Abstract
The invention relates to a ureter soft lens diagnosis and treatment method and a device integrating a guide sheath. The flexible ureteroscope handle and the tail end fixing part of the guide sheath can be combined into a whole, and the insertion operation of the guide sheath can be conveniently realized by pushing the flexible ureteroscope handle. The cystoscope is not needed to be additionally used in the operation process, the operation steps are simplified, the operation time is saved, the operation cost is reduced, and the operation risk is reduced.
Description
Technical Field
The invention relates to medical equipment, in particular to a flexible ureteroscope diagnosis and treatment method integrated with a guide sheath and an integrated medical device for realizing flexible ureteroscope diagnosis and treatment by adopting the method.
Background
The ureter soft lens is usually used in urinary tract and renal examination and urinary tract lithotripsy operation, and is a thin lens with a diameter of about 3mm, which enters into the kidney through the urethra, bladder and ureter, and the ureter calculus or kidney calculus is broken up and taken out by using a laser fiber or other tools in a working channel of the ureter soft lens. It utilizes the natural cavity of human urinary system, does not make any incision on the body, and is a urinary surgery endoscopic non-invasive or minimally invasive operation. Compared with open surgery, ureteroscope lithotripsy has the advantages of small injury, light pain, quick recovery and the like. Soft-lens surgery, however, requires a high level of manipulation by the surgeon.
The existing ureter soft lens needs a plurality of steps to finish when entering a urinary system:
step 2: using a cystoscope to find an inlet of a left ureter or a right ureter in a bladder cavity, and leading a guide wire to enter the left ureter or the right ureter through a working channel of the cystoscope;
step 4, extracting the guide wire and the guide sheath inner core;
step 5, extending the flexible ureteroscope out of the head of the guide sheath along the inner cavity of the guide sheath to enter the ureter and the kidney;
step 6, using a ureter soft lens to search and determine the position of the calculus, and extending a laser fiber into a working channel of the ureter soft lens to perform calculus breaking operation or extending a calculus taking basket into the working channel to perform calculus taking operation; repeating the step 4 and the step 5 until the steps are finished;
after the operation is finished, the flexible ureteroscope is withdrawn from the human body along the guide sheath, and then the guide sheath is withdrawn from the human body.
In the above-mentioned operation, an additional cystoscope is needed in order to guide the wire to find the entrance of the left ureter or the entrance of the right ureter. Thus, the cost and time of using the cystoscope is increased. However, if a cystoscope is not used, the time for finding the entrance of the left ureter and the right ureter is increased during the insertion process of the guide wire, and the risk that the guide wire punctures the inner wall of the urinary system is increased.
Disclosure of Invention
The technical problem to be solved by the invention is as follows: the flexible ureteroscope diagnosis and treatment method integrated with the guide sheath is provided, the flexible ureteroscope can be used only without being guided by an additional cystoscope, the setting work before the flexible ureteroscope operation can be completed, the operation process is accelerated, the operation time is saved, and the operation cost of a patient is reduced.
In addition, the invention also provides a ureter soft lens device integrated with the guide sheath, which adopts the diagnosis and treatment method.
The invention adopts the technical scheme for solving the problems as follows: a ureter soft lens diagnosis and treatment method integrated with a guide sheath is characterized in that:
the method comprises the following steps:
a preparation stage:
step 1: the flexible ureteroscope insertion tube is integrated in the guide sheath;
step 2: the tail end of the detachable hard tube is fixed with the control handle of the flexible ureteroscope, and the head of the detachable hard tube is locked with the fixing part at the tail end of the guide sheath to form a cooperating whole;
and 3, step 3: guiding and placing a guide wire in a working channel of the ureter soft lens;
and (3) operation stage:
and 4, step 4: a doctor enters a guide wire and a guide sheath into the urethra of a patient under the monitoring of a flexible ureteroscope, enters a bladder along the urethra, finds an inlet of a left ureter or an inlet of a right ureter and enters the left ureter or the right ureter;
and 5, step 5: the doctor takes down the detachable hard tube, releases the guiding sheath and keeps the guiding sheath in the urinary system to form a passage from the urethra to the left or right ureter;
and 6, step 6: the guide wire is drawn out from the working channel of the ureter soft lens and exits from the human body;
and 7, step 7: the position of the calculus is searched and determined by using a ureter soft lens, and the calculus is broken by extending a working channel of the ureter soft lens into a laser fiber or extending into a calculus extraction basket to perform calculus extraction.
The flexible ureteroscope integrated with the guide sheath by adopting the method is characterized in that: it comprises a guide sheath, a fixing part, a detachable hard tube and a ureter soft lens; the fixing part is arranged at the tail part of the guide sheath; the detachable hard tube tail is provided with a conical sleeve hole, and the conical sleeve hole is fixedly connected with a handle of the ureter soft lens.
The detachable hard tube comprises an upper guide sheath connector and a lower guide sheath connector which can be mutually buckled.
An elastic buckle is arranged on the upper guide sheath connector and can be matched with a bayonet in the fixing part.
Compared with the prior art, the invention adopts the mode of combining the guide sheath with the soft ureteroscope to replace the original method of respectively inserting the cystoscope, the guide wire, the guide sheath and the soft ureteroscope into the urinary system, directly and conveniently leads the guide sheath fixedly connected with the soft ureteroscope into the urethra, the bladder and the ureter at one time without using the cystoscope, simplifies the operation steps, saves the operation time, reduces the operation cost and reduces the operation risk.
The detachable hard tube can combine the flexible ureteroscope handle and the tail end fixing part of the guide sheath into a whole in the process of inserting the guide sheath into the urethra, and the insertion operation of the guide sheath can be conveniently realized by pushing the flexible ureteroscope handle. When the soft ureteroscope is required to further go deep into a ureter from a bladder, and the like, the detachable tube is directly separated, and when the fixed part is positioned at the entrance of the urethra, the soft ureteroscope can obtain a proper flexible pushing distance by pushing the handle of the soft ureteroscope, so that the deep inspection of the soft ureteroscope is facilitated.

The present invention is described in further detail below with reference to the attached drawing figures.
The invention directly adopts the mode of combining the guide sheath with the soft ureteroscope insertion tube, utilizes the soft ureteroscope insertion tube to replace a guide sheath inner core in the traditional soft ureteroscope technology, does not need to use an additional cystoscope guide-in guide wire and a guide sheath when in use, and can directly finish the process of entering the urethra, the bladder and the ureter by the guide wire, the soft ureteroscope and the guide sheath.
The method for enabling the ureter soft lens to enter the kidney comprises the following steps:
a preparation stage:
step 1: the flexible ureteroscope insertion tube is integrated in a guide sheath (the sheath does not need an inner core);
step 2: fixing the tail end of the detachable hard tube and the control handle of the flexible ureteroscope, and locking the head of the detachable hard tube and the fixing part at the tail end of the guide sheath to form a cooperating whole;
3, guiding a guide wire into a working channel of the ureter soft lens and placing the guide wire in the working channel;

and 7, searching and determining the position of the calculus by using a ureter soft lens, and extending a laser fiber into a working channel of the ureter soft lens to perform calculus crushing operation or extending a calculus extraction basket into the working channel to perform calculus extraction operation. The operation is carried out under the monitoring of the ureter soft lens in the whole process of entering the urethra and the bladder and entering the ureter and the kidney from the bladder without using an additional cystoscope, so that the judgment of a doctor is facilitated, and the damage to the inner wall tissue of a urinary system is avoided.
The diagnosis and treatment equipment adopting the diagnosis and treatment method comprises a guide sheath, a fixing part, a detachable hard tube and a flexible ureteroscope.
The tail part of the guide sheath is provided with a fixed part, and a bayonet is arranged in the fixed part.
The detachable hard tube comprises an upper guide sheath connector and a lower guide sheath connector which can be mutually buckled. After the guide sheath is completely inserted and placed, a protective channel from the urethral orifice of a patient to the ureter can be established, and the insertion tube containing the ureter soft lens can not generate direct friction and damage to the inner wall in the process of extending into the urethra, the bladder, the ureter and the kidney for multiple times.
After the detachable hard tube is detached, the insertion tube originally fixed between the tail of the guide sheath and the ureter soft lens handle is released, and the ureter soft lens insertion tube can be continuously pushed to further extend into the ureter and the kidney.
The invention adopts a method of integrating the guide sheath and the ureter soft lens into a whole to replace the original method of respectively inserting the cystoscope, the guide wire, the guide sheath and the ureter soft lens into the urinary system, and can better utilize the ureter soft lens to guide the guide wire and the guide sheath to enter the bladder and the ureter along the urethra; through setting up the hard tube of dismantling, connect into a whole with guide sheath and soft mirror control handle, the doctor can hold the handle and directly carry out the operation of inserting of guide wire, guide sheath, and it need not to use the cystoscope in addition, has simplified operating procedure, has practiced thrift operation time, has reduced the operation cost, has reduced the operation risk.

1. A ureter soft lens diagnosis and treatment method integrated with a guide sheath is characterized by comprising the following steps:

step 1: the flexible ureteroscope insertion tube is integrated in the guide sheath;
step 2: the tail end of the detachable hard tube is fixed with the control handle of the flexible ureteroscope, and the head of the detachable hard tube is locked with the fixing part at the tail end of the guide sheath to form a cooperating whole;

A flexible ureteroscope incorporating an integrated guide sheath for use with the method of claim 1, wherein: the flexible ureteroscope integrated with the guide sheath comprises the guide sheath, a fixing part, a detachable hard tube and a flexible ureteroscope; the fixing part is arranged at the tail part of the guide sheath.
3. The flexible ureteroscope integrated with a guide sheath according to claim 2, wherein: the detachable hard tube is formed by buckling an upper guide sheath connector and a lower guide sheath connector.
4. The flexible ureteroscope integrated with a guide sheath according to claim 3, wherein: and the upper guide sheath connector is provided with an elastic buckle which is matched with a bayonet in the fixing part.
