Abstract

背景技术
特应性皮炎(atopic dermatitis)是发生于婴儿和儿童、通常持续至成人的具有严重瘙痒和特征性皮肤表现的慢性炎症疾病。特应性皮炎的病因已知是例如遗传背景、免疫机制、和环境因素等各种因素的组合,发病机制是非常复杂的并存在各种假设。最强力的假设之一是,随着Th1/Th2细胞内稳态崩解,Th2细胞变得过度活化。由特定过敏原过度活化的Th2细胞可分泌例如IL-4和IL-31等Th2细胞因子,并且分泌的细胞因子可诱导B细胞的IgE分泌和肥大细胞的脱粒,由此释放多种炎症物质。
然而,在迄今为止的特应性皮炎的治疗中,仅使用了利用例如抗炎剂如皮质类固醇等物质抑制免疫细胞的整体活性的方法,并且上述方法涉及非常短的持续时间和严重的副作用,从而不推荐将其作为合适的治疗方法。此外,最近开发的小分子和抗体疗法在治疗例如特应性皮炎等由复杂的炎症信号传导系统介导的慢性皮炎中受限,这是因为它们仅阻断结合至一种炎症物质的受体。此外,不容易避免通过其他途径复发的风险。近来,已试图通过使用IFN-γ蛋白抑制Th2细胞的活性来改善特应性皮炎,IFN-γ蛋白是一种Th1细胞因子,但由于IFN-γ蛋白在体内极短的半衰期,其治疗效果有限。
因此,由通过根据特应性皮炎的病因而平衡Th1/Th2免疫系统的基础治疗方法来最大化治疗效果是非常重要的,并且急需开发不导致副作用的用于特应性皮炎的治疗剂。
发明内容
发明要解决的问题
本发明的目的在于提供具有60%以上的比例的表达干扰素-γ的细胞的细胞组合物。
此外,本发明的另一目的在于提供制备所述细胞组合物的方法。
此外,本发明的另一目的在于提供用于预防或治疗特应性皮炎的药物组合物,其包含所述细胞组合物。
用于解决问题的方案
为了实现上述目的,本发明中采用了以下技术方案。
1.一种生产细胞组合物的方法,所述细胞组合物具有60%以上的比例的表达干扰素-γ的细胞,所述方法包括:从人外周血分离和获得单核细胞(“monocytes”)和自体血浆;由抗CD3抗体涂布细胞培养容器;和将所述单核细胞接种至所述细胞培养容器并在含有选自由IL-2、IL-12和IL-18组成的组的至少一种的培养基中培养所述单核细胞。
2.根据上述1所述的方法,其中所述组合物中表达NKG2D的细胞的比例为60%以上。

发明的效果
根据本发明的细胞组合物具有总细胞中增加至60%以上的比例的表达干扰素-γ的细胞,并且随着连续产生干扰素-γ的细胞的比例增加,可显著改善特应性皮炎。此外,期望能够根本上地治疗特应性皮炎的免疫学异常,这是因为其可安全地长期使用而不具有副作用。

下文中,将详细描述本发明。
本发明涉及制备细胞组合物的方法。
制备本发明的细胞组合物的方法可以包括分别地从人外周血分离和获得单核细胞和自体血浆的步骤。
从外周血分离单核细胞的方法可使用本领域已知的方法。通常,使用Ficoll法,其中Ficoll是通过使糖和环氧氯丙烷彼此聚合来获得的化合物,并且通常使用约400,000的分子量。Ficoll作为形成分离细胞、病毒和细胞器等的密度梯度的材料使用,因为当其溶于水时,其变为从低粘度至高密度范围的溶液。外周血单核细胞比血液中含有的红细胞、粒细胞、和死细胞更轻,但比血浆更重,从而被分离。
单核细胞可从施用细胞组合物的个体的自体血液分离并培养。在使用分离自自体血液的单核细胞的情况中,排除了不必要的自体免疫反应,使得可有效治疗特应性皮炎而无例如炎症等副作用。
制备本发明的细胞组合物的方法可包括在含有选自由抗CD3抗体、IL-2、IL-12和IL-18组成的组的至少一种的培养基中培养单核细胞。
当培养基包含抗CD3抗体时,抗CD3抗体可涂布在培养基上。当将抗CD3抗体涂布在培养基上时,培养基可进一步包含选自由IL-2、IL-12和IL-18组成的组的一种或多种。
培养基可包含抗CD3抗体、IL-2、IL-12和IL-18。更具体地,将抗CD3抗体涂布在培养基上,IL-2、IL-12和IL-18可附加地包含在细胞培养基中。
可无限制地使用抗CD3抗体,只要其为具有结合CD3的特性的抗体即可。可以在0.1至100μg/ml、优选0.5至50μg/ml、更优选1至10μg/ml的范围内包含抗CD3抗体,但不限于此。
白细胞介素是由例如淋巴细胞、单核细胞、和巨噬细胞等免疫细胞产生的蛋白质生物活性物质的总称。本发明的组合物可包含选自由作为白细胞介素的细胞因子的IL-2、IL-12和IL-18组成的组的一种或多种。可以在100至2000IU/ml、优选500至1500IU/ml、和更优选800至1200IU/ml的范围内包含IL-2,但不限于此。可以在0.5至10ng/ml、优选1至8ng/ml、和更优选2至6ng/ml的范围内包含IL-12,但不限于此。可以在1至100ng/ml、优选10至80ng/ml、和更优选20至60ng/ml的范围内包含IL-18,但不限于此。白细胞介素不限于此,还可无限制地使用本领域技术人员已知的其它白细胞介素,只要它们符合本发明的目的即可。
培养基可进一步包含L-谷氨酰胺。对L-谷氨酰胺的浓度没有特别限制,但可在例如0.5至5mM、和优选1至3mM的范围内。
培养基可进一步包含通常用于培养其它单核细胞的组分。例如,甘氨酸、L-精氨酸、L-天冬酰胺、L-天冬氨酸、L-胱氨酸2HCl、L-谷氨酸、L-组氨酸、L-羟脯氨酸、L-异亮氨酸、L-亮氨酸、L-赖氨酸盐酸盐、L-甲硫氨酸、L-苯丙氨酸、L-脯氨酸、L-丝氨酸、L-苏氨酸、L-色氨酸、L-酪氨酸二钠盐二水合物、L-缬氨酸、生物素、氯化胆碱、D-泛酸钙、叶酸、烟酰胺、对氨基苯甲酸、吡哆醇盐酸盐、核黄素、盐酸硫胺、维生素B12、i-肌醇、硝酸钙、硫酸镁、氯化钾、碳酸氢钠、氯化钠、无水磷酸氢二钠、D-葡萄糖、谷胱甘肽、HEPES、酚红等,但不限于此。
此外,可通过添加支持血清或血浆和单核细胞增殖的附加生长因子来培养培养基。对添加至培养基的血清或血浆的类型没有特别限制,并且可使用源自各种动物的市售可得的产品,但源自人的那些更优选源自他们自身。例如,可使用本领域技术人员已知的方法,例如,添加细胞因子的组合、或刺激单核细胞增殖的凝集素。
此外,培养基可进一步包含支持血清或血浆和单核细胞增殖的附加生长因子,并且可包含血清或血浆本身。对添加至培养基的血清或血浆的类型没有特别限制,并且可使用源自各种动物的市售可得的产品,但源自人的那些优选源自他们自身。例如,可使用人AB血清或自体血浆。
培养可以以多个阶段进行,例如可以以两个或三个或更多阶段进行。
当以两个或更多步骤进行时,例如,在第二阶段中,可将第一阶段中培养的单核细胞转移至新的培养基并培养。当以两个或更多阶段进行时,第一阶段培养基可包含(自体)血浆、抗CD3抗体、L-谷氨酰胺、IL-2、IL-12和IL-18。此外,第二阶段培养基可包含(自体)血浆、L-谷氨酰胺、和IL-2。更具体地,第一阶段培养基可包含抗CD3抗体和IL-2、IL-12、IL-18,和第二阶段培养基可包含IL-2,但可不包含抗CD3抗体、IL-12和IL-18。
当以三个或更多阶段进行时,例如,在第二阶段培养后,可将培养产物转移至新的培养基并培养。第一阶段培养基可包含(自体)血浆、抗CD3抗体、L-谷氨酰胺、IL-2、IL-12和IL-18,第二阶段培养基可包含(自体)血浆、L-谷氨酰胺、和IL-2,和第三阶段培养基可包含(自体)血浆、L-谷氨酰胺、IL-2、IL-12和IL-18。更具体地,第一阶段培养基可包含抗CD3抗体、IL-2、IL-12和IL-18,第二阶段培养基可包含IL-2,但不包含抗CD3抗体、IL-12和IL-18,和第三阶段培养基可包含IL-2、IL-12、和IL-18,但不包含抗CD3抗体。
可以以在上述范围内的浓度包含抗CD3抗体、L-谷氨酰胺、IL-2、IL-12和IL-18,并且可使用上述培养基。
可以以通用细胞培养方法进行培养,例如在CO2培养箱中。CO2浓度可为例如1至10%、具体地3至7%,温度可为30至40℃、具体地35至38℃,但不限于此。
可进行培养直至充分活化并增殖单核细胞例如3至20天,和具体地8至16天,但不限于此。
为了改善培养效率,优选添加涉及在培养期间增加细胞数量的培养基。例如,添加培养基的周期可为每1至10天、具体地1至7天一次,以防止培养基的劣化,但不限于此。
通过本发明的方法提供的细胞组合物是通过培养源自外周血的单核细胞来获得的异源细胞混合物,并且可包括细胞的各种表型。
表型可为,例如,CD3(+)CD56(-)细胞(T细胞)、CD3(+)CD56(+)细胞(NKT细胞)、CD3(-)CD56(+)细胞(NK细胞)等。
通过本发明的方法提供的细胞组合物可以以60%以上的比例包含表达干扰素-γ的细胞。
表达干扰素-γ的细胞可包括,例如,细胞中表达干扰素-γ的细胞(IFN-γ(+)细胞或IFN-γ释放细胞)。就这一点而言,细胞组合物具有总细胞中非常高比例的表达干扰素-γ的细胞,并且因此,在施用至患有特应性皮炎的受试者时可改善特应性皮炎的症状。
细胞组合物可具有60%以上、和具体地60%以上、70%以上、和80%以上的比例的表达干扰素-γ的细胞。对比例的上限没有特别限制,并可为小于100%,和具体地可为90%以下。
细胞组合物中表达NKG2D的细胞的比例可为60%以上。
表达NKG2D的细胞可为在总细胞中表达NKG2D的细胞(NKG2D(+)或NKG2D释放细胞)。就这一点而言,细胞组合物具有总细胞中高比例的表达NKG2D的细胞。通过在调控对特应性疗效的免疫平衡中起关键作用的、高水平的主要在Th1细胞中表达的NKG2D,可实现针对特应性皮炎的高治疗效果。
细胞组合物中的表达NKG2D的细胞的比例可为60%以上、和具体地60%以上、70%以上、和80%以上。对上述比例的上限没有特别限制,并可为小于100%,和具体地可为90%以下。
细胞组合物可包括各自小于10%的表达IL-4和IL-13的细胞。具体地,可为小于8%、和小于7%等。细胞组合物在预防和治疗特应性皮炎中是有效的,因为总细胞中表达IL-4和IL-13的细胞的比例低,并且导致特应性皮炎的例如IL-4和IL-13等细胞因子的分泌也低。

KSP37,也被称为FGFBP2(成纤维细胞生长因子结合蛋白2),是Th1/Tc1细胞特异性分泌蛋白。该蛋白由NK细胞、γ/δT细胞、效应CD8 T细胞和Th1细胞亚群产生并分泌至血清。大部分表达Ksp37的细胞可表达穿孔素,表明Ksp37参与细胞毒性淋巴细胞介导的免疫的主要过程。
GNLY,也被称为T细胞活化蛋白519,是与穿孔素和颗粒酶一起存在于细胞毒性细胞(CTL)和NK细胞的细胞溶解颗粒中的细胞溶解性和炎性蛋白。GNLY是皂素样蛋白(SAPLIP)家族的成员,表现显著的抗微生物活性和针对肿瘤细胞的强力的细胞毒性作用,激活抗原呈递细胞并起到免疫警报素(alarmin)的作用。当细胞毒性T细胞和NK细胞粘附至感染的细胞时,释放GNLY并起到对T细胞、单核细胞和其他炎性细胞的化学引诱物的作用。此外,由于其刺激包括RANTES、IL-1、IL-6、IL-10和IFN-γ的各种细胞因子的表达,由此诱导免疫平衡。此外,由于其杀伤靶细胞的生物学功能,可通过除去受损的和畸形的炎性细胞来改善特应性皮炎的症状。
当CD74结合至编码的蛋白时,CD74起到用于起始存活途径和细胞增殖的细胞因子巨噬细胞迁移抑制因子(MIE)的细胞表面受体的作用。MIF是诱导皮肤炎症的促炎细胞因子,并且可分泌自受损细胞并释放损伤相关分子如高迁移率族蛋白B1。此外,MIF可增加T细胞活化和侵袭。
HCST(=DAP10)是涉及CD8+T细胞中表达的NKG2D信号的传输的衔接分子,并且NKG2D和HCST的结合可产生涉及免疫活性的蛋白,诱导免疫受体酪氨酸激活基序(ITAM)的磷酸化、和诱导Syk(脾酪氨酸激酶)和Zap70(ζ链相关蛋白激酶70)的信号级联放大以分泌Th1细胞因子如干扰素-γ。
当ZBP1可调节如果激活的干扰素-β时,由此影响细胞溶质模式识别系统和激活免疫应答。由ZBP1的基因的活性和表达的增加与程序性坏死(necroptosis)的活化相关。程序性坏死是指除去病毒感染的细胞或抑制病毒的传播。当发生细胞凋亡性凋亡时,细胞中的DAMP(DNA、HSP、MSU等)被释放至细胞外。此时,一些DAMP刺激树突状细胞(DC),由于刺激而成熟的DC可活化T细胞。因此,ZBP1相关的细胞凋亡可除去病毒感染的细胞,以及使DC成熟和活化T细胞,从而诱导免疫活性。
CCL5可诱导Th1细胞活化,并且当与干扰素-γ一起起作用时,可诱导NK细胞的增殖和活性。
如上所述,特应性皮炎是由于由免疫功能紊乱的免疫失衡导致的,并且可由于Th2的过度活化而发生。通过本发明的方法提供的细胞组合物可抑制Th2增殖并活化Th1细胞以平衡免疫系统,从而改善特应性皮炎。

血液在室温下保温,然后以2000RPM离心两次3分钟(Ace:4,Dec:3)。将上清液的血浆收集在一个新的50ml管中,血细胞层也收集在另一个新的50ml管中。血浆在56℃水浴中灭活30分钟,然后以2000RPM离心3分钟(Ace:4,Dec:3)。将上清液收集在另一个新的50ml管中并用作血浆。通过以1:1的比例加入ALyS505N-0培养基稀释收集在50ml管中的血细胞层。然后,将4ml Ficoll放入15ml管中,向其中加入8ml血液-培养基混合物,然后以400RCF下离心30分钟(Ace:4,Dec:0)。弃去上层血浆(约3ml)后,尽量多地分离血浆和Ficoll层之间的PBMC层,转移至50ml管中,加入无菌生理盐水,使总体积达到50ml,然后以2000RPM离心3分钟(Ace:4,Dec:3)。除去上清液后,用10ml RBC裂解缓冲液分散细胞,然后振荡反应3分钟。反应完成后,用无菌生理盐水将总体积调至50ml,然后以2000RPM离心3分钟(Ace:4,Dec:3)。此后,除去上清液,将细胞分散在1ml ALyS505N-0培养基中,计数细胞以计算细胞总数。
通过将5μl抗CD3抗体(BD,小鼠抗人,#555329)、500μl的10xHBSS和4.5ml无菌生理盐水置于T25烧瓶中,包被T25烧瓶(37℃,4小时)。从CD3包被的T25烧瓶中除去包被溶液,然后用无菌生理盐水洗涤烧瓶两次。将PBMC以1x107个细胞/5ml的量接种到洗涤过的T25烧瓶中后,加入500μl的(自体)血浆、50μl的L-谷氨酰胺(2mM)、2.8μl的IL-2(1,000IU/ml)、1.5μl的IL-12(3ng/ml)、1.5μl的IL-18(30ng/ml)和4.5ml的ALyS505N-0培养基,然后在37℃、5%CO2的条件下孵育。
烧瓶和袋子用于培养EBI。使用烧瓶培养时,PBMC接种后第3天将500μl的(自体)血浆、50μl的L-谷氨酰胺(2mM)、2.8μl的IL-2(1,000IU/ml)和4.5ml的ALyS505N-0培养基加入由上述方法制备的T25烧瓶中。接种后第4天,将混合物转移至T75烧瓶中,向其中加入1ml的(自体)血浆、100μl的L-谷氨酰胺(2mM)、5.6μl的IL-2(1,000IU/ml)和9ml的ALyS505N-0培养基。接种后第5天,向其中加入2ml的(自体)血浆、200μl的L-谷氨酰胺(2mM)、11.2μl的IL-2(1,000IU/ml)和18ml的ALyS505N-0培养基。接种后第6天,将混合物转移至T150烧瓶中,向其中加入400μl的L-谷氨酰胺(2mM)、22.4μl的IL-2(1,000IU/ml)、12μl的IL-12(3ng/ml)、12μl的IL-18(30ng/ml)和40ml的ALyS505N-0培养基。接种后第7天,向其中加入500μl的L-谷氨酰胺(2mM)、28μl的IL-2(1,000IU/ml)、15μl的IL-12(3ng/ml)、15μl的IL-18(30ng/ml)和50ml的ALyS505N-0培养基。
接种后第8天,将5ml的IL-2(200IU/ml)加入培养袋中并按摩培养袋。用夹子固定培养袋的1/3部位后,除去约30ml培养基。使用刮刀除去T150烧瓶的细胞,然后转移至袋中。用除去的培养基洗涤烧瓶,并且将培养基中的细胞也转移至袋中。只将一只袋子放入一个孵育室并压平(37℃,5%CO2)。接种后第10天,在袋子按摩后,用夹子固定袋子的2/3部位,同时除去袋子的1/3部位处的夹子。接种后第12天,在袋子按摩后除去袋子2/3部位处的夹子。接种后第14天,背部按摩后将袋子固定在扣子上。用70%EtOH擦拭收集细胞的管线,分别使用250ml和50ml管收集细胞,然后以2000RPM离心3分钟(Ace:4,Dec:3)。将一部分上清液放入50ml管中,使用培养基将细胞收集到50ml管中,使总体积达到50ml,然后以2000RPM离心3分钟(Ace:4,Dec:3)。除去上清液后,将细胞沉淀溶解并分散在1ml培养基中,然后计数细胞。
2.根据培养期的细胞表型测定
(1)细胞活化
将样品(各培养期的EBI)在室温下以400xg离心5分钟。除去上清液后,调整细胞个数为2x106个细胞/ml,,将细胞悬浮于500μl培养液中。将500μl培养液加入到微管中,并按如下加入试剂以制备活化液。在24孔板的1孔中加入500μl细胞悬浮液,并向其中加入500μl制备的活化液。反应在5%CO2和37℃的条件下在CO2孵育箱中进行4小时。将保留在孔中的细胞悬浮液转移至15ml管中,通过加入4ml的1xPBS洗涤,然后以400xg离心5分钟。除去上清液后,加入5ml的FCM染色缓冲液并洗涤,然后以400xg离心5分钟。除去上清液后,加入200μl的FCM染色缓冲液以悬浮细胞。
(2)表面抗原染色
每个样品准备2个FCM管(每个培养周期的EBI),各个管称为iso管或样品管。将2μl的CD45-FITC抗体加入各个管中。将100μl的(1)的细胞悬浮液分配到各个管中,然后用微量移液管充分混合。将溶液在室温下在暗处放置30分钟并染色。静置后,将1ml的FCM染色缓冲液加入各管,然后在室温下以400xg离心5分钟。除去上清液后,将100μl的FCM染色缓冲液加入各管中以悬浮沉淀。将100μl的IC固定缓冲液加入各管后,在室温下在黑暗中反应至少20分钟(此时不超过1小时)。
(3)细胞内染色
通过用蒸馏水将10x透化缓冲液稀释10倍来制备1x透化缓冲液。将1ml的1x透化缓冲液加入各管后,在室温下以400xg离心5分钟。除去上清液后,将100μl的1x透化缓冲液加入各管中以悬浮沉淀。将同种型抗体加入iso管中,并将FCM抗体加入样品管中。各管轻轻混合(涡旋),然后在室温下在黑暗中放置30分钟,然后染色。静置后,将1ml的1x透化缓冲液加入各管中,然后在室温下以400xg离心5分钟。除去上清液后,将300至400μl的FCM染色缓冲液加入各管中以悬浮沉淀。测定时,按iso管和样品管的顺序进行测定。
3.细胞因子分析
对于细胞因子分析,使用人细胞因子阵列(目录号ARY005B)同时测定10种或更多细胞因子的量。在准备好的4孔多盘中处理2ml分析缓冲液,并在摇动振荡器中反应1小时。用制备的测定缓冲液处理1ml样品,并将最大总体积调整为1.5ml。样品用15μl人细胞因子阵列检测抗体混合物处理、混合、室温反应1小时。小心地取下膜并用1ml的1x洗涤缓冲液冲洗。链霉亲和素-HRP在5ml测定缓冲液中稀释,用2ml稀释的链霉亲和素-HRP处理各孔。通过将膜与X射线胶片曝光10分钟来测定各细胞因子的量。
4.特应性皮炎卵清蛋白(OVA)致敏特应性模型
在7周龄雌性Balb/c小鼠中,20μg卵清蛋白(OVA,V级,Sigma,St.Louis,MO,USA)和1mg氢氧化铝(Alum,Sigma)作为免疫佐剂,两者均用生理盐水稀释,每周一次腹膜内施用,持续2周以诱导特应性皮炎致敏。OVA腹膜内施用2周后,将100μg的OVA以1×1cm贴片贴于小鼠剪毛背部皮肤致敏1周,静置2周,依次重复3次在局部诱导皮炎。OVA致敏2周后,EBI(1x106个细胞/动物)和阳性对照组,即环孢菌素A(CsA,2mg/kg)尾静脉注射6周。
正常(NOR)组和OVA组注射生理盐水(100μl)。实验完成后,采血并切开背部皮肤(图1)。
5.感官评价试验(皮损损害评价)
作为特应性皮炎中常用的临床目视评价方法,特应性皮炎的严重程度用以下五项各自的评分的总和来表示。评价项目为红斑、瘙痒及皮肤干燥、水肿及表皮脱落、糜烂、苔藓样变等,每项评分为无症状(0分)、弱(1分)、中度(2分)、重度(3分)并且,作为5个项目的分数相加的结果,给予至少0分(无症状)至最高15分(所有项目的症状均重度)的评价分数。
在实验结束后立即使用数码相机(Canon,Tokyo,Japan)拍照来调查皮肤状况。
6.止痒能力评价
在实验结束前一天测定抓挠行为,以评价EBI施用抑制由于特应性皮炎引起的瘙痒的能力。将小鼠单独隔离并观察,将后爪向上到背部和向下到地板的行为评价为一次抓挠。此外,将连续操作视为一次,如果短暂中断后再次抓挠,则该操作计入次数。
7.组织学检查
1)苏木精和伊红染色
通过苏木精和伊红染色进行形态学分析,以确认由皮肤暴露于过敏原引起的炎性反应。实验结束后,将实验动物背部组织固定在10%中性缓冲福尔马林(NBF)中,由低浓度乙醇溶液到高浓度乙醇溶液逐步脱水,制备石蜡块。将各组织块切成5μm厚,将组织切片贴在载玻片上,然后将各组织载玻片用二甲苯脱蜡,然后在酒精中水合。在载玻片上加入苏木精试剂1分钟,然后加入伊红试剂3分钟,然后染色。染色完成后,将载玻片脱水密封,然后用光学显微镜(DM750,Leica,Wetzlar,Germany)观察。
2)甲苯胺蓝染色
将组织固定在10%NBF中后制备组织石蜡块,以通过甲苯胺蓝染色测定所收集的小鼠耳组织中脱颗粒的肥大细胞数量。将各组织块切片至5μm厚,脱蜡和在酒精中水合后用蒸馏水洗涤。洗涤后的切片用甲苯胺蓝(pH0.5)染色1小时,然后用蒸馏水洗涤3-4次。随后,通过脱水和透明工艺密封后,用光学显微镜(DM750,Leica,Wetzlar,Germany)测定脱粒肥大细胞的数量。
3)免疫组化染色
小鼠皮肤组织用10%NBF固定后形成石蜡块。将各组织块切成5μm厚,并将组织切片贴在载玻片上。用二甲苯对各组织载玻片进行脱蜡后,将其在酒精中水合并在5%的正常血清中封闭1小时。然后,将载玻片在4℃下用丝聚蛋白(filaggrin)、兜甲蛋白(loricrin)、外皮蛋白(involucrin)、闭合蛋白(occludin)、TRPA1和PGP9.5抗体处理过夜,以改善皮肤屏障。用PBST洗涤后,生物素化兔抗山羊IgG(1:100,Santa Cruz Biotec)作为二抗在室温下反应24小时,然后在室温下在亲和素-生物素复合物试剂盒(Vector Lab,USA)中反应1小时。在含有0.05%的3,3'-二氨基联苯胺和0.01%HCl的0.05M tris-HCl缓冲溶液(pH7.4)中显色,然后用苏木精复染。抗TRPA1和抗PGP9.5用FITC偶联的二抗处理1小时。使用共聚焦显微镜(LSM 700,ZEISS,Jena,Germany)获取荧光图像。
8.血液中总IgE的测定
取小鼠血液1.5mL置于管中,4℃、3000rpm离心30分钟。离心后,上清液保存于-70℃备用。使用酶联免疫吸附试验(ELISA)测定血液IgE。将各样品置于使用小鼠IgE ELISA试剂盒(BD Biosciences,San Jose,CA,USA)用一抗处理的96孔板中,并用工作溶液洗涤4次。洗涤后,样品用二抗HRP偶联山羊抗小鼠IgE(1:10,000)处理1小时,然后用显色剂显色。显色完成后,用终止液终止反应,并使用ELISA在450nm处测定吸光度。
9.ELISA
将用于分析血清和淋巴液中的细胞因子(IL-1β、IL-4、IL-5、IL-6、IL-17、TNF-α和CCL2)的来自分离的血清和淋巴液的Quansys Q-PlexTM小鼠细胞因子阵列(DonginBio,Seoul,Korea)用于分析各细胞因子的含量。将捕获抗体稀释于板上的包被缓冲液(0.1M碳酸钠,PH9.5)中,用密封胶带密封,并在4℃下贴壁过夜。将200μl测定稀释液分配于各孔中,用平板振荡器封闭1小时,将100μl各浓度特异性细胞因子标准溶液和血清分配于各孔中,并在室温下再次反应2小时。反应完成后,分配100μl检测抗体溶液1小时,然后使亲和素(Avidin)-HRP溶液反应30分钟。使用洗涤缓冲液(0.05%PBS-Tween 20)对除最后一次洗涤处理之外的处理步骤之间的所有板洗涤四次。使用洗涤缓冲液进行最终洗涤5次。洗涤结束后,将100μl TMB底物分配于各孔中进行显色,并反应20分钟。分配终止液(2N H2SO4)100μl完成反应,用酶标仪测定450nm波长处的吸光度,用于分析各细胞因子的含量。
将血清稀释1/2,收集淋巴液上清液,并根据制造商的说明进行多重阵列。将50μl抗原标准品或样品一式两份分配于各孔中。孵育1小时后,用洗涤缓冲液洗涤板,与检测混合物孵育1小时,并在室温下与链霉亲和素-HRP1X反应15分钟。洗涤孔后,加入底物A和B,用ChemiDoc XRS系统(Bio-Rad Laboratories)观察板的图像并用Quansys图像分析软件进行分析。为了标准化细胞因子数据,使用2.0荧光计(Life Technologies)上的蛋白测定试剂盒确定蛋白浓度,细胞因子水平表示为总蛋白的pg/ml。
10.RNA提取和实时PCR
使用实时PCR比较和分析了特应性皮炎中涉及的细胞因子基因表达的变化。从皮肤组织和培养细胞中提取总RNA,使用Tri-RNA试剂(Favorgen biotech,Taiwan of China)分离。使用PrimeScriptTM RT Master Mix(Takara,Tokyo,Japan)从整个RNA模板合成单链cDNA。使用qPCR 2x PreMIX SYBR(Enzynomics,Seoul,Korea)和CFX-96(Bio-Rad,Hercules,CA,USA)对生成的cDNA进行实时PCR。用于扩增所有基因的PCR在40个循环(95℃10秒,60℃ 15秒,72℃ 30秒)的条件下,在95℃下进行10分钟的变性步骤。上述步骤共进行40次。使用ΔCt量化方法将表达数据计算为循环阈值(Ct)。使用GAPDH进行定量。
11.免疫印迹分析
将分离的皮肤组织溶解在PRO-PREP(iNtRON,Seongnam,Korea)中,然后以14,000g离心20分钟,上清液用于实验。使用BCA试剂盒(Fisher Scientific,hampton,NH,USA)定量蛋白浓度。将分离的上清液(总蛋白量为30μg)使用8-12%凝胶SDS-PAGE进行电泳,以分离蛋白,然后将其转移至PVDF膜(Millipore,Danvers,MA,USA)。含有转移的蛋白的PVDF膜在5%脱脂牛奶中封闭1小时,在4℃下使一抗(丝聚合蛋白、兜甲蛋白、外皮蛋白、PAR2、TSLP、TSLPR、TRPA1、β-肌动蛋白)与膜反应12小时。用TBST洗涤反应的抗体后,使对一抗具有特异性的二抗在室温下反应1小时。清洗膜后,用ECL溶液(Millipore)显色,然后使用ChemiDocTMXRS+系统(Bio-RAD,Hercules,CA,USA)进行测定。
12.统计分析
实验结果以均值±标准误均值(SEM)表示,通过单因素方差分析(ANOVA)进行显著性检验,使用Turkey HDS法进行组间后检验,P值小于0.05被确定为具有统计学意义。
实验结果
1.根据培养期的细胞表型评价
为了确认根据实施例的细胞组合物的细胞表型,收集各培养期的一些细胞样品并分析表型,并使用NKG2A和NKG2D抗体。
其结果示于图2和图3。
干扰素-γ表达细胞(IFN-γ(+)细胞、IFN-γ释放细胞)的比例如下表1所示。

下面的图4和表6显示了根据培养期的细胞外干扰素-γ分泌的累积量的测定。可以看出,根据培养期,分泌量增加。

3.细胞因子分析
测定了本发明的细胞组合物在培养期分泌的特定活化因子的分泌量的变化(图5)。
可以看出,参与免疫细胞迁移和活性的各种免疫因子的水平增加了1000倍以上,并且7天后IL8、IL16和IL18的分泌迅速增加。根据这些结果,确定其中本发明的细胞组合物可以通过增加免疫活性细胞分泌因子的分泌来抑制特应性皮炎中通常已知的过度活化的Th2细胞(用于免疫抑制)的环境,并且可对特应性皮炎有效。
4.RNA测序
对于六(6)个基因(Ksp37、GLNY、CD74、HCST、ZBP1和CCL5),确定了特应性免疫细胞活化的3天、7天和14天的日期相关表达水平(图6和7)。
特别地,可以发现,在第14天EBI中的GLNY增加了60倍以上。EBI可以过表达上述基因以诱导免疫活性,从而改善特应性皮炎。

5.EBI对卵清蛋白(OVA)诱导的特应性皮炎的改善效果
对于BALB/c小鼠,对OVA皮肤致敏诱导特应性皮炎样病变的小鼠静脉注射EBI(1×106个细胞/头)6周,确认特应性皮炎的改善效果。
如图8所示,可以看出,在施用EBI的组中,特应性皮炎的临床特征如皮肤病变形状和皮肤厚度得到改善。
OVA诱导的对照组与正常小鼠相比,皮肤病症状(干燥,红斑,酸痛,肿胀/血肿,苔藓)和表皮厚度以及皮肤屏障崩解和炎症恶化,但施用EBI的实验组中的症状显示改善的效果(图8A至8D)。评价后,皮肤组织用H&E染色并在显微镜下检查以确定改善皮肤组织学的效果。结果,与正常组相比,OVA诱导的对照组具有显著增加的表皮厚度和皮肤屏障崩解。由于水肿,表皮和真皮显著扩大,可以确认表皮的去除和厚度的增加。然而,在施用EBI的组中,与对照组相比,皮肤组织学特征得到改善。角化过度和角化不全普遍减少,表皮厚度也减少,导致表皮增生和海绵形成减少。此外,可以看出,由于炎性反应而增厚的真皮的厚度也减少了,并且退化的毛囊也新形成(图8A和8B)。
此外,在施用EBI的组中,与对照组相比,特应性皮炎的严重程度(AD严重程度评分)得到改善(图8C),并且瘙痒明显减少,同时抓挠行为相对减少(图8D))。作为测定体重变化和观察对动物的影响的结果,EBI施用不影响小鼠的体重(图8E)。
6.EBI对卵清蛋白(OVA)诱导特应性皮炎皮肤屏障的改善效果
证实通过施用EBI显著改善了表皮水分损失量(TEWL)(图9A)。此外,作为测定角蛋白1(K1)基因表达变化以确认表皮细胞过度增殖的变化的结果,在施用EBI的小鼠中观察到显著降低(图9B)。换言之,表明EBI具有抑制OVA诱导的特应性样病变中表皮细胞增殖的作用。此外,为了测定改善皮肤屏障的效果,观察构成皮肤屏障的蛋白的变化。当发生特应性皮炎时,构成皮肤屏障的蛋白表达减少,从而导致屏障受损,同时增加水分流失、过敏原渗透和感染的风险。丝聚合蛋白、外皮蛋白和兜甲蛋白是构成皮肤表皮层的主要蛋白质,对改善皮肤屏障很重要。此外,闭合蛋白是一种紧密连接蛋白,在皮肤病如特应性皮炎的情况下会减少。证实了由于施用EBI同时增加蛋白的表达,OVA诱导的特应性皮炎样病变得到改善(图9B至9D)。施用EBI与施用CsA一起改善了干燥症状,并被认为通过保护皮肤屏障有效缓解特应性皮炎的症状。
7.EBI对卵清蛋白(OVA)诱导的炎性反应的抑制效果
通过甲苯胺蓝染色,与炎性反应如肥大细胞相关的现象可以被确认为组织的深蓝色部分(图10A)。作为OVA诱导后7周(49天)检查组织的结果,在OVA组中,大量肥大细胞浸润真皮周围,表皮厚度也较厚,而EBI组观察到与对照组相比,肥大细胞浸润减少(图10A和10B)。
此外,在OVA诱导的特应性皮炎模型中,OVA致敏组的血清总IgE浓度(647.5ng/mL)与正常对照组(NOR,220.0ng/mL)相比显著升高。通过施用CsA和EBI,IgE的增加分别显著降低至328.0ng/mL和320.5ng/mL,从而证明显著效果(图10C和10D)。
8.EBI对卵清蛋白(OVA)诱导的细胞因子表达的抑制效果
为了确认EBI-M的过敏性皮肤反应和IgE抑制效果是否与Th2免疫应答有关,从OVA致敏小鼠中分离皮肤组织,并确认Th2细胞因子的mRNA表达水平。作为实验的结果,与NOR组相比,OVA处理组中Th2分化诱导细胞因子TSLP、IL-25、IL-33以及Th2和Th17相关细胞因子的表达显著增加。另一方面,在施用CsA和EBI的小鼠中,IL-4降低了46.8%,IL-13降低了47.3%,TSLP降低了53.1%,IL-33降低了49.2%,从而表现明显的抑制。此外,作为阳性对照组的CsA处理组也分别显示降低了33.6、35.4、45.8和57.9%,表明它们抑制Th2细胞因子(图11A)。此外,作为血清和淋巴液中存在的炎性细胞因子的分析结果,IL-1β、IL-4、IL-5、TNF-α、IL-17、IL-6和CCL2(MCP-1)通过EBI施用显著减少(图11B和11C)。因此,EBI抑制Th2细胞相关细胞因子的表达,被认为通过抑制肥大细胞浸润和IgE产生等来减轻特应性皮炎的症状。
9.EBI对卵清蛋白(OVA)引起的瘙痒的改善效果
为了观察EBI对OVA诱导的特应性皮炎样病变皮肤瘙痒的改善效果,研究了各种细胞因子的表达和已知与瘙痒有关的因素。可以看出,与IL-31信号传导相关的基因的表达在施用EBI的小鼠中降低(图12A)。此外,瘙痒刺激分为两种类型:组胺依赖性途径和组胺非依赖性途径。作为施用EBI的结果,证实了与瘙痒刺激信号传导相关的基因如H1R、TRPV1、TRPA1、TGF5、NK1R、MrgpA3、TSLP和TSLPR等的表达显著降低(图12A)。该结果在蛋白表达水平方面也得到证实(图12B)。
TSLP也是与特应性皮炎有关的瘙痒的介质,并且瘙痒是由组胺非依赖性途径引起的。TSLP在c神经纤维中积累钙,需要激活TRPA1,神经的激活引起瘙痒。证实EBI施用抑制了皮肤表皮神经纤维中TRPA1的表达,从而减轻了瘙痒(图12C)。此外,与对照组相比,EBI施用组皮肤中对PGP9.5表现免疫应答的神经纤维分布显著减少,该结果与TRPA1的免疫应答中的神经元数量的减少基本一致(图12C)。
10.PBMC施用组与EBI施用组之间的比较
对于OVA诱导的特应性皮炎模型,EBI以1×106个细胞/头的量每周一次静脉内施用,总共6次。在SC组的情况下,EBI以1×106个细胞/头的量每周一次皮下施用,总共6次。此外,PBMC以1×106个细胞/头的量每周一次静脉内施用,总共6次。
(1)特应性皮炎改善效果的确认
其结果示于图13。
可以看出,与不分泌干扰素-γ的PBMC施用组相比,具有高干扰素-γ和NKG2D表达率的EBI施用组的皮肤改善效果优异。
(2)根据细胞组合物施用的体重变化的确认
其结果示于图14。所有实验组的体重均无明显变化。
(3)通过细胞组合物施用的经皮厚度减少效果的确认
其结果示于图15和16。与PBMC施用组相比,具有高干扰素-γ和NKG2D表达率的EBI施用组在所有施用途径中显示经皮厚度减少。
(4)炎性反应减少效果的确认
其结果示于图17和18,与不分泌干扰素-γ的PBMC施用组相比,具有高干扰素-γ和NKG2D表达率的EBI施用组在所有施用途径中显示肥大细胞数量减少,从而减少炎性反应。
(5)血液中IgE减少的确认
其结果示于图19。与不分泌干扰素-γ的PBMC施用组相比,具有高干扰素-γ和NKG2D表达率的EBI施用组在所有施用途径中显示IgE降低。
(6)皮肤组织中特应性皮炎相关细胞因子和瘙痒相关基因的表达分析
作为结果,与PBMC施用组相比,炎性细胞因子和瘙痒相关基因的表达在具有高干扰素-γ和NKG2D表达率的EBI施用组的所有施用途径中减少,可以看出含有1×106个细胞的EBI施用组显示最高的效果(图20)。此外,干扰素-γ在淋巴结中显示大量的分泌(图21)。
Claims (14)
1.一种生产细胞组合物的方法,所述细胞组合物具有60%以上的比例的表达干扰素-γ的细胞,所述方法包括:
从人外周血分离和获得单核细胞和自体血浆;
用抗CD3抗体涂布细胞培养容器;和
将所述单核细胞接种至所述细胞培养容器并在含有选自由IL-2、IL-12和IL-18组成的组的至少一种的培养基中培养所述单核细胞。
2.根据权利要求1所述的方法,其中所述组合物中表达NKG2D的细胞的比例为60%以上。
3.根据权利要求1所述的方法,其中与外周血单核细胞(PBMC)相比,选自由以下组成的组的一种或多种基因表达5倍以上:KSP37(37kDa的杀伤特异性分泌蛋白)、GNLY(颗粒溶素)、CD74(分化簇74)、ZBP1(Z-DNA结合蛋白1)、CCL5(C-C趋化因子受体类型5)和HCST(造血细胞信号转导蛋白)。
4.根据权利要求1所述的方法,其中所述培养以3个以上的阶段进行,
其中第一阶段培养是通过将所述单核细胞接种在涂布有所述抗CD3抗体的细胞培养容器中并向其添加含有IL-2、IL-12和IL-18的培养基来进行的;
第二阶段培养是通过将第一阶段培养产物转移至未涂布有所述抗CD3抗体的细胞培养容器中并添加含有IL-2的培养基来进行的;和
第三阶段培养是通过在含有IL-2、IL-12和IL-18的培养基中培养第二阶段培养产物来进行的。
5.根据权利要求1所述的方法,其中所述抗CD3抗体的浓度为1至10μg/ml,IL-2的浓度为800至1200IU/mg,IL-12的浓度为2至6ng/ml,和IL-18的浓度为20至60ng/ml。
6.一种作为异源细胞的混合物的细胞组合物,所述细胞组合物是人外周血单核细胞的培养产物,其中总细胞中表达干扰素-γ的细胞的比例为60%以上。
7.根据权利要求6所述的细胞组合物,其中所述细胞组合物中总细胞中表达干扰素-γ的细胞的比例为80%以上。
8.根据权利要求6所述的细胞组合物,其中所述细胞组合物中总细胞中表达NKG2D的细胞的比例为60%以上。
9.根据权利要求6所述的细胞组合物,其中与外周血单核细胞(PBMC)相比,选自由以下组成的组的一种或多种基因表达5倍以上:KSP37(37kDa的杀伤特异性分泌蛋白)、GNLY(颗粒溶素)、CD74(分化簇74)、ZBP1(Z-DNA结合蛋白1)、CCL5(C-C趋化因子受体类型5)和HCST(造血细胞信号转导蛋白)。
10.根据权利要求6所述的细胞组合物,其中所述组合物中的细胞总数的范围为1x 108至1x 1010个细胞。
11.根据权利要求6所述的细胞组合物,其中所述细胞组合物是通过在含有选自由抗CD3抗体、IL-2、IL-12和IL-18组成的组的一种或多种的培养基中培养分离自人外周血的单核细胞来获得的。
12.根据权利要求11所述的细胞组合物,其中所述培养是以三个以上的阶段进行的,
其中第一阶段培养是通过将所述单核细胞接种在涂布有所述抗CD3抗体的细胞培养容器中并向其添加含有IL-2、IL-12和IL-18的培养基来进行的;
第二阶段培养是通过将第一阶段培养产物转移至未涂布有所述抗CD3抗体的细胞培养容器中并添加含有IL-2的培养基来进行的;和
第三阶段培养是通过在含有IL-2、IL-12和IL-18的培养基中培养第二阶段培养产物来进行的。
13.根据权利要求11所述的细胞组合物,其中所述抗CD3抗体的浓度为1至10μg/ml,IL-2的浓度为800至1200IU/mg,IL-12的浓度为2至6ng/ml,和IL-18的浓度为20至60ng/ml。
14.一种用于预防或治疗特应性皮炎的药物组合物,其包含根据权利要求6至13中任一项所述的细胞组合物。
