本发明公开了一种多通道经脐单孔妇科腹腔镜手术穿刺装置,包括:套筒和穿刺套,套筒由半斜面喇叭状内套管与喇叭状外套管连接形成一体,外套管的大端头设置有外固定环,外套管上端一侧设置有两个固定孔,外套管上端另一侧外壁设置有弯形挡板,弯形挡板的端头设置有方向调节孔,穿刺套由弹性圈、封闭膜及多个穿刺孔构成,弹性圈设置在封闭膜的下端,穿刺孔设置在封闭膜的上端,弹性圈套在外固定环下,使套筒与穿刺套形成一个整体。具有结构简单、操作方便、排烟效果好、降低手术难度、价格较低、经济实用、便于推广等特点。

Description

多通道经脐单孔妇科腹腔镜手术穿刺装置
技术领域
本发明涉及一种妇科腹腔镜手术辅助医疗器械,具体是一种多通道经脐单孔妇科腹腔镜手术穿刺装置。
背景技术
在妇科腹腔镜已广泛开展的今天,经脐单孔妇科腹腔镜手术具有减少和隐藏手术瘢痕、减轻术后疼痛、促进康复等优点。目前专用于经脐单孔腹腔镜手术穿刺装置(统称为Port),对单孔妇科腹腔镜手术的开展和推广起到了非常积极的作用。然而医疗改革的方向是控费降耗,减轻患者的医疗负担。2019年9月1号开始对耗材严厉控制,国家医保局开始按病种定额拨款试点,并逐渐在全国推广。近期举办的《中国医师协会妇产科分会单孔及VNOTES区域会议启动会》上,部分专家分析开展此类手术例数不多的原因之一,就是一次性Port价格昂贵,渴望能有价格低廉的替代产品诞生。目前,国内外同行都在不断探索,寻求新的替代器械。另外经脐部切口的方向与人体纵轴方向垂直,和手术操作部位(如子宫附件)成一定角度,一次性Port不能改变脐部切口的角度,手术操作时器械需向前下方倾斜,不能有效利用切口面积,操作时所有器械挤在一起,给手术增加一定的难度。其次术中产生的烟雾在盆腹腔弥散后经一次性Port脐孔外的通气管道排出,不能直接将排烟管放在手术操作部位附近,排烟效果不佳,术中常因烟雾影响视野,被迫短暂停止手术操作。
发明内容
本发明的目的是提供一种结构简单、价格低廉、能够增强排烟效果、改变脐孔切口角度、降低手术难度、容易操作、经济实用、便于推广的多通道经脐单孔妇科腹腔镜手术穿刺装置。
为了实现上述目的,本发明包括:套筒和穿刺套,套筒由半斜面喇叭状内套管与喇叭状外套管连接形成一体,外套管的大端头设置有外固定环,外套管上端一侧设置有两个固定孔,外套管上端另一侧外壁设置有弯形挡板,弯形挡板的端头设置有方向调节孔,穿刺套由弹性圈、封闭膜及多个穿刺孔构成,弹性圈设置在封闭膜的下端,穿刺孔设置在封闭膜的上端,弹性圈套在外固定环下,使套筒与穿刺套形成一个整体。
这样,在经脐单孔妇科腹腔镜手术时,切开脐孔进入腹腔,在脐部两侧用布巾钳提起腹壁组织,将套筒的内套管的长端与切口方向一致,垂直进入脐部切口,调节套筒方向旋转推进,使外套管的挡板置于腹壁切口外下方,用丝线将切口上方筋膜通过固定孔缝扎固定,下压外套管,在对侧腹壁对挡板的向上支撑双重作用下,使套管向前下方倾斜,使套管的纵轴方向和手术器械的方向保持一致,有效利用脐部切口面积,甚至可以缩小脐部的切口长度,减少创伤。由于内、外套管的上下开口均为喇叭状,手术操作时增加了手术器械的活动空间,降低了手术操作难度。如果套筒的倾斜度不能满足手术要求,可经调节孔穿入丝线缝在上腹壁两侧组织牵拉调节固定。另外根据手术需要向左或右侧倾斜时,只需牵拉对侧缝线,放松同侧缝线进行调节。如果需做上腹部手术时,将挡板和固定孔的位置进行调换即可。最后将膜状穿刺套的弹性圈套在外固定环下,用丝线结扎固定防止滑脱,经穿刺孔分别插入多个手术用套管(Trocar)及排烟管(排烟管可采用带控制开关的一端剪有小侧孔的输血器管代替),结扎暂未用的穿刺孔,实施手术。由于封闭膜具有一定的弹性和活动空间,便于手术操作时器械的摆动,腹腔充气后封闭膜膨胀又具有一定压力,手术用套管相对固定,方便手术器械进入腹腔。另外根据手术的需要,将排烟管放在手术操作部位附近,便于烟雾的排出,保持术野清晰。具有结构简单,价格低廉,能够增强排烟效果,改变脐孔切口的角度,降低手术难度,容易操作,经济实用,便于推广。

具体实施方式
由图1、图2可知,本发明包括:套筒和穿刺套,套筒由半斜面喇叭状内套管1与喇叭状外套管2连接形成一体,外套管的大端头设置有外固定环3,外套管上端一侧设置有两个固定孔4用于缝扎固定,外套管上端另一侧外壁设置有弯形挡板5,弯形挡板的端头设置有方向调节孔6。穿刺套由弹性圈7、封闭膜8及多个穿刺孔9构成,弹性圈设置在封闭膜的下端,穿刺孔设置在封闭膜的上端,弹性圈套在外固定环下,使穿刺套与套筒形成一个整体。在做手术时,将弹性圈套在外固定环下,使穿刺套与套筒形成一个整体。套管可采用卫生级的金属制品加工制作,也可采用卫生级的一次性塑料加工制作。穿刺套采用卫生级乳胶或橡胶材质制成,使其具有弹性变形性能,满足手术需要。本发明的规格可根据切口及手术需要制成系列产品。

1.多通道经脐单孔妇科腹腔镜手术穿刺装置,包括:套筒和穿刺套,其特征是套筒由半斜面喇叭状内套管(1)与喇叭状外套管(2)连接形成一体,外套管的大端头设置有外固定环(3),外套管上端一侧设置有两个固定孔(4),外套管上端另一侧外壁设置有弯形挡板(5),弯形挡板的端头设置有方向调节孔(6),穿刺套由弹性圈(7)、封闭膜(8)及多个穿刺孔(9)构成,弹性圈设置在封闭膜的下端,穿刺孔设置在封闭膜的上端,弹性圈(7)套在外固定环(3)下,使套筒与穿刺套形成一个整体。
