Abstract

技术领域
本申请涉及生物医药领域,具体的涉及一种降低细胞因子释放综合症的CAR-T细胞。
背景技术
嵌合抗原受体T(CAR-T)细胞疗法已成为治疗癌症的一种新颖的、潜在的革命性疗法。然而,CAR-T细胞疗法的广泛应用由于出现了潜在的致命毒副作用而受到限制。这包括在CAR-T治疗期间出现的细胞因子释放综合症(CRS)和神经毒性。在接受CART19细胞治疗的患者中,有高达50%的患者会出现3级或更高的CRS或神经毒性,并且已经有数起死亡报告。目前还没有预防CRS的有效疗法。
在机制上,CRS被认为是由激活的CAR T细胞最初释放的促炎症细胞因子如γ干扰素(IFN-γ)和肿瘤坏死因子α(TNF-α)所介导。这些细胞因子导致旁观者免疫细胞和内皮细胞的激活,这些细胞因子反过来激活更多的免疫细胞,最终形成细胞因子风暴。IFN-γ在CRS的病理生理学中起着重要作用。IFN-γ可刺激巨噬细胞产生过量的细胞因子,如白细胞介素6(IL-6)、TNF-α和白细胞介素10(IL-10)。
发明内容
本申请目的在于提供一种降低IFN-γ在活化的CAR-T细胞中表达的可行方法,并有助于防止CRS的发生,从而提高CAR-T疗法的治疗效果。
本申请目的还在于提供一种降低IFN-γ表达的CAR-T细胞的制备方法,有助于防止CRS的发生,并且对CAR分子的表达没有影响,对CAR-T细胞的杀伤效果几乎没有影响。
一方面,本申请提供一种分离的核酸分子,其用于在细胞内抑制IFN-γ基因的表达,所述核酸分子包反义含寡核苷酸,其中所述反义寡核苷酸包含与SEQ ID NO:1-6中任一项所示核苷酸序列相差不超过3个核苷酸的至少15个连续核苷酸。
在某些实施方式中,其中所述反义寡核苷酸包含与编码IFN-γ的mRNA的至少一部分基本上互补的互补区域。
在某些实施方式中,其中所述互补区域的长度小于30个核苷酸。

术语定义
在本申请中,“干扰素-γ(Interferon gamma、Interferon-γ、IFN-γ)”通常是指是水溶性二聚体的细胞因子。是II型干扰素的唯一成员。它主要由自然杀伤细胞(NK)和自然杀伤T细胞(NKT)细胞分泌,在固有免疫中发挥作用;在抗原特异性免疫过程中,由CD4Th1和CD8细胞毒性T细胞分泌。

在本申请中,术语“分离的”或“纯化的”通常是指至少部分从在其天然状态中通常与其结合的其他分子中分离的分子(例如多肽、核酸等)。术语“分离的核酸分子”是指从5’至3’末端阅读的脱氧核糖核苷酸或核糖核苷酸碱基的单链或双链聚合物(例如,本申请提供的IFN-γsiRNA核酸序列)或其类似物,其已与至少约50%的当从来源细胞分离总核酸时与核酸分子一起被天然发现的多肽、肽、脂质、糖类、多核苷酸或其他材料分离。在一些实施方式中,分离的核酸分子大体上不含任何其他污染性核酸分子或在核酸的天然环境中发现的可干扰其在多肽产生中的用途或其治疗性、诊断性、预防性或研究用途的其他分子。

在某些方式中,所述载体包括病毒载体,可以利用病毒载体将本申请的核酸分子引入细胞。此类病毒载体包括,例如重组逆转录病毒、腺病毒、腺伴随病毒和单纯疱疹病毒-1。通常将逆转录病毒载体和腺伴随病毒载体理解为体内转移外源性基因,特别是进入人体而选用的重组基因递送系统。或者,它们可以用于将外源基因离体引入T细胞。这些载体将基因有效递送入T细胞,并且转移的核酸被稳定整合到宿主细胞的染色体DNA中。

在另一方面,免疫疗法相关毒性包括脑疾病、损伤或功能障碍。在相关方面,脑疾病、损伤或功能障碍包括CAR-T细胞相关NT或CAR-T细胞相关脑病综合征(CRES)。在相关方面,抑制或降低脑疾病、损伤或功能障碍的发生率包括减轻受试者的头痛、谵妄、焦虑、震颤、癫痫发作活动、困惑、觉醒度改变、幻觉、言语障碍、共济失调、失用症、面神经麻痹、运动无力、癫痫发作、非抽搐EEG癫痫发作、意识水平改变、昏迷、内皮活化、血管渗漏、血管内凝血或其任何组合。在另一方面,免疫疗法相关毒性包括CAR-T诱导的细胞因子释放综合征(CRS)。在相关方面,抑制或降低CRS的发生率包括减少或抑制但不限于高烧、肌痛、恶心、低血压、低氧或休克或其组合。在相关方面,免疫疗法相关毒性是危及生命的。
在某些实施方式中,其中所述免疫疗法相关毒性是选自细胞因子释放综合征、神经毒性、神经炎症或其组合。
调节T细胞的方法
另一方面,本申请提供一种抑制细胞中IFN-γ表达的方法,该方法包括:将细胞与本申请所述的核酸分子接触。

治疗方法
另一方面,本申请提供一种免疫疗法,包括向有此需要的受试者施用有效量的经本申请所述的核酸分子修饰的嵌合抗原受体表达性T细胞(CAR-T细胞)、T细胞受体修饰的T细胞(TCR-T)、肿瘤浸润性淋巴细胞(TIL)、嵌合抗原受体修饰的天然杀伤细胞(CAR-NK)或树突状细胞或其任何组合。
细胞因子风暴包括由细胞因子与白细胞之间的正反馈回路组成的免疫反应,其中各种细胞因子的水平均很高。术语“细胞因子风暴”可以与具有所有相同性质和含义的术语“细胞因子级联”和“高细胞血症”互换使用。在一些实施例中,细胞因子风暴的特征在于IL-2释放和淋巴增殖。细胞因子风暴导致潜在的危及生命的并发症,包含心脏功能障碍、成人呼吸窘迫综合征、神经系统毒性、肾脏和/或肝功能衰竭以及弥散性血管内凝血。升高的IFN-γ水平既可以作为CRS的预测生物标志物,又可以是其严重程度的指标。
如上所述,CAR-T细胞疗法目前受到危及生命的神经毒性和CRS的风险的限制。尽管进行了积极的管理,但是所有CAR-T响应者都经历了某种程度的CRS。高达50%的用anti-CD19 CAR-T治疗的患者具有至少3级CRS或神经毒性。
另一方面,本申请提供一种用于治疗和/或预防受试者的免疫疗法相关毒性的方法,所述方法包括向有此需要的受试者施用CAR-T细胞,所述CAR-T细胞通过本申请所述的核酸分子修饰使所述IFN-γ基因表达降低(如基因沉默)。
在一些实施方式中,已经将反义寡核苷酸或siRNA转染入T细胞(如CAR-T细胞),该方法包括将转染的T细胞给予受试者。可以通过电穿孔方法将反义寡核苷酸转染或siRNA引入T细胞。

在一些实施例中,IFN-γ沉默的或基因敲除的CAR-T细胞的施用与重组IFN-γ拮抗剂一起进行,这进一步改善扩增、持久性、耐受衰老和耐受无反应性。在某些实施方式中,其中所述IFN-γ拮抗剂是抗IFN-γ抗体。
不欲被任何理论所限,下文中的实施例仅仅是为了阐释本申请的核酸分子、细胞和用途等,而不用于限制本申请发明的范围。
实施例
实施例1 CD19 FMC63 CAR mRNA的体外转录(IVT)
1.用Spe1酶消化pDA-FMC63 CAR质粒,使其线性化;
2.用PCR Cleanup试剂盒(Qiagen)纯化线性化的载体,用无RNase的水洗脱;
3.用纳米滴管测量DNA的浓度,并通过运行琼脂糖DNA凝胶来检查;
4.按照制造商的协议(Thermofisher,Cat No:AMB13455)进行IVT;具体地,将1ug模板DNA、NTP/ARCA缓冲液、T7缓冲液、GTP、T7酶和无RNase H2O以20ul的体积加入到0.2mlPCR管中,并在37℃下孵育4小时;
5. 4小时后,在每个反应中加入2ul的DNase I,并在37℃下孵育15分钟;
6.然后根据制造商的建议进行尾随程序;
7.使用RNasy试剂盒(Qiagen)纯化IVT mRNA;
8.用纳米滴管测量RNA的浓度,并通过运行PAGE凝胶检查。
实施例2将mRNA与siRNA电穿孔至A549-GFP或T细胞
1.收集A549-GFP肿瘤细胞和T细胞,用Opti-MEM培养基清洗3次;
2.用Opti-MEM培养基重悬细胞颗粒,调整细胞浓度为1×10e7/ml;
3.将5ug CD19 mRNA,10ug anti-CD19 FMC63 CAR mRNA,不加或加不同量的IFN-γsiRNA(1.5uM,4.5uM,7.5uM)或对照siRNA(4.5uM,7.5uM)等量到1.5ml EP管中,然后加入100ul T细胞或A549细胞,混合均匀;
4.在BTX ECM 830机器上设置参数:a)对于T细胞:500电压,0.7毫秒;b)对于A549肿瘤细胞:300电压,0.5毫秒;
5.在BTX电穿孔杯中加入100ul与RNA混合的细胞,轻拍以避免产生气泡;
6.进行电穿孔,然后将细胞转移到预热的培养基中,在37℃下培养,得到anti-CD19 CAR-T细胞或过表达CD19的549-GFP细胞。
实施例3 CAR-T细胞中IFN-γ细胞因子的细胞内染色
1.染色前,用50ng/ml PMA、1ug/ml Ionomycin和GolgiStop(BD Biosciences,1500倍稀释)处理实施例2获得的CAR-T细胞(采用siRNA-1电穿孔)6小时;
2. 6小时后,将CAR-T细胞转移到96孔板,在4℃,1500rpm下旋转3分钟;
3.将细胞颗粒重悬于100ul/孔的1×固定/透化缓冲液中,4℃30分钟(固定/透化缓冲液是由固定/透化浓缩液(Thermofisher,Cat.No.00-5123-43)和固定/透化稀释液(Thermofisher,Cat.No.00-5223-56)按1:3比例混合而成);
4.在每个固定/透化细胞孔中加入100ul的1X透化缓冲液(Thermofisher,Cat.No.00-8333-56),在4℃,1800rpm下旋转3分钟;
5.用200ul 1X通透性缓冲液再洗一次细胞,在4℃,1800rpm条件下旋转5分钟;
6.在1X通透性缓冲液中稀释抗IFN-γ流动抗体,CAR-T细胞在4℃下染色30分钟;
7.加入150ul/孔的1X通透缓冲液,在4℃,1800rpm下旋转5分钟;
8.用200ul 1X通透性缓冲液再洗一次细胞;
9.将细胞重悬于200ul FACS中,进行流式分析。
结果如图1所示,在采用本申请不同的IFN-γsiRNA电穿孔的T细胞中均能够下调IFN-γ的表达,其中IFN-γsiRNA-1的敲除效率是最好的。
用抗CD19 FMC63 CAR mRNA和不同数量的IFN-γsiRNA-1(1.5uM,4.5uM,7.5uM)或对照siRNA(NC)电穿孔的T细胞在不同时间段内的IFN-γ细胞因子的细胞内染色。数据通过流式细胞仪进行分析。
结果如图3所示,IFN-γsiRNA-1的抑制作用至少可以持续3天。
实施例4 CAR-T细胞的IFN-γ细胞因子释放的ELISA测试
1.在收集上清液之前,用50ng/ml PMA和1ug/ml Ionomycin在96孔板中处理CAR-T细胞6小时;
2. 6小时后,在4℃,1500rpm下旋压3分钟;
3.然后将上清液转移到新的96孔板中;
4.根据制造商的建议,用ELISA检测IFN-γ细胞因子水平(Biolegend,Cat:432004)。
结果如图2所示,在采用本申请不同的IFN-γsiRNA电穿孔的T细胞中,其中IFN-γsiRNA-1的敲除效率是最好的。
实施例5 CAR-T细胞中抗CD19 CAR的FACS染色
1.分别在T细胞电转后的第1天、第二天和第3天收集T细胞。
2.用FACS buffer稀释CD19-Fc重组蛋白,终浓度为2.5μg/ml。将CAR-T细胞在4℃下染色30分钟。
3. 30分钟后,用FACS buffer洗涤CAR-T细胞3次。
4.用FACS buffer稀释PE anti-human IgG Fc二抗,稀释比例为1:200。将CAR-T细胞在4℃下染色30分钟。
5.30分钟后,用FACS buffer洗涤细胞2次,然后将细胞重悬于200ul FACS中,进行流式分析。
结果如图4所示,电穿孔的siRNA-1对CAR分子的表达没有影响。
实施例6抗CD19 CAR-T细胞的体外细胞毒性试验
1.在共培养前12小时,将用5ug CD19 mRNA电穿孔的A549-EGFP细胞播种到平底96孔板上,3000个细胞/100ul/孔;
2.稀释CAR-T细胞至适当的浓度,将100ul/孔的细胞播种到不同E/T比例的肿瘤细胞中,如3:1和1:1;
3.将共培养板放入IncuCyte S3机器,并设置扫描参数;
4.扫描4天后,分析绿色物体综合强度(Total Green Object IntegratedIntensity,TGOII,GCU xμm2/Well),计算杀伤效率。
结果图5-6所示,采用不同含量siRNA-1电穿孔的抗CD19 CAR-T细胞均能够对肿瘤细胞产生杀伤作用,并且与单纯的抗CD19 CAR-T细胞相比,其杀伤效应没有影响。

Claims (26)
1.分离的核酸分子,其用于在细胞内抑制IFN-γ基因的表达,所述核酸分子包反义含寡核苷酸,其中所述反义寡核苷酸包含与SEQ ID NO:1-6中任一项所示核苷酸序列相差不超过3个核苷酸的至少15个连续核苷酸,所述反义寡核苷酸包含与编码IFN-γ的mRNA的至少一部分互补的互补区域,所述互补区域的长度为17-21个核苷酸。
2.根据权利要求1所述的核酸分子,其中所述互补区域的长度为19个核苷酸。
3.根据权利要求1所述的核酸分子,所述核酸分子为siRNA。
4.根据权利要求3所述的核酸分子,所述siRNA包含正义链和反义链,所述正义链包含SEQ ID NO:7-12中任一项或与其相差不超过3个核苷酸的序列,且所述反义链包含SEQ IDNO:1-6中任一项或与其相差不超过3个核苷酸的序列,所述正义链和反义链互补共同形成长度介于17到21个核苷酸的互补区域;其中每个链的长度均介于17和23个核苷酸之间。
5.根据权利要求4所述的核酸分子,所述siRNA选自以下一种或多种组合:
i)SEQ ID NO:7所示核苷酸序列的正义链,和SEQ ID NO:1所示核苷酸序列的反义链;
ii)SEQ ID NO:8所示核苷酸序列的正义链,和SEQ ID NO:2所示核苷酸序列的反义链;
iii)SEQ ID NO:9所示核苷酸序列的正义链,和SEQ ID NO:3所示核苷酸序列的反义链;
iv)SEQ ID NO:10所示核苷酸序列的正义链,和SEQ ID NO:4所示核苷酸序列的反义链;
v)SEQ ID NO:11所示核苷酸序列的正义链,和SEQ ID NO:5所示核苷酸序列的反义链;和
vi)SEQ ID NO:12所示核苷酸序列的正义链,和SEQ ID NO:6所示核苷酸序列的反义链。
6.根据权利要求5所述的核酸分子,其中每条链中不超过3个核苷酸分别被其它核苷酸所替代,同时保持抑制培养的细胞中IFN-γ表达的能力。
7.载体,其包含权利要求1-6中任一项所述的核酸分子。
8.细胞,其包括权利要求1-6中任一项所述的核酸分子或权利要求7所述的载体。
9.根据权利要求8所述的细胞,其包括免疫效应细胞。
10.根据权利要求9所述的细胞,其中所述免疫效应细胞的包括T细胞、B细胞、自然杀伤(NK)细胞、肥大细胞或吞噬细胞。
11.根据权利要求8所述的细胞,其中所述免疫效应细胞包括工程化的免疫效应细胞。
12.根据权利要求11所述的细胞,其中所述工程化的免疫效应细胞包括CAR-T细胞。
13.权利要求1-6中任一项所述的核酸分子,权利要求7所述的载体或权利要求8-12中任一项所述的细胞在制备治疗肿瘤药物中的应用。
14.根据权利要求13所述的应用,其中所述肿瘤包括实体瘤或血液瘤。
15.根据权利要求13所述的应用,其中所述肿瘤表达肿瘤相关抗原。
16.根据权利要求13所述的应用,其中所述药物包括CAR-T细胞。
17.权利要求1-6中任一项所述的核酸分子,权利要求7所述的载体或权利要求8-12中任一项所述的细胞在制备治疗CAR-T细胞治疗过程中由IFN-γ释放引起的免疫疗法相关毒性的药物中的应用。
18.根据权利要求17所述的应用,其中所述免疫疗法相关毒性选自细胞因子释放综合征、神经毒性、神经炎症或其组合。
19.组合物,其包含:
a.权利要求1-6中任一项所述的核酸分子,权利要求7所述的载体或权利要求8-12中任一项所述的细胞;和
b.药物上可以接受的载体。
20.一种调节T细胞功能的方法,所述方法包括将权利要求1-6中任一项所述的核酸分子引入T细胞。
21.根据权利要求20所述的方法,其中与野生型T细胞相比,引入权利要求1-14中任一项所述的核酸分子的T细胞表达的IFN-γ被抑制至少约20%。
22.根据权利要求20所述的方法,所述方法还包括通过向所述T细胞引入编码CAR的核酸分子来修饰所述T细胞的特异性。
23.根据权利要求22所述的方法,其中所述编码CAR的核酸分子包括mRNA。
24.根据权利要求23所述的方法,其中所述mRNA编码anti-CD19 CAR,其包含SEQ IDNO:13所示的核苷酸序列。
25.根据权利要求22所述的方法,其通过选自以下的任一方式将权利要求1-6中任一项所述的核酸分子和/或所述编码CAR的核酸分子引入T细胞:超声处理、电脉冲、电穿孔、渗透压冲击、磷酸钙沉淀、DEAE葡聚糖转染、脂质介导的递送和被动递送。
26.根据权利要求25所述的方法,其通过电穿孔方法同时将权利要求1-6中任一项所述的核酸分子和所述编码CAR的核酸分子引入T细胞。
