In the related art, in a large-scale operating room, the difficulty of the operation scheduling task is very large, and a large amount of manpower is required to complete the operation scheduling task. Based on this, the embodiment of the invention provides a surgical scheduling method, a surgical scheduling device and electronic equipment, and the technology can be applied to all surgical scheduling scenes on a certain day of a hospital.
In order to facilitate understanding of the embodiment of the present invention, a detailed description is first provided for a surgical scheduling method provided in the embodiment of the present invention, where the method is applied to an electronic device, and the electronic device may be a mobile terminal, such as a mobile phone or a tablet computer, or a computer. The electronic equipment is provided with a surgery scheduling system, and surgery information is stored in the surgery scheduling system; as shown in fig. 1, the method comprises the steps of:
step S102, a plurality of operation applications on the current date and the scheduling information of the operating room nurse on the current date are obtained.
The current date may be understood as the current day, and the plurality of surgical applications on the current date are all surgical applications on the current day, and each surgical application includes: information such as operating room, operating name, and patient information; wherein, some surgical applications also include: one or more of operation grading, estimated operation duration and surgeon information. The operating room nurse scheduling information of the current date includes a plurality of nurses on duty on the day, and the name and the on-duty time of each nurse, which may also be understood as the shift, for example, a white shift or a night shift.
In a specific implementation, a plurality of operation applications of a current date can be acquired from a preset Hospital Information System (HIS for short), which generally refers to an Information System that comprehensively manages the flows of people, logistics and financial resources of a Hospital and all departments to which the Hospital belongs by using modern means such as computer software and hardware technology, network communication technology and the like, collects, stores, processes, extracts, transmits and summarizes data generated at each stage of medical activities, and processes the data to form various Information, thereby providing comprehensive automatic management and various services for the overall operation of the Hospital.
The scheduling information of the operating room nurse on the current date can be acquired from an operating room department management system, or can be acquired from an excel table used for scheduling of operating room personnel.
And step S104, aiming at each operation application, distributing corresponding operation rooms for the operation application based on the operation room information.
In a specific implementation, for any one of a plurality of operation applications, the operation application is assigned to the same operation room as the department to which the operation application belongs, and the operation room is the operation room included in the operation information. Specifically, the surgical suite information includes the department to which each surgical suite belongs and/or the surgical type information supported by the surgical suite, and one surgical suite may belong to one department or may belong to more than one department for common use; one surgical suite may support one type of surgery or more than one type of surgery. Such departments include orthopedic, cardiac, vascular, neurosurgical, gynecological, pediatric, cardiological, thoracic, urological, and general departments, among others. For example, the surgical application is "left leg meniscal repair" or "hip replacement", which is automatically distributed to the orthopaedic operating room.
And S106, aiming at each operation application, allocating operation room nurses and operation time for the operation application based on the estimated operation duration of the operation application and the scheduling information of the operation room nurses.
During specific implementation, aiming at one of a plurality of operation applications, the operation duration of the operation application is estimated according to the operation duration of the operation type corresponding to the operation application in the past, and the estimated operation duration is determined as the preset operation duration of the operation application; or the operation duration marked in the operation application is used as the preset operation duration.
The operation time corresponding to each operation application can be determined according to the estimated operation duration of each operation application, then the operation time corresponding to each operation application and the scheduling information of the nurses in the operation room are allocated to the operation application, the nurses in the operation room are allocated, the preset principle is usually required to be followed when the nurses in the operation room are allocated, the preset principle can be set according to the user requirements, for example, the post allocated to the nurses corresponds to the post on duty of the nurses, the allocated time corresponds to the post on duty, and the like.
And step S108, generating an operation schedule of the current date according to the operation room, the operation time and the operation nurse corresponding to each operation application.
Information such as an operation room, operation time and operation nurses corresponding to each operation application is integrated, and an operation schedule of the current date can be automatically generated. On the basis, in order to improve the rationality of the operation schedule, the scheduling staff can also finely adjust the operation schedule according to the actual situation to complete the final schedule.
Firstly, acquiring a plurality of operation applications on the current date and scheduling information of operating room nurses on the current date; for each operation application, distributing a corresponding operation room for the operation application based on the operation room information; aiming at each operation application, allocating operation room nurses and operation time for the operation application based on the estimated operation duration of the operation application and the scheduling information of the operation room nurses; and generating a surgery schedule of the current date according to the surgery room, the surgery time and the surgery nurse corresponding to each surgery application. The method actively matches operating room resources through operation application, and the operation scheduling mode better meets the actual scheduling requirement because the operating room resources are fixed and the daily operation application requirement is dynamic; meanwhile, the scheduling process is informationized by the method, the operation scheduling speed is accelerated, and the operation scheduling quality is also ensured.
The embodiment of the invention also provides another operation scheduling method, which is realized on the basis of the method of the embodiment; the method mainly describes a specific process of allocating operating room nurses and operating time for each operation application based on the estimated operation duration and the scheduling information of the operating room nurses of the operation application (realized by the following steps S206-S210); as shown in fig. 2, the method comprises the steps of:
step S202, a plurality of operation applications on the current date and the scheduling information of the operating room nurse on the current date are acquired.
And step S204, aiming at the department to which each operation application belongs, distributing corresponding operation rooms for the operation applications based on the operation room information.
Step S206, aiming at each operation application, determining the estimated operation duration of the current operation application according to the historical operation statistical duration of the same operation type of the current operation application or the estimated operation duration marked in the current operation application.
In some embodiments, the estimated operation duration of the current operation application can be determined according to the operation duration of the operation with the same operation type as the current operation application in the historical operation information; the scheduling personnel can also manually estimate the operation duration according to the actual condition of the operation; the estimated operation duration of the operation application can be determined according to the estimated operation duration marked in the operation application. Any one of the plurality of surgical applications is required to be used as a current surgical application to execute the operations of the steps S206-S210, so as to obtain the surgical time and the operating room nurse corresponding to each of the plurality of surgical applications.
In a specific implementation, the historical operation information may be stored in the operation scheduling system, and the historical operation information includes the operation performed in each operation room at the historical time, and the operation duration, the operation type, the operation name, the operation patient, the operation nurse, and other information corresponding to each operation. And inquiring the operation with the same type as the operation applied by the current operation in the historical operation information, and estimating the estimated operation time length applied by the current operation according to the inquired average time length or the appointed time length of the operation.
And S208, determining the operation time of the current operation application based on the estimated operation time.
And step S210, according to the operation time of the current operation application, determining the operation room nurses meeting the preset conditions from the operation room nurse scheduling information.
The preset condition may be set according to a user requirement, for example, the preset condition may be that a post allocated to a nurse is consistent with a current duty of the nurse, and the allocated time is consistent with the on-duty time. In specific implementation, the scheduling information of the operating room nurses comprises a plurality of nurses, the name and the on-duty time of each nurse; the preset conditions include: the surgery time of the surgery application is matched with the on-duty time of the nurse in the surgery room, and each nurse works in only one surgery room on the current date; based on this, the above step S210 can be realized by the following steps 10 to 13:
and step 10, inquiring the nurse to be selected, which is matched with the operation time applied for the current operation, in the on-duty time of the nurse in the operation room nurse scheduling information. The number of the selected nurses is usually plural.
Step 11, judging whether a target nurse allocated to the surgical room to which the current surgical application belongs exists in the nurses to be selected; if so, executing step 12; if not, step 13 is performed.
And step 12, determining the target nurse as the operating room nurse of the current operation application.
If the target nurse allocated to the surgical room to which the current surgical application belongs exists in the nurses to be selected, the surgical room to which the current surgical application belongs is subjected to the surgical operation before the surgical time of the current surgical application, the target nurse of the surgical room is on duty, and the on duty time of the target nurse is consistent with the surgical time of the current surgical application, at this time, the target nurse can be determined as the surgical room nurse of the current surgical application.
And step 13, randomly drawing a specified number of nurses from the nurses to be selected as the nurses in the operating room for the current operation application.
If the target nurse which is distributed to the operation room to which the current operation application belongs does not exist in the nurses to be selected, a specified number of nurses can be randomly selected from the nurses to be selected as the operation room nurses to which the current operation application belongs. The specified number can be set according to the operation requirement, for example, it can be set to 2 or 3, etc.
Step S212, acquiring the special requirement information of the operation period corresponding to each operation application; wherein the special requirement information during the operation comprises one or more of intraoperative blood transfusion, high-value consumables, special medical instruments, intraoperative rapid pathology or special medical equipment.
In a specific implementation, the above-mentioned intra-operative special requirement information may be inputted into the operative application by the operator in advance.
Step S214, aiming at each operation application, generating a remark item corresponding to the operation application according to the special requirement information during the operation.
The remarks made above are intended to be construed as specific requirements for each surgical application.
And S216, generating a final operation schedule according to the remark item, the operation room, the operation time and the operation nurse corresponding to each operation application.
In the operation scheduling method, the operation scheduling logic conforms to the actual scheduling rule, conforms to the thinking mode of the scheduling person, is suitable for informationizing the scheduling process, converts various strategies and limiting conditions originally existing in the brain of the scheduling person into the information processing process, can greatly accelerate the speed of operation scheduling, and can also ensure the quality of operation scheduling.
The embodiment of the invention also provides another operation scheduling method, which is realized on the basis of the method of the embodiment; the method mainly describes that an operation nurse post library is stored in an operation scheduling system, the operation nurse post library comprises post types of each nurse, and the post types comprise a circulating nurse and a hand washing nurse; under the condition that each operation application corresponds to one patrol nurse and one hand washing nurse, aiming at each operation application, based on the estimated operation duration and the scheduling information of the operation room nurses of the operation application, distributing the operation room nurses and the operation time for the operation application (realized by the following steps S306-S318); as shown in fig. 3, the method comprises the steps of:
step S302, a plurality of operation applications on the current date and the scheduling information of the operating room nurse on the current date are acquired.
And step S304, aiming at the department to which each operation application belongs, distributing corresponding operation rooms for the operation applications based on the operation room information.
Step S306, aiming at each operation application, determining the estimated operation duration of the current operation application according to the historical operation statistical duration of the same operation type of the current operation application or the estimated operation duration marked in the current operation application.
And step S308, determining the operation time of the current operation application based on the estimated operation time.
Step S310, inquiring the nurse to be selected, which is matched with the operation time applied for the current operation, in the on-duty time of the nurse in the operation room nurse scheduling information.
Step S312, based on the post types stored in the operation nurse post library, determining the post type of each nurse to be selected.
Step S314, judging whether a circulating nurse and a hand washing nurse which are distributed to the operation room to which the current operation application belongs exist in the nurses to be selected; if so, step S316 is performed, otherwise, step S318 is performed.
Step S316, determining the circulating nurse and the hand washing nurse as the hand washing nurse and the circulating nurse corresponding to the current operation application; step S320 is performed.
If the circulating nurse and the hand washing nurse which are allocated to the operation room to which the current operation application belongs exist in the nurses to be selected, the operation room to which the current operation application belongs is described to have the operation before the operation time of the current operation application, the circulating nurse and the hand washing nurse of the operation room are still on duty, and the on duty time of the circulating nurse and the hand washing nurse is consistent with the operation time of the current operation application, at this time, the circulating nurse and the hand washing nurse can be determined as the circulating nurse and the hand washing nurse of the current operation application.
Step S318, randomly drawing a circulating nurse from the nurses to be selected as the circulating nurse applied for the current operation; randomly selecting one hand washing nurse from the nurses to be selected as the hand washing nurse applied for the current operation; step S320 is performed.
Step S320, generating an operation schedule of the current date according to the operation room, the operation time and the operation nurse corresponding to each operation application.
In some embodiments, a nurse seniority repository is also stored in the surgery scheduling system; wherein, the nurse qualification library stores the working years (low-grade and high-grade) of each nurse, the job title (nurse, supervisor, assistant main-task nurse, main-task nurse and the like), the competent post type, the emergency experience and the operation adaptability; the surgical application also includes a surgical grade; based on the estimated operation duration of the operation application and the scheduling information of the operating room nurses, the method for allocating the operating room nurses to the operation application comprises the following steps: and allocating operating room nurses for the operation application by combining the estimated operation duration and operation grade of the operation application, the scheduling information of the operating room nurses and the nurse qualification library.
In a specific implementation, the surgical suite information, the surgical nurse station library, the nurse qualification library and the like stored in the surgical scheduling system can be manually updated according to actual situations, for example, the surgical suite information is updated in response to an update instruction for the surgical suite information; alternatively, the nurse seniority repository is updated in response to an update instruction for the nurse seniority repository.
In order to facilitate understanding of the embodiment of the invention, a surgical scheduling method is described in detail by taking an example that 40 surgical applications are received in 1 operating room with 10 operating rooms on day 1, and the surgical scheduling method is firstly distributed to the corresponding operating rooms according to the classification of the departments to which the surgical applications belong; further estimating the operation time length according to the previous operation time length; and then, extracting lists of the circulating nurse and the hand washing nurse based on the operation nurse post library and the scheduling information of the operating room nurses, wherein the extraction nurse principle is that the allocated post is consistent with the current post, the allocated time is consistent with the on-post time, and the circulating nurse and the hand washing nurse can only be in 1 operating room all day and the like. After the 40 operations are completed, the special requirement information in the operation period in the operation application is compared with a material information base, a nurse qualification base and the like, and remark items of the operation schedule are directly generated. Meanwhile, the scheduling person can finely adjust the scheduling table according to the actual situation to complete the final scheduling.
The material information base can be stored in a surgery scheduling system, and the material information base comprises high-value consumables, instrument bags, blood bags and the like.
In this embodiment, the purpose of the operation schedule is to reasonably allocate the resources of people, materials and materials in the operating room to the operating applicant. The proper scheduling strategy is to decompose the resources such as people, materials and the like in the operating room into a module library according to indexes, and when the resources are matched, the modules are matched one by one until the requirements are all met, and the resources are not mutually exclusive: a nurse may not be scheduled to go back to a nurse's office or a nurse's office for hand washing at the same time, a specialty operating room may not be scheduled for other non-specialty operations, etc.
In the implementation, there are various changing factors in the implementation process of the surgical schedule to adjust the generated surgical schedule: 1) when the operation time exceeds the scheduled time (corresponding to the time indicated in the operation schedule), the processing procedure is as follows: in the process of performing the operation according to the operation schedule, if the operation time of the current operation reaches the operation time indicated by the operation schedule, the end time of the current operation is not received, and the delay of the current operation is determined; performing sequential delay on the operation time of the current operation after the operation in the operation schedule; and sending the message of the surgical time delay to related personnel of the surgery after the current surgery or sending the message of the surgical time delay in a form of issuing notice.
It can also be understood that: when the end time of a table operation in a scheduling plan is up and the operation scheduling system does not receive the end time of the table operation, the delay of the table operation is automatically judged, the time of the table connection and the time of the subsequent operation are delayed in real time, and meanwhile, the operation scheduling system pops up an information frame in a striking mode, and the prompting mode can comprise the following two processing schemes:
a. and (3) publishing the announcement: the operation scheduling time changes, and the related personnel can see the notice when logging in the operation scheduling system.
b. And (3) notification push: the operation time change information (message equivalent to the operation time delay) is pushed to the relevant personnel of the operation after the current operation (the relevant personnel comprises an operating doctor, an anesthetist, an operating room nurse, a patient family member and the like).
2) The treatment process of the surgical cancellation comprises the following steps: in the process of performing the operation according to the operation schedule, if a cancel instruction aiming at the target operation is received, inquiring whether the operation which is the same as the department to which the target operation belongs is supplemented in a pop-up window mode; when there is an augmented operation, operation information of the augmented operation is received, and operation schedule information of operations other than the target operation in the operation schedule is kept unchanged. When there is no supplementary surgery, adjusting the surgical time of the surgery following the target surgery in the surgical schedule forward; and pushing the message of the forward adjustment of the operation time to the related personnel corresponding to the operation of which the operation time is adjusted forward.
In the specific implementation, when the scheduled operation cannot meet the operation implementation conditions due to the change of the patient's state of illness and the like or the operation needs to be cancelled due to doctors, nurses or other conditions in the operating room, the administrator manually cancels the corresponding operation for a plurality of times, and the operation scheduling system pops up an information frame after detecting that the operation is cancelled, so as to provide a processing scheme for the administrator. Firstly, inquiring whether to cancel the operation and whether other operation supplements exist in the corresponding department, if so, filling the operation information of the supplemented operation, and keeping the information of other operations in the operation schedule unchanged. If not, the subsequent operation is advanced, and the time information of the subsequent operation is adjusted. And then the information of the change of the operation starting time is pushed to the related personnel of the corresponding operation.
3) Treatment procedure for planned surgical insertion: the operation scheduling system responds to the operation insertion instruction and inquires whether a free operation room exists currently; if yes, popping up a free surgical room list; responding to a selected instruction aiming at a specified operation room in the free operation room list, and distributing the operation to be inserted to the specified operation room; and adjusting the operation time of the operation to be inserted after the operation in the operation schedule, and sending the operation time adjusted operation message to the related personnel of the operation to be inserted after the operation or sending the operation time adjusted message in a form of announcement. If no free operation room exists, taking the current time as a reference, and listing an operation room list according to the sequence of the operation time of the next operation from less to more away from the current time; allocating the surgery to be inserted to the selected surgery room in response to the selected instruction aiming at the selected surgery room in the surgery room list; and adjusting the operation time of the operation to be inserted after the operation in the operation schedule, and sending the operation time adjusted operation message to the related personnel of the operation to be inserted after the operation or sending the operation time adjusted message in a form of announcement.
In the specific implementation, when the scheduled operation has the insertion requirement of the planned operation, the scheduling adjustment is carried out by adopting the priority principle. The operation steps are as follows:
the first step is as follows: after the administrator clicks the 'inserting' operation, the operation scheduling system automatically judges whether a free operation room exists currently (the operation is not scheduled or the operation of the previous time is finished, and the operation of the next time is not started), if so, a free operation room list is popped up, and the administrator manually judges which operation room the inserted operation is arranged in; if no free operating room exists, the system automatically moves to the second step.
The second step is that: the operation scheduling system takes the current time as a reference, lists an operation room list according to the sequence of the operation time of the next operation from less to more from the current time, and determines the operation room to which the inserted operation is arranged by the administrator according to the actual operation requirement.
The third step: because the operation that leads to receiving the delay of opening a stage influences, the system adopts two kinds of schemes to handle simultaneously: 1. and (3) publishing the announcement: the operation scheduling time changes, and the related personnel can see the announcement when logging in the scheduling system; 2. and (3) notification push: and pushing the operation time change information to related personnel at the next time.
Specifically, at the time of the insertion surgery, there are the following three cases: 1) if the inserted surgery room has a schedule originally, the nurse continues the scheduling without the need of adjusting staff. 2) If the inserted surgical suite was not scheduled, an additional nurse is required to go to the office, and the scheduled nurse prefers the second-line nurse in the scheduling list. 3) If the inserted operation is not qualified by the nurse who is on duty originally, the nurse needs to be arranged on duty additionally, and the system provides a list of information of the improper nurses by annual fund for the selection of the manager.
In some embodiments, the operation scheduling system further stores an anesthesiologist seniority library; wherein, the qualification library of the anesthesiologist stores the information of the qualification, the title and the like of each anesthesiologist; in the retrieved operation applications, part of the operation applications contain operation grade information and/or anesthesiologist remark information; according to the remark information and/or the operation grade information of the anesthetist, matching the anesthetist on the current day and shift, and preferentially assigning the operation with higher operation grade to the anesthetist with high seniority.
In some embodiments, the surgical scheduling system may further provide a scheduling strategy with the length of the surgical time as a key, including the following steps: 1) according to the historical operation record, the operation class with little change of the operation duration is scheduled preferentially; 2) a last row of simple surgeries in which the surgery time is less than a first specified duration (e.g., 1 hour), a slot schedule; 3) complex surgery with a surgery time greater than a second specified time length (e.g., 3 hours) is preferentially scheduled in the morning; 4) the type of operation should be ranked as the last infection type. The first designation attempt and the second designation duration may be set to specific values according to user requirements.
According to the operation scheduling method, the basic rule of operation scheduling is established by analyzing the requirements of various operations on resources such as people, materials and materials in an operating room, and a manager can be helped to complete all operation scheduling of 1 day quickly, reasonably and accurately.
