背景技术

针对AML化疗耐药的发生机制,围绕不同的信号通路和关键调控分子,主要形成了以下学说:肿瘤ABC转运蛋白学说、凋亡失调控学说、关键基因突变学说和骨髓微环境学说等。骨髓微环境,又称骨髓龛,此学说认为一方面AML细胞上的细胞因子、生长因子和趋化因子等受体可与骨髓龛内的成骨细胞、窦状内皮细胞和基质细胞等分泌的多种可溶性因子结合,激活不同信号传导通路,促进细胞存活、增殖和抑制细胞凋亡,介导AML化疗耐药;另一方面,AML细胞可通过一系列粘附分子及细胞外基质成分与骨内膜或基质的成骨细胞紧密结合,介导AML细胞通过“Adhesion”、“Engraftment”和“Expansion”等过程归巢并藏匿于骨髓微环境中,躲避化学药物攻击的同时促进AML细胞的自我更新及生存,产生化疗耐药。但目前AML化疗耐药仍有待于进一步研究。
发明内容
针对上述现有技术的现状,发明人前期利用CRISPR-Cas9文库筛选技术、反向蛋白微阵列技术及转录组高通量测序技术联合生物信息学分析手段筛选,经深入而广泛的研究验证,首次发现METTL3基因与急性髓系白血病化疗耐药密切相关,可作为急性髓系白血病患者临床分子生物学化学药物疗效评估的分子标记;并且发明人利用后续体外细胞实验及动物体内实验首次证实,METTL3可作为AML化疗耐药治疗的靶点,同时,发明人通过二代测序手段及AML动物模型发现并证实METTL3可以调控AML细胞的归巢。因此,METTL3及其介导的AML细胞归巢可能是AML化疗耐药的关键机制之一。
基于上述发现,本发明提供以下技术方案。

本发明第三个方面,提供一种非诊断目的是否存在METTL3基因及其定量检测方法,步骤如下:

与现有技术相比,本发明的技术方案具有如下有益效果:
本发明首次发现METTL3基因表达量与急性髓系白血病化学药物治疗耐药密切相关。本发明评估急性髓系白血病化学药物治疗反应及预后的试剂盒检测简单、快速、准确、特异性强,对于提高诊断精确度,实现急性髓系白血病的个体化精准诊疗,增加治愈或延长急性髓系白血病患者生存期有着重要意义。
本发明进一步明确了METTL3在AML化疗耐药中的作用方式,依据本发明研究成果,抑制METTL3的功能可以通过抑制AML细胞归巢,抑制AML细胞化疗耐药。本发明研究结果进一步明确了METTL3介导AML化疗耐药的作用机制,有利于AML化疗耐药临床治疗药物的开发,具有十分重要的临床意义。

应该指出,以下详细说明都是例示性的,旨在对本发明提供进一步的说明。除非另有指明,本文使用的所有技术和科学术语具有与本发明所属技术领域的普通技术人员通常理解的相同含义。
正如背景技术所介绍的,提高急性髓系白血病患者预后和生存的关键是寻找其化学药物治疗耐药机制并指导其临床治疗,因此寻找可预判化学药物疗效的关键分子标记,提高急性髓系白血病分层诊断精度,完善急性髓系白血病预后分层评估体系,对于高危或耐药易感人群实施合理有效的治疗方案,最终实现急性髓系患者个体化精准诊疗具有十分重要的意义。此外,针对现有技术中AML难治、AML化疗后复发及AML化疗耐药的现象,为了解决如上的技术问题,本发明目的在于进一步明确AML化疗耐药的发生机制,为其治疗提供新的治疗靶点。
本发明第一个方面,提供检测METTL3表达水平的试剂在制备评估急性髓系白血病患者化疗效果或预后的产品中的用途。
METTL3作为m6A修饰系统的核心甲基转移酶,即m6A甲基化修饰系统包括“写入器”,其与“橡皮擦”和“读取器”等组分一同介导RNA特定腺苷酸的甲基化修饰、去甲基化调控和功能识别等动态调节过程。METTL3为诸多肿瘤的癌基因。但METTL3能否调控AML化疗耐药,目前尚无相关报道。
本发明优选的实施例中,所述METTL3在难治或化疗后复发急性髓系白血病患者中表达上调。
进一步的,所述试剂包括RT-PCR、qRT-PCR或高通量测序平台检测用试剂。
本发明第二个方面,提供了一种评估急性髓系白血病化疗效果及预后的产品,所述产品包括试剂盒,所述试剂包括可以检测METTL3表达水平的PCR反应试剂。
本发明优选的实施例中,所述PCR反应试剂包括扩增METTL3的引物;优选的,所述引物序列如SEQ ID NO.1-2所示。其它PCR反应试剂为dNTP、DEPC水和SYBR Green。PCR反应试剂中各组分的用量及配比关系为常规用量配比关系。
进一步的,扩增METTL3的PCR条件为:95℃1min;95℃15s,62℃15s,72℃45s,40个循环;95℃15s,60℃1min,95℃持续。产物长度为104bp。
本发明第三个方面,提供一种非诊断目的是否存在METTL3基因及其定量检测方法,步骤如下:
①采用扩增METTL3的引物扩增待检样品,得到扩增产物;
②对扩增产物进行测序测定,检测扩增产物中是否存在METTL3;

发明优选的实施例中,提供一种对AML病患者临床化学药物治疗反应或预后评估进行定量检测的方法,步骤如下:
定量检测该个体的METTL3基因,并与化疗敏感患者(即2周期化疗后可达CR的患者)进行比较,存在差异就表明该患者临床诊断为难治及复发耐药的AML的可能性高于化疗敏感个体。所述差异是指METTL3基因表达量的高低。
本发明第四方面,提供METTL3作为AML化疗耐药治疗靶点在制备AML化疗耐药性产品中的应用。
依据本发明研究结果表明,首次化疗即达CR的AML患者的初诊时骨髓样本相比,难治患者的相应初诊骨髓样本及复发患者的骨髓样本中METTL3及m6A甲基化水平显著增多;AML化疗耐药细胞株THP-1/IDA,Kasumi-1/IDA,K562/A02及HL-60/ADR细胞mRNA中METTL3及m6A甲基化水平较敏感株明显增加。为了验证METTL3作为AML化疗耐药治疗靶点的可行性,本发明针对AML疾病药敏模型进行METTL3过表达,结果显示体外实验中AML细胞化疗耐药性显著提升,体内实验中AML药敏模型小鼠化疗耐药显著,生存期显著缩短。其实METTL3可作为AML化疗耐药的治疗靶点。
本发明优选的实施例中,所述METTL3作为AML化疗耐药治疗靶点的应用方式包括METTL3抑制剂用于制备抗AML化疗耐药药物。
进一步优选的,所述METTL3抑制剂为STM2457。
本发明的实施例中,所述AML化疗耐药所针对的化疗药为临床一线化疗药物,例如去甲氧柔红霉素(IDA)及阿糖胞苷(Ara-C)。
本发明第五方面,提供METTL3在制备抑制AML细胞粘附及归巢的治疗产品中的应用。
AML细胞可通过一系列粘附分子及细胞外基质成分与骨内膜或基质的成骨细胞紧密结合,介导AML细胞通过粘附等过程归巢并藏匿于骨髓微环境中,联合粘附分子所介导的物理粘附作用激活系列信号级联反应,共同促进AML细胞的自我更新及生存,产生AML化疗耐药。参与AML细胞归巢的粘附分子包含整合素,其可通过与多种配体结合增强AML细胞间及AML细胞与骨髓基质细胞间的粘附。如AML细胞表面的α4β1整合素(ITGA4)可以通过与骨髓基质配体结合,激活下游信号通路,促进AML细胞和骨髓基质细胞的生存及增殖。
为了验证METTL3作为抑制AML细胞粘附及归巢的治疗靶点的应用,本发明针对AML模型进行METTL3的过表达,结果显示过表达METTL3后,AML细胞内粘附相关通路显著改变;AML细胞内ITGA4表达显著提高;AML细胞向HUVEC细胞的迁移及粘附能力显著提高;归巢至小鼠骨髓内的AML细胞比例增高。提示METTL3可作为抑制AML细胞粘附及归巢的治疗靶点,通过抑制METTL3的功能可实现抑制AML细胞归巢的治疗作用。
本发明优选的实施例中,METTL3作为抑制AML细胞归巢治疗靶点的应用方式包括METTL3抑制剂用于制备抗AML细胞归巢药物。
进一步优选的,所述METTL3抑制剂可作为AML细胞内ITGA4抑制剂。
本发明第六方面,提供METTL3抑制剂在制备抗AML化疗耐药活性成分的应用。
为了验证METTL3抑制剂可作为抗AML化疗耐药活性成分进行应用的可行性,本发明针对AML化疗耐药模型施用了METTL3抑制剂,本发明研究证实AML化疗耐药细胞中的METTL3功能抑制后,AML化疗耐药细胞株的耐药受到抑制;AML化疗耐药细胞向HUVEC细胞的迁移及粘附能力显降低;归巢至AML化疗耐药小鼠骨髓内的AML化疗耐药细胞比例显著降低。启示METTL3抑制剂可作为AML化疗耐药的活性成分进行应用。
所述METTL3抑制剂作为抗AML化疗耐药活性成分的应用方式包括但不限于将所述METTL3抑制剂应用于制备抗AML化疗耐药药物,或保健品。
上述优选技术方案的一些实施方式中,所述抗AML化疗耐药药物由所述METTL3抑制剂及药学上所必须的辅料组成。
上述优选技术方案的另一些实施方式中,所述抗AML化疗耐药的药物中包括所述METTL3抑制剂及药学上所必须的辅料,还包括其他具有AML化疗耐药活性的成分。
为了使得本领域技术人员能够更加清楚地了解本发明的技术方案,以下将结合具体的实施例详细说明本发明的技术方案。
实施例1 METTL3基因的检测及与急性髓系白血病患者临床化学药物治疗反应及预后评估的关联分析
1.1研究对象
选取急性髓系白血病患者37例,2019年9月至2021年9月于山东大学齐鲁医院确诊治疗,临床资料从病历获得。大部分受试者均为山东省内汉族居民。取受试人员骨髓3mL,分离单个核细胞。所有受试人员均按照山东大学齐鲁医院伦理委员会要求,征得受试者同意。
1.2 RNA提取
①在PBS洗涤后的细胞沉淀中加入1mLTRIzol,枪头吹打混匀至样品完全溶解,样品体积不应超过TRIzol体积的10%。
②每1mLTRIzol裂解液中加入0.2mL氯仿,上下颠倒混匀15s,冰上静置10min。
③4℃15,000×g离心15min。样品分为三层:底层为红色有机相,上层为无色水相,中间层为白色。RNA主要存在于上层水相中。
④将上层水相转移到新的EP管中(约300uL),加入等体积异丙醇沉淀水相中的RNA,室温静置10min。
⑤4℃1,5000×g离心10min,离心后在管侧和管底出现白色胶状沉淀,移去上清。
⑥加入1mL 75%乙醇洗涤RNA沉淀,4℃1,5000×g离心5min,弃上清。
⑦室温放置干燥RNA沉淀5-10min,加入20μL DEPC水,用无酶枪头轻柔吹打混匀,溶解RNA。
⑧测定RNA浓度和纯度。
1.3引物、PCR扩增、测序和定量
①从GenBank下载METTL3基因组序列(Gene ID:56639,NM_019852),根据序列设计引物。具体引物详细信息见表1。

PCR扩增条件:
a.反应条件:95℃1min;95℃15s,62℃15s,72℃45s,40个循环;95℃15s,60℃1min,95℃持续。
b.扩增反应结束后,从60℃缓慢加热到99℃,建立PCR产物的熔解曲线,分析扩增产物特异性。
②PCR扩增产物送交上海铂尚生物技术公司测序,采用了正向和反向测序,测序结果应用软件4Peaks及BLAST进行检验和分析,经分析发现PCR扩增产物为目的基因METTL3。
③METTL3的定量:与内参基因GAPDH相比,利用2-ΔCt方法,计算METTL3的表达量。
1.4 METTL3表达关联分析
在GraphPad Prism6中应用非配对T检验对受试人员骨髓样本PCR结果进行统计分析。所有P值均为双侧概率,当P<0.05时认为有显著统计学意义。AML化疗敏感患者(即2周期化疗后可达CR的患者)与AML难治复发患者进行非配对检验,P=0.0002。结果发现METTL3表达量与急性髓系白血病患者的临床化学药物治疗反应及预后显著相关,难治复发急性髓系白血病患者METTL3表达量(中位值0.03561)较化疗敏感患者METTL3的表达量(中位值0.01341)明显增高。可作为急性髓系白血病患者化学药物治疗反应及预后评估的分子标记。详细结果见图2。
实施例2急性髓系白血病患者化学药物治疗反应及预后评估检测试剂盒
由于METTL3与急性髓系白血病患者的临床化疗反应及预后显著相关,可基于此设计METTL3基因特异性引物,以患者的cDNA为模板进行扩增检测。
制备一试剂盒(100人次),它含有如表2所示的物质构成:

抽取受试者骨髓3mL,分离表达CD34的单个核细胞,常规方法提取RNA。利用急性髓系白血病患者化学药物治疗反应及预后评估检测试剂盒进行PCR反应,将反应产物进行测序,将测序结果利用BLAST软件进行检验和分析,同时进行定量检测。单独构建验证集(3例)结果显示,与AML化疗敏感受试者相比,METTL3检测结果显著增高的患者临床评估为化学药物治疗相对不敏感,其为难治及复发急性髓系白血病。
实施例3 METTL3对人源化AML小鼠药敏模型的影响
为明确METTL3作为AML化疗耐药治疗靶点的可行性,本实施例在AML细胞株内利用慢病毒转染技术构建过表达METTL3及METTL3甲基转移酶功能突变体METTL3-CD,利用THP-1构建稳定过表达AML细胞株,并通过尾静脉注射至NSG小鼠体内,构建AML模型,并于注射后第42天给予化疗药物IDA治疗。结果发现,METTL3过表达的AML小鼠IDA治疗时显著出现耐药表现,而METTL3甲基转移酶功能突变后,其诱导AML化疗耐药的能力随之丧失(图3)。提示METTL3可作为AML化疗耐药的治疗靶点。
实施例4 METTL3对AML小鼠模型细胞粘附及归巢的影响
为了明确METTL3作为抑制AML细胞粘附及归巢治疗靶点的可行性,本实施例利用METTL3过表达的人源THP-1及Kasumi-1细胞株,分析METTL3过表达前后AML细胞株内ITGA4的表达,发现METTL3可促进ITGA4的表达(图5);利用METTL3及METTL3甲基转移酶功能突变体METTL3-CD转染后稳定构建的THP-1及Kasumi-1细胞进行迁移及粘附实验,结果表明METTL3可促进AML细胞迁移、粘附至HUVEC细胞(图6);通过尾静脉注射至NSG小鼠体内,分析16小时后NSG小鼠骨髓及脾脏内AML细胞比例,即归巢细胞比例,结果发现METTL3可促进AML细胞的归巢(图7)。以上结果均提示,METTL3可作为AML细胞粘附及归巢的治疗靶点。
实施例5 METTL3抑制剂对AML化疗耐药小鼠模型的影响
为了明确METTL3抑制剂可作为抗AML化疗耐药活性成分进行应用的可行性,本实施例利用METTL3抑制剂STM2457分别对AML化疗耐药细胞及敏感细胞进行处理,检测处理后细胞膜表面ITGA4的表达变化示METTL3抑制剂可显著降低AML化疗耐药细胞膜表面的ITGA4表达(图8);METTL3抑制剂可抑制迁移、粘附至HUVEC细胞的AML化疗耐药细胞比例,尾静脉注射16h后流式细胞术检测发现METTL3抑制剂可显著降低NSG小鼠骨髓及脾脏内AML化疗耐药细胞比例,即AML化疗耐药细胞归巢比例(图9);IDA处理下的EDU实验证实,METTL3抑制剂可显著抑制AML化疗耐药细胞的化疗耐药(图10)。以上结果均提示:METTL3抑制剂可作为抗AML化疗耐药活性成分进行应用。

1.检测METTL3表达水平的试剂在制备评估急性髓系白血病患者化疗效果或预后的产品中的用途。
2.根据权利要求1所述的用途,其特征在于,所述METTL3在难治或化疗后复发急性髓系白血病患者中表达上调。
3.根据权利要求1所述的用途,其特征在于,所述试剂包括RT-PCR、qRT-PCR或高通量测序平台检测用试剂。
4.一种评估急性髓系白血病化疗效果及预后的产品,所述产品包括试剂盒,所述试剂包括可以检测METTL3表达水平的PCR反应试剂。
5.根据权利要求4所述的产品,其特征在于,所述PCR反应试剂包括扩增METTL3的引物;优选的,所述引物序列如SEQ ID NO.1-2所示;进一步的,PCR反应试剂还包括dNTP、DEPC水和SYBR Green;进一步的,扩增METTL3的PCR条件为:95℃1min;95℃15s,62℃15s,72℃45s,40个循环;95℃15s,60℃1min,95℃持续。
6.一种非诊断目的是否存在METTL3基因及其定量检测方法,步骤如下:
①采用扩增METTL3的引物扩增待检样品,得到扩增产物;
