Abstract

背景技术
慢性伤口护理给美国医疗保健体系和个人付款人都带来了巨大的财务负担。目前,与不愈合/难以愈合的皮肤伤口相关的年度费用总计约为250亿美元(Witherel CE etal.,Wound Repair and Regeneration 24.3(2016):514-524)。然而,尽管付出了所有代价,但患者对没有任何痛苦或疤痕情况下的正常生活的需求几乎得不到满足。尽管自体移植是皮肤置换中最常用的方法之一,但在发病率、死亡率和可用于移植的供体皮肤面积方面的大小限制仍然存在问题。

发明内容
在一方面,本发明涉及一种支架,包含:多孔材料,该多孔材料包含:具有第一表面、第二表面以及它们之间的厚度的大豆分离蛋白(SPI)纤维;其中第一表面上的平均孔径小于第二表面上的平均孔径;并且其中平均孔径从第一表面到第二表面穿过(through)材料逐渐增加。
在一个实施方式中,植物蛋白选自由大豆分离蛋白、小麦谷蛋白、玉米醇溶蛋白和豌豆蛋白组成的组。在一个实施方式中,纤维的直径为0.5μm至5μm。在一个实施方式中,支架的厚度为500μm至2000μm。在一个实施方式中,第一表面的平均孔径在直径上为1μm至20μm。在一个实施方式中,第二表面的平均孔径为10μm至200μm。在一个实施方式中,平均孔径的逐渐增加是线性的。在一个实施方式中,平均孔径的逐渐增加是非线性的。

在一个实施方式中,电处理是电纺丝。在一个实施方式中,SPI溶液包含溶解于1,1,1,3,3,3-六氟-2-丙醇中的SPI。在一个实施方式中,牺牲材料包含水溶性材料。在一个实施方式中,水溶性材料是溶于乙醇的PEO。在一个实施方式中,水性溶液是水。在一个实施方式中,冷冻步骤是在-80℃下进行2小时。在一个实施方式中,冻干步骤是在-60℃和0.08mbar下进行。

支架
本发明提供具有分级孔径的纤维状支架,其有助于细胞浸润。该支架可用于工程化具有固有分级结构的组织,例如皮肤或骨骼。与其他多孔支架相比,分级的多孔结构模仿了这些组织的细胞外基质的天然生物结构,从而允许更自然的各向异性的细胞渗透到支架中,并改善了结构内的细胞运动性和迁移性。整个支架中孔的大小分布增强了不同细胞类型的分离和排列。例如,真皮成纤维细胞和角质形成细胞可以位于支架中紧密模拟其各自天然组织的固有分层结构的位置处。此外,孔允许有效运输生物活性分子、营养因子和废物。

支架10可以具有任何合适的形状。在一些实施方式中,支架10基本上是平面的,例如片状。在其它实施方式中,支架10可以成形为三维结构,例如管或球体。支架10可以具有任何合适的厚度,例如小于100μm或高达数毫米的厚度。在一个实施方式中,仿生支架的厚度为500μm至5000μm,或在另一实施方式中,为500μm至2000μm。支架10可以具有任何几何形状。在各个实施方式中,支架10可以经修剪或调整尺寸以适应任何合适的形状。
在各个实施方式中,支架10包含大豆分离蛋白(SPI),减轻宿主体内的免疫反应,减少和缩短进行组织重塑的炎症过程。如本文所用,术语“大豆分离蛋白”以大豆蛋白工业的常规意义使用。例如,大豆分离蛋白是这样的大豆材料:在无水分基础上,其具有的蛋白质含量至少为90%的大豆蛋白。本领域中使用的“分离的大豆蛋白”具有与本文中使用的和本领域中使用的“大豆分离蛋白”相同的含义。大豆分离蛋白是由大豆形成的,方法是从子叶中去除大豆的壳和胚芽,将子叶剥落或磨碎并且从剥落或磨碎的子叶中去除油,从子叶纤维和脂质中分离出子叶的大豆蛋白和碳水化合物,然后从碳水化合物中分离大豆蛋白。在某些实施方式中,所得材料用乙醇洗涤以除去一定百分比的异黄酮。在一个实施方式中,大豆基组合物包含含有大豆蛋白和大豆子叶纤维的纤维材料。纤维材料通常包括脱脂的大豆蛋白材料和大豆子叶纤维。通过挤压大豆蛋白材料和大豆子叶纤维来生产纤维材料。
在各个实施方式中,支架可以修饰有一个或多个官能团,用于共价结合各种蛋白质(例如胶原蛋白)或化合物如治疗剂。可以与支架相连的治疗剂包括但不限于:镇痛药、麻醉药、抗真菌药、抗生素、消炎药、驱虫药、解毒剂、止吐药、抗组胺药、抗癌药、降压药、抗疟药、抗微生物药、抗精神病药、退烧药、防腐剂、抗关节炎药、抗结核药、镇咳药、抗病毒药、心脏活性药物、导泻剂、化学治疗剂、彩色或荧光成像剂、皮质激素(例如类固醇)、抗抑郁药、抑制剂(depressant)、诊断辅助剂、利尿剂、酶、祛痰剂、激素、催眠药、矿物质、营养补充剂、拟副交感神经药、钾补充剂、辐射敏化剂、放射性同位素、荧光纳米颗粒(例如纳米金刚石)、镇静剂、磺胺类药物、兴奋剂、拟交感神经药、镇定剂、尿液抗感染剂、血管收缩剂、血管扩张剂、维生素、黄嘌呤衍生物等。治疗剂也可以是其他有机小分子、天然分离的实体或其类似物、有机金属试剂、螯合金属或金属盐、肽基药物、或肽或非肽受体靶向或结合剂。预期治疗剂与支架的连接可以通过蛋白酶敏感性连接子或其他可生物降解的连接进行。可掺入仿生支架的分子包括但不限于,维生素和其他营养补充剂;糖蛋白(例如胶原蛋白);纤连蛋白;肽和蛋白质;碳水化合物(简单的和/或复杂的);蛋白聚糖;抗原;寡核苷酸(有义和/或反义DNA和/或RNA);抗体(例如针对传染剂、肿瘤、药物或激素的抗体);和基因治疗试剂。

在一个实施方式中,支架可进一步包含一种或多种天然或合成药物,诸如非甾体抗炎药(NSAID)。在一个实施方式中,支架可以进一步包含抗生素,例如青霉素。在一个实施方式中,支架可进一步包含天然肽,例如甘氨酰-精氨酰-甘氨酰-天冬氨酰-丝氨酸(GRGDS)、精氨酰甘氨酰天冬氨酸(RGD)和釉原蛋白。在一个实施方式中,支架可以进一步包含蛋白质,例如壳聚糖和丝。在一个实施方式中,支架可进一步包含蔗糖、果糖、纤维素或甘露醇。在一个实施方式中,支架可以进一步包含细胞外基质蛋白,例如纤连蛋白、玻连蛋白、层粘连蛋白、胶原蛋白和vixapatin(VP12)。在一个实施方式中,支架可进一步包含解联蛋白,例如VLO4。在一个实施方式中,支架可进一步包含脱细胞或脱盐的组织。在一个实施方式中,支架可进一步包含合成肽,例如釉基质蛋白(emdogain)。在一个实施方式中,支架可进一步包含营养物,例如牛血清白蛋白。在一个实施方式中,支架可进一步包含维生素,诸如维生素B2、维生素Ad、维生素D、维生素E和维生素K。在一个实施方式中,支架可以进一步包含核酸,例如mRNA和DNA。在一个实施方式中,支架可进一步包含天然或合成的类固醇和激素,例如地塞米松、氢化可的松、雌激素及其衍生物。在一个实施方式中,支架可以进一步包含生长因子,例如成纤维细胞生长因子(FGF)、转化生长因子β(TGF-β)和表皮生长因子(EGF)。在一个实施方式中,支架可以进一步包含递送载体,例如纳米颗粒、微粒、脂质体、病毒和非病毒转染系统。
在一个实施方式中,支架是无细胞提供的。在另一个实施方式中,提供的支架预接种了一个或多个细胞群以形成人工组织构建体。该人工组织构建体可以是自体的(其中细胞群来源于患者自身的组织)或同种异体的(其中细胞群来自与患者相同物种内的另一个受试者)。人造器官构建体也可以是异种的,其中不同的细胞群来源地不同于受试者的哺乳动物物种。例如,这些细胞可以来源于哺乳动物的器官,例如人、猴子、狗、猫、小鼠、大鼠、牛、马、猪、山羊和绵羊。
可以从多种来源分离细胞,包括例如,活体受试者的活组织检查和从尸体回收的完整器官。分离的细胞优选是自体细胞,其通过活检从意欲成为接受者的受试者中获得。活检可以使用活检针进行,该活检针是一种快速动作的针,可以快速、简单地进行手术。
可以使用本领域技术人员已知的技术分离细胞。例如,可以机械分解和/或用消化酶和/或螯合剂处理组织,这些消化酶和/或螯合剂会削弱相邻细胞之间的连接,从而可以将组织分散为单个细胞的悬浮液而不会造成明显的细胞破坏。可以通过切碎组织并用多种消化酶中的任何一种单独或组合处理切碎的组织来完成酶解离。这些包括但不限于胰蛋白酶、胰凝乳蛋白酶、胶原蛋白酶、弹性蛋白酶和/或透明质酸酶、脱氧核糖核酸酶、链酶和分散酶。机械破坏也可以通过多种方法来完成,包括但不限于刮擦组织表面,使用研磨机、搅拌机、筛子、均质机、压力传感器或超声仪。
一旦组织被还减小成单个细胞的悬浮液,就可以将悬浮液分成亚群,从中可以得到细胞成分。这也可以使用标准的细胞分离技术来完成,包括但不限于克隆和选择特定细胞类型,选择性破坏不需要的细胞(否定选择),混合群体中基于差异细胞凝集的分离,冻融程序,混合群体中细胞的差异粘附特性,过滤,常规和分区离心,离心淘析(逆流离心),单元重力分离,逆流分布,电泳和荧光激活细胞分选。
例如,当供体患有诸如癌症或其他肿瘤转移至所需组织等疾病时,也可能需要进行细胞分级分离(fractionation)。可以对细胞群进行分类以将恶性细胞或其他肿瘤细胞与正常非癌细胞分离。从一种或多种分选技术中分离出来的正常非癌细胞可以用于组织重建。
分离的细胞可以在体外培养,以增加可用于接种仿生支架的细胞数量。为了防止组织排斥,优选使用同种异体细胞,更优选自体细胞。但是,如果在植入人工器官后在受试者中确实发生了免疫反应,则可以使用免疫抑制剂(例如环孢菌素或FK506)对受试者进行治疗,以减少排斥的可能性。在某些实施方式中,可将嵌合细胞或来自转基因动物的细胞接种到生物相容性支架上。
分离的细胞可以在用遗传物质包覆之前被转染。有用的遗传物质可以是例如能够减少或消除宿主免疫应答的遗传序列。例如,可以抑制细胞表面抗原(例如I类和II类组织相容性抗原)的表达。这可以使移植的细胞减少了被宿主排斥的机会。另外,转染也可以用于基因递送。
分离的细胞可以是正常的,也可以是基因改造的,以便提供附加或正常的功能。可以使用利用逆转录病毒载体、聚乙二醇进行基因工程改造细胞的方法或本领域技术人员已知的其他方法。这些包括使用在细胞中转运和表达核酸分子的表达载体(参见Goeddel;Gene Expression Technology:Methods in Enzymology 185,Academic Press,SanDiego,Calif.(1990))。可通过常规转化或转染技术将载体DNA引入原核生物或细胞中。可以在Sambrook et al.(Molecular Cloning:A Laboratory Manual,3nd Edition,ColdSpring Harbor Laboratory press(2001))和其他实验室教科书中找到适用于转化或转染宿主细胞的方法。
可以根据标准方法来将细胞接种到支架上。例如,已经报道了将细胞接种到聚合物基材上以用于组织修复(参见例如Atala,A.et al.,J.Urol.148(2Pt 2):658-62(1992);Atala,A.,et al.J.Urol.150(2Pt 2):608-12(1993))。培养物中生长的细胞可以用胰蛋白酶消化以分离细胞,并且可以将分离出的细胞接种在支架上。或者,从细胞培养物中获得的细胞可以从培养板上提起作为细胞层,并且可以将细胞层直接接种到支架上而无需事先分离出细胞。
在一个实施方式中,将100万至5000万个细胞悬浮在培养基中,并施加到每平方厘米支架表面上。在标准培养条件(诸如例如37℃5%CO2)下将支架培育一段时间直到细胞附着。然而,应当理解的是,接种到支架上的细胞的密度可以变化。例如,更高的细胞密度由接种细胞促进更大的组织再生,而较小的密度可通过从宿主浸润移植物的细胞而允许相对较大的组织再生。根据基质或支架和细胞,也可以使用其他接种技术。例如,可以通过真空过滤将细胞应用于基质或支架。根据本文的教导,细胞类型的选择以及细胞到支架上的接种对于本领域普通技术人员而言将是常规的。
在一个实施方式中,用一个细胞群来接种支架以形成人造组织构建体。在另一实施方式中,使用两个不同的细胞群在两侧接种支架。这可以通过首先在支架的一侧接种,然后在另一侧接种来执行。例如,可以将支架的一侧位于上部放置并进行接种。然后可以重新放置支架以便第二侧位于上部。然后可以在第二侧接种第二细胞群。或者,可以同时对支架的两侧进行接种。例如,可以将两个细胞室放置在支架的两侧(即夹层)。两个细胞室可以填充有不同的细胞群以同时在支架的两侧进行接种。可以旋转或频繁翻转夹层的支架,以使两个细胞群的附着机构相等。
在另一实施方式中,两个单独的支架可以接种不同的细胞群。在接种后,可以将两个支架附接在一起以形成在两侧具有两个不同细胞群的单个支架。可以使用诸如纤维蛋白胶、液体共聚物、缝合线等标准程序来进行支架彼此的附接。
为了促进本发明支架上的细胞生长,支架可以涂覆有一种或多种细胞粘附增强剂。这些药剂包括但不限于胶原蛋白、层粘连蛋白和纤连蛋白。支架还可以包含在支架上培养的细胞,以形成目标组织替代物。或者,可以在本发明的支架上培养其他细胞。
制造方法
本发明还涉及制造本发明的支架的方法。该方法将电处理与水合电处理的支架、冷冻浸泡的构建体和将其冻干的后处理步骤组合在一起。该方法产生水合冻干的支架,其组合牺牲材料,产生本发明的独特分级多孔支架。
现在参考图2,示出了制造分级多孔支架的示例性方法100。方法100从步骤110开始,其中用牺牲材料对植物蛋白溶液进行电处理以形成复合支架。重要的是要注意,将植物蛋白溶液和牺牲材料同时但分别地电沉积到同一旋转基板上。在某些实施方式中,植物蛋白溶液和牺牲材料经沉积以便植物蛋白溶液和牺牲材料沉积的轨迹相互成90度角。在步骤120中,将牺牲材料从复合支架中去除以留下植物蛋白支架。在步骤130中,将植物蛋白支架在水性溶液中水合。在步骤140中,从水性溶液中取出后,将植物蛋白支架冷冻。在步骤150中,将冷冻的植物蛋白支架冻干以形成分级多孔支架。冻干可以在-60℃和0.08mbar下进行。
可以任何适合的方式制备SPI溶液。例如,SPI溶液可以包含溶解于1,1,1,3,3,3,-六氟-2-丙醇(HFP)溶液中的SPI。在一些实施方式中,SPI溶液可以包含一定量的牺牲材料。在一个实施方式中,SPI溶液包含溶解于HFP中的7%(w/v)SPI和0.05%(w/v)聚环氧乙烷(PEO)。
可以任何适合的方式制备牺牲材料溶液。在某些实施方式中,牺牲材料是水溶性的,使得步骤120和步骤130可同时进行(在水合步骤中去除牺牲纤维)。在一个实施方式中,牺牲材料包含溶解于90%乙醇中的3%(w/v)PEO。在其它实施方式中,牺牲材料可以包含聚环氧乙烷、聚乙烯醇或葡聚糖。
在一个实施方式中,电处理是电纺丝。进行SPI溶液和牺牲材料溶液共纺的条件可以在任何合适的范围内进行,例如本文公开的那些。例如,电纺过程中使用的电场可以为5至约50kV,更优选约10至约30kV。纺丝溶液向喷丝头的进料速率可以为约0.1至约3mL/小时,更优选约0.5至约1.5mL/小时。喷丝头可以单独或同时补充有一个或多个额外的空气喷射口。喷丝头到旋转基材的距离可以相同,或者对于SPI溶液和牺牲材料溶液,它们可以处于不同的距离。
本领域技术人员将理解,旋转基板通常包括通常通过钻夹头机械地附接到电机的心轴。在各个实施方式中,电机以大约每分钟1转(rpm)至约40,000rpm的速度旋转心轴。在一个示例性实施方式中,电机转速为约1000rpm至约4000rpm。在另一示例性实施方式中,电机转速为约1rpm至约300rpm。

在一些实施方式中,SPI溶液、牺牲溶液或两者可进一步包含附加动物或植物蛋白。非限制性例子包括:明胶,基质胶,角蛋白,胶原蛋白,弹性蛋白,纤维蛋白,透明质酸,糖胺聚糖,蛋白聚糖,纤连蛋白,玻连蛋白,层粘连蛋白,壳聚糖和大豆壳聚糖。
本发明的试剂盒

在一些实施方式中,试剂盒可进一步包含置于占组合物总重量约0.005%至2.0%的防腐剂中的支架。该防腐剂用于防止在暴露于环境污染物的情况下变质。根据本发明使用的防腐剂的例子包括但不限于从苯甲醇、山梨酸、对羟基苯甲酸酯、丙二脲及其组合中选择的那些。在一个实施方式中,防腐剂是约0.5%至2.0%苯甲醇和0.05%至0.5%山梨酸的组合。

实施例1:分级水合冻干的(GHL)支架
本研究的目的是开发纤维支架,其具有足够大的孔以允许细胞穿透到支架的纵深,据报道这会启动/促进巨噬细胞的表型转换为组织重塑M2表型并从而增强再生性伤口愈合(Garg K et al.,Biomaterials 34.18(2013):4439-4451;Wang Z et al.,Biomaterials 35.22(2014):5700-5710;Sussman EM et al.,Annals of biomedicalengineering 42.7(2014):1508-1516)。
纤维支架的主要组分是纯化的大豆分离蛋白(SPI),其是一种范例的(paradigmatic)生物相容性生物材料,已被证明其允许细胞在体外附着、扩散和增殖(LinL et al.,Journal of tissue engineering and regenerative medicine 7.12(2013):994-1008)。支架的牺牲组分是聚环氧乙烷(PEO),其是一种廉价的高分子化合物,容易获得并且可溶于水。至于纺丝溶液,将7%(w/v)SPI和0.05%(w/v)聚环氧乙烷(PEO)溶解于1,1,1,3,3,3-六氟-2-丙醇(HFP)中,而至于PEO纺丝溶液,将3%(w/v)PEO溶解于90%乙醇中。将两种组分进行共电纺丝并且将得到的支架进行后处理。在水合步骤期间,去除PEO;冻干步骤产生分级孔隙率。
具体地,使用两个均连接到13kV高压静电场的独立SPI和PEO喷丝头建立共纺丝系统。PEO喷丝头连接到在1bar压力下运行的附加空气喷射口。使用垂直旋转心轴收集纤维。两个喷嘴相互成90度角并垂直于心轴布置(图3)。这种布置创造的纤维支架含有PEO的大纤维(直径~20μm)和SPI的小纤维(直径~1.2μm)。为了从复合SPI/PEO支架中去除牺牲纤维,将支架浸没在水中(40℃)过夜(图4A,图4B)。然后将浸泡的支架转移至-80℃下保持2小时。将冷冻的样品在-60℃和0.08mbar下冻干过夜。图5A和图5B分别显示了所得的GHL支架和常规纺SPI的横截面。对于常规纺SPI支架,将7%(w/v)SPI和0.05%(w/v)聚环氧乙烷(PEO)溶解于1,1,1,3,3,3-六氟-2-丙醇(HFP)中并且电纺到固定矩形铝靶上。电纺丝在13kV的高压静电场上进行,并且喷丝头到铝靶的距离为15cm。
体外细胞研究表明,与常规电纺SPI支架相比,我们新开发的支架的穿透性有了显著改善(图6A,图6B,图7A,图7B)。进行了Sprague Dawley大鼠的动物研究,以比较GHL支架会影响的炎症反应与常规电纺SPI支架引起的炎症反应。制作两个皮下袋,分别位于下背部区域的左侧和右侧,并每个中插入一个10mm x 10mm的方形支架。一个是GHL支架,且另一个是常规纺SPI对照。手术后3、7和10天对动物实施安乐死,收集支架周围的组织并准备进行组织学检查。
用泛巨噬细胞抗体将组织染色,该抗体可识别存在于支架周围的胶质囊附近区域中的所有巨噬细胞,显示在术后10天,与对照SPI组相比,靶向GHL支架的巨噬细胞数明显更少(图8A,图8B)。

Claims (8)
1.一种制备分级多孔支架的方法,包括以下步骤:
用牺牲材料溶液电处理植物蛋白溶液,将所述植物蛋白溶液和所述牺牲材料同时并分别沉积到旋转的基材上,以形成复合支架;
从所述复合支架中去除所述牺牲材料以形成植物蛋白支架;
在水性溶液中将所述植物蛋白支架水合;
冷冻所述植物蛋白支架;和
将冷冻的植物蛋白支架冻干以形成分级多孔支架。
2.根据权利要求1所述的方法,其中所述电处理是电纺丝。
3.根据权利要求1所述的方法,其中所述植物蛋白溶液包含溶解于1,1,1,3,3,3-六氟-2-丙醇中的SPI。
4.根据权利要求1所述的方法,其中所述牺牲材料包括水溶性材料。
5.根据权利要求4所述的方法,其中所述水溶性材料是溶解于乙醇中的PEO。
6.根据权利要求1所述的方法,其中所述水性溶液是水。
7.根据权利要求1所述的方法,其中所述冷冻步骤在-80℃下进行2小时。
8.根据权利要求1所述的方法,其中所述冻干在-60℃和0.08mbar下进行。
