不考虑癌症类型,而把具有相同基因突变的病人归为一类,进行精准治疗,已是行业共识。跨癌种靶向NTRK基因变异病人的药物已经有多款上市药物,并且有更多的药物进入临床实验阶段。例如拉罗替尼(larotrectinib),其作为首例泛癌种靶向药,用于治疗携带NTRK基因融合的局部晚期或转移性实体瘤的成人和儿童患者。拉罗替尼主要通过与细胞内TRKB(NTRK2编码)的ATP位点竞争性结合,进而抑制TRK的催化活性和自磷酸化,阻断下游的信号通路传导,从而发挥抗肿瘤的作用。
NTRK基因融合是当前最明确的致癌原因,现有的泛癌种靶向药主要针对该靶点-通路。显然地,寻找更多的致癌靶点,并明确其相关机理是精准医疗能够大规模进行的基本前提。
MUC5B(粘蛋白5B)是粘蛋白家族成员,其在多种肿瘤中过表达,被报道与肿瘤的EMT相关(Journal of Clinical Oncology 2020 38:15_suppl,e16562-e16562)。但是其突变在肿瘤发生中的功能及与药物药效的关系尚未被报道。
综上所述,寻找更多的精准治疗的作用靶点,最终实现精准的个体化治疗是有效的治疗策略。当前,对获得潜在的实现与基因突变相关的肿瘤的治疗的化合物存在需求。
发明内容
本发明的技术方案是在以下研究成果的基础上提出的:本发明人发现与MUC5B野生型肿瘤细胞系相比,具有MUC5B基因突变的肿瘤细胞系对mTOR C1/2抑制剂ATG-008(化学结构见以下式I)高度敏感。ATG-008在MUC5B基因突变的小鼠异位移植肿瘤模型中显示出了出乎意料的药效。
因此,本发明的目的是提供mTOR抑制剂在制备用于预防和/或治疗肿瘤的药物中的用途。本发明还提供了包含mTOR抑制剂的预防和/或治疗肿瘤的药物组合物。
一方面,本发明提供了mTOR抑制剂在制备用于预防和/或治疗具有MUC5B基因突变的肿瘤的药物中的用途。
根据本发明所述的用途,其中所述mTOR抑制剂可以选自西罗莫斯、依维莫斯、Onatasertib(ATG-008)、雷帕霉素、Sapanisertib(TAK-228)或Vistusertib(AZD2014)中的一种或多种。

与现有技术相比,本发明具有以下有益效果:
本发明的发明人发现,与MUC5B野生型肿瘤细胞系相比,具有MUC5B基因突变的肿瘤细胞系对mTOR抑制剂,特别是mTORC 1/2抑制剂ATG-008高度敏感。ATG-008在MUC5B基因突变的小鼠异位移植肿瘤模型中显示了极佳的药效。因此,本发明mTOR抑制剂,例如ATG-008用于具有MUC5B基因突变的肿瘤的精准治疗提供了理论和实验依据。

实施例1ATG-008对多种肺癌细胞的抑制作用
本发明人在多种肺癌细胞系中对ATG-008的抑瘤效果进行了检测,结果发现相对于MUC5B野生型细胞,ATG-008能有效地抑制MUC5B基因突变的肺癌细胞的增殖、自我更新和侵袭能力。方案如下:
试验方法:
1.细胞培养
将细胞复苏并培养于各自的培养液中。
2.细胞铺板(第一天):
2.1收获处于对数生长期的细胞并采用细胞计数仪进行细胞计数。用台盼蓝排斥法检测细胞活力,确保各细胞系活力在80%以上。
2.2用培养液稀释和调整细胞浓度,添加90μL细胞悬液至96孔细胞板中(包括药物处理当天的细胞对照T0),使细胞密度达到指定的浓度。
2.3 96孔板中的细胞置于37℃、5%CO2和95%湿度条件下培养过夜。其中,具体培养步骤均按照ATCC的产品说明书进行。
3.T0细胞活力读数(第二天):
3.1在对照细胞培养板中每孔加入10μL培养液。
3.2将CellTiter-Glo试剂和细胞培养板置于室温下平衡30分钟。
3.3每孔加入50μL CellTiter-Glo试剂。
3.4在定轨摇床上振动5分钟使细胞充分裂解。
3.5将细胞培养液置于室温下平衡20分钟。
3.6用EnVision读取化学发光值。
4.药物处理(第二天):
4.1用溶剂(0.5%(w/v)CMC/0.25%(v/v)Tween80)溶解被测化合物ATG-008形成储存液。
4.2在已接种细胞的96孔板中每孔加入10μL溶液,每个细胞浓度设置三个复孔。被测化合物最高浓度为31.6μM,9个浓度,3.16倍稀释。
4.3将已加药的96孔板中的细胞置于37℃、5%CO2、95%湿度条件下继续培养72小时。
5.T3细胞活力读数(第五天)
5.1将CellTiter-Glo试剂和药物处理的细胞培养板置于室温下平衡30分钟。
5.2每孔加入50μL CellTiter-Glo试剂。
5.3在定轨摇床上振动5分钟使细胞充分裂解。
5.4将细胞培养液置于室温下平衡20分钟。
5.5用EnVision读取化学发光值。
数据处理:
使用GraphPad Prism 5.0软件分析数据,利用非线性S曲线回归来拟合数据得出剂量—效应曲线,并由此计算IC50值。
细胞存活率(%)=(Lum待测药-Lum培养液对照)/(Lum细胞对照-Lum培养液对照)×100%,其中Lum细胞对照-Lum培养液对照设为100%。
扩增倍数=(第五天的Lum待测药-Lum培养液对照)/(第二天的Lum待测药-Lum培养液对照)
生物标志物分析:
使用上述细胞生长抑制曲线,通过下面公式计算线下面积(AUC),作为药物敏感度的指标,AUC越小,药物敏感度越高。
其中,Inhibition%代表抑制率;Bottom代表最低读数;Top代表最高读数;EC50代表半数效应浓度;HillSlope代表曲线坡度。
在所有细胞系中,AUC最小的十种细胞被定义为ATG008敏感细胞(Sensitive),AUC最大的十种细胞被定义为ATG008不敏感细胞(Insensitive)。使用Wilcoxon rank sumtest统计模型对比分析敏感细胞与不敏感细胞的基因突变,将与ATG-008药效相关性最大的突变进行排序。
实验结果:
1.ATG-008对多种肺细胞体外生长具有抑制作用,半数生长抑制浓度IC50和AUC如表1、图1和图2所示。根据AUC将细胞系分为敏感细胞和不敏感细胞。

基因突变分析显示,包括MUC5B基因突变在内,多个基因突变和ATG-008的药效显著相关。

实施例2ATG-008显著抑制MUC5B基因突变的肿瘤
ATG-008在小鼠异位移植肿瘤模型中显著抑制携带MUC5B基因突变的肿瘤的生长,药效优于MUC5B野生型的肿瘤模型。

2.细胞培养及转接
实验动物:BALB/c Nude小鼠,雌性,购自江苏集萃药康生物科技有限公司,生产许可证号:SCXK(苏)2018-0008。饲养环境:SPF级。
实验动物饲养室的环境条件:实验动物均饲养于恒温恒湿的独立通风盒内,饲养室温度20-26℃,湿度40-70%,10-20次/小时换气,昼夜明暗交替时间12h/12h;持续供给钴60放射灭菌鼠全价颗粒饲料,不限量自由摄取,饮用自来水(高压蒸汽灭菌后使用),饮水瓶不间断供水,自由摄取。饲养鼠盒是聚砜鼠盒,高压灭菌后使用,规格为325mm×210mm×180mm;垫料是高压灭菌玉米芯,每盒4只动物,笼卡上标明IACUC批准号、实验编号、实验开始时间、课题负责人、实验人员、动物来源、组别和动物号等;实验动物打耳号进行标记。
每种细胞培养在如下特定的培养液中进行。收集指数生长期的细胞,模型SK-MES-1和NCI-H526的细胞用PBS重悬至适合浓度后用于小鼠皮下肿瘤接种,模型NCI-H82和NCI-H209的细胞用PBS重悬至适合浓度后与matrigel 1:1混合用于小鼠皮下肿瘤接种,具体如表3所示。

3.动物造模和随机分组
每个模型,每只雌性BALB/c Nude小鼠右前侧背部皮下接种细胞。下表4提供具体分组信息。

4.实验观察和数据收集
肿瘤接种后,常规监测包括肿瘤生长及治疗对动物正常行为的影响。具体内容有实验动物的活动性,摄食和饮水情况,体重增加或降低情况,眼睛、被毛及其它异常情况。实验过程中观察到的临床症状均记录在原始数据中。肿瘤体积计算公式:肿瘤体积(mm3)=1/2×(a×b2)(其中a表示长径,b表示短径)。实验中使用StudyDirectorTM(版本号3.1.399.19,供应商Studylog System,Inc.,S.San Francisco,CA,USA)软件收集数据,包括肿瘤的长短径的测量和动物体重的称量。原始数据由天平和游标卡尺测量后直接导入软件,数据的任何变动都将被记录在此软件中。开始给药后,每周测量两次小鼠的体重和肿瘤大小。
统计分析:
为比较不同处理组某天的肿瘤体积,首先我们使用Bartlett检验来验证所有组间的方差齐性假设。当Bartlett检验的p值不小于0.05时,单因素方差分析将被用于检验所有组均值是否相等。若单因素方差分析的p值小于0.05,我们将用Tukey HSD检验进行所有组之间两两比较,或用Dunnett’s t检验进行每个治疗组和对照组之间的两两比较。当Bartlett检验的p值小于0.05时,Kruskal Wallis检验将被用于检验所有组的中位数是否相等。若Kruskal Wallis检验的p值小于0.05,我们将用Conover检验进行所有组之间两两比较或每个治疗组和对照组之间的两两比较,并根据多重检验的组数进行相应的p值校正。
此外,基于探索性数据分析的目的,我们进行了任意时间点所有组之间的两两比较。因为这种比较只使用了特定时间点待比较两组的肿瘤体积数据,所以不需要进行多重检验校正。首先我们使用Bartlett检验来验证两组之间的方差齐性假设,当Bartlett检验的p值不小于0.05时,我们用Welch’s t检验来比较两组均值是否相等;当Bartlett检验的p值小于0.05时,我们用Mann Whitney U检验来比较两组中位数是否相等。
所有的统计分析和图形绘制都在R语言环境中进行(3.3.1版)。在非特别说明的情况下,所有检验均为双尾检验,p值小于0.05时被认为具有统计显著性。
5.实验结果:
测试药ATG-008在4个CDX肺癌模型中的抗肿瘤作用研究结果:
本次实验评价测试药物ATG008在4个CDX皮下异种移植模型NCI-H82、NCI-H209、SK-MES-1和NCI-H526中的BALB/c Nude小鼠动物模型中的抗肿瘤作用。
本次实验中,对照组和测试药组的小鼠均未出现明显的整体体重下降。ATG-008在各肿瘤模型中均有抗肿瘤生长效果(表5、图4-7)。
表5显示ATG-008在MUC5B基因突变肿瘤模型中的药效优于在MUC5B野生型模型中的药效。图4显示在MUC5B突变的NCI-H82肺癌模型中,使用本发明的mTOR抑制剂ATG-008和对照分别处理小鼠后,小鼠的肿瘤体积的生长曲线(注:数据以“平均值±标准误差”表示)。图5显示在不具有MUC5B基因突变的NCI-H209肺癌模型中,使用本发明的mTOR抑制剂ATG-008和对照分别处理小鼠后,小鼠的肿瘤体积的生长曲线(注:数据以“平均值±标准误差”表示)。图6显示在MUC5B突变的SK-MES-1肺癌模型中,使用本发明的mTOR抑制剂ATG-008和对照分别处理小鼠后,小鼠的肿瘤体积的生长曲线(注:数据以“平均值±标准误差”表示)。图7显示在不具有MUC5B基因突变的NCI-H526肺癌模型中,使用本发明的mTOR抑制剂ATG-008和对照分别处理小鼠后,小鼠的肿瘤体积的生长曲线(注:数据以“平均值±标准误差”表示)。
由上述结果可知,平均肿瘤生长抑制率(TGI)为67.09%(突变)和28.28%(野生)。结果证明,MUC5B基因突变的肿瘤模型对mTOR抑制剂更为敏感。

实施例3MUC5B基因突变在多种肿瘤中的发生频率分析
使用TCGA数据库分析,证明MUC5B在多种肿瘤类型中出现频率较高,如图8所示。从高到低依次是黑色素瘤、子宫内膜癌、结直肠癌、食管腺癌、宫颈癌、非小细胞肺癌、B细胞淋巴瘤、头颈肿瘤、胆管癌、膀胱及尿路癌、胃癌、食管鳞癌、肝癌、肉瘤、乳腺癌、脑胶质瘤和肾癌等。
这些数据说明本申请的mTOR抑制剂ATG-008可以用于各种与MUC5B基因突变相关的肿瘤的精准治疗,从而进一步提高临床治疗效果。
最后应说明的是,以上实施例仅用以说明本发明的技术方案而非限制。尽管参照实施例对本发明进行了详细说明,本领域的普通技术人员应当理解,对本发明的技术方案进行修改或者等同替换,都不脱离本发明技术方案的范围。均应涵盖在本发明的权利要求范围当中。

4.根据权利要求1至3中任一项所述的用途,其中所述具有MUC5B基因突变的肿瘤选自黑色素瘤、子宫内膜癌、结直肠癌、食管腺癌、宫颈癌、肺癌如非小细胞肺癌、B细胞淋巴瘤、头颈肿瘤、胆管癌、膀胱及尿路癌、胃癌、食管鳞癌、肝癌、肉瘤、乳腺癌、脑胶质瘤或肾癌中的一种或多种;
优选地,所述具有MUC5B基因突变的肿瘤为肺癌,更优选为非小细胞肺癌。
5.根据权利要求1至4中任一项所述的用途,其中所述预防和/或治疗肿瘤为抑制肿瘤细胞生长、迁移和/或侵袭,和/或抑制肿瘤生长和/或定殖。
6.一种用于预防和/或治疗具有MUC5B基因突变的肿瘤的药物组合物,其包含预防和/或治疗有效量的mTOR抑制剂,以及用于检测MUC5B基因突变的试剂。
8.根据权利要求6或7所述的药物组合物,其中所述MUC5B基因突变选自以下与野生型MUC5B基因相比的突变中的一种或多种:
g.chr11:1267232C>G;g.chr11:1267928C>T;g.chr11:1271857T>A;g.chr11:1272810G>A;g.chr11:1267206C>A;g.chr11:1271781G>A;g.chr11:1263045C>A;g.chr11:1268918C>A;g.chr11:1268708C>G;
优选地,所述MUC5B基因突变为以下与野生型MUC5B基因相比的突变:
g.chr11:1263045C>A或g.chr11:1268918C>A。
9.根据权利要求6至8中任一项所述的药物组合物,其中所述具有MUC5B基因突变的肿瘤选自黑色素瘤、子宫内膜癌、结直肠癌、食管腺癌、宫颈癌、肺癌如非小细胞肺癌、B细胞淋巴瘤、头颈肿瘤、胆管癌、膀胱及尿路癌、胃癌、食管鳞癌、肝癌、肉瘤、乳腺癌、脑胶质瘤或肾癌中的一种或多种;
优选地,所述具有MUC5B基因突变的肿瘤为肺癌,更优选为非小细胞肺癌。
10.根据权利要求6至9中任一项所述的药物组合物,其中所述预防和/或治疗肿瘤为抑制肿瘤细胞生长、迁移和/或侵袭,和/或抑制肿瘤生长和/或定殖。
