Abstract
本发明由用于宫腔内放置的扩张导管组成,有着能适应子宫器官在其内腔的不同形态的符合解剖学特性的特征,因此在运用不同手术手段进行的刮宫术导致腔的内壁变薄后能够对腔的内壁进行高效地扩张和分离,以作为预防宫腔粘连的分离器和隔离器,提高设备内部和向四周引流的能力。本设备包含了处于收缩状态的扩张设备,并在推进器的辅助下通过插入设备放置在子宫内,而推进器和插入设备将在部件放置到位后一并丢弃。
Description
技术领域
本发明涉及用于子宫内放置的扩张导管,所述扩张导管具有可适应于子宫器官在其内腔中呈现的不同形态的解剖特性,有效地允许子宫腔的内壁的扩张和分离,从而达成两个目的:一方面是分离子宫壁从而预防宫腔粘连,即避免子宫壁的贴合;另一方面是挤压子宫壁,以达到止血的效果。
背景技术
子宫腔在医学上被定义为虚拟的腔体。这意味着子宫壁之间在没有人工分离的作用下是紧密接触的。子宫壁的这种贴合可能会导致在内部损伤或者是受伤的情况下,例如在宫腔手术的情况下,在对立面间形成粘连或瘢痕。
过去已经应用了许多不同的方法来降低这类粘连的发生率。在不同的方法当中,物理或屏障法是最广泛使用和有效的,林(Lin)的对比研究(Lin,X.,Wei,M.,Li,T.C.,Huang,Q.,Huang,D.,Zhou,F.,&Zhang,S.(2013).A comparison of intrauterineballoon,intrauterine contraceptive device and hyaluronic acid gel in theprevention of adhesion reformation following hysteroscopic surgery forAsherman syndrome:a cohort study.European Journal of Obstetrics&Gynecologyand Reproductive Biology,170(2),512-516.doi:10.1016/j.ejogrb.2013.07.018)证明了这一点,发现使用宫内导管时的粘连率低于使用宫内节育器或者是抗粘连凝胶时的粘连率。
这种类型的导管体现在一种可充气体,这种充气体包含了一系列的体外外管,并且在植入体内后通过这些体外外管,设备能够充气,形成单一的中央引流通道。
这样的设计也成为了这类设备的问题(不符合子宫解剖学),即放置困难和腔内积血引流不良。
本发明还和发明专利WO2016074647的方法相似,其中提供了一种可充气球囊用于分离子宫壁,而带有两个推进阀的球囊用于宫颈。这种方法有着相同的问题,即放置困难及由于仅设置内部引流导致的腔内积血引流不良。
同样的,在发明专利US 4552557A中,还提供一种带有内部引流的可充气球囊,其中涉及单个呈三角形的内部充气室,但也和前面的设计有着一样的局限性,即涉及放置困难及由于仅设置内部引流导致的腔内积血引流不良。
本发明除了这些问题外还考虑到,这些设备没有为固定在子宫的导管和侧向引流至宫颈的后侧基底提供引导系统。
发明内容
这里提出的宫腔扩张器设备解决了经阴道的放置和内部定位的问题,而且避免了由于壁的充气过度导致的患者不适,除没有体外外管之外,其许多引流通道均有显著改进,以消除因液体积聚和主引流通道的堵塞而造成的多余压力。
为此,并且更具体地说,基于以上对此类设备概念结构的描述,本发明的设备的独特性在于它是由可充气主体构成,所述可充气主体具有在其中央区域带有钝性尖端的倒矛头外形的结构,并与通过中央管内引流的中央引流通道相吻合,特别地,所述主体由几个单独的充气腔室叠加而成,这些充气腔室确定了两个带有圆形边缘的柔性侧翼,这些柔性侧翼在插入设备内初始为折叠状态且占据体积最小,而插入设备则为管状和开口的结构,类似于护套。因此,它实质性地促进了设备的植入与其经子宫颈的通行,在插入设备的引入下确保了末端位置的深度,即在构成其解剖结构的末端正确地放置在子宫内,并通过子宫颈到达构成子宫腔本身的空间的底部,且一旦插入设备通过内部推进器被放置在工作区域,所述插入设备能够轻易地从设备上轴向地移除,所述内部推进器也与插入设备一并丢弃,所述设备可以通过其腔室的选择性充气轻易地展开,而其腔室与通过主引流管内部的小充气管相关联,且小充气管的长度远大于主引流管的长度,因此小充气管在其下端采用螺旋状结构,一旦设备在适应了患者的身体后,小充气管便可以回缩,并且小充气管是在腔内的而非外部的,因此,在隔开的腔室充气后,由于这些管因其螺旋结构而可缩回,因此它们将保留在患者阴道内,在阴道内而非外部(这在本发明的背景技术部分中进行了描述),以减少了穿戴时由于设备悬挂在外而导致的不适感,因此避免了可能的感染源和患者活动的限制;所述管终止在相应的止回阀中。
因此,主可充气体呈现了符合人体工程学的结构,其变量能够适应器官的不同的形态,从而在充气时在两个主面和两个副面上均限定了弯曲的轮廓部分,使得它在子宫壁的支撑下确定了通往中央引流通道外的多个附加的引流管道。
更具体地说,在主体中采用了双中心和四侧充气腔室来生成更多的引流通道;除了统一的中央通道之外,从而覆盖了所有的壁内分泌物液体排出的角度,且因此避免了压力过大和上皮组织粘连。
接着,所述主体在末端设置了三重气囊用于附着于子宫颈,用不连续的侧附着环和引流管道网来防止在肌肉收缩阶段的移位和可能产生的脱落。
最后,在三重气囊露出上述的中央引流管后,所述中央引流管延伸到上边缘以作为不同充气通道补充,即除了由于腔室膨胀而形成的侧引流通道,增添了中央管道用于内部液体的排空,从而限定了七个引流通道:一个在中央,两个在上方,两个在下方,以及两个在侧向。
这种特殊的设备结构使得它能够有更小的体积和重量,而不需要外部尖锐形状的塑料部件。
相似地,不同的充气腔室可以为更好地适应腔内畸形而进行局部和不规律的充气。
至于主体在护套或插入器内的折叠方式,它被折叠为枢轴的形式,以更好地放置在适用于宫腔的侧壁上的导管内。
该设备的形态为带有内部管道网路径和前部充气限制器的侧向漂浮物,保证了更好的适应子宫腔的能力,并且能产生更大的分离体积,而且在放置过程中不会有任何的内部不适,从而避免了可能的总体或局部粘连而需要妇科学家随后再次介入;避免了在恢复阶段增加子宫肌肉收缩阶段的不适,例如非自愿收缩引起的不适。

具体实施方式
根据上述的附图,可以看出所提出的宫腔扩张器设备由具有圆形边缘的倒置矛头状的结构的可充气主体(1)构成,其中在上方限定了流入中央管(3)的中央引流通道(2),而中央通道(2)的末端部分地被钝性尖端(4)堵塞,因此所述可充气主体(1)由几个单独的充气腔室构成,这些充气腔室确定了两个在插入设备(6)中初始为折叠状态占体积最小的带有圆形边缘的柔性侧翼(5),而如图5所示,插入设备为带有圆形和开口顶端的管状结构,类似于护套,极大促进了设备的植入和其经过子宫颈的通行。

通过这种方式,本设备能够通过腔室充气轻易地进行展开,其腔室与通过中央引流管(3)内部的小充气管(7)相关联,而小充气管呈现的长度远大于中央引流管的长度,因此在它们的下端采取螺旋状结构,这使得设备一旦适应了患者的身体,它们能够进行回缩,且其端部止于相应的止回阀(图中未示出)。
主体(1)呈现了可充气且符合人体工程学的构造以适应器官的不同形态,其特点在于在充气时会在两个主面和两个副面上均限定了弯曲的轮廓部分,在子宫壁的支撑下,确定了除中央引流通道(2)和中央管(3)外的多个引流管道或通道(8)。

Claims (4)
1.宫腔扩张器设备,其特征在于,所述宫腔扩张器设备由可充气主体(1)构成,所述可充气主体具有圆形边缘的倒置矛头状的结构,其中在上方限定了流入中央管(3)的中央引流通道(2),中央通道(2)的末端部分地由钝性尖端(4)堵塞,而所述可充气主体(1)由几个单独的充气腔室构成,所述充气腔室限定了两个带有圆形边缘的柔性侧翼(5),所述主体在充气状态时的主面上和副面上均呈现出弯曲的轮廓部分,轮廓部分支撑子宫壁,并限定除由中央管(3)限定的中央引流通道外的多个引流管道或通道(8),所述可充气主体(1)的腔室由通过中央引流管(3)内部的小充气管(7)进行辅助充气,其独特点在于在主体(1)的下方设有用于附着在宫颈的一系列气囊(9),优选为三个,所述气囊带有不连续的环(10),以利于向四周引流。
2.根据权利要求1所述的宫腔扩张器设备,其特征在于,其包含了带有管状和开口结构的如护套的插入设备(6),所述主体(1)最初插入并折叠在其中,所述插入设备作为设备穿过宫颈的插入元件,在借助内部推进元件(11)插入后,相对于所述设备轴向可移除。
