Abstract

Description
技术领域
本披露总体上涉及眼睛的屈光治疗、更具体地涉及通过将材料打印到靶区上的眼睛屈光治疗。
背景技术
眼睛的屈光治疗是指为改善视力而改变眼睛的屈光特性以减少屈光不正所进行的手术。当眼睛的形状没有正确弯曲光线时屈光不正发生,从而导致图像模糊。屈光不正的主要类型是近视(近视眼)、远视(远视眼)、老花眼(近视力随年龄增长而丧失)、以及散光。典型的屈光治疗包括激光原位角膜磨削术(LASIK)、准分子激光屈光性角膜切削术(PRK)、放射状角膜切开术(RK)、散光性角膜切开术(AK)、自动板层角膜移植术(ALK)、激光角膜热成形术(LTK)、传导性角膜成形术(CK)、以及角膜内环(Intacs)。
发明内容

图1展示了用于眼睛12a的屈光治疗的系统10的实例。系统10包括打印机14,该打印机根据被设计成为眼睛12a提供屈光治疗的图案16来将材料(例如,生物或生物相容性材料)打印到靶区12的打印区域上。在一些情况下,靶区12可以是眼睛12a,打印机14将材料直接打印到眼睛12a上,以进行屈光治疗。在一些情况下,靶区12可以是植入物基材12b。当用材料打印植入物基材12b时,它产生待植入眼睛12a中以用于屈光治疗的植入物。

在一些实施例中,靶区12是眼睛12a,比如人或其他动物的眼睛。在其他实施例中,靶区12是眼睛植入物的植入物基材12b。眼睛植入物是通过手术植入眼睛12a中以改善视力的人工辅助物。植入物基材12b是可以将材料打印到其上以形成植入物的基材。植入物基材12b可以包括任何合适的透明生物相容性材料,例如,透明质酸(又称为玻璃酸)或胶原蛋白。植入物基材12b可以具有任何合适的尺寸和形状。例如,植入物基材12b可以是圆形或环形的,其直径在0.5毫米至12毫米(mm)的范围内,或者在比如0.5mm至5mm、5mm至8mm、和/或8mm至12mm等子范围内。靶区12的打印区域可以是在其上打印材料的区域。
为了有助于描述,本说明书使用了眼科手术中常用的坐标系。在这个坐标系中,在眼睛上操作的激光束限定了z轴,而xy平面垂直于z轴。总体上,xy平面与眼睛的瞳孔、尖端或顶点所限定的平面重合。

激光器22使眼睛12a准备好进行屈光治疗。激光器22可以是任何合适的激光手术装置,该激光手术装置产生并发射与眼睛12a的角膜相互作用(例如,光致破裂或光切除)的激光束。激光手术装置典型地包括产生激光束的激光源(例如,飞秒或准分子)、以及引导激光束聚焦至靶区的特定点的扫描部件(例如,光学器件)。激光器22通过以任何合适的方式与角膜相互作用来使眼睛12a准备好进行治疗。例如,激光器22可以通过在角膜中创建膜瓣或去除眼睛12a的全部或部分上皮组织(例如,光治疗性角膜切削术(PTK))来使眼睛12a的角膜的打印区域暴露。作为另一个实例,激光器22可以在角膜内制作切口(例如,凹部)以接纳植入物。作为另一个实例,激光器22可以进行治疗的后续步骤。例如,激光器22可以使角膜或所打印的材料成形为产生处方屈光特性,或者执行其他操作以完成治疗。在某些实施例中,激光器22可以包含产生不同激光束的不同激光源,例如,激光器22可以具有产生使角膜或所打印的材料光致破裂的光束、以及切除角膜或所打印的材料的光束的源。
打印机14将材料打印到靶区12的打印区域上,并且可以包括任何合适的、被配置成根据数字指令将材料沉积到打印区域上的打印机。例如,打印机14可以是3D、或增材制造打印机,该打印机沉积连续的材料层以产生以特定形状和尺寸配置的材料。打印机14包括打印机头44和打印机控制器46。打印机头44将材料引导至打印区域,并且可以是任何合适的、将材料沉积到表面上的打印机挤出机。打印机控制器46使打印机头沿x、y、z方向移动,以将材料引导至打印区域的特定位置,并且可以接收计算机20的指令,以根据图案16移动打印机头44。
材料包括任何合适的生物和/或生物相容性的透明或半透明材料。这种材料的实例包括培养的胶原蛋白材料、人或动物细胞材料、生物相容性塑料、或透明质酸。在某些情况下,也可以使用上皮组织可以在其上生长的材料。这种材料可以为角膜细胞和细胞外材料提供最佳营养,在使用期内具有光学透明性,并具有支持上皮组织生长的表面特性。
眼球跟踪器24跟踪靶区12的移动,以帮助在靶区12上精确打印材料。眼球跟踪器24可以跟踪眼睛12a,因为靶区眼睛12a比靶区植入物基材12b更可能移动和需要跟踪。然而,眼球跟踪器24可以用于跟踪任何类型的靶区12。眼球跟踪器检测眼睛12a的平移移动和/或角度(或旋转)移动。在某些实施例中,图像处理用于定位眼睛12a的中心点,例如瞳孔,以确定平移移动。图像处理用于定位眼睛12a的特征(例如血管、虹膜特征或任何其他适当的特征),以确定角度移动。
当眼球跟踪器24检测到靶区12的移动时,眼球跟踪器24通知计算机20,该计算机调整对打印机控制器46的指令,以对靶区12的移动进行补偿。例如,如果靶区12平移和/或旋转特定量,则打印机控制器46通过平移和/或旋转图案16那个特定量来对移动进行补偿。
在其他实施例中,接口可以将靶区12固定或保持在希望的地点和位置,使得可以不需要眼球跟踪器24。例如,患者接口可以通过使用例如吸力来将眼睛12a保持在位。
固化照明器30用使材料固化的光照射打印区域。光可以通过促进材料和可选的眼睛角膜的交联来使材料固化。固化光的实例包括紫外光或400nm与500nm之间的光(比如LED光)。
分配器32在手术过程中将液体沉积到靶区12上。例如,分配器32将角膜交联溶液引导至眼睛12a,该角膜交联溶液促进眼睛12a的角膜交联。角膜交联溶液的实例包括核黄素溶液或其他合适的溶液。作为另一个实例,分配器32将粘合剂引导至打印区域,该粘合剂促进材料粘合到打印区域上。粘合剂可以包括纤维蛋白。
相机28生成打印区域的图像,以监视材料的打印。相机28可以包括任何合适的、可以生成物体图像的系统。相机28的实例包括OCT系统(比如时域或频域OCT系统),该系统生成可用于创建打印区域图像的OCT扫描。
图2展示了通过将材料打印到靶区12上来对眼睛12a进行屈光治疗的方法的实例,该方法可以由图1的系统10执行。该方法在步骤110开始,其中激光器22使眼睛12a准备好进行屈光治疗。取决于手术,激光器22可以:在眼睛12a的角膜中创建膜瓣来为LASIK手术做好准备;在角膜中创建被设计成用于接纳角膜植入物的凹部;或者去除眼睛12a的上皮组织来为PRK手术做好准备。
在步骤112,准备靶区12的打印区域。靶区12可以是眼睛12a本身或待植入眼睛12a中的植入物的植入物基材12b。可以以任何合适的方式准备打印区域。例如,分配器32可以将粘合剂引导至打印区域,该粘合剂促进材料粘合到打印区域上。在步骤114,相机28生成打印区域的图像(比如OCT扫描图像),以监视材料的打印。
在步骤116,打印机14根据被设计成为眼睛12a提供屈光治疗的图案16来将材料打印到打印区域上。材料可以是任何合适的生物和/或生物相容性的透明材料。注意的是,如果靶区12是植入物基材12b,步骤116可以在手术之前执行,使得步骤116发生在步骤110之前。
在步骤117,眼球跟踪器24跟踪靶区12的移动。在步骤118,可以检测移动。如果检测到移动,计算机20接收描述靶区12的移动的信息,并指导打印机14对移动进行补偿。然后,该方法移动至步骤122。如果没有检测到移动,该方法直接移动至步骤122。在使用患者接口将靶区12固定在位的实施例中,该方法通常不执行步骤117、118和122。
在步骤122,固化照明器用使材料固化的光照射打印区域。步骤124取决于手术以及靶区12是眼睛12a还是植入物基材12b。如果靶区12是植入物基材12b,该方法移动至步骤126,其中植入物插入眼睛12a中。然后,该方法移至步骤128。如果靶区12是眼睛12a,该方法直接移动至步骤128。
在步骤128,分配器32将角膜交联溶液引导至眼睛12a,该角膜交联溶液促进角膜交联。在步骤130,完成手术,该步骤取决于手术。例如,在LASIK手术中,完成手术可以包括闭合膜瓣。然后,所述方法结束。该方法结束之后,愈合过程开始。例如,如果该方法去除上皮组织,则其在所打印的植入物上重新生长。
图3至图6展示了植入物和沉积的材料,这些植入物和沉积的材料可以使用指导创建3D形状的图案16来创建。图案16可以存储为3D可打印文件,比如出自由3D系统创建的立体光刻CAD软件的STL文件格式。
图3展示了系统10创建的用于矫正远视的植入物54的实例。在该实例中,眼睛12a具有角膜50和上皮组织52。植入物54的形状可以类似于接触透镜的形状,并且插入角膜50的凹部56中。
图4展示了通过系统10沉积的用于矫正远视的材料60的实例。在该实例中,部分上皮组织52被去除。系统10以矫正远视的形状将材料60沉积到角膜50的暴露的基质上。沉积的材料60可以具有沉积在基质上的薄层形状。上皮组织52在材料60上生长。
图5展示了通过系统10沉积的用于矫正散光和/或提高生物力学稳定性(例如,圆锥形角膜)的材料60的实例。在该实例中,部分上皮组织52被去除。系统10以矫正散光和/或提高生物力学稳定性的形状将材料60沉积到角膜50的暴露的基质上。沉积的材料60可以具有布置在基质上的圆顶形状。上皮组织52在材料60上生长。
图6展示了通过系统10沉积的用于矫正近视的材料60的实例。在该实例中,部分上皮组织52被去除。系统10以矫正近视的形状将材料60沉积到角膜50的暴露的基质上。沉积的材料60可以具有沉积在基质上的环形圈形状。上皮组织52在材料60上生长。

Claims (20)

调整对该打印机控制器的指令,以对该靶区的移动进行补偿。
