Abstract
本发明涉及一种利用乙肝肝硬化患者和正常人外周血内γδT细胞的数量和表型分布差异评估外周血的免疫情况并利用此数据提供乙肝肝硬化发病情况信息的方法,其特征包括:(A)在外周血免疫细胞中分析γδT细胞占淋巴细胞(lymphocytes)比例和该细胞绝对数量的阶段;(B)检测外周血γδT细胞激活程度,杀伤能力等,并分析乙肝肝硬化患者和正常人的外周血内免疫细胞分布情况的阶段;(C)在乙肝肝硬化和正常人当中,为判定肝硬化发病情况,而在患者及正常人两个组群中辨别其有意义的标记结果值显示出统计学上的显著性和差异的组合的阶段;以及(D)分析乙肝肝硬化患者外周γδT细胞比例与疾病进展阶段相关性。

技术领域
本发明涉及一种方法,通过测量和诊断乙肝肝硬化患者和正常人的外周血内γδT细胞的数量和表型分布差异,找出两个群组之间显示出明显差距的标记,分析乙肝肝硬化患者外周血γδT细胞比例和乙型肝炎肝硬化发病进程的相关性,并提供利用外周血鉴定乙型肝炎肝硬化发病进程的相关标准。
背景技术
乙型肝炎病毒(HBV)感染是多种肝病的主要危险因素,乙型肝炎肝硬化是乙型肝炎肝硬化发展的结果。肝硬化必须依靠病理诊断、超声、腹腔镜检和CT等检查,但是这些检测手段在疾病发生早期的诊断中作用有限。近年来,随着免疫治疗的的快速发展,免疫诊断作为一种新兴的诊断技术,也成为帮助临床查找病因、了解病情进展的重要方法。
越来越多的证据表明,乙型肝炎肝硬化患者的肝脏中发生免疫失调。这些变化包括肝脏中炎性环境的恶化,表现为浸润的非病毒特异性白细胞以及偏斜的细胞因子和趋化因子谱。炎性白细胞浸润肝脏在确定HBV发病和发展中起重要作用。肝脏是具有大量固有免疫细胞的器官,包括天然杀伤(NK)细胞,天然杀伤T(NKT)和γδT细胞。γδT细胞是一种重要的免疫细胞类型,根据T细胞受体链的表达可分为Vδ1和Vδ2γδT细胞两种亚型。γδT细胞亚群可直接识别在肿瘤细胞或病原体感染细胞表面表达的分子,而不是由专业抗原呈递细胞加工的肽。通常,γδT细胞已被赋予针对造血和实体瘤以及微生物感染的保护作用。有关人类病毒感染中γδT细胞的可用数据最初是从病毒感染研究中获得的,认为γδT细胞反应可实现初始控制和控制,有效发展适应性免疫反应。
除肝脏内部,血样里也包括几乎大部分的免疫细胞群及其亚型免疫细胞,研究外周血γδT细胞免疫活化和迁移状态对揭示CHB发病机制和进程具有重要意义。CD38是一种表达在早期T细胞表面的糖蛋白,成熟以后消失,但是T细胞活化后又重新出现;静止的T淋巴细胞一般不表达CD69,在受到某些刺激后T淋巴细胞表面就合成和表达一些新的糖蛋白,其中最早表达的是CD69。CD38和CD69作为T细胞免疫活化的标志,在许多急性或慢性感染中均会表达升高,如人类免疫缺陷病毒(HIV)、EB病毒(EBV)等感染。同时,淋巴细胞(包括T细胞)的迁移不仅取决于各种粘附分子的联合作用,还受趋化因子与其受体之间相互作用的控制。细胞表面的趋化因子受体CCRs、CXCRs与相应配体结合后能够促进细胞的迁移、浸润。循环中的T细胞响应趋化因子的活化而迁移,这些趋化因子在激活正常T细胞表达和分泌,如CXCR 3和CXCR4。
因此本发明针对外周血免疫细胞中γδT细胞的数量和表型分布差异,其中包括在外周血免疫细胞中分析γδT细胞占淋巴细胞比例和该细胞绝对数量,外周血γδT细胞激活程度,迁移能力,与正常人的外周血中γδT细胞数量和表型分布做对比,将显示出统计学上的显著性和差异的组合进行标记,分析出乙肝肝硬化患者外周γδT细胞与乙肝肝硬化疾病进程的相关性,建立利用外周血鉴定乙型肝炎肝硬化发病进程的相关标准。
发明内容
本发明的目的在于,为诊断乙肝肝硬化的发病情况,在外周血的γδT细胞当中找出乙肝肝硬化患者较正常人具有差异的标记。从而诊断个体免疫活性,提供相应的免疫治疗方法。
本发明涉及一种利用乙型肝炎肝硬化患者和正常人外周血内免疫细胞的分布差异评估外周血的免疫力并利用此数据提供乙型肝炎肝硬化发病情况信息的方法,其特征在于,包括:(A)分析正常人以及乙型肝炎肝硬化患者外周血免疫细胞中γδT细胞不同表型的细胞频率的变化;(B)分析正常人以及乙型肝炎肝硬化患者的外周血内γδT细胞表面活化因子表达情况;(C)分析正常人以及乙型肝炎肝硬化患者的外周血内γδT细胞表面趋化因子表达情况;(D)在正常人以及乙型肝炎肝硬化患者当中,为判定乙型肝炎肝硬化发病,而在正常人以及乙型肝炎肝硬化患者两个组群中辨别其有意义的标记结果值显示出统计学上的显著性和差异的组合的阶段,诊断乙型肝炎肝硬化的发病情况的阶段。
根据本发明,可选择血样内含有的免疫细胞中有意义的标记,利用流式细胞计分析上述标记,并通过算法可诊断出乙型肝炎肝硬化免疫力。出于预防目的,比起接受精密体检,上述检查方法不仅非常简单快捷,且节省相当的费用,还可提供充分高的准确性。与此同时,较病理诊断、超声、腹腔镜检和CT等检查,可实现高效的早期诊断,对提高乙型肝炎肝硬化患者生存率和生存质量,提高乙型肝炎肝硬化治愈率,降低死亡率具有重要意义。

具体实施方式
下面结合具体实施例对本发明作进一步的说明,但本发明不限于此。
实施例1:
1)细胞分离:将获取的外周血离心1500rpm,10min离血浆后,用等量PBS缓冲液充分混匀剩余的静脉血轻轻加样于淋巴细胞分离液之上,2500rpm,离心20min;小心取出离心管,吸取单个核细胞层使用PBS重悬,离心2000rpm,离心15min;弃上清;最后所得细胞重悬于1ml培养基或是PBS中计数备用。
2)以107细胞/10μl抗体的量向分离好的PBMC悬液中加入足量抗体,吹匀后染色15min后向细胞悬液中以107细胞/2ml比例加入缓冲液,吹匀离心弃上清。
3)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者(LC为三种疾病进程的平均数)外周血中γδT细胞频率,以γδT细胞频率统计学上的显著性和差异,诊断乙型肝炎肝硬化的发病情况的阶段(图1)。在多例健康人的外周血中γδT细胞平均可达9.87±1.38%,在乙型肝炎肝硬化患者外周血中γδT细胞平均可达6.72±0.66%(P<0.001),分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞分别可达6.05±0.85%(P<0.001),4.99±0.88%(P<0.001),10.264±1.54%。由此分析可得当外周血中γδT细胞在6.72±0.66%范围内时,表明该样本极大可能为乙型肝炎肝硬化患者;与此同时,当外周血中γδT细胞下降至6.05±0.85%和4.99±0.88%范围内,表明该样本极大可能步入为Child A、B乙型肝炎肝硬化进程。
实施例2:
1)细胞分离和染色步骤同实施案例1,此处不再赞述。
2)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞中的Vδ1和Vδ2T细胞亚群表型细胞频率,以Vδ1和Vδ2T细胞频率统计学上的显著性和差异,诊断乙型肝炎肝硬化的发病情况的阶段(图2,3)。在多例健康人的外周血中Vδ1细胞平均可达0.74±0.31%,而在乙型肝炎肝硬化患者外周血中γδT细胞平均可达1.40±0.16%,分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中Vδ1细胞分别可达1.26±0.16%,1.08±0.25%,2.08±0.46%。但都未表现出显著性差异;除此之外,在多例健康人(HC)的外周血中Vδ2细胞平均可达8.15±1.54%,在乙型肝炎肝硬化患者外周血中Vδ2细胞平均可达5.11±1.45%(P<0.001),分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞分别可达4.58±0.85%(P<0.001),3.91±0.83%(P<0.001),7.68±1.06%。由此分析可得当外周血中Vδ2细胞在5.11±1.45%范围内时,表明该样本极大可能为乙型肝炎肝硬化患者;与此同时,当外周血中Vδ2细胞上升至4.58±0.85%和3.91±0.83%范围内,表明该样本极大可能步入为Child A、B乙型肝炎肝硬化进程。

2)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞中的CD38+γδT、CD69+γδT细胞频率,以CD38+γδT、CD69+γδT细胞频率统计学上的显著性和差异,诊断乙型肝炎肝硬化的发病情况的阶段(图4,5)。在多例健康人的外周血中CD38+γδT细胞平均可达2±0.27%,而在乙型肝炎肝硬化患者外周血中CD38+γδT细胞平均可达7.54±0.42%(P<0.01),分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中CD38+γδT细胞分别可达5.62±0.18%(P<0.01),3.55±0.46%,13.44±0.41%(P<0.01);除此之外,在多例健康人的外周血中CD69+γδT细胞平均可达3.6±0.93%,在乙型肝炎肝硬化患者外周血中CD69+γδT细胞平均可达4.79±0.77%,分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中CD69+γδT细胞分别可达6.38±0.65%(P<0.05),3.64±0.82%,4.17±0.56%。由此分析可得当外周血中CD38+γδT细胞在7.54±0.42%范围内时,表明该样本较大可能为乙型肝炎肝硬化患者;与此同时,当外周血中CD38+γδT细胞上升至5.62±0.18%和13.44±0.41%范围内,表明该样本极大可能步入为Child A、C乙型肝炎肝硬化进程;CD69+γδT在6.38±0.65%范围内时,表明该样本可能为乙型肝炎肝硬化患者,该范围也可辅助CD38+γδT细胞对疾病的Child A进程进行诊断。
实施例4:

2)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞中的CXCR3+γδT、CXCR4+γδT细胞频率,以CXCR3+γδT、CXCR4+γδT细胞频率统计学上的显著性和差异,诊断乙型肝炎肝硬化的发病情况的阶段(图6,7)。在多例健康人的外周血中CXCR3+γδT细胞平均可达90.75±0.89%,而在乙型肝炎肝硬化患者外周血中CXCR3+γδT细胞平均可达62.68±3.36%(P<0.001),分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中CXCR3+γδT细胞分别可达62.44±4.47%(P<0.001),63.31±7.89%(P<0.001),62.30±6.33%(P<0.001);除此之外,在多例健康人的外周血中CXCR4+γδT细胞平均可达86.87±1.71%,在乙型肝炎肝硬化患者外周血中CXCR4γδT细胞平均可达61.49±3.23%(P<0.001),分别在10例以上的Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中CXCR4+γδT细胞分别可达61.64±5.17%(P<0.001),64.66±6.47%(P<0.001),56.91±5.43%(P<0.001)。由此分析可得当外周血中CXCR3γδT和CXCR4+γδT细胞分别在62.68±3.36%和61.49±3.23%范围内时,表明该样本极大可能为乙型肝炎肝硬化患者;与此同时,当外周血中CXCR3+γδT细胞上升至62.44±4.47%,63.31±7.89%和62.30±6.33%范围内,表明该样本极大可能步入为Child A、B、C乙型肝炎肝硬化进程;CXCR4+γδT分别在61.64±5.17%,64.66±6.47%和56.91±5.43%范围内时,也可辅助CXCR3+γδT细胞对疾病的三个进程进行诊断。
实施例5:

2)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞、Vδ1、Vδ2表型细胞频率,以这三种细胞频率统计学上的置信区间的重叠范围,诊断乙型肝炎肝硬化。结果分析得,当γδT细胞占外周血T细胞的比例<9.87%(P<0.001),Vδ2表型细胞占外周血T细胞的比例<7.68%(P<0.001),Vδ1表型细胞占外周血T细胞的比例>1.08%时,我们认为该样本属于乙型肝炎肝硬化患者(P<1×10-6)。

2)利用流式细胞术分析检测健康人(HC)及A、B、C三种疾病乙型肝炎肝硬化患者外周血中γδT细胞、Vδ1、Vδ2、CD38+γδT、CD69+γδT表型细胞频率,以这五种细胞频率统计学上的置信区间的重叠范围,诊断乙型肝炎肝硬化的发病。结果分析得,当γδT细胞占外周血T细胞的比例<9.87%(P<0.001),Vδ2表型细胞占外周血T细胞的比例<7.68%(P<0.001),Vδ1表型细胞占外周血T细胞的比例>1.08%,CD38+γδT占外周血T细胞的比例>3.55%(P<0.01)时,我们认为该样本属于乙型肝炎肝硬化患者(P<1×10-8)。

2)利用流式细胞术分析检测健康人(HC)及Child A、B、C三种疾病进程乙型肝炎肝硬化患者外周血中γδT细胞、Vδ1、Vδ2、CD38+γδT、CD69+γδT、CXCR3+γδT、CXCR4+γδT细胞频率,以这七种细胞频率统计学上的置信区间的重叠范围,诊断乙型肝炎肝硬化的发病情况。结果分析得,当γδT细胞占外周血T细胞的比例<9.87%(P<0.001),Vδ2表型细胞占外周血T细胞的比例<7.68%(P<0.001),CXCR3+γδT占外周血T细胞的比例<63.3%(P<0.001),CXCR4+γδT占外周血T细胞的比例<64.7%(P<0.001),CD38+γδT占外周血T细胞的比例>3.55%(P<0.01)时,我们认为该样本属于乙型肝炎肝硬化患者(P<1×10-14)。
本发明将乙型肝炎肝硬化患者作为事例,利用其外周血免疫力诊断出正常人(HC)、Child A、B、C三种疾病进程。但从方法论上,不受限于乙型肝炎肝硬化,当然可以适用于其他炎症。同时,利用免疫细胞标记得出的置信区间,可在今后开发更多标记后,提高其敏感度和特异度来实现乙型肝炎肝硬化诊断适用性上的最大化。除本发明揭示的诊断细胞模型项目外,还可通过新的组合得出分析模型,并适用于乙型肝炎肝硬化诊断。以上,说明了本发明的具体构成因素等特定内容,以及有限的实施例及示意图,但上述说明只是为提高对本发明的整体理解,而本发明不受限于上述实施例,在本发明所属的技术领域拥有通常知识的技术人员应可进行各种修改和变形。因此,本发明的精神不局限与上述实施例,不仅前述的专利权利所求项,而且与上述权利所求项范围同等或等价的变形等,均应包含在本发明的保护范围内。
Claims (14)
1.一种利用乙型肝炎肝硬化患者和正常人外周血内免疫细胞的分布差异评估外周血的免疫力并利用此数据提供乙型肝炎肝硬化发病情况信息的方法,其特征在于,包括:
(A)在外周血免疫细胞中分析γδ、Vδ1、Vδ2、CD38+γδ、CD69+γδ、CXCR3+γδ、CXCR4+γδT细胞的频率来鉴定乙型肝炎肝硬化发展阶段,并分析乙型肝炎肝硬化患者和正常人的外周血内免疫细胞分布情况的阶段;
(B)在乙型肝炎肝硬化患者和正常人当中,为判定乙型肝炎肝硬化发病情况,而在乙型肝炎肝硬化患者及正常人两个组群中辨别其有意义的标记结果值显示出统计学上的显著性和差异的组合的阶段;
(C)利用上述标记检测单位血液中的免疫力,诊断乙型肝炎肝硬化患者的发病情况的阶段以及患病的机率。
2.根据权利要求1所述的方法,其特征在于,在上述(A)阶段,利用流式细胞计完成γδ、Vδ1、Vδ2、CD38+γδ、CD69+γδ、CXCR3+γδ、CXCR4+γδT细胞的频率分析。
3.根据权利要求1所述的方法,其特征在于,依据免疫细胞显现的标记,分析对应各标记的免疫细胞显型的细胞频率。
4.根据权利要求1所述的方法,其特征在于,在上述(B)阶段,根据可区分乙型肝炎肝硬化患者和正常人的有意义的标记组合,通过统计学方法计算出P值。
5.根据权利要求1所述的方法,其特征在于,以细胞频率为准,外周血中γδT细胞中显示的比例范围满足一定标准。
6.根据权利要求1所述的方法,其特征在于,以细胞频率为准,外周血中Vδ1、Vδ2Τ细胞中显示的比例范围满足一定标准。
7.根据权利要求1所述的方法,其特征在于,以细胞频率为准,外周血中CD38+γδT、CD69+γδT细胞中显示的比例范围满足一定标准。
8.根据权利要求1所述的方法,其特征在于,以细胞频率为准,外周血中CXCR3+γδT、CXCR4+γδT细胞中显示的比例范围满足一定标准。
9.根据权利要求1所述的方法,其特征在于,以细胞频率为准,在上述(C)阶段,γδ细胞、Vδ1、Vδ2Τ细胞中显示的上述P值满足一定标准。
10.根据权利要求1所述的方法,其特征在于,以细胞频率为准,在上述(C)阶段,γδ、Vδ1、Vδ2、CD38+γδ、CD69+γδT细胞中显示的上述P值满足一定标准。
11.根据权利要求1所述的方法,其特征在于,以细胞频率为准,在上述(C)阶段,γδ、Vδ1、Vδ2、CXCR3+γδ和CXCR4+γδT细胞中显示的上述P值满足一定标准。
12.根据权利要求1所述的方法,其特征在于,以细胞频率为准,在上述(C)阶段,CD38+γδT、CD69+γδT、CXCR+3γδT、CXCR4+γδT细胞中显示的上述P值满足一定标准。
13.根据权利要求1所述的方法,其特征在于,以细胞频率为准,在上述(C)阶段,γδ、Vδ1、Vδ2、CD38+γδ、CD69+γδ、CXCR3+γδ和CXCR4+γδT细胞中显示的上述P值满足一定标准。
14.根据权利要求1至13任一项所述的方法提供乙型肝炎肝硬化发病情况信息的诊断工具。
