Background
Extracorporeal membrane oxygenation (ECMO) is an extracorporeal life support technology, can replace cardiopulmonary function, provides stable circulating blood volume for patients with cardiopulmonary failure, timely and effectively recovers blood supply and oxygen supply of important organs such as heart, brain and the like, and gains time and opportunity for the treatment of critically ill patients. The ECMO system mainly comprises an oxygenator (membrane lung), a centrifugal pump and a pipeline, wherein the pipeline is used as a bridge between the oxygenator and the centrifugal pump and is connected with a patient, specifically, an arteriovenous cannula is connected with a blood vessel of the patient to lead venous blood of the patient out of the body, the venous blood is changed into arterial blood after gas exchange (carbon dioxide discharge and oxygen absorption) is completed by the oxygenator, and then the arterial blood is sent into an arterial system of the patient through the arterial cannula, so that stable circulating blood volume is provided for tissue organs of the patient suffering from cardiopulmonary failure.
Continuous Renal Replacement Therapy (CRRT) refers to blood purification for 24 hours each day to replace the function of the damaged kidney, and has strong blood purification effects by drawing the blood out of the body, removing some metabolic wastes or toxic substances in the blood through the diffusion, convection and adsorption of a filter membrane, and drawing the blood back into the body.
At present, many diseases, such as viral pneumonia caused by novel coronavirus COVID-19, are not only cardiopulmonary dysfunction, but also accompanied with kidney injury, in such a situation, the purpose of rescuing patients is difficult to achieve by using ECMO or CRRT alone, and ECMO and CRRT are usually used simultaneously, but if two blood diversion passages are established for ECMO and CRRT, two groups of wound are required to be opened on the patients; and a larger dose of anticoagulation is required during the use of CRRT, so this approach increases patient pain and increases the risk of infection, mechanical injury and difficulty of anticoagulation management. Another concept is to connect the CRRT device with the ECMO device, but it should be noted that the ECMO sends blood into the body to maintain the blood flow of the tissues and organs of the body, and needs to provide great power, so the blood flow rate and pressure in the blood line of the ECMO are high; in normal hemodialysis, the blood flow rate is often slow, because too fast a circuit blood flow update may lead to a blood filter clearing transition on the one hand, and too high a flow pressure may cause a machine arterial pressure to alarm continuously on the other hand. The current conventional method is to add a flow restrictor, but the direct access of the flow restrictor is prone to turbulence, which carries potential hemolysis and thrombus risk.
Disclosure of Invention
In order to overcome the defects existing in the process of using ECMO equipment and CRRT equipment in the prior art, the invention provides a pipeline adapter suitable for a blood purification device in an ECMO period, which can divide blood in an ECMO drainage tube in proportion and respectively convey the blood to an oxygenator and a CRRT filter of ECMO according to different flow rates so as to realize the functions of oxygenating the blood of a patient and purifying the blood.
In order to achieve the purpose, the invention adopts the following technical scheme:

one end of the main pipe is connected to the ECMO drainage pipe, and the other end of the main pipe is provided with a shunt valve;
the two branch pipes are respectively a first branch pipe and a second branch pipe, and the first branch pipe and the second branch pipe are respectively connected to the flow dividing valve;
the first branch pipe is provided with a first peristaltic pump, the outlet of the first branch pipe is connected to the ECMO oxygenator, the second branch pipe is provided with a second peristaltic pump, and the outlet of the second branch pipe is connected to the CRRT filter;
and the blood to be oxygenated and purified is led out from the ECMO drainage tube and then respectively enters the first branch tube and the second branch tube through the header tube, and respectively enters the ECMO oxygenator and the CRRT filter under the driving of the first peristaltic pump and the second peristaltic pump.
Preferably, the ratio of the flow rates of the first branch pipe and the second branch pipe is 8: 2.
compared with the prior art, the pipeline adapter provided by the invention has the advantages that the first branch pipe and the second branch pipe are respectively matched with the requirements of the flow rate and the pressure of the pipeline through the organic combination of the flow dividing valve and the first peristaltic pump and the second peristaltic pump, particularly, the second branch pipe does not cause high-pressure alarm due to too large flow rate, a separate flow limiter is not needed for changing the flow rate, and the risk of hemolysis and thrombosis is reduced.
The ECMO equipment and the CRRT equipment are connected by adopting the pipeline adapter provided by the invention, so that the two equipment do not need to be respectively provided with pipes, namely, the two blood diversion passages which are not mutually influenced are prevented from being respectively formed on the body of a patient to construct, the pain of the patient is reduced, and the risks of infection and mechanical injury are reduced.

In the invention, the blood led out from the ECMO drainage tube is divided by the pipeline adapter according to a proportion, and one strand of blood passes through the CRRT filter 40 to realize the purification effect on the blood; the other blood flow passes through an ECMO oxygenator 30 to complete blood oxygenation, carbon dioxide in the blood is exchanged, and fresh oxygen is dissolved; finally, the blood is converged into the body of the patient. Specifically, the shunt valve 11 disposed at the end of the manifold 10 shunts the blood into the first branch tube 21 and the second branch tube 22, and the first peristaltic pump 211 and the second peristaltic pump 221 are used to deliver the blood to the ECMO oxygenator 30 and the CRRT filter 40 according to a certain flow ratio, thereby achieving oxygenation and purification of the blood.
Further, according to the present invention, the ratio of the flow rates of the first branch pipe 21 and the second branch pipe 22 is 8: 2. the first peristaltic pump and the second peristaltic pump provided by the invention are not sensitive to pressure, have very high precision and can accurately control the blood flow of the first branch pipe and the second branch pipe; the first and second peristaltic pumps are used as power sources of the pipeline adapter, and strictly control the blood flow of the first and second branch pipes, so that the blood flow of the second branch pipe 22 flowing into the CRRT filter 40 can be effectively controlled, and the pressure can be obviously reduced, so as to meet the filtering requirements of the CRRT filter 40.
