本发明涉及一种距骨融合表面型人工踝关节假体,包括:胫骨髓腔内固定假体柄,所述胫骨髓腔内固定假体柄的上端用于与胫骨髓腔紧固连接;假体节段,锥接在所述胫骨髓腔内固定假体柄的下端;关节衬垫,镶嵌在所述假体节段的下端,且所述关节衬垫的下表面为凹弧面结构;距骨表面假体,所述距骨表面假体的下部与距骨紧固连接,所述距骨表面假体的上表面为抛光凸弧面结构且与所述关节衬垫的下表面适形匹配形成光滑的运动关节。本发明能够广泛应用于胫骨远端肿瘤切除后肢体连续性中断的骨缺损重建,其可实现肢体结构恢复目的,同时实现胫距关节即刻稳定和远期稳定的完美结合,从而使患者远期踝关节功能得到显著改善。

Description

一种距骨融合表面型人工踝关节假体
技术领域
本发明涉及一种人工假体,具体涉及一种基于3D打印技术的距骨融合表面型人工踝关节假体,属于医疗器械技术领域。
背景技术
胫骨是仅次于股骨的骨肉瘤第二好发部位,胫骨骨肉瘤约占全身骨肉瘤的19%,其中20%的胫骨骨肉瘤发生于胫骨远端。胫骨远端由于较多肌腱、血管和神经,很难获得良好的肿瘤边界。因此,有学者推荐膝下截肢作为胫骨远端骨肉瘤首选的手术方式。但是,随着化疗和外科技术的进步,胫骨远端骨肉瘤的保肢治疗同样能够获得良好的生存率,并被广大骨肿瘤医生所认可和推荐。
胫骨远端恶性肿瘤瘤段切除后重建方式多样,主要包括异体骨重建、自体骨灭活再植、带血管蒂自体腓骨移植、不带血管蒂自体腓骨移植以及踝关节假体置换等。异体骨重建存在感染风险高、异体骨吸收及排异反应等问题,从而增加伤口感染等并发症风险。自体骨灭活再植方法多样,国内外最为常用的三种灭活方式为酒精、液氮和高温法,不同灭活方式在并发症方面各有利弊。踝关节承受全身重量,并且通过踝关节的正常活动可完成日常行走活动,因此对于胫骨远端恶性肿瘤患者重建踝关节的功能性对于术后疗效具有重要意义。踝关节融合的优点是可以恢复骨性结构的连续性,提供良好的踝关节稳定性,减轻负重时的疼痛。但依靠钢板螺钉系统实现胫骨远端固定和踝关节融合的患者,随着植骨后自异体骨的吸收,可增加术后灭活自体骨和大段异体骨的骨折发生率。虽然目前胫骨远端恶性肿瘤瘤段切除后重建方式众多,但大部分重建方式均以牺牲踝关节活动度为代价,对于大多数患者尤其是年轻患者而言,牺牲正常踝关节活动度仍然较难接受。但在切除胫骨远端恶性肿瘤大段瘤骨后,能够实现踝关节稳定和良好活动度的完美结合的重建方式,仍是骨肿瘤科医师面临的重大挑战。
踝关节置换术并不像髋膝关节置换一样得到广泛推广,重要原因是远期假体机械性失效和切口并发症发生率高。同时踝关节置换术后假体松动失效与踝周的疼痛、肿胀,严重影响患者术后功能。研究显示,人工踝关节机械性失效很大原因在于假体距骨侧距骨塌陷及假体骨界面骨吸收造成。
目前3D打印技术在骨科领域广泛应用,大量研究表明多孔结构金属不仅具有良好的组织相容性以及理想的生物力学结构,采用3D打印技术个体化复制患者骨小梁结构及密度,保证理想的孔隙率和孔隙直径,可有利于宿主骨长入,提供假体和宿主骨的融合率。人工关节假体重建胫骨下段骨缺损的优势在于金属的力学强度远优于异体骨和灭活骨,可以实现早期稳定,重建一个可活动的踝关节。但其缺点也显而易见,即很难实现自体骨与金属假体之间的有效融合。随着数字骨科的飞速发展,3D打印技术可以解决这一困扰。近年来,增材制造技术为个体化、定制型人工假体的加工带来了技术上的进步,3D打印人工假体的技术优势主要体现在两个方面,一是适形匹配,二是骨整合功能。对于胫骨远段的修复重建来说,3D打印技术提供了一个较为理想的策略。通过3D打印可以制作出大小、形状匹配的胫骨下段缺损人工假体,在距骨表面假体与距骨的接触界面上可以制作出供骨长入的金属骨小梁结构,可以实现距骨表面假体与自体距骨形成骨性融合。
发明内容
针对上述问题,本发明的目的是提供一种基于3D打印技术的距骨融合表面型人工踝关节假体。
为实现上述目的,本发明采取以下技术方案:
本发明第一方面提供的一种距骨融合表面型人工踝关节假体,主要适用于胫骨远端肿瘤切除后肢体连续性中断的骨缺损重建,该人工踝关节假体包括:胫骨髓腔内固定假体柄,所述胫骨髓腔内固定假体柄的上端用于与胫骨髓腔紧固连接;假体节段,锥接在所述胫骨髓腔内固定假体柄的下端;关节衬垫,镶嵌在所述假体节段的下端,且所述关节衬垫的下表面为凹弧面结构;距骨表面假体,所述距骨表面假体的下部与距骨紧固连接,所述距骨表面假体的上表面为抛光凸弧面结构且与所述关节衬垫的下表面适形匹配形成光滑的运动关节。
本发明第二方面提供的一种距骨融合表面型人工踝关节假体,主要适用于踝关节损伤、类风湿性关节炎的病例,该人工踝关节假体包括:胫骨髓腔内固定假体柄,所述胫骨髓腔内固定假体柄的上端用于与胫骨髓腔紧固连接;关节衬垫,镶嵌在所述胫骨髓腔内固定假体柄的下端,且所述关节衬垫的下表面为凹弧面结构;距骨表面假体,所述距骨表面假体的下部与距骨紧固连接,所述距骨表面假体的上表面为抛光凸弧面结构且与所述关节衬垫的下表面适形匹配形成光滑的运动关节。
所述的距骨融合表面型人工踝关节假体,优选地,根据患者胫骨远端瘤段切除范围、皮肤缺损情况和软组织张力自由选择不同长度的所述假体节段。
所述的距骨融合表面型人工踝关节假体,优选地,所述关节衬垫采用高分子聚乙烯材料制备而成。
所述的距骨融合表面型人工踝关节假体,优选地,所述胫骨髓腔内固定假体柄与所述胫骨髓腔采用水泥固定或钛浆喷涂生物固定两种类型。
所述的距骨融合表面型人工踝关节假体,优选地,所述距骨表面假体由Ti6Al4V合金粉通过电子束熔融3D打印技术生产而成,所述距骨表面假体留有3个放射方向的钉孔用于与所述距骨通过螺钉进行加压固定。
所述的距骨融合表面型人工踝关节假体,优选地,所述距骨表面假体与所述距骨的接触面制备有孔隙大小为200-800μm并且完全连通的多孔金属骨小梁结构,且孔隙率为50%-80%。
本发明由于采取以上技术方案,其具有以下优点:
1、本发明提供的肿瘤型距骨融合表面型人工踝关节假体采用组配式设计,可以根据患者胫骨远端瘤段切除范围、皮肤缺损情况和软组织张力自由调节踝关节假体胫骨侧部件长度,从而个体化重建胫骨远端缺损段。
2、本发明可以重建胫距关节跖屈和背伸功能,重建踝关节的正常活动功能。
3、本发明通过3D打印技术设计的孔隙结构,提高假体距骨底座与宿主骨界面间的融合率,从而为良好的骨长入和假体稳定性提供重要基础。

实施例一:
图1展示了一种肿瘤型距骨融合表面型人工踝关节假体,主要适用于胫骨远端肿瘤切除后肢体连续性中断的骨缺损重建,该肿瘤型距骨融合表面型人工踝关节假体包括:胫骨髓腔内固定假体柄1,胫骨髓腔内固定假体柄1的上端用于与胫骨髓腔5紧固连接;假体节段2,锥接在胫骨髓腔内固定假体柄1的下端;关节衬垫3,镶嵌在假体节段2的下端,且关节衬垫3的下表面为凹弧面结构;距骨表面假体4,距骨表面假体4的下部与距骨6紧固连接,距骨表面假体4的上表面为抛光凸弧面结构且与关节衬垫3的下表面适形匹配形成光滑的运动关节,由此可以重建胫距关节跖屈和背伸功能,实现踝关节活动功能的良好恢复。

Claims (7)

1.一种距骨融合表面型人工踝关节假体,主要适用于胫骨远端肿瘤切除后肢体连续性中断的骨缺损重建,其特征在于,该人工踝关节假体包括:
胫骨髓腔内固定假体柄(1),所述胫骨髓腔内固定假体柄(1)的上端用于与胫骨髓腔(5)紧固连接;
假体节段(2),锥接在所述胫骨髓腔内固定假体柄(1)的下端;
关节衬垫(3),镶嵌在所述假体节段(2)的下端,且所述关节衬垫(3)的下表面为凹弧面结构;
距骨表面假体(4),所述距骨表面假体(4)的下部与距骨(6)紧固连接,所述距骨表面假体(4)的上表面为抛光凸弧面结构且与所述关节衬垫(3)的下表面适形匹配形成光滑的运动关节。
2.一种距骨融合表面型人工踝关节假体,主要适用于踝关节损伤、类风湿性关节炎的病例,其特征在于,该人工踝关节假体包括:
胫骨髓腔内固定假体柄(1),所述胫骨髓腔内固定假体柄(1)的上端用于与胫骨髓腔(5)紧固连接;
关节衬垫(3),镶嵌在所述胫骨髓腔内固定假体柄(1)的下端,且所述关节衬垫(3)的下表面为凹弧面结构;
距骨表面假体(4),所述距骨表面假体(4)的下部与距骨(6)紧固连接,所述距骨表面假体(4)的上表面为抛光凸弧面结构且与所述关节衬垫(3)的下表面适形匹配形成光滑的运动关节。
3.根据权利要求1所述的距骨融合表面型人工踝关节假体,其特征在于,根据患者胫骨远端瘤段切除范围、皮肤缺损情况和软组织张力自由选择不同长度的所述假体节段(2)。
4.根据权利要求1或2所述的距骨融合表面型人工踝关节假体,其特征在于,所述关节衬垫(3)采用高分子聚乙烯材料制备而成。
5.根据权利要求1或2所述的距骨融合表面型人工踝关节假体,其特征在于,所述胫骨髓腔内固定假体柄(1)与所述胫骨髓腔(5)采用水泥固定或钛浆喷涂生物固定两种类型。
6.根据权利要求1或2所述的距骨融合表面型人工踝关节假体,其特征在于,所述距骨表面假体(4)由Ti6Al4V合金粉通过电子束熔融3D打印技术生产而成,所述距骨表面假体(4)留有3个放射方向的钉孔用于与所述距骨(6)通过螺钉进行加压固定。
7.根据权利要求6所述的距骨融合表面型人工踝关节假体,其特征在于,所述距骨表面假体(4)与所述距骨(6)的接触面制备有孔隙大小为200-800μm并且完全连通的多孔金属骨小梁结构,且孔隙率为50%-80%。
