Abstract

本发明公开了一种个体化3D打印钛合金骶骨假体,包括腰椎结合部、髂骨结合部和连接部。腰椎结合部为按照骶1椎体形状仿生设计的带有扇状尾翼的中空柱状多孔结构,与腰5椎体下终板的骨结合面为仿骨小梁多孔结构设计,并设计有2个螺钉孔;髂骨结合部为仿髂骨耳状面设计的中空多面体,其髂骨结合面为仿骨小梁多孔结构设计,并设计有4个呈纵向对称分布的螺钉孔;连接部为中空、多孔、管状结构,将腰椎结合部和髂骨结合部连接。本发明通过钛合金骶骨假体重建骨盆环结构,并通过螺钉固定和在中空结构中植骨,达到早期力学稳定、后期坚强的生物愈合效果;使骶骨缺损重建后外形美观、力学稳定,降低远期假体失败率。

Description

一种个体化3D打印钛合金骶骨假体
技术领域
本发明属于医用内固定物制造技术领域,尤其涉及一种用于全骶骨切除手术后重建骨盆环稳定性的个体化3D打印钛合金骶骨假体。
背景技术
人体骶骨包含5块骶椎(骶1-骶5椎体),位于脊柱的末端,与双侧髂骨组成坚强的骨盆环结构,向上连接腰椎,将上半躯体重量传递到双侧下肢。除骨折外,骶骨最常见的病变为肿瘤(原发肿瘤或转移瘤)。虽然发病率低,但因为毗邻盆腔脏器、重要的血管神经束,病变常常压迫骶神经,出现会阴、肛周麻木、大小便失禁,进一步发展可导致下肢瘫痪,危及生命。
骶骨肿瘤的治疗上通常根据患者病情选择后方入路全骶骨切除手术。该手术方式严重破坏了人体的负重结构。骶骨切除后,腰5椎体以上的人上半身和骨盆残余的两侧髂骨之间无骨性连接,整个上身重量无法通过骨盆传导到下肢,人体无法站立、行走。这种躯干和下肢之间异常加大的活动范围导致神经根过度牵张,对神经的刺激性加大。因此,临床上多采用后路椎弓根螺钉固定腰椎,髂骨椎弓根螺钉固定双侧髂骨,并依靠钛棒将上述两种螺钉连接,从而将躯干和骨盆连接起来。但是这种机械性固定由于没有重建骶骨,存在以下问题:1)切除骶骨后局部会形成较大的腔隙,如果不植入骶骨假体会导致长期存在残腔和积液,引发感染;2)骶骨切除后会导致盆腔内脏器如直肠、结肠向后侧(背侧)疝出,导致病人大便障碍,3)螺钉与骨之间是机械性连接,两者之间不能行成骨长入及达到最终生物愈合,长期活动必然导致螺钉从骨内拔出和钛棒/螺钉断裂。这些都会增加术后感染率和再手术风险,导致病人无法站立和行走。
全骶骨切除手术后,骶骨的重建无法通过常规内植物来完成。目前仅有北京大学第三附属医院(例如,CN206151634U)和北京大学人民医院(例如,CN205286615U)设计的3D打印钛合金骶骨假体。但其采用的是解剖模拟,假体尺寸较大,术中需要较大范围的组织剥离和显露,增加创伤;同时,其设计的假体没有用于植骨的结构,后期不能达到骨性融合和生物固定,最终易导致内固定失败。上海长征医院提出了一种组配式人工骶骨假体(CN107496901A)。但其假体主体并非个性化定制,其与腰椎骨面结合度和后期骨长入较3D打印假体要差,同时缺乏增加假体的远期生物固定的结构设计,容易导致内固定失败。综上所述,目前国内外尚未成功设计出不仅满足全骶骨切除术后人体结构力学需求,而且植入后可促进腰椎和双侧髂骨骨性融合,从而重建肢体力学支撑,并缩短病人恢复负重和运动所需时间的骶骨假体。
发明内容
本发明的目的在于提供一种个体化3D打印钛合金骶骨假体,可以克服全骶骨切除术后植入假体存在的重建困难、远期效果差的弊端,并缩短病人恢复负重和运动所需时间。
为达到上述目的,本发明采用了以下技术方案:
一种个体化3D打印骶骨假体,该骶骨假体包括腰椎结合部、髂骨结合部和连接部;所述腰椎结合部为仿骶椎中空柱状结构(包括中空柱状部分),腰椎结合部(中空柱状部分)的上端设置有腰椎终板连接件,连接部包括设置在腰椎结合部(中空柱状部分)侧面上并向完整或残存的髂骨耳状面伸出的中空通道结构,髂骨结合部包括设置于所述髂骨耳状面与对应侧中空通道结构之间的中空多面体,所述中空多面体上设置有髂骨结合面连接件。
优选的,所述腰椎结合部的顶盖(即中空柱状部分上端面)按照患者骶椎(例如,骶1椎体形状)CT数据进行设计(例如,顶盖的形状和尺寸按照骶1椎体上终板的CT数据设计,大小和弧度一致),所述腰椎结合部(中空柱状部分)的侧面为多孔状,腰椎结合部(中空柱状部分)的内部通过位于其背侧的植骨窗口植入人工骨、异体骨或自体骨,腰椎结合部采用3D金属打印制备而成。
优选的,所述腰椎结合部(中空柱状部分)的下端背侧设置有扇状尾翼(具有骶骨仿生曲面的板体,使腰椎结合部形成仿骶椎中空柱状结构),利用扇状尾翼表面的多孔结构可与残存的盆底肌群缝合,防止骶骨切除术后肠管疝出。
优选的,所述腰椎结合部的顶盖上具有仿骨小梁多孔结构设计,利于骨长入,使腰椎结合部与腰椎(例如,腰5椎体下终板)骨质结合,并与腰椎(例如,腰5椎体下终板)最终达到生物愈合;腰椎结合部的顶盖上还具有腰椎终板连接件安装孔(例如,2个对称的锁定螺钉孔),用于腰椎终板连接件(例如,松质骨螺钉)与腰椎(例如,腰5椎体下终板)的固定,增加初始强度。

本发明的骶骨假体是在骶骨解剖形态特征基础上经过重新设计而成,其以中空连接部连接具有髂骨结合面的中空髂骨结合部和仿骶椎中空腰椎结合部,通过连接件固定骨接触面于髂骨耳状面、腰椎终板,并通过中空结构中植骨,达到早期力学稳定、后期坚强的生物愈合效果,可加快重建骨盆环结构,使骶骨缺损重建后具有外形美观、力学稳定的特点,显著地降低了远期假体内固定的失败率。
进一步的,本发明通过采集患者骨盆CT扫描数据,并与腰椎和残存的髂骨耳状面的解剖形状进行匹配,使骶骨假体能够与脊柱和髂骨部分对应骨质贴合,提高了固定的稳定性。并通过接触面上的多孔结构设计,引导骨长入,最终达到假体和骨之间的生物固定。
进一步的,本发明通过有限元力学分析和拓扑结构优化设计(并非简单的模拟解剖外形),能够使假体使用材料最少、体积最小、重量最轻、力学强度最强。

进一步的,本发明的骶骨假体与骨相接触的面上,增加了羟基磷灰石涂层,可增加骨长入几率,提高假体的稳定性。

具体实施方式
下面结合附图和实施例对本发明做进一步详细说明。
参见图1、图2、图3及图8、图9,针对全骶骨切除的手术特点,结合骨盆环的解剖结构特征,本发明提出了一种能够将腰椎和双侧髂骨固定的3D打印个性化钛合金假体。
上述3D打印个体化钛合金假体包括与腰椎11(具体为腰5椎体)下终板接触的腰椎结合部1及与残存髂骨10接触的髂骨结合部2,所述腰椎结合部1的侧面与其两侧的髂骨结合部2之间连接有解剖形状相适应的中空连接部3;腰椎结合部1的后侧中部具有植骨窗口6,腰椎结合部1的后侧下端具有扇状尾翼7。
其中髂骨结合部2和腰椎结合部1的外形(例如,与骨的接触面)设计,均来自于患者的骶椎和髂骨的CT数据,可与上述骨质(腰椎和髂骨)的外形紧密贴合。同时,以假体使用材料最少、体积最小、重量最轻、力学强度最强为目标,通过有限元力学分析和拓扑结构进行了优化设计,从而形成了自腰椎结合部1向后侧伸出的两个管状结构(中空连接部3)和与管状结构伸出端对应连接的耳状中空多面体(髂骨结合部2)。
所述腰椎结合部1的顶盖上具有2个4.5mm螺钉孔(位于顶盖的中心线上,距离顶盖中心10mm),每侧髂骨结合部2上各有两个纵向排列的6.5mm螺钉孔5(位于髂骨结合部上下两端且靠外侧,即远离连接部一侧)。
人体正常的骨盆环结构如图4、图5所示,骶骨9上端与腰椎11(具体为腰5椎体)连接,骶骨9两侧分别与对应侧的髂骨10连接,形成稳定的环路,将人体上半身的重量传递到下肢。
全骶骨切除后,人体上半身的重量传递到腰椎11(具体为腰5椎体)后无法再向两侧髂骨10传导,导致无法站立和行走(图6、图7)。
将上述3D打印个体化钛合金假体植入(图8、图9)时,假体的中空、表面多孔的腰椎结合部1与腰椎11(具体为腰5椎体的下终板)紧密贴合,两枚4.5mm松质骨拉力螺钉4通过腰椎结合部1的顶盖上的螺钉孔5打入腰椎11(具体为腰5椎体)内以固定,表面多孔的耳状髂骨结合部2利用位于其外侧的仿髂骨耳状面(形状与髂骨耳状面相应)与髂骨的耳状面紧密贴合,腰椎结合部1与髂骨结合部2通过中空、表面多孔、管状的连接部3连接为一体。每侧髂骨结合部2使用其上纵向分布的2枚6.5mm椎弓根螺钉8固定于髂骨,髂骨耳状面上的椎弓根螺钉8与固定在腰椎上的6.5mm椎弓根螺钉8通过钛棒相连,从而获得更牢固的初始稳定性。骶骨假体与腰椎和髂骨的多孔结合面(顶盖、仿髂骨耳状面)上具有羟基磷灰石涂层,利于骨长入及假体远期达到生物愈合和固定。
本发明提出的3D打印个体化钛合金假体,其外形设计来自于患者的CT扫描数据并结合了有限元力学分析和拓扑结构优化,真正达到了“量身定做”;不仅能够与人腰椎和双侧髂骨的解剖结构很好的贴合,而且植入时需要显露的范围小、组织刺激性小、创伤小。通过自身带有的螺钉孔设计,可植入松质骨螺钉和椎弓根螺钉,获得了坚强的初始稳定性;利用中空结构进行植骨,利用接触面的多孔结构诱导骨长入,最终达到生物愈合。该3D打印个体化钛合金假体,集植骨与固定于一身,可降低假体内固定失败率,缩短术后恢复负重和行动能力的时间,因此更加实用。

Claims (10)

1.一种个体化3D打印骶骨假体,其特征在于:该骶骨假体包括腰椎结合部(1)、髂骨结合部(2)和连接部(3);所述腰椎结合部(1)包括中空柱状结构,腰椎结合部(1)的中空柱状结构上端设置有腰椎终板连接件,连接部(3)包括设置在腰椎结合部(1)的中空柱状结构侧面上并向髂骨耳状面伸出的中空通道结构,髂骨结合部(2)包括设置于所述髂骨耳状面与对应侧中空通道结构之间的中空多面体,中空多面体上设置有髂骨结合面连接件。
2.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述腰椎结合部(1)的中空柱状结构侧面为多孔状,腰椎结合部(1)的中空柱状结构背侧设置有植骨窗口(6)。
3.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述腰椎结合部(1)的中空柱状结构下端设置有扇状尾翼(7)。
4.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述腰椎结合部(1)与腰椎的接触面上设置有仿骨小梁多孔结构以及腰椎终板连接件安装孔。
5.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述髂骨结合部(2)与髂骨的接触面上设置有仿骨小梁多孔结构以及髂骨结合面连接件安装孔。
6.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述髂骨结合面连接件采用椎弓根螺钉(8);该椎弓根螺钉(8)与设置于腰椎上的椎弓根螺钉(8)通过躯干骨盆连接件相连。
7.根据权利要求6所述一种个体化3D打印骶骨假体,其特征在于:所述躯干骨盆连接件选自钛棒。
8.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述连接部(3)的中空通道结构的表面呈多孔状,该中空通道结构自腰椎结合部(1)向背侧伸出,并分别连接在腰椎结合部(1)与两侧髂骨结合部(2)之间。
9.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述骶骨假体的髂骨结合部(2)和连接部(3)的空间相对位置和形状是通过有限元力学分析和拓扑结构优化设计而成,骶骨假体采用3D打印一体成型。
10.根据权利要求1所述一种个体化3D打印骶骨假体,其特征在于:所述骶骨假体采用钛合金制成,该骶骨假体的骨接触面上设置有羟基磷灰石涂层。
