Description

Clinical path forming system, method, corresponding equipment and storage medium
Technical Field
The present application relates to the field of electronic digital data processing, and more particularly to a clinical path forming system, method, and corresponding apparatus and storage medium.
Background
Clinical pathway (Clinical pathway) is a set of standardized treatment patterns and procedures established for a disease/disease type. The medical behaviors are controlled through the clinical path, so that the effects of standardizing the clinical diagnosis and treatment behaviors, guaranteeing the medical quality and safety and reducing the cost can be achieved. The national health and security commission has issued more than 1000 clinical routes of diseases for all levels of hospitals to reference for medical process guidance, medical quality control and management, and has also included the relevant content of clinical route management into performance assessment management, and clinical route relevant indexes such as the diameter entering rate of the clinical routes, average hospitalization days of single disease types, average cost of single disease types and the like also influence the performance score of the hospitals.
Based on the published clinical paths of the state, a plurality of hospitals are improved on the basis of the published clinical paths, so that own clinical paths are formed for in-hospital management, but diagnosis and treatment projects in the clinical paths are mostly obtained based on expert consensus discussion or guidelines, data support is lacking, and meanwhile, along with the continuous development of medical technology and the continuous updating and refinement of disease diagnosis and treatment schemes, the phenomenon that a large number of out-of-clinical-path projects are required to be applied to patients frequently occurs in the actual application of the current clinical paths, and the phenomenon that the currently used clinical paths cannot adapt to the development and change of the disease diagnosis and treatment is required to be continuously optimized and updated to improve the diameter entering rate and the management and control effect is also described.
Furthermore, the establishment of clinical routes for countries and hospitals is also aimed at optimizing medical and care protocols by disease-specific design. The diagnosis and treatment operation which is carried out conventionally is required in the normal diagnosis and treatment process, unnecessary and unreasonable diagnosis and treatment behaviors are reduced, the hospitalization time is standardized, the medical resource consumption is rationalized, and the medical expense is controlled to be increased. However, although clinical path forms issued by the country and each hospital contain diagnosis and treatment items of applicable objects, diagnoses and diagnosis and treatment stages, specific medication, consumables and cost have no clear guidance scheme for selection of diagnosis and treatment schemes, and particularly, the diagnosis and treatment items and the specific medication and consumable guidance schemes are not available in scenes with larger resource consumption such as surgery, anesthesia and the like. The method also enables the traditional clinical path to not control the resource consumption and diagnosis and treatment behaviors under the operation and anesthesia, and for the diagnosis and treatment behaviors mentioned in the clinical path, the resource consumption under the diagnosis and treatment behaviors cannot be controlled, so that the consumption difference of each department and each doctor is large, and the accurate cost control cannot be achieved.

Detailed Description
Embodiments and examples of the present application will be described in detail below with reference to the accompanying drawings.
The scope of applicability of the present application will become apparent from the detailed description given hereinafter. It should be understood, however, that the detailed description and specific examples, while indicating preferred embodiments of the application, are given by way of illustration only.
Fig. 1 shows a block diagram of a preferred embodiment of a clinical path forming system according to the present application. The system comprises:
the disease seed selection module 102 is used for selecting disease seeds needing to be optimized or generated into a clinical path according to own requirements by a patio or a researcher.
The disease-specific clinical path determination module 104 is configured to determine whether a disease-specific clinical path of the selected disease species exists, wherein the disease-specific clinical path refers to a national version clinical path issued by national defense commission.
The first disease-specific clinical path tag usage rate calculation module 106 is configured to calculate, when determining a disease-specific clinical path having a selected disease type, a usage rate of each disease-specific clinical path tag according to a pre-constructed disease-specific clinical path tag, wherein the disease-specific clinical path tag represents a clinical diagnosis and treatment item and is matched with a charge item or order data of the selected disease type of the corresponding hospital, respectively.
The disease-specific clinical pathway signature may be constructed as follows. For diagnosis and treatment projects related in the clinical path of the disease type (selected disease type) of the national edition, the project names in the clinical path of the disease type of the national edition are applied as the names of all the labels, and charging project/doctor's advice data related to the disease type are matched to all the labels. For the projects not related to the national version clinical path of the disease, the standardization classification is carried out according to the following standardization rules, the names are created by referring to the national clinical path expression mode, and the charging projects or orders are matched to the labels. One label may contain multiple charge items or multiple orders, but one label may contain only charge items or orders, not both, and one charge item/order may correspond to only one label.
Items not covered by the clinical path of the disease species of the national edition can construct tag names and match charged items/orders according to the following labeling rules. The clinical path labels of the specific diseases to be constructed are divided into three categories of consumable materials, medicines and medical service items. For medicines, for example, the charge items classified as medicines can be classified according to categories, and the label names are expressed as medicine categories (such as antacids, anti-emetics, etc.) with reference to the national clinical route expression mode, referring to, for example, the 2019 edition of the national medical insurance medicine catalogue. For consumable, for example, reference may be made to "medical instruments catalogue", and the charging items classified as consumable are classified according to the category, and a certain diagnosis and treatment item object (such as an oxygen absorbing object, an infusion object, etc.) related to the tag is preferentially adopted, if there is no consumable of the corresponding diagnosis and treatment item, the tag name is expressed as the consumable category (such as dressing, hemostatic material, etc.). For medical service projects, firstly, the medical orders except the description medical orders in the medical order list are standardized, and the expression modes of the medical orders are standardized to form standardized medical orders. All orders except the orders (or at least orders covering examination, electronic application, consultation) will then be described for classification, with the tag name expressed as a diagnostic item (e.g. abdominal ultrasound, nebulized inhalation, etc.) with reference to the national clinical pathway expression. Judging whether the 'standardized medical advice' covers all the charging items with the category of medical service items by using a medical advice and charging item mapping relation table, and for the charging items without the corresponding standardized medical advice, classifying the charging items according to the diagnosis and treatment items to form a label, wherein the label name is expressed as the diagnosis and treatment items (such as II-level nursing and the like) by referring to the national clinical path expression mode, for example, the 2012 version of the national medical service item is used as a reference.
The specific clinical path label usage can be calculated, for example, as follows:
specific clinical pathway label usage = 100% of the number of cases/total number of cases involved in the specific clinical pathway label. The number of medical records related to the label is calculated from a hospital charge item list or a doctor's advice list.
In an embodiment, the dedicated disease clinical path labeling physician usage and/or dedicated disease clinical path labeling department usage may also be calculated:
specific clinical path label a doctor's rate of use = 100% of the number of doctor's cases/total number of doctor's cases referred to by the specific clinical path label.
Specific clinical path label specific department use rate = 100% of the number of department cases/total number of department cases related to the specific clinical path label.
The first specific disease clinical path tag recommendation module 108 is configured to generate a first specific disease recommendation removal and/or inclusion and/or replacement tag according to a first predetermined specific disease criterion according to the calculated usage of each specific disease clinical path tag. That is, the application of the "dedicated disease clinical path tag" is compared with the national version clinical path, and recommendation inclusion/recommendation removal/recommendation alternatives are generated on the basis of the national clinical path. For example, a "dedicated clinical path label" that is the same as the item name in the national clinical path and has a usage rate of less than 30% is a recommended removal label; the "special disease clinical path label" which is different from the item name in the national clinical path but has the use rate of 50% or more is a recommended inclusion label; and for a specific disease clinical path label corresponding to the charging item of the operation and anesthesia, if the usage rate is more than or equal to 50%, recommending the specific disease clinical path label to optimize and replace the operation/anesthesia. Criteria for specific disease criteria such as usage thresholds may be set according to hospital practices.
The first hospital clinical path generating module 110 is configured to modify the clinical path of the specific disease according to the first specific disease recommendation, remove and/or incorporate and/or replace the label, and take the modified clinical path of the specific disease as the clinical path in the hospital, so that the clinical path is more consistent with the reality and science of the hospital.
In an embodiment, the clinical pathway forming system further comprises:
an intra-hospital clinical path determination module for determining whether an intra-hospital clinical path for the selected disease species is present. "hospital" refers to an application in a hospital;
and the intra-hospital clinical path label utilization rate calculation module is used for calculating the utilization rate of each intra-hospital clinical path label according to the pre-constructed intra-hospital clinical path label in response to determining the intra-hospital clinical path with the selected disease, wherein the intra-hospital clinical path label represents a clinical diagnosis and treatment project and is matched with the charge project or doctor's advice data of the disease of the corresponding hospital respectively. Specifically, the name of the item in the clinical path of the disease in the hospital can be used as the name of each label of the clinical path in the hospital, if the hospital has a corresponding relation table of the clinical path of the disease, the medical advice and the charging item mapping relation table is consulted, and the charging item/medical advice corresponding to the item in the clinical path of the hospital after the medical advice and the charging item in the mapping relation table are removed is the charging item/medical advice contained in the clinical path. If the hospital does not have a corresponding relation table of clinical paths, medical orders and charging items, the clinical path items are used as names, experience and clinical path item expression modes are combined to distinguish the clinical path items as medicines and other diagnosis and treatment items, for medicine diagnosis and treatment items, the national medical insurance catalogue can be used as a reference, the charging item names are classified and matched to the clinical path items, for other diagnosis and treatment items, for example, the national medical service item 2012 can be used as a reference, and the medical orders/charging items are matched to the clinical path items. The matching is similar to the healthcare item targeting rules in the targeting rules described above. Likewise, one label may contain multiple charge items or multiple orders, but one label may contain only charge items or orders, not both, and one charge item/order may correspond to only one label. The clinical path label usage rate in hospital is calculated as follows: clinical route label usage in a hospital = 100% of the number of medical records/total medical records referred to by the clinical route label in the hospital.
And the intra-hospital clinical path label recommending module is used for generating intra-hospital recommended removal and/or inclusion and/or replacement labels according to the preset intra-hospital criteria according to the calculated use rate of the clinical path labels of the specific diseases and the use rate of the clinical path labels of the specific diseases. For example, the "clinical path label in hospital" is recommended to be removed, the usage rate is less than 30%, and the corresponding "clinical path label in specific disease" is less than 50%; the utilization rate of the special disease clinical path label is greater than or equal to 50%, and the corresponding hospital clinical path label does not belong to the items in the hospital clinical path, and the special disease clinical path label is a recommended inclusion label. For a 'special disease clinical path label' of which the scene of a charging item belongs to surgery and anesthesia (an execution department/an opening department in a charging list is an operating room or an anesthesia department, or the charging item of which the application scene is surgery/anesthesia can be judged according to product use instructions, medical service item interpretation and the like), if the utilization rate is more than or equal to 50%, recommending the 'special disease clinical path label' to optimally replace surgery/anesthesia; for the use ratio of the clinical path label in hospital is less than 30%, but the use ratio of the clinical path label in specific disease is more than or equal to 50%, the clinical path label in specific disease is recommended to replace the clinical path label in hospital. Similarly, criteria such as usage thresholds for in-hospital criteria may be set based on hospital practices.
And the intra-hospital clinical path updating module is used for updating the intra-hospital clinical path according to the intra-hospital recommended removal and/or inclusion and/or replacement labels.
In an embodiment, the clinical pathway forming system further comprises:
a second disease-specific clinical path tag usage calculation module for calculating usage of each disease-specific clinical path tag from pre-constructed disease-specific clinical path tags in response to determining that there is no disease-specific clinical path of the selected disease species;
and the second special disease clinical path label recommending module is used for generating a second special disease recommendation inclusion label according to a second preset special disease criterion according to the calculated use rate of each special disease clinical path label. For example, the labels with the usage rate of 50% or more of the "special disease clinical path labels" are recommended inclusion labels;
and the second hospital clinical path generation module is used for generating a hospital clinical path according to the second special disease recommendation inclusion label.
In an embodiment, the clinical path forming system further comprises an expert criticizing module for accepting a secondary confirmation of the recommended label by the expert. And (3) the expert critizes the recommended inclusion/removal/replacement items based on clinical experience and the traditional clinical path, judges whether to include/remove/replace or not, and then generates or updates a clinical path based on the result after the expert critizes, wherein the clinical path comprises the optimized items in the clinical path, the associated names of the charging items and standard schemes (upper limit, lower limit and average value of the usage).
In an embodiment, the clinical path forming system further comprises a viewing module for viewing at least the calculated dedicated disease clinical path label usage, intra-hospital clinical path label usage, dedicated disease clinical path label doctor usage, and/or dedicated disease clinical path label department usage results.
In the embodiment, a recommended scheme of specific medication, consumable and usage under the diagnosis and treatment item in the clinical path can be provided, and particularly, the recommended scheme of specific medication item and specific medication, consumable and usage applied to special scenes with larger resource consumption such as surgery, anesthesia and the like replaces a mode of only controlling the surgery name in the traditional clinical path, so that accurate fee control is realized.

In step S204, the usage rate of each disease-specific clinical path label is calculated according to pre-constructed disease-specific clinical path labels, wherein the disease-specific clinical path labels represent clinical diagnosis and treatment projects and are respectively matched with the charge projects or doctor orders data of the disease types of the corresponding hospitals, and each charge project or doctor orders can only correspond to one disease-specific clinical path label.
In step S206, a first specific disease recommendation removal and/or inclusion and/or replacement label is generated according to the calculated usage of each specific disease clinical path label according to a first predetermined specific disease criterion.
In step S208, the dedicated disease clinical path is modified according to the first dedicated disease recommendation removal and/or inclusion and/or replacement labels and the modified dedicated disease clinical path is taken as an intra-hospital clinical path.
In step S222, the use rate of each disease-specific clinical path tag is calculated from the pre-constructed disease-specific clinical path tags.
In step S224, a second specific disease recommendation inclusion tag is generated according to the calculated usage rate of each specific disease clinical path tag and a second predetermined specific disease criterion.
In step S226, an in-hospital clinical pathway is generated from the second specific disease recommendation inclusion tag.
In an embodiment, the clinical pathway forming method further comprises: determining whether there is an intra-hospital clinical pathway for the disease species; in response to determining an in-hospital clinical path having the disease, calculating a utilization rate of each in-hospital clinical path tag according to pre-constructed in-hospital clinical path tags, wherein the in-hospital clinical path tags represent clinical diagnosis and treatment items and are respectively matched with charge items or orders data of the disease of a corresponding hospital, wherein each charge item or order can correspond to only one in-hospital clinical path tag; generating a hospital recommended removal and/or inclusion and/or replacement label according to a preset hospital criterion according to the calculated use rate of each specific disease clinical path label and the use rate of each hospital clinical path label; the intra-hospital clinical path is updated according to the intra-hospital recommendation to remove and/or incorporate and/or replace tags.
