细胞进行生物学功能的分型分析;所述分型包括:
Q1-活跃期效应型细胞:PS6蛋白上调,FOXP3蛋白下调;
Q2-活跃期调节型细胞:PS6蛋白上调,FOXP3蛋白上调;
Q3-静止期调节型细胞:PS6蛋白下调,FOXP3蛋白上调;

具体实施方式
本发明提供了标志物及其在制备评价机体免疫功能的产品中的应用。本领域技术人员可以借鉴本文内容,适当改进工艺参数实现。特别需要指出的是,所有类似的替换和改动对本领域技术人员来说是显而易见的,它们都被视为包括在本发明。本发明的方法及应用已经通过较大样本量的临床实施进行验证,相关人员明显能在不脱离本发明内容、精神和范围内对本文的方法和应用进行改动或适当变更与组合,来实现和应用本发明技术。
本发明采用的试材皆为普通市售品,皆可于市场购得。
本发明中,多通道流式细胞仪检测方法包括:抽取患者外周血5ml,利用Ficoll密度梯度离心法按2000r/分钟速度离心30分钟,分离提取外周血单个核细胞(PBMC)(图1A)。70-90%的PBMC都是淋巴细胞(70-90%),其中CD3+T细胞又占了淋巴细胞中绝大部分(45-70%)。通过提取PBMC能够有效的针对CD3+CD4+T细胞进行进一步的分析,避免其他血液成分对检验结果造成影响(图1B)。由于FOXP3属于细胞核内表达因子,而PS6主要在胞浆表达,在进行抗体孵育前需要进行细胞固定和破膜处理,该步骤采用商业化的固定破膜液套装(如:BD公司)。按照常规临床流式细胞仪检测方法,以2-5x106细胞/mL的浓度重悬细胞,并根据说明书推荐参考值范围加入一抗(0.1-10μg/mL)在室温或4℃避光孵育30-60分钟。本技术方案采用4通道以上流式细胞仪检测,商业化公司可提供多种自带荧光标记的一抗或荧光素二抗,抗体选择原则为每个通道1种荧光素,各个通道之间的荧光素搭配根据抗原表达强弱合理分配并选择光谱重叠小的荧光素,可采用优化荧光素抗体搭配方案:CD3-PerCP,CD4-PE,FOXP3-FITC和PS6-APC。通过多通道流式细胞仪能够稳定、高效的检测PBMC中CD3+CD4+T细胞的FOXP3和PS6的表达水平,检测荧光信号值的设定如图1C和D所示。
本发明基于FOXP3与PS6表达水平的联合分析技术,将CD3+CD4+T细胞按照功能不同分为效应型(促进炎症反应)和调节型(抑制炎症反应)两大亚群。效应型CD3+CD4+T细胞是机体抗感染的主要效应细胞;调节型CD3+CD4+T细胞主要功能是控制机体的炎症反应程度,避免感染等致病因素导致正常组织过度炎症损伤。因此,促炎型和调节型CD3+CD4+T细胞的稳态平衡是决定机体在健康或疾病状态下免疫功能的最关键因素。如前所述,目前临床上缺乏高效检测这一关键指标的可靠灵敏的方法。发明人的研究证实,mTOR通路下游效应分子PS6是判断不同亚型CD3+CD4+T细胞生物学活性的灵敏指标,根据其表达水平的高低可将细胞分为活跃期(PS6高表达)和静止期(PS6低表达)两个状态。同时,目前的研究成果一致证实:FOXP3是调节型CD3+CD4+T细胞的最重要标志物,在效应型CD3+CD4+T细胞中极低表达。因此,根据FOXP3表达水平的高低可将CD3+CD4+细胞分为调节型(抑制炎症)和效应型(促进炎症)两种细胞。通过上述流式细胞学检测方法,根据流式细胞学检测的二维等高线图或散点图的不同象限(Q1,Q2,Q3和Q4)或象限组合可将外周CD3+CD4+T细胞分为具备不同生物学功能的八个亚群,分别为:Q1-活跃期效应型细胞:PS6高表达(>120荧光道数),FOXP3低表达(<140荧光道数);Q2-活跃期调节型细胞:PS6高表达(>120荧光道数),FOXP3高表达(>140荧光道数);Q3-静止期调节型细胞:PS6低表达(<120荧光道数),FOXP3高表达(>140荧光道数);Q4-静止期效应型细胞:PS6低表达(<120荧光道数),FOXP3低表达(<140荧光道数);Q1+Q2-活跃期细胞:PS6高表达(>120荧光道数),FOXP3不分级;Q2+Q3-调节型细胞:FOXP3高表达(>140荧光道数),PS6不分级;Q3+Q4-静止期细胞:PS6低表达(<120荧光道数),FOXP3不分级;Q4+Q1-效应型细胞:FOXP3低表达(<140荧光道数),PS6不分级(图1E)。
以上所述,本发明提供一种用于免疫细胞亚群分型、评估患者或正常人的机体免疫功能或评价药物治疗效果或疾病预后的产品;本发明中机体免疫功能的评价指标包括以上所述八个亚型的CD3+CD4+T细胞的数量比例及其生物学活性评价。
本研究发现通常情况下,外周成熟的CD3+CD4+T细胞在没有受到抗原刺激时处于功能静止状态(Q3+Q4)。当发生感染时,外源性微生物的抗原刺激诱导静止期的细胞向效应型细胞(Q1)转化;同时,机体为控制炎症反应强度避免过度的组织损伤,部分细胞开始转变为活跃的调节型细胞(Q2),因此感染情况下活跃期细胞(Q1+Q2)比例显著增加。当机体免疫细胞清除致病菌后,活化的CD3+CD4+T细胞又重新转化为静止期细胞(Q3+Q4)。因此,CD3+CD4+T细胞的功能稳态是机体发挥正常免疫功能抵抗感染且同时避免过度炎症损伤的关键因素。在疾病或药物作用的情况下,CD3+CD4+T细胞的功能稳态受到破坏,导致机体免疫力下降。通过本技术可以系统有效的判断机体在正常或疾病状态下的细胞免疫功能,并作为药物治疗效果和疾病预后的判断指标。该技术的灵敏度、有效性以及相关分子机制已在临床实施中得到验证。下面结合具体临床实施案例,进一步阐述本发明:
实施例1
糖皮质激素(Glucocorticoid,GC)是医学领域具有里程碑意义的代表性药物,在所有临床专科中均广泛应用,对结缔组织疾病、炎症、过敏等系统性疾病的治疗具有不可替代的作用。以GC治疗肾病综合征(Nephrotic syndrome,NS)为例,GC的常规治疗方案要求起始剂量大、维持疗程长和减量过程慢。这虽然提高了GC疗效,但同时也容易因为GC累积剂量大而出现显著的药物不良反应。其中,由于GC广泛的免疫抑制作用而引起的机体抵抗力低下,最终导致重症感染危及患者生命是最严重的并发症。这在NS患者中时常发生。单一的减少GC剂量和疗程是目前唯一可行的降低风险的方法,但这又可能降低了GC治疗疾病的效果。因此,临床医生有必要在GC治疗过程中密切监测患者CD3+CD4+T细胞的免疫表型变化。
发明人团队对接受常规GC方案治疗的NS患者进行队列研究发现,通过上述多通道流式细胞检测技术联合分析CD3+CD4+T细胞FOXP3及PS6的表达水平能够准确评价患者的免疫功能。临床医生通过结果可以判断GC治疗NS患者发生感染与否情况下免疫细胞的功能表型发生显著变化(图2A),其中:无激素情况下感染能够显著促进Q1-活跃期效应型细胞(图2B),这有助于清除外源性致病菌;而接受GC治疗的患者机体的这一保护机制显著减弱;无激素情况下,感染轻微增加Q2-活跃期调节型细胞以控制过度炎症反应,但接受GC治疗的患者在发生感染时活跃期调节型细胞比例显著增加(图2C),提示免疫细胞促炎/消灭细菌的功能被明显抑制;GC或感染对于Q3-静止期调节型细胞的影响较轻微(图2D),但对Q4-静止期效应细胞影响显著(图2E)。通过进一步联合分析可以发现,虽然一般情况下感染诱导活跃期细胞(Q1+Q2)比例增加(图2F),但接受GC治疗的患者在发生感染时其静止期细胞(Q3+Q4)向调节型细胞(Q2+Q3)而不是向效应型细胞(Q4+Q1)转化(图2G-I),导致GC治疗患者发生感染时调节型细胞增加而效应型细胞减少,即抵抗感染的免疫功能受损。以上临床数据证实,本发明技术能够高效评价接受GC治疗的患者CD3+CD4+T细胞生物学功能活性的变化及其抵抗感染的免疫力水平。
实施例2
为进一步验证以上临床结果的分子机制,发明人检测了不同情况下患者外周CD3+CD4+T细胞的抗炎因子(IL10)和炎症因子(IFNG)表达水平,研究结果证实:GC主要是通过增加抗炎因子IL10的表达水平影响感染情况下细胞的免疫功能(图3A),其对炎症因子IFNG表达干扰较小(图3B)。发明人进一步分离GC治疗不同阶段的患者CD4+T细胞进行体外抗原刺激实验,同样证实GC治疗的CD3+CD4+T细胞在抗原刺激后分泌更高水平的IL10;更为重要的是,IL10的高表达状态在8个月的GC减量过程中持续存在(图3C)。目前的研究结果一致认为:高表达FOXP3的调节型CD4+T细胞是产生IL10的主要细胞。因此,以上体内和体外研究结果进一步从细胞分子水平论证了基于FOXP3和PS6分层联合分析技术评价CD3+CD4+T细胞免疫功能的分子机制。更重要的是,本发明在GC治疗不同时期以及感染与否情况下均能直观、高效的评价CD3+CD4+T细胞的功能活性变化。
以上仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员来说,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。

8.评价CD3+CD4+T细胞生物学功能的方法,其特征在于,利用多维度流式细胞学方法联合分析免疫细胞表达PS6蛋白及FOXP3蛋白的水平,根据两种蛋白的表达水平评价CD3+CD4+T细胞的生物学功能。
9.一种免疫细胞分型的方法,其特征在于,通过流式细胞学技术检测待测样本外周CD3+CD4+T细胞的FOXP3和PS6表达水平,根据二维等高线图或散点图不同象限及象限组合的结果,对CD3+CD4+T细胞进行生物学功能的分型分析;所述分型包括:
