Abstract
本发明提供了一种直肠组织氧饱和度监测仪,包括电极、球囊和体外监控部件,其中所述电极包括近红外光谱电极,所述电极贴附在所述球囊的内壁,并且所述近红外光谱电极与所述体外监控部件通过线缆连接。本发明的监测仪可以有效地预警局部组织的缺血缺氧现象,可以干预局部组织的氧供需失衡,进而防止围术期肠道因缺血缺氧导致术后肠梗阻、肠屏障损害相关的内毒素血症等并发症,对加速术后肠功能恢复和快速摄食摄饮具有重要临床价值。
Description
技术领域
本发明涉及一种医疗器械,特别涉及近红外光谱连续直肠组织氧饱和度监测仪。
背景技术
肠道组织在围术期应激下极易出现微循环紊乱和组织氧供需失衡,使术后肠功能恢复延迟以及因肠道缺血缺氧导致肠道屏障功能损害,导致内毒素入血以及全身炎性反应,延迟术后康复进程。
围术期肠道组织微循环障碍和肠道组织氧供需失衡是影响手术患者术后并发症的关键因素。而且,肠道组织氧供需失衡也是影响胃肠手术术后吻合口漏发生的关键因素。因此,加强动态实时监测和管理对于降低此类外科并发症至关重要。虽然已有设备可用于胃腔内弥散性二氧化碳分压和pHi(胃肠粘膜内酸度)的监测,但该设备为间断测定方法,且为间接测定方法,存在监测延迟和受到多种外界因素影响的缺陷,导致其难以满足临床实际需要。
因此,当前仍然缺乏临床可用的连续、实时、有效监护肠道组织围术期氧供需平衡的设备和相关耗材,用于监测围术期肠应激状态以及肠道组织血流灌注及氧供需平衡;加强动态实时监测和管理,降低外科并发症和肠屏障功能障碍相关的内毒素血症。
发明内容
本发明提供一种直肠组织氧饱和度监测仪,包括电极、球囊和体外监控部件,其中所述电极包括近红外光谱电极,所述电极贴附在所述球囊的内壁,并且所述近红外光谱电极与所述体外监控部件通过线缆连接。
根据本发明的实施方案,近红外光谱电极可以选自单束近红外谱发射和接受端感应电极。例如,可以将单束近红外谱发射和接受端感应电极整合在电极片上。
根据本发明的实施方案,所述电极片的尺寸为2cm×4cm。
根据本发明的实施方案,所述近红外光谱电极选自近红外谱氧饱和度监测电极。
根据本发明的实施方案,所述电极胶粘在球囊的内壁上。

根据本发明的实施方案,所述球囊为可充气球囊,其可以包含囊体,所述囊体内部设置有空腔,空腔内部空间形成充气空间。优选地,所述充气空间根据所充入气体的体积而发生变化,优选在充气后形成与肠道内壁贴合的形状。
根据本发明的实施方案,多个所述传感器位于所述球囊的直型端。在检测仪工作时,传感器所处区域为平缓的平面,以保证传感器与局部组织充分的紧密粘贴,有利于更有效和准确地监测组织氧饱和度的变化。
根据本发明的实施方案,所述导管内嵌所述线缆。
根据本发明的实施方案,所述球囊包含多个充气口,多个所述充气口均在所述导管上,且位于所述球囊内部。所述充气口可在需要时用于自动向球囊内部通入气体,并通过监控部件确保一定囊内压力,并保持恒定,确保可充气球囊与直肠内壁紧密贴附,但不造成直肠内壁受压造成缺血;避免出现压力过高造成组织壁承压过大的局部组织缺血,或避免出现压力过小时传感器没有和组织壁充分粘贴;监测结束前排出球囊内部气体,便于取出监测套件,所述监测套件包括电极和球囊。

(1)使用上述直肠组织氧饱和度监测仪的电极采集直肠组织氧饱和度数据;
(2)将采集到的数据通过体外监控部件进行数据处理;
(3)将处理后的数据通过显示部件显示。
本发明还提供所述直肠组织氧饱和度监测仪用于处理直肠组织氧饱和度数据的用途。
有益效果:
(1)本发明设备通过单束近红外谱发射和接受端感应电极,整合在2cm×4cm的电极片上,电极片使用缆线与体外设备相连,整个电极片贴敷在可充气的球囊内侧壁上,保证与肠道内壁紧密贴合,确保数据准确性,并防止外界因素干扰,连续、实时动态监测肠道组织氧饱和度;能够实时、动态、连续监测肠道组织氧供需平衡。
(2)本发明的单束近红外光谱监测电极设计,具有简洁、实用特点.
(3)本发明中监测电极设计和保护套件确保监测过程可靠,能够避免外界干扰;对防止术后内毒素血症、推动围术期肠道微循环功能监测、加速术后康复、预防术后严重并发症(如胃肠道手术术后吻合口漏发生)。
(4)本发明能够整合肠道中心体温监测电极,避免重复监测,节约耗材费用。
(5)本发明的监测仪,可以早期预警局部组织缺血缺氧,并及时干预局部组织的氧供需失衡,对术后快速肠功能恢复并降低因肠道缺血性肠屏障功能损害相关的内毒素血症发生,具有非常重要的临床价值。

实施例1
如图1-3所示的一种直肠组织氧饱和度监测仪,包括电极8、球囊2和体外监控部件7,其中所述电极8包括近红外光谱电极,所述电极8贴附在所述球囊2的内壁,并且所述近红外光谱电极与所述体外监控部件7通过线缆连接。
近红外光谱电极选自单束近红外谱发射和接受端感应电极,可以将单束近红外谱发射和接受端感应电极整合在电极片上。
所述电极胶粘在球囊的内壁上。
所述近红外光谱电极选自近红外谱氧饱和度监测电极。
所述监测仪还包括温度感应器4。
所述球囊在充气检测时,其中心具有一直型端。

1.一种直肠组织氧饱和度监测仪,其特征在于,包括电极、球囊和体外监控部件,其中所述电极包括近红外光谱电极,所述电极贴附在所述球囊的内壁,并且所述近红外光谱电极与所述体外监控部件通过线缆连接;
优选地,所述球囊为可充气球囊。
2.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,近红外光谱电极可以选自单束近红外谱发射和接受端感应电极;例如,可以将单束近红外谱发射和接受端感应电极整合在电极片上。

优选地,另设一根导管,所述导管的一端与空气相连,另一端贯穿所述球囊,与温度感应器相连。
4.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,所述球囊在充气检测时,其中心具有一直型端。
5.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,所述球囊内还设有多个传感器,所述传感器通过线缆与温度感应器和外部的监控部件相连。
优选地,所述传感器设有2个,2个传感器在所述球囊内壁上对称设置。
优选地,多个所述传感器位于所述球囊的直型端。
6.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,所述导管内嵌所述线缆。
7.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,所述球囊包含多个充气口,多个所述充气口均在所述导管上,且位于所述球囊内部。
所述球囊包含2个充气口。
优选地,2个所述充气口在导管上对称设置。
8.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,球囊内设置多个压力感应器,所述压力感应器通过线缆与外部的监测部件相连。
优选地,所述压力感应器设有2个。
9.根据权利要求1所述的直肠组织氧饱和度监测仪,其特征在于,所述体外监控部件包括数据传输部件、数据处理部件和显示部件。
优选地,所述近红外光谱电极与所述体外监控部件通过线缆连接,其中所述线缆可以选自电线、数据传输线中的一种或多种。
优选地,将监测套件至直肠适当位置后,与体外监控部件相连接。
优选地,所述直肠组织氧饱和度监测仪选自近红外光谱连续直肠组织氧饱和度监测仪。
10.权利要求1-9任一项所述的直肠组织氧饱和度监测仪检测到的数据的连续处理方法,其特征在于,包括:
(1)使用上述直肠组织氧饱和度监测仪的电极采集直肠组织氧饱和度数据;
(2)将采集到的数据通过体外监控部件进行数据处理;
(3)将处理后的数据通过显示部件显示。
优选地,权利要求1-9任一项所述直肠组织氧饱和度监测仪用于处理直肠组织氧饱和度数据的用途。
