Abstract
本发明涉及一种通用型供临床多次回输异体DNT细胞的高效体外扩增方法。在本发明的方法中,在连续培养过程中分阶段递减T细胞活化剂的用量,而无需额外添加白细胞介素‑4或AB血清。使用本发明方法所获得的通用型DNT细胞,具有纯度高、体外杀瘤活性高、可复苏培养、可多次回输或可用于多位患者使用的显著优势,具有较大的临床应用前景。

免疫细胞治疗是一种新兴的、具有显著疗效的肿瘤治疗模式,是一种免疫抗癌的新型治疗方法。它是运用生物技术和生物制剂对从病人或健康捐赠者体内采集的免疫细胞进行体外培养和扩增后回输到病人体内的方法,用以激发、增强机体自身免疫功能,从而达到治疗肿瘤目的。
免疫细胞治疗是许多危及人类严重疾病的重要治疗手段,包括癌症、自身免疫疾病、器官移植排斥反应、严重过敏性疾病,甚至严重病毒感染都有使用免疫细胞治疗成功的案例。
目前免疫细胞治疗大多是采集自体免疫细胞于体外培养扩增,然后进行回输。如果需要多次回输治疗,则要多次采集人外周血中的免疫细胞进行培养扩增,才能满足临床回输需求。。由于病情严重,多数需要治疗的患者常无法提供足够数量的免疫细胞进行培养,且患者由于患病体质和各种用药治疗,导致免疫细胞在体外培养扩增非常困难,甚至无法培养。
因此,本领域需要一种从健康捐赠者获取免疫细胞且可用于多次回输的通用型免疫细胞体外扩增方法,方便临床治疗使用并避免多次采集-多次回输给临床带来的不便,同时消除由于肿瘤病人外周血质量不高甚至存在肿瘤细胞的风险。
发明内容
本发明的目的就是提供一种全新的通用型供临床多次回输异体DNT细胞的高效体外扩增方法。
在本发明的第一方面,提供了一种通用型DNT细胞的体外扩增方法,其特征在于,包括步骤:
(i)提供一获自供体的外周血起始样品I;
(ii)去除起始样品中的CD4+和CD8+T细胞,从而获得样品II;
(iii)在含有适合DNT细胞生长的培养基的培养体系中,培养样品II,从而获得样品III;其中,所述培养体系中包括固定的T细胞促分裂原;
(iv)在含有适合DNT细胞生长的培养基的培养体系中,培养样品III,从而获得样品IV;其中,所述培养基中含有浓度为30-80ng/mL(较佳地40-60ng/mL,更佳地50ng/mL)的可溶性T细胞促分裂原;
(v)在含有适合DNT细胞生长的培养基的培养体系中,培养样品IV,其中,所述培养基中含有浓度为15-40ng/mL(较佳地20-30ng/mL,更佳地25ng/mL,更佳地为亚步骤(va)中所述T细胞促分裂原的添加量的1/2)的可溶性T细胞促分裂原,从而获得所需量的通用型DNT细胞,为样品V;和
(vi)在含有适合DNT细胞保存的溶液体系中,收集样品V;其中,所述溶液中含有人血白蛋白,从而获得临床使用的通用型DNT细胞,为DNT细胞制剂;
其中,所述步骤(iii)至(v)的培养体系中均含有200-1000IU/mL(较佳地为300-700IU/mL,更佳地为500IU/mL)的重组人白介素2,并且均不含有重组人白介素4,并且均不含AB血清。
在另一优选例中,所述步骤(i)中,所述的供体是健康供体。
在另一优选例中,所述样品I的量为10-300ml,优选地为20-200ml。
在另一优选例中,所述样品II是冻存后复苏的细胞溶液。
在另一优选例中,所述步骤(ii)包括:去除起始样品中的CD4+和CD8+T细胞,从而获得样品IIa(富集的DNT细胞);离心收集样品IIa(富集的DNT细胞)后,以冻存液重悬,程控降温后置于液氮保存,此为样品IIb(富集冻存的DNT细胞);样品IIb冻存后,于37℃化冻,以培养基洗涤一次,再以培养基重悬,此为样品IIc(富集冻存再复苏的DNT细胞)。
在另一优选例中,在所述步骤(iii)至(v)的培养体系中,培养条件为37℃、5%CO2。
在另一优选例中,在所述步骤(iii)至(v)的培养基中,还包括选自下组的细胞因子:IL-7、IL-12、IL-15,或其组合。
在另一优选例中,所述步骤(iii)之前,还包括步骤(iia):用0.9%生理盐水洗涤样品II。
在另一优选例中,所述T细胞促分裂原选自下组:结合CD3的抗体、凝集素、能够刺激DNT细胞扩增的化合物,或其组合。
在另一优选例中,凝集素选自下组:植物凝集素伴刀豆球蛋白A(ConA)、植物凝集素(PHA)、大豆凝集素(SBA),或其组合。
在另一优选例中,所述能够刺激DNT细胞扩增的化合物选自下组:IPP、帕米膦酸(Pamidronate)、唑来膦酸(Zoledronate),或其组合。
在另一优选例中,所述T细胞促分裂原是结合CD3的抗体。
在另一优选例中,所述步骤(iii)中的培养体系位于培养瓶中,优选地为T25培养瓶。
在另一优选例中,所述固定的T细胞促分裂原是指包被于培养瓶或微孔板上的T细胞促分裂原。

在另一优选例中,在步骤(iv)中,包括2-5次(较佳地为3-4次,更佳地为3次)DNT细胞的扩增,在每次扩增中,待扩增的DNT细胞的初始浓度各自独立地为1×106至3×106个细胞/mL,并且每次扩增的培养时间为24-72小时,较佳地为36-60小时,更佳地为48小时。
在另一优选例中,在步骤(iv)中,还包括检测DNT细胞表型和活率。
在另一优选例中,所述步骤(iv)的培养时间为72-168小时,较佳地为96-144小时,更佳地为120小时。
在另一优选例中,在步骤(v)中,包括2-4次(较佳地为2次)DNT细胞的扩增,在每次扩增中,待扩增的DNT细胞的初始浓度各自独立地为1×106至3×106个细胞/mL,并且每次扩增的培养时间为24-72小时,较佳地为36小时。
在另一优选例中,在步骤(v)中,还包括检测DNT细胞表型和活率。
在另一优选例中,所述步骤(v)的培养时间为48-96小时,较佳地为72小时。
在另一优选例中,在步骤(vi)中,包括步骤:
(via)离心收集样品V中的DNT细胞;
(vib)以含2.5%人血白蛋白的生理盐水(或“溶媒”)洗涤DNT细胞;和
(vic)以含2.5%人血白蛋白的生理盐水(或“溶媒”)将DNT细胞调整为1×108个细胞/mL的浓度。
在本发明的第二方面,提供了一种有效量的如本发明第一方面所述的方法获得的DNT细胞的用途,用于制备一药物组合物或制剂,所述药物组合物或制剂用于:
(a)预防和/或治疗肿瘤;
(b)预防和/或治疗感染性疾病;
(c)预防和/或治疗自身免疫性疾病;
(d)预防和/或治疗变态反应;
(e)预防和/或治疗移植物抗宿主疾病;和/或
(f)调节免疫应答。
在另一优选例中,所述肿瘤为与所述DNT细胞异体的肿瘤。

(a)预防和/或治疗肿瘤;
(b)预防和/或治疗感染性疾病;
(c)预防和/或治疗自身免疫性疾病;
(d)预防和/或治疗变态反应;
(e)预防和/或治疗移植物抗宿主疾病;和/或
(f)调节免疫应答。
应理解,在本发明范围内中,本发明的上述各技术特征和在下文(如实施例)中具体描述的各技术特征之间都可以互相组合,从而构成新的或优选的技术方案。限于篇幅,在此不再一一累述。

具体实施方式
本发明人经过广泛而深入的研究,经过大量的工艺优化实验,首次开发了一种实用型一次获取供者外周血供临床多次回输异体DNT细胞的高效体外扩增方法。具体地,本发明优化了DNT细胞的培养配方和培养工艺,相比于现有的DNT细胞的体外扩增方法,本发明方法中无需额外添加IL-4,并且在连续培养过程中分阶段递减T细胞活化剂的用量(此处为抗人CD3抗体),可获得高纯度的通用型DNT细胞,大大减少终产品中杂质含量。
结果表明,本发明的方法可用于从人外周血新鲜采集并富集DNT细胞的体外高效扩增,也可以用于先行冻存的上述DNT细胞,再后续复苏后体外高效扩增DNT细胞。使用本发明工艺扩增所得的DNT细胞数量大,并且其中DNT细胞特征性表面标志CD3+CD4-CD8-T细胞的纯度高(>85%)。由于DNT细胞的杀瘤活性不依赖T细胞受体,因此,使用同一供者制备的DNT细胞可以提供给不同患者临床治疗使用。
在此基础上,完成了本发明。
术语
为了可以更容易地理解本公开,首先定义某些术语。如本申请中所使用的,除非本文另有明确规定,否则以下术语中的每一个应具有下面给出的含义。在整个申请中阐述了其它定义。
如本文所用,术语“约”可以是指在本领域普通技术人员确定的特定值或组成的可接受误差范围内的值或组成,其将部分地取决于如何测量或测定值或组成。
如本文所用,术语“给予”、“施用”可互换使用,是指使用本领域技术人员已知的各种方法和递送系统中的任一种将本发明的产品物理引入受试者,包括静脉内、肌内、皮下、腹膜内、脊髓或其它肠胃外给药途径,例如通过注射或输注。
本发明的体外扩增方法
在本发明中,提供了一种通用型DNT细胞的体外大量扩增方法,所述的方法包括步骤:
(i)提供一获自供体的外周血起始样品I;
(ii)去除起始样品中的CD4+和CD8+T细胞,从而获得样品II;
(iii)在含有适合DNT细胞生长的培养基的培养体系中,培养样品II,从而获得样品III;其中,所述培养体系中包括固定的T细胞促分裂原;
(iv)在含有适合DNT细胞生长的培养基的培养体系中,培养样品III,从而获得样品IV;其中,所述培养基中含有浓度为30-80ng/mL(较佳地40-60ng/mL,更佳地50ng/mL)的可溶性T细胞促分裂原;
(v)在含有适合DNT细胞生长的培养基的培养体系中,培养样品IV,其中,所述培养基中含有浓度为15-40ng/mL(较佳地20-30ng/mL,更佳地25ng/mL,更佳地为亚步骤(va)中所述T细胞促分裂原的添加量的1/2)的可溶性T细胞促分裂原,从而获得所需量的通用型DNT细胞,为样品V;和

如本文所用,术语“双阴性T细胞”和“DNT细胞”可互换使用,是指一种T淋巴细胞亚群,其细胞表面表达CD3分子,但缺乏CD4、CD8分子以及CD16/CD56分子。其中,CD4是辅助性T细胞特征性表面分子,CD8是细胞毒T细胞特征性表面分子,CD16/CD56是NK细胞的特征性表面分子。DNT细胞表达T细胞受体(TCR),可以是αβ或者γδTCR。通过本发明方法扩增的DNT细胞亚群可以包括αβ+和γδ+T细胞的混合物。在一个优选的实施方式中,DNT细胞是来源于人外周血的细胞。
起始样品可以来源于含有双阴性T细胞或者其前体的任何生物学样品。这类样品包括但不限于新鲜或者冷藏保存的血液、骨髓、淋巴组织、胸腺、肝、脾、淋巴结组织、肿瘤组织、胎儿组织细胞和其级分或者富集的部分,也可以来自诱导性多潜能干细胞(inducedPluripotent Stem Cell,iPSC)。
在一个优选的实施方式中,起始样品是血液,优选人血液,更优选人外周血。
在培养起始样品或者其级分前,起始样品基本上去除CD4+和CD8+T细胞。“基本上”是指这些细胞中的大部分被去除但不排除这些细胞中的小部分仍然残留。
可以利用本领域己知的技术去除样品中的这些细胞类型。具体地,可以向起始样品中添加抗体,所述抗体结合待去除的CD8+和CD4+细胞但不结合双阴性T细胞。在一个优选的实施方式中,向样品中添加特异结合CD4和CD8的标记的抗体。
在另一个优选的实施方式中,向样品中添加特异结合CD4和CD8的磁珠。
一旦起始样品中己经去除CD4+和CD8+T细胞,本发明的方法可以立即继续或者该样品可以被冷冻并保存,可以是低温冰箱或设备(-80℃以下)或液氮,用于以后的使用。因此当需要DNT细胞时,DNT细胞的体外扩增可以从这个时间的冷冻样品开始。
在本发明的一个实施方式中,提供了一种从健康供者一次获取外周血(100-400ml),在体外富集DNT细胞后立即冻存,按临床回输方案复苏该富集的DNT细胞,用于本发明的体外扩增的方法。
优选地,所述的本发明方法的步骤(ii)可包括:去除起始样品中的CD4+和CD8+T细胞,从而获得样品IIa(富集的DNT细胞);样品IIa可以直接以本发明的体外扩增方法进行培养,也可以液氮冻存后复苏再培养;样品IIa(富集的DNT细胞)离心收集后,以冻存液重悬,程控降温后置于液氮保存,此为样品IIb(富集冻存的DNT细胞);样品IIb冻存后,依临床需求进行复苏培养;于37℃化冻后,以培养基洗涤一次,再以培养基重悬,此为样品IIc(富集冻存再复苏的DNT细胞)。
在培养基中培养己经基本上去除CD4+和CD8+细胞的样品,所述培养基包括固定的T细胞促分裂原以及能够刺激DNT细胞生长的试剂。
固定的T细胞促分裂原可以是能够刺激双阴性T细胞的任意试剂,包括但不限于结合CD3或者T细胞受体的抗体以及凝集素包括植物凝集素伴刀豆球蛋白A(ConA)和植物凝集素(PHA)或者能够刺激DNT细胞扩增的任意化合物例如但不限于IPP、帕米膦酸(Pamidronate)和唑来膦酸(Zoledronate)。优选地,T细胞促分裂原是CD3的抗体例如OKT3。
可以利用本领域己知的技术固定T细胞促分裂原。优选地,T细胞促分裂原被包被到固相支持物,包括但不限于微孔板、培养皿、培养袋,或者培养瓶。优选地,T细胞促分裂原是固定的抗CD3抗体,更优选地固定在微孔板上。
能够刺激DNT细胞生长的试剂可以是任意合适的试剂,优选是细胞因子,例如白介素。优选地,细胞因子包括白介素-2(IL-2)、白介素-7(IL-7)、白介素-12(IL-12)、白介素-15(IL-15)或者这些中两种或更多种的混合物。
特别值得注意的是,在本发明中,扩增培养所用的培养基中不包含IL-4,并且不含AB血清。
试剂的浓度应当适于促进双阴性细胞的扩增。优选地,细胞因子给药范围从大约200IU/mL到1000IU/mL。在一个特别优选的实施方式中,IL-2在培养基中的浓度为500IU/mL。
培养基可以是适于T细胞培养的任意培养基,包括但不限于AIM-V培养基、RPMI培养基、X-VIVOI0和X-VIVOI5、和任何其他商业化无动物血清培养基。培养基优选地含有其他合适试剂包括抗生素。
在一个实施方式中,在用能够刺激DNT细胞生长的试剂和固定的T细胞促分裂原培养时,细胞可与DC细胞共培养或用抗原和细胞因子刺激。在抗肿瘤DNT细胞的制备中,可以使用灭活的肿瘤细胞或者来自肿瘤特异的或者肿瘤相关的抗原、肽或新生抗原(neoantigens)。
细胞优选地在步骤(ii)-(v)中的任意一个步骤培养一段时间,范围从约2-8天。优选地,每个步骤进行大约3-7天,更优选4-6天。
通过本方法制备的DNT细胞的纯度可以利用本领域己知的技术例如流式细胞术或其它活细胞表型鉴定技术进行证实。
本发明制得的DNT细胞的用途
本发明还包括通过本发明方法获得的双阴性T细胞在任意和所有应用中的用途。本发明方法所制得的异体T细胞可以用于多种肿瘤治疗,可以大规模制备,质量稳定可控,可及时给任何适用患者使用。
在本发明的实施方式中,通过本发明方法扩增的双阴性T细胞具有较强的抗肿瘤作用。因此,在一个实施方式中,本发明提供一种治疗肿瘤的方法,包括给予需要其的动物有效量的通过本发明方法获得的通用型DNT细胞。本发明还包括通过本发明方法获得的有效量的通用型DNT细胞用于治疗肿瘤的用途。本发明还包括有效量的通过本发明方法获得的通用型DNT细胞在制备治疗肿瘤的药物中的用途。
此处使用的术语“有效量”是指能够在剂量和需要的时间内有效地实现所需结果例如治疗癌症的剂量。
此处使用的术语“动物”包括动物界的所有成员,包括人。在一个优选的实施方式中,所述动物是人。
术语“治疗”包括但不限于疾病或者病症(例如癌症、移植排斥和移植物抗宿主疾病、自身免疫病、变态反应、感染等等)的一种或多种症状的减轻或者改善、疾病程度的减轻、疾病的稳定化状态、预防疾病传播、疾病进展的延迟或者减缓和疾病状态的改善或者缓解,可检测的或者不能检测的症状缓解和/或与如果不接受治疗的预期存活相比延长的存活。
在肿瘤或者癌症的治疗中,能够治疗的肿瘤是可以单独用双阴性T细胞或者与其他治疗例如手术、放疗或者化疗,包括各种靶向治疗药物、免疫检验点抑制剂、免疫细胞增强剂和增强DNT细胞在肿瘤组织浸润的纳米材料等联合应用进行治疗的任何肿瘤。
可治疗的癌症包括没有被血管化或基本上还没有被血管化的肿瘤,以及血管化的肿瘤。癌症可包括非实体瘤(诸如血液学肿瘤,例如白血病和淋巴瘤)或可包括实体瘤。用本发明治疗的癌症类型包括但不限于癌、胚细胞瘤和肉瘤,和某些白血病或淋巴恶性肿瘤、良性和恶性肿瘤、和恶性瘤,例如肉瘤、癌和黑素瘤。也包括成人肿瘤/癌症和儿童肿瘤/癌症。
血液学癌症为血液、骨髓、或淋巴组织的癌症。血液学(或血原性)癌症的例子包括白血病,包括急性白血病(诸如急性淋巴细胞白血病、急性髓细胞白血病、急性骨髓性白血病和成髓细胞性、前髓细胞性、粒-单核细胞型、单核细胞性和红白血病)、慢性白血病(诸如慢性髓细胞(粒细胞性)白血病、慢性骨髓性白血病和慢性淋巴细胞白血病)、真性红细胞增多症、淋巴瘤、霍奇金氏疾病、非霍奇金氏淋巴瘤(无痛和高等级形式)、多发性骨髓瘤、瓦尔登斯特伦氏巨球蛋白血症、重链疾病、骨髓增生异常综合征、多毛细胞白血病和脊髓发育不良。
实体瘤为通常不包含囊肿或液体区的组织的异常肿块。实体瘤可为良性或恶性的。不同类型的实体瘤以形成它们的细胞类型命名(诸如肉瘤、癌和淋巴瘤)。实体瘤诸如肉瘤和癌的例子包括纤维肉瘤、粘液肉瘤、脂肪肉瘤间皮瘤、淋巴恶性肿瘤、胰腺癌卵巢癌。
本发明还包括DNT细胞的其他治疗用途,例如感染性疾病的治疗和例如在自身免疫疾病、变态反应、移植排斥和移植物抗宿主疾病的治疗中用于调节免疫应答。在这种情况下,制备DNT细胞的方法可以包括添加合适的感染物、变态反应原细胞或者组织作为抗原。
因此,在一个实施方式中,本发明提供一种治疗感染性疾病的方法,包括给予有效量的通过本发明方法获得的双阴性T细胞。本发明还包括用有效量的通过本发明方法获得的双阴性T细胞治疗感染性疾病的用途。本发明还包括通过本发明方法获得的有效量的双阴性T细胞在制备治疗感染性疾病的药物中的用途。
在进一步的实施方式中,本发明提供一种调节免疫应答的方法,包括给予有效量的通过本发明方法获得的双阴性T细胞。本发明还包括有效量的通过本发明方法获得的双阴性T细胞用于调节免疫应答的用途。本发明还包括有效量的通过本发明方法获得的双阴性T细胞在制备调节免疫应答的药物中的用途。
在一个实施方式中,DNT细胞用于治疗自身免疫病。根据本发明可以治疗的自身免疫疾病包括但不限于糖尿病、关节炎、多发性硬化、红斑狼疮、炎症性肠病、皮炎、脑膜炎、血栓性血小板减少性紫癫、综合症、脑炎、眼葡萄膜炎、白细胞粘附缺陷、风湿热、Reiter’s综合征、进行性全身性硬化、原发性胆汁性肝硬变、坏死性脉管炎、重症肌无力、多发性肌炎、结节病、肉芽肿病、血管炎、恶性贫血、CNS炎性病症、抗原抗体复合物介导的疾病、自身免疫溶血贫血、淋巴瘤性甲状腺肿、突眼性甲状腺肿、习惯性白发流产、Raynaud’s综合征、肾小球性肾炎、皮肌炎、慢性活动性肝炎、乳麽泻、组织特异性自身免疫、退行性自身免疫迟发性超敏反应、AIDS的自身免疫并发症、萎缩性胃炎、强直性脊柱炎和Addison’s病。
在另一个实施方式中,DNT细胞可用于治疗移植物抗宿主疾病,其中移植物中的免疫细胞对受体正常组织的免疫攻击。当被移植的组织含有免疫细胞时这种情况可能存在,例如当治疗白血病、再生障碍性贫血和酶或者免疫缺陷而移植健康供者的骨髓或者淋巴组织时。
在进一步的实施方式中,DNT细胞可用于治疗变态反应。在变态反应中,免疫系统对通常无毒无害的抗原或者变态反应原的攻击。利用本发明方法可以预防或者治疗的变态反应包括但不限于枯草热、哮喘、异位性湿疹和对槲叶毒葛和常春藤、室内尘螨、蜂花粉、坚果、甲壳类动物、青霉素和许多其他物质的变态反应。
通过本发明方法制备的DNT细胞可以配制成以适于体内给药的生物相容性形式给予对象的药物组合物。“适于体内给药的生物相容性形式”是指被给予物质的一种形式,其中治疗效果超过任意毒性作用。物质可以给予活的生物,包括人和动物。组合物可以适当的方式给药,优选通过注射例如静脉内、皮下、肌内等等。
此处描述的组合物可以通过本来己知的用于制备药学上可接受的组合物的方法制备,所述组合物能够给予对象,这样有效量的细胞与药学上可接受载体组合为混合物。例如Remington’s Pharmaceutical Sciences(Remington’s Pharmaceutical Sciences,20th ed.,Mack Publishing Company,Easton,Pa.,USA2000)中描述了合适的载体。在此基础上,组合物包括但不限于物质与一或多种药学上可接受的载体或者稀释剂结合的溶液,在具有合适pH和等渗的缓冲溶液中含有生理溶液。
根据因素例如个体的疾病状态、年龄、性别和体重和细胞在个体引发所需应答的能力,组合物的有效量可能不同。可以调节剂量方案以提供最适治疗应答。例如,数个分开的剂量可以每周给药,或者剂量可以按照治疗情况需要所示按比例减少。
可以和本发明联合应用的药物包括可用于治疗待治疾病或者病症的其他活性物质。例如,在肿瘤治疗中,其他抗癌剂可以在相同组合物或者分离的组合物中给予。
本发明的药物组合物可以以适于待治疗(或预防)的疾病的方式施用。施用的数量和频率将由这样的因素确定,如患者的病症、患者疾病的类型和严重度——尽管适当的剂量可由临床试验确定。
当指出“免疫学上有效量”、“抗肿瘤有效量”、“肿瘤-抑制有效量”或“治疗量”时,待施用的本发明组合物的精确量可由临床试验确定,其考虑患者(对象)的年龄、重量、肿瘤大小、感染或转移程度和病症的个体差异。可通常指出:包括本文描述的T细胞的药物组合物可以以104至109个细胞/kg体重的剂量,优选106至108个细胞/kg体重的剂量(包括那些范围内的所有整数值)施用。T细胞组合物也可以这些剂量多次施用。细胞可通过使用免疫疗法中公知的注入技术(见例如Rosenberg等,New Eng.J.of Med.319:1676,1988)施用。对于具体患者的最佳剂量和治疗方案可通过监测患者的疾病状况获得,因此治疗调节由医学领域技术人员容易地确定。
对象组合物的施用可以任何方便的方式进行,包括通过喷雾法、注射、吞咽、输液、植入或移植。本文描述的组合物可被皮下、皮内、瘤内、淋巴结内、脊髓内、肌肉内、通过静脉内(i.v.)注射或腹膜内施用给患者。在一个实施方式中,本发明的T细胞组合物通过皮内或皮下注射被施用给患者。在另一个实施方式中,本发明的T细胞组合物优选通过i.v.注射施用。T细胞的组合物可被直接注入肿瘤,淋巴结或感染位置。
在本发明的某些实施方式中,利用本文描述的方法或本领域已知的其他将T细胞扩展至治疗性剂量的方法活化和扩增的细胞,与任何数量的有关治疗形式结合(例如,之前、同时或之后)施用给患者,所述治疗形式包括但不限于用以下试剂进行治疗:所述试剂诸如抗病毒疗法、西多福韦和白细胞介素-2、阿糖胞苷(也已知为ARA-C)或对MS患者的那他珠单抗治疗或对牛皮癣患者的厄法珠单抗治疗或对PML患者的其他治疗。在进一步的实施方式中,本发明的T细胞可与以下结合使用:化疗、辐射、免疫抑制剂,诸如,环孢菌素、硫唑嘌呤、甲氨喋呤、麦考酚酯和FK506,抗体或其他免疫治疗剂。在进一步的实施方式中,本发明的细胞组合物与骨髓移植、利用化疗剂诸如氟达拉滨、外部光束放射疗法(XRT)、环磷酰胺结合(例如,之前、同时或之后)而施用给患者。例如,在一个实施方式中,对象可经历高剂量化疗的标准治疗,之后进行外周血干细胞移植。在一些实施方式中,在移植后,对象接受本发明的扩增的免疫细胞的注入。在一个额外的实施方式中,扩展的细胞在外科手术前或外科手术后施用。
施用给患者的以上治疗的剂量将随着治疗病症的精确属性和治疗的接受者而变化。人施用的剂量比例可根据本领域接受的实践实施。通常,每次治疗或每个疗程,可将1×109个至1×1011个本发明的双阴性T细胞,通过例如静脉输注的方式,施用于患者。
本发明的主要优点包括:
1)配方与工艺改进,适合临床用人DNT细胞的体外高效扩增方法。
2)配方与工艺改进,不需使用人AB血清,也不需胎牛血清(FBS)。
3)配方与工艺改进,可培养新鲜富集的DNT细胞,也可培养冻存后复苏的DNT细胞。
4)配方与工艺改进,可达成一次采集/冻存,多次复苏培养,提供多次回输或是多位患者使用。
5)配方与工艺改进,细胞扩增数量可以达到1x109个细胞/mL全血及以上。
6)配方与工艺改进,保证终产品DNT细胞纯度(CD3+CD4-CD8-)≥85%。
7)配方与工艺改进,保证细胞体外高效杀瘤活性(生物学效力)≥50%。
下面结合具体实施例,进一步阐述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。下列实施例中未注明具体条件的实验方法,通常按照常规条件,例如Sambrook等人,分子克隆:实验室手册(New York:Cold Spring HarborLaboratory Press,1989)中所述的条件,或按照制造厂商所建议的条件。除非另外说明,否则百分比和份数是重量百分比和重量份数。
实施例1:磁珠富集DNT细胞体外扩增培养
1.1健康捐赠者DNT细胞以磁珠富集
从健康捐赠者收集20~200ml外周血到含肝素钠管中。以淋巴细胞分离液分离白膜层细胞,先加入CD4磁珠去除CD4+细胞,再加入CD8磁珠去除CD8+细胞,如此获得的细胞即为去除CD4+与CD8+的DNT细胞。
1.2体外扩增培养DNT细胞
先以抗人CD3单克隆抗体(10μg/mL)包被T25培养瓶,将步骤1.1获得的DNT细胞以AIM-V培养基(含10%捐赠者血浆和500IU/mL的重组人白介素2)调整成1×106~4×106个细胞/mL的浓度,放入前述包被好的T25培养瓶中。于37℃的5%CO2培养箱中培养。
第3天以新鲜AIM-V培养基(含10%捐赠者血浆和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~1×106个细胞/mL的浓度继续培养。
第5天以新鲜AIM-V培养基(含10%捐赠者血浆和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~2×106个细胞/mL的浓度继续培养。
第7天检测DNT细胞表型与活率,并以新鲜AIM-V培养基(含50ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度继续培养。
第9天以新鲜AIM-V培养基(含50ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度继续培养。
第10天检测DNT细胞表型与活率,并以新鲜AIM-V培养基(含50ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度转移到培养袋继续培养。
第12天以新鲜AIM-V培养基(含50ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度转移到培养袋继续培养。
第14天检测DNT细胞表型与活率,为降低终产品的杂质干扰并提高DNT细胞纯度,降低AIM-V培养基中抗人CD3单克隆抗体浓度为25ng/mL。以新鲜AIM-V培养基(含25ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度继续培养。
第15~16天视需要,以新鲜AIM-V培养基(含25ng/mL的抗人CD3单克隆抗体和500IU/mL的重组人白介素2)将DNT细胞调整成1×106~3×106个细胞/mL的浓度继续培养。
第17~18天视需要,收获DNT细胞。
1.3收获DNT细胞以制成DNT细胞制剂
第17~18天,视需要,收获DNT细胞。用250ml尖底离心瓶收集细胞,900×g离心10分钟,再以溶媒洗涤。将收集到的DNT细胞以溶媒调整为0.5~1×108个细胞/mL的浓度,此为DNT细胞制剂成品,质检合格后可供临床使用。
实施例2:磁珠富集DNT细胞冻存复苏后进行体外扩增培养
2.1健康捐赠者DNT细胞以磁珠富集
同上述1.1。
2.2获得的DNT细胞进行冻存
步骤2.1获得的DNT细胞,以0.9%生理盐水洗涤1次,以冻存液重悬并调整细胞浓度为3~5×106个细胞/mL,冻存于液氮中,视需要复苏进行体外扩增培养。
2.3冻存DNT细胞复苏
步骤2.2冻存的DNT细胞由液氮中取出,于37℃恒温箱中化冻。化冻的DNT细胞以AIM-V培养基(含500IU/mL的重组人白介素2)洗涤1次,再以AIM-V培养基(含10%捐赠者血浆和500IU/mL的重组人白介素2)重悬。
2.4体外扩增培养DNT细胞
同上述1.2。
第17~18天视需要,收获DNT细胞。
2.5收获DNT细胞以制成DNT细胞制剂
同上述1.3。
实施例3:沉降富集DNT细胞体外扩增培养
3.1健康捐赠者DNT细胞富集
从健康捐赠者收集20~200ml外周血到含肝素化管中。利用CD4/CD8淋巴细胞去除剂(RossettSep,StemCell),去除CD4+与CD8+T细胞,再以等体积Ficoll-Hypaque溶液进行PBMC分离。如此获得的细胞即为去除CD4+与CD8+的DNT细胞。分离得到的细胞,以0.9%生理盐水洗涤1次,进行体外扩增培养。
3.2体外扩增培养DNT细胞
同上述1.2。
第17~18天,视需要,收获DNT细胞。
3.3收获DNT细胞以制成DNT细胞制剂
同上述1.3。
实施例4:沉降富集DNT细胞冻存复苏后进行体外扩增培养
4.1健康捐赠者DNT细胞富集
同上述3.1。
4.2获得的DNT细胞进行冻存
同上述2.2。
4.3冻存DNT细胞复苏
同上述2.3。
4.4体外扩增培养DNT细胞
同上述1.2。
第17~18天,视需要,收获DNT细胞。

实施例5:应用例以及效果数据
在本实施例中,以CD3+CD4-CD8-T(Double Negative T,DNT)细胞为例,验证经上述通用型临床用人DNT细胞的高效体外扩增方法进行体外扩增的过程与效果。
应用例1
依实施例1的方法,将以磁珠去除CD4+与CD8+得到的DNT细胞,直接利用本发明的方法(重组人白介素2浓度为500IU/mL,两阶段降低抗人CD3单克隆抗体用量(第一阶段50ng/mL、第二阶段25ng/mL),不使用白介素4,也不使用AB血清)进行体外扩增培养,于第17~20天收集细胞,检测细胞活性、表型与细胞扩增总数。
应用例2
依实施例2的方法,将以磁珠去除CD4+与CD8+得到的DNT细胞,经冻存后复苏再利用本发明的方法(重组人白介素2浓度为500IU/mL,两阶段降低抗人CD3单克隆抗体用量(第一阶段50ng/mL、第二阶段25ng/mL),不使用白介素4,也不使用AB血清)进行体外扩增培养,于第17~20天收集细胞,检测细胞活性、表型、与细胞扩增总数。

对比例1
依照现有技术,于培养基中添加重组人白介素2(500IU/mL)、重组人白介素4(2ng/mL)、重组人白介素7(8ng/mL)、重组人白介素12(5ng/mL)、5%自体血浆和6%AB血清。具体的细胞培养过程同上述应用例。
对比例2
参考已有文献(Lee JB等,Clin Cancer Res.25(7):2241-2253,2019),使用250IU/mL的白介素2与100ng/mL抗人CD3单克隆抗体。具体的细胞培养过程同上述应用例。
将前述应用例与对比例的细胞,分别于培养第1天、第7天、第14天、与第17天取样检测细胞总数,比较细胞总数的变化,得到应用例与对比例的细胞生长曲线如附图1。由附图1可见,应用例的细胞由第7天开始细胞数显着增加,而对比例1要到第14天才有明显细胞数增加的情况;对比例2则是第7天到第14天细胞数小幅增加,第14天细胞数才开始明显增加。显示在相同的培养时间点,本发明方法相较于对比例可以扩增得到更多的DNT细胞。
于第17天收集DNT细胞,比较应用例与对比例的细胞活率、表型纯度与DNT扩增倍数,结果列于表1。

表1中应用例1及应用例3与对比例1及对比例2皆为新鲜分离DNT细胞后直接培养。比较DNT细胞扩增倍数,以本发明方法采用磁珠富集和沉降富集DNT细胞后体外扩增所得的DNT细胞扩增倍数分别为15873.68倍与20948.08倍,远高于对比例1的8827.93倍与对比例2的9402.65倍,而细胞表型与细胞活率差别不大。因此,采用两种不同方法富集的DNT细胞以本发明扩增技术可获得更多的DNT细胞产品。
将分离所得DNT细胞进行冻存,复苏后以本发明方法体外扩增,得到应用例2及应用例4(表1)。与未冻存直接培养的对比例1及对比例2比较,采用两种不同方法富集DNT细胞,然后冻存复苏DNT细胞后体外扩增所得的DNT细胞扩增倍数分别为10778.95倍与10819.41倍,与未冻存直接培养的对比例1(8827.93倍)和对比例2(9402.65倍)相当,且DNT细胞纯度和细胞活率差别不显著。
应用例5
将外周血去除CD4+与CD8+后富集得到的DNT细胞,经液氮冻存7天、14天、与30天后复苏,再利用本发明的扩增方法(重组人白介素2浓度为500IU/mL,两阶段降低抗人CD3单克隆抗体用量(第一阶段50ng/mL、第二阶段25ng/mL),不使用白介素4,也不使用AB血清)进行体外扩增培养,于第17天收集细胞,检测细胞活率、纯度、细胞生物学效力、和DNT细胞扩增倍数。
比较新鲜全血富集DNT细胞与冻存后复苏DNT细胞的细胞活率、纯度、细胞生物学效力以及DNT细胞扩增倍数,结果列于表2。

由表2可见,与新鲜富集DNT细胞后连续培养17天的DNT细胞批次相比,DNT细胞在冻存不同时间(7天、14天、30天)后复苏再体外培养的DNT细胞批次相比,在细胞活率(>85%)、细胞纯度(CD3+CD4-CD8-,>90%)、肿瘤杀伤活性(>50)和体外扩增倍数上均无明显差异,其中各批次在扩增倍数上的差异,可能与不同供者间DNT细胞的特质有关,但均可符合临床输注需求。整体而言,本发明可以适用于新鲜富集的DNT细胞以及冻存后复苏的DNT细胞体外大量扩增DNT细胞,不仅终产品DNT细胞活率在85%以上,纯度超过90%,生物学效力超过50%,DNT细胞扩增倍数也可达到8,000倍以上。
以上实施例证实,本发明所提供的在day0冻存DNT细胞再后续高效体外扩增工艺,提供了适合临床治疗方案在细胞制备上的灵活性,在已有专利扩增技术基础上,对生产工艺又做了进一步改进,使DNT细胞的体外扩增水平在17天内提高到8,000倍数量级。使一次扩增所得的DNT细胞可以提供给多个患者临床治疗剂量。
在本发明提及的所有文献都在本申请中引用作为参考,就如同每一篇文献被单独引用作为参考那样。此外应理解,在阅读了本发明的上述讲授内容之后,本领域技术人员可以对本发明作各种改动或修改,这些等价形式同样落于本申请所附权利要求书所限定的范围。
Claims (10)
1.一种通用型DNT细胞的体外扩增方法,其特征在于,包括步骤:
(i)提供一获自供体的外周血起始样品I;
(ii)去除起始样品中的CD4+和CD8+T细胞,从而获得样品II;
(iii)在含有适合DNT细胞生长培养基的培养体系中,培养样品II,从而获得样品III;其中,所述培养体系中包括固定的T细胞促分裂原;
(iv)在含有适合DNT细胞生长培养基的培养体系中,培养样品III,从而获得样品IV;其中,所述培养基中含有浓度为30-80ng/mL(较佳地40-60ng/mL,更佳地50ng/mL)的可溶性T细胞促分裂原;
(v)在含有适合DNT细胞生长的培养基的培养体系中,培养样品IV,其中,所述培养基中含有浓度为15-40ng/mL(较佳地20-30ng/mL,更佳地25ng/mL,更佳地为亚步骤(va)中所述T细胞促分裂原的添加量的1/2)的可溶性T细胞促分裂原,从而获得所需量的通用型DNT细胞,为样品V;和
(vi)在含有适合DNT细胞保存的溶液体系中,收集样品V;其中,所述溶液中含有人血白蛋白,从而获得临床使用的通用型DNT细胞,为DNT细胞制剂;
其中,所述步骤(iii)至(v)的培养体系中均含有200-1000IU/mL(较佳地为300-700IU/mL,更佳地为500IU/mL)的重组人白介素2,并且均不含有重组人白介素4,并且均不含AB血清。
2.如权利要求1所述的体外扩增方法,其特征在于,所述样品II是冻存后复苏的细胞溶液。
3.如权利要求1所述的体外扩增方法,其特征在于,在所述步骤(iii)至(v)的培养基中,还包括选自下组的细胞因子:IL-7、IL-12、IL-15,或其组合。
4.如权利要求1所述的体外扩增方法,其特征在于,所述T细胞促分裂原选自下组:结合CD3的抗体、凝集素、能够刺激DNT细胞扩增的化合物,或其组合。
5.如权利要求1所述的体外扩增方法,其特征在于,在所述步骤(iii)中,培养体系中待扩增的DNT细胞的初始浓度为1×106至4×106个细胞/mL。
6.如权利要求1所述的体外扩增方法,其特征在于,在步骤(iv)中,还包括检测DNT细胞表型和活率。
7.如权利要求1所述的体外扩增方法,其特征在于,在步骤(iv)中,还包括检测DNT细胞表型和活率。
8.如权利要求1所述的体外扩增方法,其特征在于,在步骤(vi)中,包括步骤:
(via)离心收集样品V中的DNT细胞;
(vib)以含2.5%人血白蛋白的生理盐水(或“溶媒”)洗涤DNT细胞;和
(vic)以含2.5%人血白蛋白的生理盐水(或“溶媒”)将DNT细胞调整为1×108个细胞/mL的浓度。
9.一种有效量的如权利要求1所述的方法获得的DNT细胞的用途,其特征在于,用于制备一药物组合物或制剂,所述药物组合物或制剂用于:
(a)预防和/或治疗肿瘤;
(b)预防和/或治疗感染性疾病;
(c)预防和/或治疗自身免疫性疾病;
(d)预防和/或治疗变态反应;
(e)预防和/或治疗移植物抗宿主疾病;和/或
(f)调节免疫应答。
10.如权利要求9所述的用途,其特征在于,所述肿瘤为与所述DNT细胞异体的肿瘤。
