背景技术
癌症是全球第二大死因,全球约六分之一的死亡由癌症引起。对于大多数恶性肿瘤患者来说手术是有效的首选治疗方法。研究表明,在对肿瘤进行更彻底切除的同时对周围健康组织尽可能多地加以保护,对于患者的预后至关重要,例如,显微镜下根治性切除的前列腺癌患者(R0,20.3个月)的平均存活时间是姑息性切除患者(R1,10.3个月)的两倍(Garcea,G.,Dennison,A.,Pattenden,C.,Neal,C.,Sutton,C.,Berry,D.(2008).SurvivalFollowing Curative Resection for Pancreatic Ductal Adenocarcinoma.ASystematic Review ofthe Literature.JOP:Journal ofthe pancreas.9.99-132.)。完整切除需要在术中对肿瘤边缘和残留病变进行有效的检测。在大多数手术情况下,外科医生只能依靠视觉和触感反馈来区分肿瘤与周边正常组织之间的细微差别,而微创手术又限制了医生的视觉和触感,给手术带来挑战。这些原因造成实体瘤手术治疗的临床效果在过去十几年没有发生显著的改进。在对美国国家癌症数据库(NCDB)中2008-2012年6,495,889例包括乳腺癌、结直肠癌等十种实体瘤手术治疗的分析中,发现阳性切除边缘的发生率为8.85%,在卵巢癌和前列腺癌中更分别高达35.00%和21.03%。阳性切除边缘通常需要额外的辅助治疗,大幅度增加患者和医疗系统的负担。(Orosco,R.K.,Tapia,V.J.,Califano,J.A.,Clary,B.,Cohen,E.E.W.,Kane,C.,…Nguyen,Q.T.(2018).Positive SurgicalMargins in the 10Most Common Solid Cancers.Scientific Reports,8(1).doi:10.1038/s41598-018-23403-5)。MRI、CT和PET-CT等分子影像虽然已经广泛应用于癌症早期检测以及原位、区域性和远端复发监测,但它们无法在术中提供实时准确的影像学指导。快速冷冻病理是术中即时诊断的主要手段,可将诊断时间缩短至30min,但仍有取样代表性差、准确度偏低、时间过长的缺陷。
荧光影像设备易于在手术室内安装。荧光分子从受激到可视只需几毫秒,可以用于肿瘤组织的体内实时成像。与可见光相比,近红外光(700-900nm)能够穿透更深的生物组织、减小组织的光学吸收散射和自发荧光,从而增强信噪比。近红外荧光成像系统可以把荧光信号与普通的RGB彩色视频合并在一起,帮助医生获取解剖定位。因此荧光影像在手术导航中有着巨大的发展潜力。荧光造影剂在荧光影像的发展过程中起了至关重要的作用。近红外荧光造影剂吲哚菁绿(ICG)1959年获得FDA批准,常规用于血管灌注造影成像,评估心脏和肝脏功能,在腹腔镜和机器人手术中,ICG由于敏感度高、可以弥补触感缺失,而在前哨淋巴结定位和肝肿瘤成像等方面得到广泛的应用。2017年,5-氨基乙酰丙酸盐酸盐(5-ALA)作为荧光造影剂获得FDA批准用于高级神经胶质瘤(HGG)的手术治疗,这一批准是在5-ALA在欧洲获批使用十年之后,基于多中心3期临床试验的结果作出的。5-ALA可以辅助恶性组织可视化,帮助医生更彻底地切除肿瘤,改善患者的预后。它的获批标志着:(1)术中对肿瘤组织和切除边缘的实时检测正在得到重视;(2)监管机构已经把用于手术导航的荧光造影剂与治疗药物加以区别对待,并着手制定合理的标准推动荧光造影剂的审批。5-ALA的获批是一个重要的里程碑,为荧光造影剂的发展创造了有利条件。
从成像原理上,造影剂分为“always-on”和“可激活”两类,基于抗体的靶向造影剂属于“always-on”类型,其在体内循环和在与靶点作用时荧光信号的强度基本保持不变。相反“可激活”造影剂只有在靶点才发射荧光。为了提高靶向造影剂的TBR,不仅只有提高靶向造影剂在肿瘤组织中的特异性积累一条路可行,把“always-on”型靶向造影剂转化成“可激活”型靶向造影剂也是合理的途径。我们构建理想造影剂的思路如下:通过调节DoL使荧光基团在造影剂中由于聚集而发生荧光淬灭,造影剂在体内稳定循环,在正常组织中停留短暂,背景荧光信号强度低。当造影剂时到达肿瘤细胞时,靶向基团与细胞表面受体的结合导致造影剂被细胞内吞,在细胞中抗体载体裂解,释放荧光基团,使荧光信号得到恢复。从而达到理想的TBR。
LUM015由Lumicell公司开发,它作用于肿瘤微环境中的组织蛋白酶。
人组织蛋白酶(cathepsins)家族包含12个半胱氨酸、2个天冬氨酸和2个丝氨酸组织蛋白酶。主要存在于溶酶体中,正常生理状态下起蛋白水解的作用,但通常能在病变组织胞外检测到组织蛋白酶,会导致胞外蛋白的水解。
在肿瘤中,组织蛋白酶可降解胞外基质和基底膜等,使肿瘤细胞能够离开肿瘤组织,实现转移和扩散。目前研究发现,组织蛋白酶在多种肿瘤中表达均有升高。
在分子结构上,LUM015由一个荧光淬灭基团QSY21通过GGRK肽(可被组织蛋白酶剪切),与一个20kD的PEG和Cy5荧光基团组成。
在酶作用之前,由于荧光淬灭基团的存在,LUM015呈现光学惰性状态;经过组织蛋白酶K、L、S或B的水解作用后,淬灭剂被剪切分离,剩余带有Cy5的片段具有光学活性。对于乳腺癌患者而言,切缘干净的保乳术无疑能提高术后生存质量,但这也对手术提出了巨大的挑战。采用常规手术方法,保乳术的阳性切缘率约为20%至40%,通常会导致二次手术,从而给患者带来的不必要的痛苦。
在一项可行性研究(NCT02438358)中,研究人员招募了15例浸润性乳腺癌或原位导管癌患者,采用LUM 2.6手持装置,对切除腔体和切除组织进行成像,以评估LUM015检测保乳术后切缘残余肿瘤的效果。结果显示,LUM015检测灵敏度为100%,并能检测浸润性导管癌、浸润性小叶癌和导管原位癌。胃肠癌也是LUM015研究的主要适应症之一。为了确定药物标记结直肠和食管肿瘤的有效性,研究人员在一项单中心研究(NCT02584244)中招募了9名患者,进行结直肠肿瘤切除或内镜下黏膜切除术,采用LUM 2.6手持装置对切除组织进行成像,结果显示LUM015耐受良好,可特异性标记肿瘤部位。
诸多研究结果表明,LUM015具有良好的安全性,配合LUM 2.6设备,在成像特异性、成像采集速度、视野范围等方面均有很大的优势,目前,针对乳腺癌、结直肠癌、食管癌、前列腺癌、胰腺癌和脑癌等多种肿瘤的临床试验均在进行中。此外,2020年11月,LUM015获得FDA快速通道审评资格认定,可能是继OTL38之后最具临床转化前景的荧光成像药物(Whitley,M.J.et al.Science Translational Medicine 8,320ra4-320ra4(2016);Smith,B.L.et al.Ann Surg Oncol 27,1854–1861(2020);Esfahani,S.A.et al.Am JNuclMed Mol Imaging 9,230–242(2019);Smith,B.L.et al.Breast Cancer Res Treat 171,413–420(2018)等)。
LUM015由四个部分:Cy5荧光基团、QSY21荧光淬灭基团、分子量为20000的mPEG、可以被组织蛋白酶MMP切断的位点,其中的寡肽序列-Gly-Gly-Arg-mPEG增加造影剂的循环稳定性,并且帮助造影剂通过高渗透长积累(EPR)效应在肿瘤组织中积累。在正常情况下,Cy5发出的荧光被QSY21荧光淬灭基团吸收,LUM015不发光。
肿瘤组织中有高表达的组织蛋白酶MMP,可切断LUM015,使QSY21荧光淬灭基团离开,Cy5发出的荧光不再被荧光淬灭基团吸收,所以LUM015发光。由于组织蛋白酶MMP只有在肿瘤组织中高表达,所以LUM015只在肿瘤组织中发光,也就是说其高表达仅仅限于肿瘤组织实体的内,而对组织边界、组织游离响应不明显。

发明内容
本发明的目的在于提供一种靶向造影剂,其能够通过提供新的靶点而获得了一种针对FAP的靶向造影剂,从而对多种肿瘤的高效响应,增强了响应的范围,满足实体识别和边界识别要求。
肿瘤相关成纤维细胞(carcinoma associated fibroblasts,CAFs)是肿瘤微环境的重要组成成分。研究发现CAFs通过分泌各种生长因子、细胞因子、趋化因子等促进肿瘤细胞的增殖、侵袭和转移(Yamamura,Y.,Asai N,EnomotoA,Kato T,Mii S,Kondo Y,etal.Akt-Girdin signaling in cancer-associated fibroblasts contributes to tumorprogression.Cancer research.2015 Mar 1;75(5):813-23.PubMed PMID:25732845)。将近90%的上皮原性恶性肿瘤(包括乳腺癌、卵巢癌、结肠癌、胰腺癌、皮肤黑色素瘤等)的CAFs出现FAP-α的过表达,FAP-α主要定位于包膜和胞质。其几乎不表达于正常组织及良性上皮肿瘤的间质中静息的成纤维细胞中。HuaX等研究发现在349切除的乳腺标本中,正常组织中FAP-α表达阴性,导管内癌组织中表达阳性率为19.27%,在导管内癌伴微浸润的组织中表达阳性率为82.72%,而在浸润性导管癌组织中其表达阳性率为100%(Hua X,Yu L,Huang X,Liao Z,Xian Q.Expression and role of fibroblast activation protein-alpha in microinvasive breast carcinoma.Diagnostic pathology.2011 Nov 8;6:111.PubMed PMID:22067528.Pubmed Central PMCID:3228672.)。目前研究发现FAP-α促进肿瘤的发生、发展主要通过下面几个方式(Bhowmick NA,Moses HL.Tumor-stromainteractions.Current opinion in genetics&development.2005Feb;15(1):97-101.PubMed PMID:15661539.Pubmed Central PMCID:2819733):(1)释放血管内皮生长因子,白细胞介素-8等血管生长相关因子,诱导肿瘤组织血管增生,从而有利于癌细胞的生长、侵袭;(2)通过细胞与细胞之间的接触,释放生长因子,分泌基质成分等促进肿瘤生长;(3)提供免疫隔离的间给肿瘤细胞,协助其逃逸机体防御系统;(4)表达细胞因子、蛋白酶及黏附分子等协助肿瘤、转移。
为实现上述目的,本发明的实施例提供了一种靶向造影剂,具有如下结构:

作为一种实施方式,L1、L2、L3、L4、L6、L7可以为选自包括而不限于如下一种或者一种的重复或者多种的排列(多种的排列,包括几组重复单一结构排列,包括不限于AABBCC、ABC、AABC等情形,其中A、B、C各自指如下任一的不同基团,下同)或者多种的重复(多种的重复,包括不限于ABCABCABC等的情形):-CH2-、-CH2CH2O-、-C(=O)-、-C(=O)O-、-C(=O)NH2-、-S-S-、-O-、-S-、-NH-、-SC-、-HC=CH-、-HC≡CH-、

作为一种实施方式,靶向造影剂在制备前列腺癌诊疗药物中的应用。
本发明还提供了一种组合物产品,包括靶向造影剂和药学上可接受的载体。其中载体,本领域技术人员可根据需要进行选择,例如可选自包括而不限于磷酸二氢钠、磷酸氢二钠、氯化钠、柠檬酸、柠檬酸钠、聚山梨酯20、聚山梨酯80、注射用水等。
作为一种实施方式,本发明设计了针对FAP的靶向造影剂:
这个靶向造影剂具有与LUM015一样的结构,如图1所示,不同之处是结构示意图中可降解位点4的-Gly-Gly-Arg-(可被MMP切断)变成了-Gly-Gly-Pro-(可被FAP切断)。
改变荧光基团和淬灭基团,可以得到新的分子设计,不同荧光基团发射波长不同,有的发射波长可以更好地和现有获批设备匹配。
(2)新的基团取代mPEG2k,消除造影剂的异质性:
mPEG2k是高分子混合物,分子量范围在+/-10%,ODDA是白蛋白结合基团,与白蛋白结合后同样具有EPR效应,但同时消除了产物的异质性,ODDA可以替代mPEG2k,直接引入到LUM015的结构中:
或引入到含有-Gly-Pro-的造影剂中
(3)可选的可以进一步引入附加的靶向基团,进一步增强靶向选择性:
附加的靶向基团包括GX1、PSMA、cyclic RGD、叶酸:
均针对在肿瘤中高表达的标记物,在合成上,可通过其带有反应活性基团的衍生物,连接在如下结构上:
举例而言,在结构式如下的靶向造影剂中:
Cy5发射的荧光信号被淬灭基团QSY21所淬灭,连接Cy5和QSY21的-Gly-Pro-可被在多种癌症中过度表达的成纤维细胞活化蛋白降解,激活荧光,分子量20,000的mPEG使靶向造影剂通过高渗透长滞留效应(EPR)在肿瘤组织中积累。
合成的靶向造影剂的醋酸盐形态为蓝紫色的冻干的粉末,由于上面连接的mPEG的分子量为20000,而且具有10%的变化空间,这一靶向造影剂为一混合物。
合成采用Fmoc固相合成方法,产物由MALDI-TOF验证,(1)合成保护的多肽;(2)固相荧光基团的引入;(3)从树脂上切除;(4)纯化;(5)mPEG偶联;(6)造影剂纯化。
固相合成在室温下进行,树脂活化:将化合物1(Fmoc-Lys(Dde)-Wang resin,购于南京肽业生物科技有限公司,R61124,0.3-0.8mmol/g,100-200mesh,1g)首先用六氢吡啶/DMF混合溶剂I(体积比25%:75%)去除N端Fmoc保护基使N端成为自由氨基。
下面结合如下具体的实施例,进一步阐述本发明:
1通过标准固相肽合成技术组装:
QSY21-Ahx-Gly-Gly-Arg-Lys-PEG2Ac-Cys
在三环酰胺接头树脂(DL形式,200-400目,0.6mmol/g负载,18.0mmol规模)上进行固相合成。在受保护的多肽的固相合成中使用下列受保护的氨基酸:Fmoc-AEEAc-OH;Fmoc-Pro-OH;Fmoc-8-Ahx-OH;Fmoc-Cys(Trt)-OH;Fmoc-Gly-OH;Fmoc-Lys(Boc)-OH。将大约3倍当量的Fmoc保护的氨基酸与3倍当量的HBTU,3倍当量的HOBt和3倍当量的DIC(即Fmoc保护的氨基酸、HBTU、HOBt和DIC的用量均为树脂的摩尔量的3倍)混合并溶解于DMF中,在摇动20分钟后,将活化的氨基酸加入树脂中。再将6倍当量的DIPEA加入反应器中的偶联溶液中,并进行偶联反应大约2.0小时。使用20/80哌啶/DMF溶液进行Fmoc脱保护。其中,指应用于树脂,合成多肽用的微球,下同。

2多肽QSY21-Ahx-Gly-Gly-Arg-Lys-PEG2Ac-Cys-CONH2从树脂上的切割和分离
完成在树脂上的多肽组装之后,从树脂上切除多肽并去除Cys和Lys的侧链保护基团,用含有体积比为TFA∶水∶EDT∶TIPS(92.5∶2.5∶2.5∶2.5)的裂解溶液处理受保护的肽树脂。将裂解溶液在冰浴中冷却并在使用前解冻至室温。将裂解反应混合物在室温下搅拌约2小时。滤出废树脂并用90∶10的TFA∶水混合物洗涤。然后将合并的滤液和洗涤液沉淀到冷乙醚中并离心以收集肽。倾析乙醚,用冷乙醚洗涤固体沉淀物三次。将未纯化的线性单体在真空下干燥至恒重,产率65%。
3合成QSY21-Ahx-Gly-Gly-Arg-Lys(Cy5)-PEG2Ac-Cys-CONH2
化合物QSY21-Ahx-Gly-Gly-Arg-Lys-PEG2Ac-Cys-CONH2溶于少量DMF(2mL)中(3.5mM),向溶液中加入8倍当量的DIPEA。系统用氩气吹拂后,向混合物中加入同样当量的Cy5 NHS活化酯。混合物在避光条件下搅拌6小时。通过减压蒸发除去溶剂,产物用MeCN沉淀并用MeCN洗涤两次,通过HPLC纯化,洗脱液:H2O(90%)/MeCN(10%)=>H2O(0%)/MeCN(100%)),收集产物并冻干,产率50%。
4合成:
QSY21-Ahx-Gly-Gly-Arg-Lys(Cy5)-PEG2Ac-Cys(mPEG20k)-CONH2
化合物QSY21-Ahx-Gly-Gly-Arg-Lys(Cy5)-PEG2Ac-Cys-CONH2溶解于含有3个当量TCEP的pH 7的PBS缓冲液(0.5mM),加入1.1当量的mPEG-MAL(MW 20,000),在室温下反应12小时。产物通过半制备型HPLC纯化,产率50%。产物通过MALDI-TOF光谱法表征。
采用相同或者相近(包括而不限于以本发明公开的方案为参照经过合理推断获得的技术方案)的方法,还可以获得包括而不限于如下的造影剂,其具体的制备和表征这里就不再一一赘述:
包括而不限于上述示例的本发明的造影剂,可以与包括而不限于如下所示的物质为载体,如磷酸二氢钠、磷酸氢二钠、氯化钠、柠檬酸、柠檬酸钠、聚山梨酯20、聚山梨酯80、注射用水等,来配制应用试剂或者药剂。从而可以实现在肿瘤组织的肿瘤实体识别或边界识别或淋巴结显影等领域的应用,特别是肿瘤组织边界等的识别。肿瘤组织可以包括肉瘤组织或食管癌组织或乳腺癌组织或胆管癌组织或肺癌组织或嗜铬细胞瘤组织或肾癌组织或分化型甲状腺癌组织或腺样囊性癌组织或胃癌组织或肝癌组织或结直肠癌组织或头颈部癌组织或卵巢癌组织或胰腺癌组织或前列腺癌组织等。
