一种眼内液细胞因子检测试剂,检测试剂包括检测眼内液中的Eotaxin、IL-10、IL-12p70、IL-1β、IL-2、IL-5、IL-6、IL-8、MCP-1、TNF、IFN-γ、VEGF、IL-17a、ICAM-1、VCAM-1、TGF-β1和bFGF的试剂中的至少两种。
通过采用上述技术方案,Eotaxin有助于嗜酸性粒细胞募集到响应寄生虫感染以及过敏性和自身免疫性疾病的炎症部位。
IL-10是一种多效性细胞因子,可以在多种类型细胞中发挥免疫抑制或免疫刺激的作用。
IL-12p70促进 Th1细胞的生成和细胞免疫应答, 抑制 Th2 细胞的生成, 刺激多种细胞因子的分泌尤其是 IFN-γ,并且可增强NK细胞、巨噬细胞和CTL细胞的杀瘤效应,通过间接作用抗肿瘤血管生成,增强机体抗肿瘤免疫反应。

IL-6在感染和组织损伤的反应中迅速而短暂地产生,通过刺激急性期反应、造血和免疫反应促进宿主防御,失调的IL-6持续合成对慢性炎症和自身免疫具有病理学作用。
IL-8是趋化性细胞因子,可以促进炎症细胞趋化和诱导细胞增殖,并在癌组中高表达。
MCP-1 是调节单核细胞/巨噬细胞迁移和浸润的关键趋化因子之一。TNF感染时能激活中性粒细胞和巨噬细胞成熟活化,导致炎症;参与机体抗肿瘤免疫反应。
TNF-a即TNF,是葡萄膜炎发病机理中的重要因素。抗TNF-a治疗可调节眼后段眼内炎症患者的外周血T细胞,从而有助于视觉功能的恢复
IFN-γ诱导巨噬细胞可诱导性一氧化氮合酶产生,促进NO的合成,可能与中枢神经系统的某些疾病的发生或保护作用有关。
VEGF在维持生理稳态和视网膜血管疾病(如(DR)和(AMD))中起重要作用。
IL-17a具促炎性作用;刺激上皮细胞、内皮细胞、纤维母细胞表达TNF\IL-1β、IL-6、IL-8、G-CSF等因子。
ICAM-1可募集促炎性淋巴细胞和促进I型超敏反应的肥大细胞。ICAM-1在RPE上表达较强,而VCAM-1表达较弱。VCAM-1在内源性BRB上表达最强,而ICAM-1在外源性BRB上表达最强。
VCAM-1是参与炎症反应的关键细胞粘附分子,介导淋巴细胞、单核细胞、嗜酸性粒细胞和嗜碱性粒细胞与血管内皮的粘附,在细胞免疫应答中对白细胞募集起关键作用。
TGF-β1是一种主要的多潜能细胞因子,具有明显的免疫抑制作用。
bFGF在眼睛中,扮演着晶状体形态形成的角色,不同水平的bFGF刺激会引起不同的晶状体细胞反应。
本申请通过对以上十七种细胞因子进行筛选组合,可满足眼科疾病辅助诊断和分型的需求。
可选的,检测试剂包括荧光编码微球、带有荧光标记的特异性抗体。
通过采用上述技术方案,检测试剂适用于流式细胞多因子检测技术。
第二方面,本申请提供一种眼内液细胞因子检测方法,采用如下技术方案:
一种眼内液细胞因子检测方法,选用检测试剂并采用流式细胞法对眼内液细胞因子进行检测。
通过采用上述技术方案,眼内液细胞因子的检测方法有生物活性检测法、免疫学法和分子生物学检测法。目前科研和临床上酶联免疫吸附法应用较多,但是酶联免疫吸附法只能检测眼内液样本中1-2个细胞因子的浓度,难以进行多个细胞因子间的比较。如果用大量样本检测多个细胞因子,检测费时费力的问题。
对此,本申请采用流式细胞多因子检测技术。该技术利用聚苯乙烯荧光微球连结特定的捕获抗体,捕捉待测物,再加入荧光标记的检测抗体,形成“三明治”夹心复合物,最后通过流式细胞仪对特异性目的蛋白进行检测。该技术可以同时检测多种细胞因子,检测效率高,且样本量需求小。
可选的,眼内液细胞因子检测方法包括如下步骤:
步骤一:将眼内液样本和微球溶液混合均匀,孵育得到预混样本;
步骤二:将预混样本和抗体溶液混合均匀,孵育完成后洗涤,得到待测样本;
步骤三:通过流式细胞仪对待测样本进行荧光检测,对照标准曲线,得到各细胞因子含量。
可选的,所述微球溶液内含有多种不同的捕获微球,不同的捕获微球分别包被有不同的捕获抗体并具有不同的荧光强度,不同的捕获抗体对应于不同的细胞因子。
可选的,所述抗体溶液内含有多种不同的荧光抗体,不同的荧光抗体对应于不同的细胞因子。
通过采用上述技术方案,步骤一,对应受检系统中的每一个检测指标都设有不同的捕获微球,不同的捕获微球上包被有特异的捕获抗体,并具有不同的荧光强度,通过微球溶液与眼内液样本混合后,捕获抗体结合对应的细胞因子;步骤二,通过预混样本和抗体溶液混合,细胞因子结合带有荧光标记的检测抗体,形成“三明治”夹心复合物;步骤三,通过流式细胞仪进行荧光检测,便可对样品中的各检测因子的含量进行分析。
可选的,用于检测TGF-β1时眼内液样本先经过酸化,样本酸化的过程如下:配制酸化试剂,再将眼内液样本和酸化试剂混合,孵育并中和后进行后续检测。
通过采用上述技术方案,测TGF-β1要求样本要被酸化而测其他细胞因子不需要,且酸化样本加稀释的TGF-β1微球孵育时间需2个小时,而其他微球加样本孵育时间需1个小时,故TGF-β1细胞因子的检测单独进行。
第三方面,本申请提供一种眼内液细胞因子检测试剂的应用,采用如下技术方案:
可选的,一种眼内液细胞因子检测试剂在炎症性眼病中的应用,检测试剂包括检测眼内液中的Eotaxin、IL-10、IL-12p70、IL-1β、IL-6、IL-8、MCP-1、TNF、IFN-γ和VEGF的试剂。
通过采用上述技术方案,对于非肿瘤患者且未经抗生素治疗时,若患者IL-1β、IL-6、IL-8均显著升高,提示可能细菌感染,若IL-6>160pg/ml且IL-10>50pg/ml,提示革兰氏阴性菌感染;IL-6>46pg/ml, IL-10<50pg/ml,可能提示革兰氏阳性菌感染。IFN-γ升高<100pg/ml,并且其他细胞因子如IL-1β、IL-6等也有升高,提示病毒感染可能。如果单纯IL-10升高10倍以上,并且IFN-γ也有升高,可能提示嗜血,需排除EB病毒感染等可能。
IFN-γ明显下降,IL-6明显上升,IL-10和TNF-α升高与结核感染相关;TNF含量显著升高,IL-6含量显著升高,与弓形虫感染相关。
可选的,一种眼内液细胞因子检测试剂在免疫性眼病中的应用,检测试剂包括检测眼内液中的IL-17a、IL-1β、IL-5、IL-6、TGF-β1、TNF、IFN-γ、IL-2和VEGF的试剂。
通过采用上述技术方案,IL-1β分泌增加与Behcet's病和与年龄有关的黄斑变性有关;IFN-γ升高,IL-17a升高,IL-1β升高,IL-2升高,TNF升高,TGF-β1降低与葡萄膜炎相关;IFN-γ升高,TNF升高,IL-6升高与急性葡萄膜炎相关;IFN-γ升高,IL17a升高与交感性眼炎相关;TNF升高,IL-6升高,IL-1β升高与干眼病相关。
可选的,一种眼内液细胞因子检测试剂在血管性眼病中的应用,检测试剂包括检测眼内液中的TNF、ICAM-1、IL-10、IL-6、IL-8、TGF-β1、VCAM-1和VEGF的试剂。
通过采用上述技术方案,VEGF升高,ICAM升高,TGF-β1升高与糖尿病视网膜病变(DR)相关,与DR黄斑水肿相关;ICAM升高与糖尿病性白内障相关;VEGF升高与黄斑水肿和血管通透性增加相关。
综上所述,本申请具有以下有益效果:
1、本申请优选检测Eotaxin、IL-10、IL-12p70、IL-1β、IL-2、IL-5、IL-6、IL-8、MCP-1、TNF、IFN-γ、VEGF、IL-17a、ICAM-1、VCAM-1、TGF-β1和bFGF的细胞因子组合,获得该组合细胞因子的具体含量,无法直接获得诊断结果,而作为中间结果辅助医生诊断并分型;
2、本申请中优选采用流式微球检测法,适用的细胞因子试剂种类多样,所需样本量极少,如果各种细胞因子的位置不同,样本量50μl即可满足检测需求,并且可以同时检测多种因子含量,同时进行多因子分析,操作方便,节省人力。

本申请的实施例采用如下原料:
Capture Bead Diluent(即微球稀释液)、Detection Reagent Diluent(即抗体稀释液)均购买自美国BD公司,货号558264,Human Soluble Protein Master Buffer Kit。清洗液为1×PBS缓冲液。
眼内液细胞因子检测试剂包括Human Eotaxin Flex Set、Human IL-10 FlexSet、Human IL-12p70 Flex Set、Human IL-1β Flex Set、Human IL-2 Flex Set、HumanIL-5 Flex Set、Human IL-6 Flex Set、Human IL-8 Flex Set、Human MCP-1 Flex Set、Human TNF Flex Set、Human IFN-γ Flex Set,Human VEGF Flex Set、Human IL-17aFlex Set、Human Soluble CD54 (ICAM-1) Flex Set、Human Soluble CD106 (VCAM-1)Flex Set、 Human TGF-β1 Single Plex Flex Set和Human bFGF Flex Set中的至少两种。
根据不同类型的眼病检测需要,选用相应的检测试剂,以实施例1-3进行具体说明,实施例1-3中眼内液样本数量均为1。
以上检测试剂均购买自美国BD公司,每一检测试剂均包括对应于特定细胞因子的微球试剂、抗体试剂和冻干粉人体重组蛋白。微球试剂为带有特定捕获抗体的聚苯乙烯荧光微球,每个微球大小一致,具有特定的荧光强度。不同的捕获微球具有不同的荧光强度,荧光标记为APC和APC/Cy7。抗体试剂为带有荧光标记的特异性抗体,不同的荧光抗体对应于不同的细胞因子,荧光标记为PE。
标准品曲线制作:采用冻干粉人体重组蛋白制作标准品曲线。
实施例1:
一种用于辅助诊断炎症性眼病的眼内液细胞因子的检测试剂,选用HumanEotaxin Flex Set、Human IL-10 Flex Set、Human IL-12p70 Flex Set、Human IL-1βFlex Set、Human IL-6 Flex Set、Human IL-8 Flex Set、Human MCP-1 Flex Set、HumanTNF Flex Set、Human IFN-γ Flex Set和Human VEGF Flex Set检测试剂。
检测方法包括如下步骤:
眼内液样本制备:取玻璃体液,并于10000rpm下离心10min,得到眼内液样本;
微球溶液制备:取每种微球试剂0.25 μl并混合,添加微球稀释液10μl,混合均匀,得到微球溶液;
抗体溶液制备:取每种抗体试剂0.25 μl并混合,添加抗体稀释液10μl,混合均匀,得到抗体溶液;
样本孵育:将微球溶液装到空白流式管,每管12.5μl,向流式管内加入20μl眼内液样本,混匀,室温孵育1h,得到预混抗体;
抗体孵育:向流式管内加入12.5μl抗体溶液,混合均匀,室温孵育2h,得到待测样本;
洗涤:向流式管内加入1ml清洗液,以2200rpm的转速离心5min,去除上清液,再加入300μl清洗液,重悬;
检测:通过流式细胞仪对待测样本进行荧光检测,对照标准曲线,得到各细胞因子含量。
实施例2:
一种用于辅助诊断免疫性眼病的眼内液细胞因子的检测方法,选用Human IL-1βFlex Set、Human IL-2 Flex Set、Human IL-5 Flex Set、Human IL-6 Flex Set、HumanTNF Flex Set、Human IFN-γ Flex Set,Human VEGF Flex Set、Human IL-17a Flex Set和Human TGF-β1 Single Plex Flex Set检测试剂。
除TGF-β1外的细胞因子检测方法包括如下步骤:
眼内液样本制备:取房水,并于10000rpm下离心10min,得到眼内液样本;
微球溶液制备:取除TGF-β1外的每种微球试剂0.25 μl并混合,添加微球稀释液10.5μl,混合均匀,得到微球溶液;
抗体溶液制备:取除TGF-β1外的每种抗体试剂0.25 μl并混合,添加抗体稀释液10.5μl,混合均匀,得到抗体溶液;
样本孵育:将微球溶液装到空白流式管,每管12.5μl,向流式管内加入20μl眼内液样本,混匀,室温孵育1h,得到预混抗体;
抗体孵育:向流式管内加入12.5μl抗体溶液,混合均匀,室温孵育2h,得到待测样本;
洗涤:向流式管内加入1ml清洗液,以2200rpm的转速离心5min,去除上清液,再加入300μl清洗液,重悬;

TGF-β1的检测方法包括如下步骤:
眼内液样本制备:取房水,并于10000rpm下离心10min,得到眼内液样本;
样本酸化:取20μl眼内液样本和4μl 1N HCL混合,在室温下孵育10min,再加入4μl1.2N NAOH/0.5M HEPES中和,得到酸化样本;
微球溶液制备:取微球试剂0.25 μl,添加微球稀释液12.25μl,混合均匀,得到微球溶液;
抗体溶液制备:取抗体试剂0.25 μl,添加抗体稀释液12.25μl,混合均匀,得到抗体溶液;
样本孵育:将微球溶液装到空白流式管,每管12.5μl,向流式管内加入20μl酸化样本,混匀,室温孵育2h,得到预混抗体;
抗体孵育:向流式管内加入12.5μl抗体溶液,混合均匀,室温孵育2h,得到待测样本;
洗涤:向流式管内加入1ml清洗液,以2200rpm的转速离心5min,去除上清液,再加入300μl清洗液,重悬;
检测:通过流式细胞仪对待测样本进行荧光检测,对照标准曲线,得到各细胞因子含量。
实施例3:
一种用于辅助诊断血管性眼病的眼内液细胞因子的检测方法,选用Human IL-10Flex Set、Human IL-6 Flex Set、Human IL-8 Flex Set、Human TNF Flex Set、HumanVEGF Flex Set、Human Soluble CD54 (ICAM-1) Flex Set、Human Soluble CD106 (VCAM-1) Flex Set、 Human TGF-β1 Single Plex Flex Set检测试剂。
除TGF-β1外的细胞因子检测方法包括如下步骤:
眼内液样本制备:取房水,并于10000rpm下离心10min,得到眼内液样本;
微球溶液制备:取除TGF-β1外的每种微球试剂0.25 μl并混合,添加微球稀释液10.75μl,混合均匀,得到微球溶液;
抗体溶液制备:取除TGF-β1外的每种抗体试剂0.25 μl并混合,添加抗体稀释液10.75μl,混合均匀,得到抗体溶液;
样本孵育:将微球溶液装到空白流式管,每管12.5μl,向流式管内加入20μl眼内液样本,混匀,室温孵育1h,得到预混抗体;
抗体孵育:向流式管内加入12.5μl抗体溶液,混合均匀,室温孵育2h,得到待测样本;
洗涤:向流式管内加入1ml清洗液,以2200rpm的转速离心5min,去除上清液,再加入300μl清洗液,重悬;

根据细胞因子含量变化给出的提示信息,医生结合具体病情分析最终诊断为急性视网膜坏死综合征。
应用例2:
应用实施例2的检测方法对患者B进行检测,辅助诊断免疫性眼病。
检测结果如图2所示,从左至右依次为FSC/SSC图,APC/APC-Cy7图,PE/SSC图,上面三个图关于除TGF-β1外的细胞因子,下面三个图关于TGF-β1。所得荧光强度如表3所示,对照标准曲线后所得的细胞因子含量如表4所示。

根据细胞因子含量变化给出的提示信息,医生结合具体病情分析最终诊断为糖尿病性视网膜病变。
应用例4:
参照实施例1的检测方法,对受试者的IL-5、IL-6、IL-8、IL-17A、MCP-1、VEGF、VCAM-1、ICAM-1和bFGF的细胞因子组合进行检测。
检测结果如图4所示,从左至右依次为FSC/SSC图,APC/APC-Cy7图,PE/SSC图。经检测所得荧光强度如表7所示,对照标准曲线后所得的细胞因子含量如表8所示。

1.一种眼内液细胞因子检测方法,其特征在于:选用检测试剂并采用流式细胞法对眼内液细胞因子进行检测,得到各细胞因子含量;
所述检测试剂包括检测眼内液中的Eotaxin、IL-10、IL-12p70、IL-1β、IL-6、IL-8、MCP-1、TNF、IFN-γ和VEGF的试剂;
对于非肿瘤患者且未经抗生素治疗时,若患者IL-1β、IL-6、IL-8均显著升高,提示可能细菌感染;若IL-6>160pg/ml且IL-10>50pg/ml,提示革兰氏阴性菌感染;若IL-6>46pg/ml,IL-10<50pg/ml,可能提示革兰氏阳性菌感染;若IFN-γ升高<100pg/ml,并且其他细胞因子也有升高,提示病毒感染可能;如果单纯IL-10升高10倍以上,并且IFN-γ也有升高,可能提示嗜血;
若IFN-γ明显下降,IL-6明显上升,IL-10和TNF-α升高,与结核感染相关;若TNF含量显著升高,IL-6含量显著升高,与弓形虫感染相关;

步骤一:将眼内液样本和微球溶液混合均匀,孵育得到预混样本;
步骤二:将预混样本和抗体溶液混合均匀,孵育完成后洗涤,得到待测样本;
步骤三:通过流式细胞仪对待测样本进行荧光检测,对照标准曲线,得到各细胞因子含量。
4.根据权利要求3所述的一种眼内液细胞因子检测方法,其特征在于:所述微球溶液内含有多种不同的捕获微球,不同的捕获微球分别包被有不同的捕获抗体并具有不同的荧光强度,不同的捕获抗体对应于不同的细胞因子。
