Description

Clinical pathway formation system, method, and corresponding apparatus and storage medium
Technical Field
The present application relates to the field of electrical digital data processing, and more particularly to clinical pathway formation systems, methods, and corresponding devices and storage media.
Background
The Clinical pathway (Clinical pathway) refers to a standardized set of treatment modalities and procedures established for a disease/disease species. The medical behavior is controlled through the clinical path, and the effects of standardizing the clinical diagnosis and treatment behavior, ensuring the medical quality and safety and reducing the cost can be achieved. The national health record committee has released more than 1000 clinical routes of diseases for all levels of hospitals to refer to at present, and the clinical routes are used for guidance of medical process, control and management of medical quality, and has brought relevant contents of clinical route management into performance assessment management, and relevant indexes of clinical routes such as access rate of clinical routes, average number of days of hospitalization of single disease category, average cost of single disease category and the like will influence performance scoring of hospitals.
Based on the clinical path published by the country, a plurality of hospitals are improved on the basis of the clinical path, and the clinical path of each hospital is formed for in-hospital management and control, but most diagnosis and treatment items in the clinical path are obtained based on expert consensus discussion or guidelines and lack of data support, and meanwhile, with the continuous development of medical technology and the continuous updating and refining of disease diagnosis and treatment schemes, in the practical application of the current clinical path, a phenomenon that a patient needs to apply a large number of items outside the clinical path often occurs, which also indicates that the currently used clinical path cannot be suitable for the development and change of disease diagnosis and treatment, and needs to be continuously optimized and updated to improve the path entry rate and the management and control effect.
In addition, the establishment of clinical routes in countries and hospitals also aims to design optimal medical and care plans by disease category. The diagnosis and treatment operation which needs to be carried out conventionally in the diagnosis and treatment process is standardized, unnecessary and unreasonable diagnosis and treatment behaviors are reduced, the hospitalization time is standardized, the medical resource consumption is rationalized, and the increase of medical expenses is controlled. However, although clinical path forms published by countries and hospitals include diagnosis and treatment items of applicable objects, diagnoses and various diagnosis and treatment stages, specific medicines, consumables and cost for selecting diagnosis and treatment schemes do not have a clear guidance scheme, and particularly, no diagnosis and treatment items and specific medicine and consumable guidance schemes are provided for scenes with high resource consumption such as operations and anaesthesia. This also makes traditional clinical route can't control operation, resource consumption and the action of diagnosing under anesthesia, for the action of diagnosing mentioned in clinical route, can't control the resource consumption under the action of diagnosing, cause each department, each doctor to consume the difference great, can't do accurate accuse and spend.

Detailed Description
Embodiments and examples of the present invention will be described in detail below with reference to the accompanying drawings.
The scope of applicability of the present invention will become apparent from the detailed description given hereinafter. It should be understood, however, that the detailed description and the specific examples, while indicating preferred embodiments of the invention, are given by way of illustration only.
FIG. 1 shows a block diagram of a preferred embodiment of a clinical pathway formation system in accordance with the present invention. The system comprises:
and the disease species selection module 102 is used for selecting the disease species needing to be optimized or generating the clinical path according to the self requirements by the academies or researchers.
A disease specific clinical pathway determination module 104 for determining whether there is a disease specific clinical pathway for the selected disease category, wherein the disease specific clinical pathway refers to a national version of the clinical pathway promulgated by the national council.
The first specific clinical path tag usage rate calculation module 106 is configured to calculate, when a specific clinical path with a selected disease category is determined, a usage rate of each specific clinical path tag according to a pre-constructed specific clinical path tag, where the specific clinical path tag represents a clinical diagnosis and treatment item and is respectively matched with a charging item or medical order data of the selected disease category of a corresponding hospital.
The specific clinical pathway label can be constructed as follows. For the diagnosis and treatment items related in the clinical path of the disease category (selected disease category) of the national edition, the item names in the clinical path of the disease category of the national edition are used as the names of all labels, and the charging items/medical advice data related to the disease category are matched to all the labels. For the items not related to the national version of the disease clinical pathway, the standardized classification is carried out according to the following standardized rules, names are created by referring to the national clinical pathway expression mode, and the charging items or medical orders are matched to the labels. One tag may contain multiple charge items or multiple orders, but one tag may contain only charge items or orders, not both, and one charge item/order may correspond to only one tag.
The national version of the item not covered by the clinical pathway of the disease category can construct a label name and match the billing item/order according to the following standardization rules. The clinical path labels of the special diseases to be constructed are divided into three categories of consumables, medicines and medical service items. For example, the pharmaceutical products may be classified according to their categories by referring to the catalog of national medicals 2019, and the label names may be expressed as pharmaceutical product categories (e.g., anti-acid drugs, anti-emetic drugs, etc.) by referring to the national clinical pathway expression. For consumables, for example, refer to "medical device classification catalog", and classify the billing items classified as consumables according to categories, and preferentially adopt a certain medical item object (such as an oxygen inhalation object, an infusion object, etc.) related to a label, and if there is no corresponding medical item consumable, the label name is expressed as a consumable category (such as a dressing, a hemostatic material, etc.). For the medical service items, firstly, the medical orders except for the description medical orders in the medical order detail table are standardized, and the expression modes of the medical orders are standardized to form standardized medical orders. All orders (or at least covering examinations, electronic application forms, consultation orders) other than the description order are then classified, and the label names are expressed as medical items (e.g., abdominal ultrasound, aerosol inhalation, etc.) with reference to the national clinical pathway expression. Whether the standardized medical advice covers all the charging items of which the categories are medical service items or not is judged by applying the mapping relation table of the medical advice and the charging items, for the charging items without the corresponding standardized medical advice, for example, the 'national medical service items' 2012 edition is taken as a reference, the charging items are classified according to the diagnosis and treatment items to form labels, and the label names are expressed as the diagnosis and treatment items (such as II-level nursing and the like) according to the national clinical path expression mode.
The clinical pathway label usage for a specific disease can be calculated, for example, as follows:
the usage rate of the clinical path label of a specific disease is 100% of the number of cases related to the clinical path label of the specific disease/the total number of cases. The number of cases referred to by the label is calculated from the hospital charge item or order details.
In embodiments, the specific illness clinical pathway label physician usage and/or specific illness clinical pathway label department usage may also be calculated:
the usage rate of a doctor in a special clinical path label is 100% of the number of doctor cases related to the special clinical path label/the total number of doctor cases.
The usage rate of a department in a special clinical path label is 100% of the number of the department records related to the special clinical path label/the total number of the department records.
A first disease-specific clinical pathway label recommendation module 108 for generating a first disease-specific recommended removal and/or inclusion and/or replacement label according to a first predetermined disease criterion based on the calculated usage rate of each disease-specific clinical pathway label. That is, applying the "specialty clinical pathway label" against the national version clinical pathway generates a recommendation inclusion/recommendation removal/recommendation alternative based on the national clinical pathway. For example, a "specialty clinical pathway label" that is the same as the name of the item in the national clinical pathway and has a usage rate of less than 30% is a recommended removal label; the special clinical pathway label which is different from the item name in the national clinical pathway but has the use rate of more than or equal to 50 percent is a recommended inclusion label; and for the special disease clinical path label corresponding to the charging item with the scene of the charging item being the operation and the anesthesia, if the utilization rate is more than or equal to 50%, the special disease clinical path label is recommended to be optimized to replace the operation/anesthesia. Criteria for the specific criteria, such as a usage threshold, can be set based on the actual conditions of the hospital.
A first nosocomial clinical pathway generation module 110 for removing and/or including and/or replacing the label according to the first nosocomial recommendation to modify the nosocomial clinical pathway and treating the modified nosocomial clinical pathway as a nosocomial clinical pathway, thereby making the clinical pathway more in line with hospital reality and science.
In an embodiment, the clinical pathway formation system further comprises:
an in-hospital clinical pathway determination module to determine whether there is an in-hospital clinical pathway for the selected disease category. "in-hospital" refers to use in a certain hospital;
and the hospital internal clinical path label utilization rate calculation module is used for responding to the determination of the hospital internal clinical path with the selected disease category, and calculating the utilization rate of each hospital internal clinical path label according to the pre-constructed hospital internal clinical path label, wherein the hospital internal clinical path label represents clinical diagnosis and treatment items and is respectively matched with the charging items or the medical order data of the disease category of the corresponding hospital. Specifically, the name of the item in the clinical path of the patient type in the hospital can be used as each label name of the clinical path in the hospital, and if the hospital has a corresponding relation table of the clinical path of the patient type, the medical order and the charging item, the mapping relation table of the clinical path of the hospital is referred to, and the medical order with consumable material in the category in the mapping relation table and the charging item/medical order corresponding to the item in the clinical path of the hospital after the charging item are removed are the charging item/medical order contained in the clinical path. If the hospital has no corresponding relation table of clinical paths, medical orders and charging items, the items in the clinical paths of the disease are used as names, the items in the clinical paths are distinguished into medicines and other diagnosis and treatment items by combining experience and the expression mode of the items in the clinical paths, for the medicine diagnosis and treatment items, the names of the charging items can be classified and matched to the items in the clinical paths by taking national medical insurance catalogue as reference, for the other diagnosis and treatment items, the medical orders/charging items can be matched to the items in the clinical paths by taking national medical service item 2012 as reference. The matching is similar to the targeting rules for healthcare items in the targeting rules described above. Likewise, a tag may contain multiple charge items or include multiple orders, but a tag may contain only charge items or orders, not both, and only one charge item/order may correspond to a tag. The in-hospital clinical pathway label usage was calculated as follows: the usage rate of the clinical pathway label in a hospital is 100% of the number of cases related to the clinical pathway label in the hospital.
And the hospital clinical pathway label recommendation module is used for generating a hospital recommended removal and/or inclusion and/or replacement label according to the calculated utilization rate of each special disease clinical pathway label and the utilization rate of each hospital clinical pathway label and a preset hospital criterion. For example, a "hospital clinical pathway label" usage rate of less than 30% corresponds to a "specific illness clinical pathway label" of less than 50%, the "hospital clinical pathway label" being a recommended removal label; the usage rate of the special clinical pathway label is more than or equal to 50%, and the corresponding in-hospital clinical pathway label does not belong to the item in the hospital clinical pathway, and the special clinical pathway label is a recommended inclusion label. For a 'special disease clinical path label' of which the scene of the charging item is surgery and anesthesia (the executive department/the order department in the charging detail table is an operating room and an anesthesia department, or the application scene can be judged to be the charging item of surgery/anesthesia according to product use instructions, medical service item interpretation and the like), if the utilization rate is more than or equal to 50%, the 'special disease clinical path label' is recommended to be optimized to replace the surgery/anesthesia; for a "clinical pathway label in the hospital" usage rate of less than 30%, but a "clinical pathway label for a specific disease" usage rate of 50% or more, the "clinical pathway label for a specific disease" is recommended to replace the "clinical pathway label in the hospital". Similarly, criteria within the hospital, such as a usage threshold, may be set based on the actual conditions of the hospital.
An in-hospital clinical pathway update module to update an in-hospital clinical pathway according to the in-hospital recommendation removal and/or inclusion and/or replacement label.
In an embodiment, the clinical pathway formation system further comprises:
the second special clinical pathway label utilization rate calculation module is used for responding to the special clinical pathway without the selected disease category and calculating the utilization rate of each special clinical pathway label according to the pre-constructed special clinical pathway label;
and the second special disease clinical path label recommending module is used for generating a second special disease recommendation inclusion label according to the calculated utilization rate of each special disease clinical path label and a second preset special disease criterion. For example, the labels with the usage rate of the "special clinical pathway label" being greater than or equal to 50% are all recommended inclusion labels;
and the second nosocomial clinical pathway generation module is used for generating a nosocomial clinical pathway according to the second special disease recommendation inclusion label.
In an embodiment, the clinical pathway formation system further comprises an expert review module for accepting a secondary confirmation by an expert of the recommended label. And performing review on recommended inclusion/removal/replacement items by an expert based on clinical experience and a traditional clinical path, judging whether the items are included/removed/replaced, and generating or updating a clinical path based on the result after the review by the expert, wherein the clinical path comprises the optimized items in the clinical path, the associated charging item names and standard schemes (upper limit, lower limit and mean value of the usage).
In an embodiment, the clinical pathway formation system further comprises a viewing module for viewing at least the calculated specific clinical pathway label usage, in-hospital clinical pathway label usage, specific clinical pathway label physician usage, and/or specific clinical pathway label department usage results.
In the embodiment, a recommendation scheme of specific medication, consumables and usage under the clinical path internal diagnosis item can be provided, and particularly, a mode of only controlling the name of an operation in the traditional clinical path is replaced by the recommendation scheme of applying the specific diagnosis and treatment item and the specific medication, consumables and usage to a special scene with high resource consumption such as operation and anesthesia, so that accurate cost control is realized.

In step S206, a first disease specific recommendation removal and/or inclusion and/or replacement label is generated according to a first predetermined disease specific criterion based on the calculated usage of each disease specific clinical pathway label.
In step S208, the disease specific clinical pathway is modified according to the first disease specific recommendation by removing and/or including and/or replacing the label and the modified disease specific clinical pathway is taken as the in-hospital clinical pathway.
