Abstract

本发明涉及益生菌微生物及其在医学,尤其是在细菌感染的治疗和预防中的用途的领域。本发明还涉及目的是改善治疗效果,尤其是在治疗和预防幽门螺旋杆菌(Helicobacter pylori)感染的领域中的益生菌微生物的修饰。
现有技术
幽门螺旋杆菌是一种革兰氏阴性、耐酸和嗜二氧化碳的微生物,在人类胃中发现其龛影并且其与人一起进化。目前,该菌种感染超过50%的成年全球人口(Hooi)。胃黏膜的定植刺激强烈的局部和全身炎症和免疫应答,其与由细菌产生的有毒物质一起引起10%至20%感染个体的消化性溃疡和1%至3%感染个体的胃癌(Suerbaum S,Michetti P)。
能够通过使用与诸如阿莫西林、克拉霉素、硝基咪唑、氟喹诺酮和四环素的抗生素相关的质子泵抑制剂(PPI)的治疗来实现感染的根除,所述抗生素通常组合使用(Malfertheiner)。这些抗生素的长期使用以产生化学药抗性菌株而结束,并导致治愈率的持续降低。此外,给予抗生素通常产生在许多情况下可能导致治疗暂停的不利影响(Alba)。

动物乳双歧杆菌(Bifidobacterium animalis lactis)(B94,Lallemand Health Solutions)的菌株也用于治疗除幽门螺旋杆菌感染之外的疾病,例如肠易激综合征。益生菌还可以改善对肠道病原体的免疫应答。能够通过抑制免疫细胞产生促炎和/或刺激免疫细胞分泌消炎细胞因子来获得免疫调节(Foligne)。然而,在该领域中获得的结果已经是不一致的,在某些研究中改善了幽门螺旋杆菌的根除,但在其他研究中没有显著影响(Cekin)。尤其是,已经在不同来源的嗜酸乳杆菌和动物乳双歧杆菌中报道了对幽门螺旋杆菌的活性的大变化,从而阻止了这些菌株中活性的普遍性(Dang)。益生菌对通常用于治疗幽门螺旋杆菌的抗生素的敏感性也是限制益生菌/抗生素组合治疗的实际有效性的因素;尤其是,包括嗜酸乳杆菌的乳杆菌菌种通常易受阿莫西林和克拉霉素的影响,所述阿莫西林是用于幽门螺旋杆菌根除的所有治疗方案的关键/>乳双歧杆菌也易受阿莫西林影响(Morovic)。对红霉素(代表大环内酯)的敏感性是中等的,即低;但是胃中大环内酯的含量(在给予治疗有效量之后)非常可能远高于它们对幽门螺旋杆菌的最小抑制浓度(MIC)。发现双歧杆菌的菌株易变地易受克拉霉素的影响,这可以视为其在治疗幽门螺旋杆菌感染中作为该抗生素的佐剂的有限的成功(Collado)。包括乳杆菌和双歧杆菌菌种的益生菌的组合被用作对幽门螺旋杆菌感染的抗生素治疗的佐剂,然而在这种情况下也具有不一致的结果(McFarland中的综述)。
因此,仍然亟需对涉及使用益生菌试剂的幽门螺旋杆菌感染进行可用的有效治疗。具体需要寻找持续有效的益生菌,即显示稳定和可再现水平的活性,特别是当与特定抗生素疗法组合使用时。其他需求是强烈改善已知抗生素混合物在幽门螺旋杆菌的治疗中的有效性,例如,通过整体幽门螺旋杆菌根除率的强增强和/或协同作用。其他需求是鉴别可靠地能够减少幽门螺旋杆菌感染的治疗方案中使用的抗生素的副作用的试剂,而不损害抗生素的活性水平。
概述

能够通过常规方法来评估本克隆抑制幽门螺旋杆菌生长的活性,例如,通过在幽门螺旋杆菌的固体培养物上接种相关克隆并验证与接种部位对应的生长抑制区的形成/延伸;作为替代示例,可以通过“琼脂孔扩散方法”来评估克隆活性,其中将益生菌混悬物嵌入其上培养幽门螺旋杆菌的琼脂板中挖出的孔中。孵育后,抑制区是可检测的。

能够通过标准方式评估幽门螺旋杆菌生长的抑制中本克隆对抗生素活性的佐剂效应,通常通过比较存在和不存在相关克隆下抗生素对幽门螺旋杆菌的MIC,其中MIC的降低对应于杀菌活性的增加;具体地,在幽门螺旋杆菌培养物上的体外条件下,能够引起本克隆的佐剂效应:这是特别重要的,因为其支持本益生菌对幽门螺旋杆菌生长的强“直接”抑制效果,即不仅仅是由宿主上的间接免疫刺激效果介导的。如实验部分所支持的,本克隆导致所测试抗生素的杀菌活性显著增加,即相应MIC的100到1000倍降低。该效果也是重要的,因为其允许在益生菌-抗生素组合治疗中实质减少给予抗生素的量,而不会导致幽门螺旋杆菌根除率中功效的降低;较少量地给予抗生素在降低对患者抗生素的累积/副作用的风险上,以及在降低幽门螺旋杆菌的抗生素抗性菌株的形成的风险上具有有利于患者和环境的明显的优点。

如本文所报道的,当使用本发明的新颖的益生菌克隆时,或者当与彼此组合使用时,或者当与对幽门螺旋杆菌感染的抗生素治疗组合使用时,本发明的新颖的益生菌克隆有效抑制幽门螺旋杆菌的生长。
关于给予本益生菌微生物的剂量和方案,没有预见具体限制;这些能够根据患者的具体需求(疾病的程度/过程、患者的通常状态等)而容易地适用。作为非限制性实例,本益生菌微生物的日剂量将包括109至5×1011菌落形成单位(CFU)/天,优选5×109至2.5×1011CFU/天,更优选8×109至1.5×1011CFU/天的,例如,能够提及大约1010;当组合使用一种或多种益生菌微生物时,上述剂量区间意味着涉及使用的所有微生物。

为了实施组合的益生菌-抗生素治疗,除了本发明的益生菌微生物和引用的赋形剂/成分/食料之外,所有上述组合物可以包含与治疗幽门螺旋杆菌感染有关的一种或多种抗生素;所述抗生素优选选自克拉霉素、阿莫西林以及它们的类似物,后者在本文中被称为“克拉霉素抗生素”(即具有大环内酯结构的抗生素)或“阿莫西林抗生素”(即具有青霉素结构的抗生素)。

实验
1.抗阿莫西林和克拉霉素的新颖的克隆的分离
将野生型嗜酸乳杆菌La-14和动物乳双歧杆菌Bl-04(益生菌,DupontDanisco)用作起始材料。
对阿莫西林的抗性(MIC)相对较低,即0.5μg/ml嗜酸乳杆菌和0.25μg/ml乳双歧杆菌。对克拉霉素的抗性MIC也低,即0.125μg/ml嗜酸乳杆菌和0.016μg/ml乳双歧杆菌。在重复冷冻/解冻之后和冻干之后,抗性状态是稳定的。
通过琼脂孔扩散法检测对幽门螺旋杆菌的抑菌活性;通过时间-杀菌曲线法检测杀菌活性。
我们在具有增加的抗生素浓度的MRS(Deman Rogosa Sharpe)琼脂中传代培养两种野生型生物体之后分离五种抗生素抗性克隆,从原始MIC的1/4开始,对于阿莫西林高达8μg/ml并且对于克拉霉素高达6μg/ml。将具有最高可能抗性水平的菌株传代培养几次,在甘油肉汤中冷冻或冻干。在冷冻/冻干之后,进行化学药敏感性测试以确认抗性水平的稳定性。对于嗜酸乳杆菌La-14和动物乳双歧杆菌Bl-04,我们能够鉴别并分离对克拉霉素或阿莫西林具有高抗性的克隆;此外,对于动物乳双歧杆菌Bl-04,我们可以鉴别并分离对两种抗生素均具有高抗性的克隆。下面列出了五个分离的克隆,具有抗生素抗性的标记(CLA=克拉霉素;AMO=阿莫西林)和根据布达佩斯条约的保藏编号:

2.DNA测序
对原始野生型菌株和五个分离的克隆进行DNA测序。原始菌株的DNA和新颖的化学药抗性益生菌的DNA由Dr.Greta Immobile Muraro,Polo d’Innovazione di Genomica,Genetica e Biologia Società Consortile R.L.,Siena,使用MiSeq Reagent Micro Kit在Illumina MiSeq仪器上通过“微生物全基因组测序2x150 PE”进行测序(包括通过高达100倍的生物分析器覆盖率的文库制备、清理和定量)。也将序列与在基因组文库BLAST/NCBI中保藏的野生菌株的那些进行比较,以证明在突变的化学药抗性菌株中完全不存在外源DNA。
与野生嗜酸乳杆菌La-14的DNA序列相比,抗阿莫西林的嗜酸乳杆菌克隆包含了能够导致112种可能的表型变体的11个突变位点。抗克拉霉素的嗜酸乳杆菌克隆包含能够导致205种表型变体的18个突变位点,其。
与野生乳双歧杆菌Bl-04的DNA序列相比,抗阿莫西林和克拉霉素的乳双歧杆菌克隆包含能够导致199种可能的表型变体的19个突变位点,其。抗克拉霉素的乳双歧杆菌克隆包含能够导致199种表型变体的19个突变位点。抗阿莫西林的乳双歧杆菌克隆包含能够导致109种表型变体的11个突变位点。
在原始和抗生素抗性菌株中没有检测到质粒。

3.抗生素敏感性测试
使用96孔的棋盒,用肉汤稀释法(金标准法)进行对幽门螺旋杆菌和益生菌菌株的抗生素敏感性测试(图1)。所获得的结果表明克隆LAA8和BLA8的8μg/mL阿莫西林抗性(MIC)、克隆LAC6和BLC6的6μg/mL克拉霉素抗性(MIC)以及克隆BLAC86的分别为8μg/mL和6μg/mL的双阿莫西林/克拉霉素抗性(MICs);所有这些值表明高水平的抗生素抗性。
4.协同作用的评价
为了研究在本发明的框架中选择的嗜酸乳杆菌和乳双歧杆菌菌株之间的生长抑制的可能的协同作用,我们执行了十字形接种。
在微有氧条件下,于37℃,将幽门螺旋杆菌菌株在选择性琼脂中培养72小时,然后以对应于3McFarland浊度标准的密度悬浮在无菌盐溶液中,并且用棉签在三个方向上向MHF(Mueller Hinton Fastidious)琼脂板上划线。在每一琼脂板中,沿着两条垂直线接种嗜酸乳杆菌La-14和乳酸动物双歧杆菌Bl-04;在每一线中接种1010个细胞。然后,将板在37℃下孵育48至72小时。之后,检查板是否存在抑制生长的芦荟(参见图3)。
将La-14菌株和克隆与Bl-04菌株和克隆共同接种以确定协同作用的存在。在阴性对照中,在垂直接种线(La-La或Bl-Bl)中使用相同的菌株/克隆。在板的边缘处和板半径的1/2处(我们的板中2cm)用毫米尺测量抑制区。
在板边缘处的抑制区源自单个菌株的抑菌活性,靠近板中部的抑制区源自两个菌株的活性(图3)。如果抑制区倾向于朝向板的中部增大(虚线),则抑菌效果是协同的。具体地,我们确定板的1/2半径是距接种的交点的合理的距离,使得在此测量的抑制不源自仅添加相应的抑制区,突出真正的协同作用。我们确实观察到抑制区从板的边缘向中心的增加,当La菌株或克隆与Bl菌株或克隆共同接种时,在观察区(板半径的中间,距中心2cm处)中变得完全明显。这在阴性对照中不发生(接种于两条垂直线的相同菌株/克隆)。在不存在或存在抗生素(阿莫西林和/或克拉霉素,仅用于克隆)的情况下,在不同培养基(Muller HiltonFastidious或Agar Pylori)中进行的独立实验中一致地观察到该协同作用。在以下表格中报告所获得的数据:

以上报告的测试证实在本发明菌株(或相应克隆)之间抑制幽门螺旋杆菌生长的协同作用,因为它们的组合接种将抑制区从边缘增加到读取点(半径的1/2),而源自阴性对照(菌株或对其自身的克隆)的抑制区在相同的读数点保持稳定。这些数据进一步确认观察到的增加不取决于较高浓度的细胞(事实上,阴性对照含有与阳性对照相同量的细胞)。
也将板的边缘处抑制区的存在考虑为每个菌株“本身”对幽门螺旋杆菌的抗菌活性的重要指标,即不考虑与共同给予的抗生素试剂的可能效果和/或对生物体可能的通用免疫刺激效果。在“琼脂孔扩散”实验中进一步证实了该活性。即使在体外观察到杀菌活性,也实施“时间杀菌曲线”。La-14和Bl-04在3小时内引起三个对数(从108cfu/ml到105cfu/ml)的幽门螺旋杆菌活计数的减少,并且在72小时内引起幽门螺旋杆菌的全部杀灭。这里重要的是,注意到仅免疫刺激活性(对某些益生菌偶尔在文献中报告)通常不足以根除感染,特别是在幽门螺旋杆菌的情况下;事实上,该细菌导致免疫应答的弱激活,其本身负责该感染的致病性的一部分。相反地,本发明菌株对幽门螺旋杆菌的直接抑制活性的存在使得它们不仅用作佐剂,而且用作针对幽门螺旋杆菌感染的主要治疗剂。此外,当在与抗生素的组合治疗中使用时,本益生菌不具有仅作为辅助免疫刺激剂的资格,因为这些益生菌施加直接的抗菌活性,该抗菌活性增强共同给予的抗生素试剂的抗菌活性,从而为有效的组合治疗提供了基础。
5.益生菌上清液对抗生素活性的佐剂效应
为了研究嗜酸乳杆菌La-14和动物乳双歧杆菌Bl-04对阿莫西林和克拉霉素的抗螺旋杆菌活性的可能的佐剂或有利效应,将棋盘法用在96孔板上。将MRS肉汤中培养的200μl益生菌上清液倒入第一列的第一个孔(H1,起始浓度,1:2)中,并将200μl的抗生素倒入第一列的其他孔(H2-H12)中;将含有10%的灭活胎牛血清的100μl布鲁杆菌(Brucella)肉汤添加到剩余的孔中。在水平方向上将所有物质双倍稀释后,将100μl益生菌上清液添加到含有不同浓度的抗生素的第三列的每一孔(H3-A3)中;然后,在垂直方向上进行萃取物的稀释。A12孔是阴性对照(没有任何物质,图2)。以这种方式,从第三列向下的每一不同孔包含不同浓度的两种物质。将来自48小时板培养物的细菌以每ml 108个生物体悬浮在布鲁杆菌肉汤中。将2微升的细菌混悬液滴加至含有不同浓度的待检查物质的96个孔中的每一孔中。37℃下在微氧环境中将微板孵育24小时。确定肉汤稀释液的MIC(首先不是浑浊的孔)。然后将3μl混合物从每一孔传代培养到含有幽门琼脂的Petri培养皿上;37℃下在微有氧环境中将培养皿孵育3天。MBC被认为是抗生素的最低浓度,抗生素在琼脂上的传代培养物显示了完全不存在细菌生长。结果是两次测定的平均值。所有测试一式二份进行并重复三次。
所获得的结果如下:
针对幽门螺旋杆菌菌株,克拉霉素MIC为12μg/ml(MBC:24μg/ml)。阿莫西林的MIC为0.25μg/ml(MBC:0.5μg/ml)。当将益生菌上清液添加到抗生素时,我们观察到MIC值和MBC值两者的强烈降低:
-克拉霉素MIC变成0.125μg/ml,几乎是原始MIC的1/100(以相同的方式,MBC变成0.25μg/ml)。
-阿莫西林MIC变成0.00025μg/ml,是原始MIC的1/1000(以相同的方式,MBC变成0.0005μg/ml)。
因此,针对幽门螺旋杆菌的抗生素活性增加了100倍(克拉霉素)和1000倍(阿莫西林)。
以上数据系指嗜酸乳杆菌La-14;对乳双歧杆菌Bl-04发现相同的结果。
总之,本嗜酸乳杆菌和乳双歧杆菌菌株证明以巨大的方式增加针对幽门螺旋杆菌的抗微生物活性。此外,当在体外一起测试时,它们表现出针对幽门螺旋杆菌的强抗菌协同作用。DNA测序证明质粒完全不存在。两个化学药抗性菌种的核苷酸突变的数目将其配置为与原始克隆完全不同的新变体。
