本发明涉及一种通用型人工全肘关节假体,包括:肱骨柄,所述肱骨柄的上端用于插入肱骨髓腔并与之紧固连接;关节体部,所述关节体部的上端与所述肱骨柄连接;尺骨柄,所述尺骨柄的下端用于插入尺骨髓腔并与之紧固连接,所述尺骨柄的上端与所述关节体部通过单纯屈伸机制的铰链结构形成铰链关节。本发明能够广泛应用于肘关节创伤、退行性关节病、类风湿关节炎、肿瘤等重建,实现肢体结构恢复的目的,同时实现肘关节即刻稳定和应力降低等优点的完美结合,从而使患者远期肘关节功能得到显著改善。

Description

一种通用型人工全肘关节假体
技术领域
本发明涉及一种人工假体,具体涉及一种基于3D打印技术的通用型人工全肘关节假体,属于医疗器械技术领域。
背景技术
肘部骨肿瘤占全身骨肿瘤发病率的1%。随着放化疗技术的发展,使得局部切除肿瘤、保留上肢成为可能。然而,由于肘部重要的解剖结构密集,局部切除术后会导致肘腕手部关节不稳定及功能丧失。肘关节周围肿瘤切除术后需要重建肘关节使其功能稳定,但可供选择的重建方法不多。在人工全肘关节出现以前,仅有为数不多的几种重建方法可供选择。关节融合术、切除成形术、异体骨关节移植等各自存在弊端,导致术后肘关节运动受限、关节不稳定等并发症。对于较小的骨缺损,常选用关节融合术或切除成形术。关节融合术导致手术后肘关节运动受限,切除成形术导致术后关节不稳定。对于较大的缺损,可应用同种异体肘关节移植,但术后并发症发生率可高达70%,会出现感染、关节不稳定、肘关节脱位。虽然有坚实的内固定,但术后出现骨不连的机会仍很高(约15%),另外部分患者可出现异体骨吸收。对于肘关节周围肿瘤切除术后功能重建而言,目前尚没有比人工肘关节置换更好的方法。
人工肘关节置换多用于创伤、退行性关节病、类风湿关节炎等,少数肘部肿瘤的病例也采用人工肘关节置换,显示其重建效果较好。尺骨近端或肱骨远端肿瘤切除而接受肘关节人工假体重建的病例,一般均需要使用个体化定制的肘关节假体置换,所出现的并发症多为神经损伤、感染和假体机械性失败。肘关节切除后使用铰链式人工关节置换失败率较高,原因包括:①尺骨髓腔很细,很难注入理想量的骨水泥,尺骨柄固定不牢靠;②肘关节日常屈伸活动频繁,对人工肘关节柄的应力大、容易造成松动。
目前,现有文献中报道的18例肱骨远端肿瘤型人工肘关节置换的患者,并发症包括无菌性松动3例、感染2例、桡神经损伤l例、假体周围骨折1例。我中心报道18例肘关节周围肿瘤切除后通过人工全肘关节置换重建肘关节功能的病例,其术后早期并发症少,松动发生率仅为l例,假体柄穿出骨皮质2例,未发现假体柄折断和假体感染的病例。但全肘关节假体存在的问题也是显而易见的。由于尺骨髓腔较细且有弯曲弧度,导致全肘关节远期极易出现机械松动,从而导致肿瘤切除后肘关节重建失败。由于假体与自体骨接触面采用了3D打印金属骨小梁结构的设计,使得该人工假体能够很快实现与肱骨远端及尺骨近端骨整合,有效降低了肘关节屈伸时对假体造成的应力集中,从而极大地减少了假体松动、折断等机械性并发症。
发明内容
针对上述问题,本发明的目的是提供一种基于3D打印技术的通用型人工全肘关节假体。
为实现上述目的,本发明采取以下技术方案:一种通用型人工全肘关节假体,包括:肱骨柄,所述肱骨柄的上端用于插入肱骨髓腔并与之紧固连接;关节体部,所述关节体部的上端与所述肱骨柄连接;尺骨柄,所述尺骨柄的下端用于插入尺骨髓腔并与之紧固连接,所述尺骨柄的上端与所述关节体部通过单纯屈伸机制的铰链结构形成铰链关节。
所述的通用型人工全肘关节假体,优选地,所述关节体部包括向肱骨侧延伸的延伸部以及与所述延伸部一体连接的铰接部。
所述的通用型人工全肘关节假体,优选地,所述肱骨柄的下端形成锥接端,所述肱骨柄的下端直接与所述关节体部的延伸部相锥接;
或者,在所述肱骨柄的下端锥接合适长度的缺损段,所述缺损段再与所述关节体部的延伸部相锥接。
所述的通用型人工全肘关节假体,优选地,所述肱骨侧和尺骨侧假体设计有8°的外翻提携角。
所述的通用型人工全肘关节假体,优选地,在所述关节体部的延伸部同肱骨内外髁自体骨面相接触的部位以及所述尺骨柄插入所述尺骨髓腔内的柄部近端均采用3D打印技术制备有孔隙大小为200-800μm并且完全连通的多孔金属骨小梁结构,且孔隙率为50%-80%。
所述的通用型人工全肘关节假体,优选地,所述肱骨柄与所述肱骨髓腔以及所述尺骨柄与所述尺骨髓腔采用水泥固定或钛浆喷涂生物固定两种类型。
本发明由于采取以上技术方案,其具有以下优点:
1、本发明设计为非定制式铰链型肘关节假体,能够广泛应用于肘关节创伤、退行性关节病、类风湿关节炎、肿瘤切除后的重建,实现肢体结构恢复目的,同时实现肘关节即刻稳定和应力降低优点的完美结合,从而使患者远期肘关节功能得到显著改善。
2、本发明可以重建肘关节屈伸功能,恢复肢体正常长度,为上肢骨缺损后正常的力学传导提供条件。
3、本发明为了抵抗向后方移位和轴向旋转的应力,设计了位于肱骨内外髁之间的关节体部,同时保留肱骨内外髁,以尽量保留内外髁上的肌肉附着点。
4、本发明在关节体部的延伸部同肱骨内外髁自体骨面相接触的部位以及尺骨柄插入尺骨髓腔5内的柄部近端表面采用3D打印孔隙结构来增加假体与骨接触面的整合,有效降低了肘关节屈伸时对全肘关节假体造成的应力集中,从而极大的减少了全肘关节假体松动、折断等机械性并发症。
5、本发明采用了组配式设计,因而可以适用于肱骨远端或尺骨近端肿瘤切除后造成的大段骨缺损。

如图1、图2所示,本发明提供的通用型人工全肘关节假体,包括:肱骨柄1,肱骨柄1的上端用于插入肱骨髓腔4并与之紧固连接;关节体部2,关节体部2的上端与肱骨柄1连接;尺骨柄3,尺骨柄3的下端用于插入尺骨髓腔5并与之紧固连接,尺骨柄3的上端与关节体部2通过单纯屈伸机制的铰链结构形成铰链关节,由此可以重建肘关节屈伸功能,恢复肢体正常长度,为上肢骨缺损后正常的力学传导提供条件。
上述实施例中,优选地,关节体部2包括向肱骨侧延伸的延伸部以及与延伸部一体连接的铰接部,术中可将该延伸部紧固连接在肱骨内外髁之间,同时保留肱骨内外髁,以尽量保留肱骨内外髁上的肌肉附着点,从而可以有效抵抗向后方移位和轴向旋转的应力。
上述实施例中,优选地,肱骨柄1的下端形成锥接端,肱骨柄1的下端直接与关节体部2的延伸部相锥接(适用于病变切除范围较短的情形);又或者,可在肱骨柄1的下端锥接合适长度的缺损段(图中未示出),缺损段再与关节体部2的延伸部相锥接(适用于病变切除范围较长的情形),由此实现术中不同缺损长度的组配。
上述实施例中,优选地,肱骨柄1和尺骨柄3设计有8°的外翻提携角,由此可以允许全肘关节假体有8°的外翻。
上述实施例中,优选地,在关节体部2的延伸部同肱骨内外髁自体骨面相接触的部位以及尺骨柄3插入尺骨髓腔5内的柄部近端均采用3D打印技术制备有孔隙大小为200-800μm并且完全连通的多孔金属骨小梁结构,且孔隙率为50%-80%,以为新生骨组织提供长入空间,使得新生骨组织可以爬行长入孔隙结构中,由此能够很快实现与肱骨远端及尺骨近端的骨整合,有效降低了肘关节屈伸时对全肘关节假体造成的应力集中,从而极大的减少了全肘关节假体松动、折断等机械性并发症。
上述实施例中,优选地,肱骨柄1与肱骨髓腔4以及尺骨柄3与尺骨髓腔5可以采用水泥固定或钛浆喷涂生物固定两种类型。

关节体部(2),所述关节体部(2)的上端与所述肱骨柄(1)连接;
尺骨柄(3),所述尺骨柄(3)的下端用于插入尺骨髓腔(5)并与之紧固连接,所述尺骨柄(3)的上端与所述关节体部(2)通过单纯屈伸机制的铰链结构形成铰链关节。
2.根据权利要求1所述的通用型人工全肘关节假体,其特征在于,所述关节体部(2)包括向肱骨侧延伸的延伸部以及与所述延伸部一体连接的铰接部。

或者,在所述肱骨柄(1)的下端锥接合适长度的缺损段,所述缺损段再与所述关节体部(2)的延伸部相锥接。
4.根据权利要求1所述的通用型人工全肘关节假体,其特征在于,所述肱骨柄(1)和尺骨柄(3)设计有8°的外翻提携角。
5.根据权利要求2所述的通用型人工全肘关节假体,其特征在于,在所述关节体部(2)的延伸部同肱骨内外髁自体骨面相接触的部位以及所述尺骨柄(3)插入所述尺骨髓腔(5)内的柄部近端均采用3D打印技术制备有孔隙大小为200-800μm并且完全连通的多孔金属骨小梁结构,且孔隙率为50%-80%。
6.根据权利要求1至5任一项所述的通用型人工全肘关节假体,其特征在于,所述肱骨柄(1)与所述肱骨髓腔(4)以及所述尺骨柄(3)与所述尺骨髓腔(5)采用水泥固定或钛浆喷涂生物固定两种类型。
