Abstract
本发明涉及医药技术领域,尤其涉及并公开了一种表面活性蛋白‑D在制备治疗角膜炎药物中的新用途,包括用于促进金黄色葡萄球菌的聚集,辅助中性粒细胞杀灭金黄色葡萄球菌,治疗角膜炎。本发明有以下有益效果:SP‑D可促进金葡菌的聚集并被眼部中性粒细胞杀灭,同时保护角膜上皮组织,减少金葡菌对上皮的损伤,从而在金葡菌角膜炎时,调动和补充眼部的固有免疫防御机制,使眼部金葡菌被更快清除,局部细菌总量明显下降,并通过减少角膜上皮组织的损伤,有效增加角膜的防御功能,促进炎症的控制和消褪,SP‑D非常适用于作为金葡菌角膜炎的辅助治疗药物。

背景技术
细菌性角膜炎是临床常见的眼表面疾病,据2018年WHO的报告,角膜盲是发展中国家第二位的致盲性眼病,而金黄色葡萄球菌引起的感染性角膜炎是当前发病率和致盲率最高的角膜病之一。在浙江省的感染性角膜炎病人中,列首位是金黄色葡萄球菌感染引起的,占总数的18.6 %。而在发达国家,据美国眼科学会报告的2018年全美国3万例感染性角膜炎中,由金黄色葡萄球菌感染引起的亦位列第一,占总数的11.3%(www.aao.org/ppp)。此外,金黄色葡萄球菌的耐药性问题十分严重和棘手,这导致其引起的角膜炎治愈率明显下降,致盲率显著提高,因此对金黄色葡萄球菌性角膜炎的深入研究就显得尤为重要。
近年来,有关金黄色葡萄球菌性角膜炎治疗的研究中,对金葡菌的致病因素、治疗策略方面,以及眼表面固有免疫系统,防御细菌感染的作用机制方面取得了较大的进展和突破,并逐渐成为研究的热点。其中,眼表面固有免疫系统,作为眼部最重要的防御力量,在金葡菌性角膜炎的发生发展中的地位尤其受到重视。目前,对眼表面固有免疫因素防御金黄色葡萄球菌感染的机制的认识,主要包括以下两方面,⑴ 免疫分子与细菌的相互作用:结膜中的巨噬细胞、中性粒细胞,角膜中的树突状细胞,泪液中的分泌型免疫球蛋白A(IgA)、防御素、乳铁蛋白等因子,通过与细菌的相互作用,发挥免疫防御作用。⑵ 眼表面上皮细胞与细菌的相互作用:眼表面的上皮细胞是防御眼部细菌感染的重要屏障,而金葡菌性角膜炎的发生和发展与这一屏障的破坏密切相关。当金葡菌侵入上皮细胞内,并在细胞内存活,细菌就能有效地逃避人体免疫系统和抗菌素的杀伤,使感染性角膜炎迁延不愈;而金葡菌穿越眼表面上皮细胞层的保护,到达深层组织,感染就发生扩散。上述眼表面固有免疫机制在正常情况下,发挥保护眼表面的作用,而一旦该免疫机制失灵,则会发生角膜炎。因此,加深对固有免疫分子在眼表面发挥免疫作用机制的认识,对指导我们将来建立更高效的治疗金葡菌性角膜炎的新方案非常重要。
眼表面的固有免疫系统的成员中,除了原来已知的溶菌酶、乳铁蛋白、IgG、IgA、补体等固有免疫蛋白外,新近又发现了另一种固有免疫蛋白——表面活性蛋白,其中的SP-D在防御细菌感染性疾病中发挥重要作用。SP-D是含类胶原结构的C型凝集素家族成员,最初发现其在肺组织中表达。SP-D的强大免疫功能主要表现在,对病毒中和作用,清除肺部的细菌、真菌、凋亡的细胞,调节过敏反应,抑制组织的炎症反应等作用。在肺外组织中,如:消化道,生殖道,泌尿道及眼部的粘膜表面,亦有SP-D表达,并且同样参与防御病原菌的固有免疫过程。SP-D分子的有两个末端,分别称C末端和N末端,C末端的糖基识别区可以识别并结合微生物表面的糖基,N末端的类胶原结合域则与宿主细胞表面的受体结合,这样SP-D就成为细菌和免疫细胞之间的桥梁,从而启动机体清除病原菌的免疫机制。当眼表面的固有免疫功能不良,会表现出SP-D水平下降,细菌感染扩散,而外源性地补充SP-D将有益于平衡固有免疫功能,抑制细菌感染发展。
目前,现有技术中未见SP-D作为药物,用于治疗细菌感染性角膜炎的报道。
发明内容
针对现有技术存在的抗菌药物对难治性金葡菌性角膜炎治疗效果不佳,细菌耐药性问题难以解决,治疗药物存在明显副作用等技术缺陷,本发明提供一种有效的,无明显不良反应的表面活性蛋白-D制备治疗角膜炎药物从而辅助杀灭金黄色葡萄球菌的新应用。
为实现上述发明目的,本发明采用如下的技术方案:
一种将表面活性蛋白-D用于制备治疗角膜炎药物的应用。其中表面活性蛋白-D(SP-D),属于C型凝素家族成员,由相对分子质量为130KD的三个亚单位组成,其亚单位由3个相同的相对分子质量为43KD的多肽链组成。SP-D分子一级结构分为4部分从N端到C端依次为氨基端区、胶原样区、颈区和糖基识别域(CRD)。N端区含半胱氨酸连接亚单位中各肽链胶原样区缠绕,形成三联螺旋体结构,颈区参与蛋白质的折叠,CRD上有钙离子依赖识别位点,在钙离子参与下,识别结合病原体表面的碳水化合物的羟基,并通过C-型CRD和病原微生物表面的碳水化合物的羟基相结合。SP-D的三级结构是由3条相同肽链组成的三聚体。四级结构一般由4个三聚体构成十二聚体排列成十字形。
作为优选,所述药物用于辅助杀灭金黄色葡萄球菌(金葡菌)。
作为优选,所述药物用于辅助中性粒细胞杀灭金黄色葡萄球菌。主要是辅助眼表面中性粒细胞杀灭金葡菌。
作为优选,所述药物用于促进金黄色葡萄球菌的聚集。
作为优选,所述药物剂型为眼药水、眼用凝胶或眼药膏,以及不脱离本发明构思的类似剂型改进或变化。可将SP-D与一种或多种药学上可接受的赋形剂和/或载体结合,制成适于人或动物使用的任何剂型。
具体药理实验测试结果如下:
(一)SP-D促进金葡菌的聚集并辅助中性粒细胞吞噬金葡菌。
1)SP-D促进金葡菌聚集的体外实验:
采用LB液体培养基法进行体外实验,结果表明,SP-D有显著抑制金葡菌活动,促进金葡菌聚集作用,并且随着时间推移,其作用更明显。SP-D溶液作用10、20、30 min,反映金葡菌聚集程度的荧光光度值与对照组的比值分别为(116±7)%、(154±7)%和(179±8)%,均显著大于同样条件下未加入SP-D的对照组。SP-D有显著的促进金葡菌聚集作用。
2)SP-D促进聚集的金葡菌被中性粒细胞吞噬的体外实验:
采用LB液体培养基法获取金葡菌,进行中性粒细胞吞噬细菌的体外实验。结果表明,金葡菌与中性粒细胞共同培养1 h后,SP-D干预组,反映中性粒细胞吞噬金葡菌数量多少的荧光光度值为947±49,显著高于同样条件下未加入SP-D对照组的700±27。SP-D有明显的促进单核细胞吞噬并杀灭金葡菌作用。
3)SP-D促进小鼠眼表面中性粒细胞吞噬金葡菌实验:
采用小鼠金葡菌角膜炎感染模型进行SP-D促进中性粒细胞吞噬金葡菌的体内实验。实验结果表明,经SP-D眼药水处理的小鼠,在金葡菌感染3、6 h后,反映中性粒细胞吞噬金葡菌多少的吞噬分数(中性粒细胞吞噬分数:100个中性粒细胞中吞噬的细菌数量乘以吞噬了细菌的中性粒细胞总数)分别为368±52和896±64,显著高于同样条件下未经SP-D眼药水处理的对照组的126±32和574±60。在角膜炎时,SP-D有显著的促进小鼠眼表面的中性粒细胞吞噬金葡菌作用,并且随时间推移,SP-D的上述作用更加明显。
(二)SP-D促进小鼠金葡菌角膜炎的眼表面金葡菌清除。
采用小鼠金葡菌角膜炎模型进行体内实验,结果表明,在SP-D眼药水作用3、6小时(h)后,小鼠眼表面金葡菌数量分别为(2.3±0.7)×105 CFU和(5.2±1.5)×104 CFU,较同样条件下未经SP-D眼药水处理的对照组的(4.8±1.3)×106 CFU和(7.5±1.4)×105 CFU显著下降。在金葡菌角膜炎时,SP-D可有效促进小鼠眼表面金葡菌清除,使细菌总量明显下降,并且随时间推移,细菌总量下降更明显。
(三)SP-D减少金葡菌对小鼠角膜上皮细胞损伤。
采用小鼠金葡菌角膜炎感染模型,进行SP-D影响金葡菌对小鼠角膜上皮损伤的体内实验。实验结果表明,在SP-D眼药水作用3、6 h后,反映金葡菌损伤小鼠角膜程度的角膜荧光素染色评分,分别为3.3±0.4和6.2±1.1,显著低于同样条件下未经SP-D眼药水处理的对照组的5.8±1.6和9.5±2.5。在金葡菌角膜炎时,SP-D可有效减少细菌对小鼠角膜上皮的损伤,并且随时间推移,SP-D的上述保护作用更加明显。
综上所述,本发明有以下有益效果:SP-D可促进金葡菌的聚集并被眼部中性粒细胞杀灭,同时保护角膜上皮组织,减少金葡菌对上皮的损伤,从而在金葡菌角膜炎时,调动和补充眼部的固有免疫防御机制,使眼部金葡菌被更快清除,局部细菌总量明显下降,并通过减少角膜上皮组织的损伤,有效增加角膜的防御功能,促进炎症的控制和消褪,SP-D非常适用于作为金葡菌角膜炎的辅助治疗药物。
具体实施方式
下面结合具体实施方式对本发明做进一步的说明。应该理解,这些描述只是示例性的,而并非要限制本发明的范围。此外,在以下说明中,省略了对公知技术的描述,以避免不必要地混淆本发明的概念。
除有定义外,以下实施例中所用的技术和科学术语具有与本发明所属领域技术人员普遍理解的相同含义。
实施例1:SP-D促进金葡菌聚集
1.实验材料
试验菌株:金葡菌(ATCC 25923),购自美国ATCC公司。
试验样品: SP-D(实验浓度1 µg/ml),购自美国R&D Systems公司。
磷酸盐缓冲液(PBS):0.03mol/L,PH 7.2。
细菌培养基:LB液体培养基。
荧光分光光度计:日立F-7000荧光分光光度计(日本岛津公司)。
异硫氰酸荧光素 (FITC):1 mg/ml,PH 9.6
2.实验方法
将LB液体培养基中的金葡菌置于37℃温箱中培养16-18 h,随后收集细菌并重悬于1ml的PBS,达细菌浓度为2.1×109 CFU,加入FITC染色1h后,PBS清洗,再制成1ml的菌液,然后实验组加入1ml SP-D(终浓度1 µg/ml),对照组仅加入1ml的PBS,不加入SP-D,均孵育30min,于此同时分别在10、20、30min,测定荧光光度值,实验重复3次。
3.实验结果:金葡菌聚集程度如表一所示:

经SP-D处理后,反映金葡菌聚集程度的荧光光度值明显增加,并且随时间推移,细菌聚集程度更明显。
4.实验结论:SP-D有明显促进金葡菌聚集作用。
实施例2:SP-D促进聚集的金葡菌被中性粒细胞吞噬
1.实验材料
试验菌株:金葡菌(ATCC 25923),购自美国ATCC公司。
试验样品: SP-D(实验浓度1µg/ml),购自美国R&D Systems公司。
磷酸盐缓冲液(PBS):0.03 mol/L,PH 7.2。
细菌培养基:LB液体培养基。
异硫氰酸荧光素 (FITC):购自美国BD公司。
Hanks平衡盐溶液(Hank's Balanced Salt Solution,HBSS):购自美国Gibco公司。
台盼蓝:购自美国Sigma公司。
Ficoll-Paque PLUS:购自瑞典GE healthcare公司。
Wright’s 染液:购自南京建成公司。
多聚甲醛:购自美国Sigma公司。
流式细胞仪: BD FACSCanto II流式细胞仪(美国BD公司)
2.实验方法
中性粒细胞悬液的制备:抽取1名健康成年供血者的外周静脉血8ml置于抗凝管中,加入等体积的右旋糖酐,充分混匀,室温静置20min沉淀红细胞,取上清液离心10min,随后去上清液,用5ml的1×HBSS重悬细胞,而后在细胞悬液底层加入等体积Ficoll-Paque PLUS,此时可以看到细胞重悬液分为两层,离心35min,离心后可看到细胞分层,移液器完全抽弃中间层的单核细胞层,4ml灭菌注射用水重悬中性粒细胞,加入等体积2×HBSS混匀,离心10min,弃上清液,重复上一步骤裂解红细胞,最后底层为外周血中性粒细胞。实验所得中性粒数约为6-7×106/ml。提取的中性粒细胞采用Wright’s染色,用光学显微镜对其进行鉴定是否为中性粒细胞。中性粒细胞悬液加入等体积的台盼蓝溶液,充分混匀,置于载玻片上,光学显微镜观察中性粒细胞活性并进行计数。
中性粒细胞吞噬金葡菌:将LB培养基中的金葡菌置于37℃温箱中培养16-18h,随后收集细菌并重悬于1ml的PBS,达细菌浓度为3.3×109 CFU,加入FITC染色1h后,PBS清洗,再制成1ml的菌液,然后实验组加入1 ml的SP-D(终浓度1 µg/ml),对照组仅加入1ml的PBS,不加入SP-D,均孵育30 min。随后,每1µl细菌菌液,加入20µl中性粒细胞悬液,37℃培养1h后,2%多聚甲醛固定,用流式细胞仪检测,对每个样本测定2000个中性粒细胞,实验重复3次。
3.实验结果:中性粒细胞吞噬金葡菌后的荧光光度值如表二所示:

经过SP-D处理后,反映中性粒细胞吞噬金葡菌数量的荧光光度值明显增加。
4.实验结论:SP-D有明显的促进中性粒细胞吞噬金葡菌的作用。
实施例3:SP-D促进小鼠眼表面中性粒细胞吞噬金葡菌
1.实验材料
试验菌株:金葡菌(ATCC 25923),购自美国ATCC公司。
试验样品: SP-D(实验浓度1 µg/ml),购自美国R&D Systems公司。
磷酸盐缓冲液(PBS):0.03 mol/L,PH 7.2。
细菌培养基:LB液体培养基。
Hema-3 染色试剂盒:购自美国Fisher scientific公司。
C57BL/6小鼠:SPF级,体重20-22 g,6-8周龄,雄性,购自浙江省医学实验动物中心。
2.实验方法
①金葡菌的培养:将LB液体培养基中的金葡菌置于37℃温箱中培养16-18h,随后收集细菌并重悬于PBS,达细菌浓度为2.2×107 CFU。
②SP-D对小鼠眼表面中性粒细胞吞噬金葡菌的影响:雄性小鼠,每组3只,经腹腔注射麻醉后,用30G针头,在小鼠角膜中央区划出3道平行的划痕,长约1mm,深度超过角膜上皮层而不穿透内皮层,其中一组小鼠,将2.5µl细菌浓度为2.2×107 CFU的金葡菌液加入2.5 µl的PBS混合后,滴入小鼠角膜表面,作为对照组;另一组小鼠,将培养的2.2×107 CFU的金葡菌液2.5µl加入2.5µl的SP-D眼药水(SP-D终浓度1µg/ml)后,滴入小鼠角膜表面。在小鼠角膜感染后3、6 h,先在小鼠结膜囊内滴入5 µl的PBS,随后用毛细管收集小鼠泪液5µl,涂于载玻片上,干燥后,用Hema-3 染色试剂盒进行染色,光学显微镜下观察中性粒细胞吞噬细菌的数量。随机挑选视野中的100个中性粒细胞,将这些中性粒细胞中吞噬的细菌数量乘以吞噬了细菌的中性粒细胞数,记录为中性粒细胞吞噬分数,实验重复3次。
其中SP-D眼药水的制备
处方:SP-D共2mg、三氯叔丁醇 0.1g、灭菌生理盐水加至1000ml,并利用NaHCO3调节pH值至7.0-7.2。
制备方法:SP-D共2mg、三氯叔丁醇 0.1g,将二者溶于灭菌生理盐水中,使全部溶解,加灭菌生理盐水至1000ml,NaHCO3调节pH值至7.0-7.2,滤过,灌装即得。
3.实验结果:小鼠眼表面中性粒细胞吞噬金葡菌后的吞噬分数值如表三所示:

经过SP-D处理后,小鼠眼表面中性粒细胞吞噬金葡菌的吞噬分数明显增加,并且随时间推移,吞噬分数增加程度更明显。
4.实验结论:SP-D促进小鼠眼表面中性粒细胞吞噬金葡菌。
实施例4:SP-D促进金葡菌角膜炎小鼠眼表面的金葡菌数量下降
1.实验材料
试验菌株:金葡菌(ATCC 25923),购自美国ATCC公司。
C57BL/6小鼠:SPF级,体重20-22g,6-8周龄,雄性,购自浙江省医学实验动物中心。
试验样品: SP-D(实验浓度1 µg/ml),购自美国R&D Systems公司。
磷酸盐缓冲液(PBS):0.03mol/L,PH 7.2。
细菌培养基:LB液体培养基、琼脂培养基。
2.实验方法
①金葡菌的培养:将LB液体培养基中的金葡菌置于37℃温箱中培养16-18h,随后收集细菌并重悬于PBS,达细菌浓度为2.2×107 CFU。
②SP-D对金葡菌角膜炎小鼠眼表面金葡菌数量的影响:雄性小鼠,每组3只,经腹腔注射麻醉后,用30 G针头,在小鼠角膜中央区划出3道平行的划痕,长约1mm,深度超过角膜上皮层而不穿透内皮层,其中一组小鼠,将2.5 µl细菌浓度为2.2×107 CFU的金葡菌液加入2.5 µl的PBS混合后,滴入小鼠角膜表面,作为对照组;另一组小鼠,将培养的2.2×107CFU的金葡菌液2.5µl加入2.5µl的SP-D眼药水(SP-D终浓度1µg/ml)后,滴入小鼠角膜表面。在小鼠角膜感染后3、6h,先在结膜囊滴入5µl的PBS,随后用毛细管收集小鼠泪液5µl,随即接种于琼脂平板上,培养并检测金葡菌数量。实验重复3次。
其中SP-D眼药水的成分、含量、制备方法与实施例3相同。
3.实验结果:SP-D对眼表面金葡菌数量的影响如表四:

经过应用SP-D液后,金葡菌角膜炎小鼠眼表面细菌数量明显下降,并且随时间推移,细菌下降程度更明显。
4.实验结论:SP-D溶液滴眼可有效促进金葡菌角膜炎小鼠眼表面金葡菌清除。
实施例5:SP-D减少金葡菌对小鼠角膜上皮损伤
1.实验材料
试验菌株:金葡菌(ATCC 25923),购自美国ATCC公司。
C57BL/6小鼠:SPF级,体重20-22g,6-8周龄,雄性,购自浙江省医学实验动物中心。
试验样品: SP-D(实验浓度1 µg/ml),购自美国R&D Systems公司。
磷酸盐缓冲液(PBS):0.03mol/L,PH 7.2。
细菌培养基:LB液体培养基、琼脂培养基。
2.实验方法
①金葡菌的培养:将LB液体培养基中的金葡菌置于37 ℃温箱中培养16-18h,随后收集细菌并重悬于PBS,达细菌浓度为1.6×109 CFU。
②SP-D影响金葡菌对小鼠角膜上皮的损伤:雄性小鼠,每组3只,经腹腔注射麻醉后,其中一组小鼠,将2.5µl细菌浓度为2.2×107 CFU的金葡菌液加入2.5µl的PBS混合后,滴入小鼠角膜表面,作为对照组;另一组小鼠,将培养的2.2×107 CFU的金葡菌液2.5µl加入2.5µl的SP-D眼药水(SP-D终浓度1µg/ml)后,滴入小鼠角膜表面。在小鼠角膜感染后6、12h,先在结膜囊滴入1µl的1%荧光素钠溶液,随后在裂隙灯显微镜在钴蓝光下照相,根据角膜着染的情况,进行荧光素染色计分(角膜荧光素染色评分标准参考2007年国际干眼研讨报告(Ocul Sure,2007,5(2):75-92),将角膜分为4个象限,染色水平为0~3个级别,没有染色定义为0,1分为染色<5个点,2分为密集型点状染色,3分为片状染色或出现丝状物,共分为0~12分。),实验重复3次。
其中SP-D眼药水的成分、含量、制备方法与实施例3相同。
3.实验结果:SP-D影响金葡菌感染导致角膜损伤的荧光素染色情况如下表五:

经过应用SP-D干预后,金葡菌导致的角膜荧光素染色评分明显下降,并且随时间推移,评分下降程度更明显。
4.实验结论:SP-D可有效减少金葡菌对小鼠角膜上皮的损伤。
综上所述仅为本发明的较佳实施例,并非用来限定本发明的实施范围,凡依本申请专利范围的内容所作的等效变化与修饰,都应为本发明的技术范畴。

1.一种将表面活性蛋白-D用于制备治疗角膜炎药物的应用。
2.根据权利要求1所述的应用,其特征在于:所述药物用于辅助杀灭金黄色葡萄球菌。
3.根据权利要求2所述的应用,其特征在于:所述药物用于辅助中性粒细胞杀灭金黄色葡萄球菌。
4.根据权利要求3所述的应用,其特征在于:所述药物用于促进金黄色葡萄球菌的聚集。
5.根据权利要求1所述的应用,其特征在于:所述药物剂型为眼药水、眼用凝胶或眼药膏。
