Abstract
本发明公开了一种带吸引功能的气囊动脉插管,属于医疗器械技术领域。该气囊动脉插管包括插管本体、蝶形气囊、气囊充气管、引流口、插管壁夹层和吸引出口;插管本体用于降主动脉灌注,蝶形气囊用于阻闭插管本体与降主动脉管腔之间的空隙,同时可能存在的反流血液全部被收集到蝶形气囊与插管连接部位最低点的引流口处;气囊充气管与蝶形气囊相连通,用于向蝶形气囊内充入气体或液体;引流口用于吸引反流至引流口处的血液;所述引流口设置于插管本体外表面与蝶形气囊外表面连接处的环形线上;插管壁夹层为反流血液引出的通道。该插管用于行主动脉弓部手术低温停循环后进行降主动脉远端灌注,并显著缩短停循环时间。
Description
技术领域
本发明涉及医疗器械技术领域,具体涉及一种带吸引功能的气囊动脉插管。
背景技术
累及主动脉弓部的主动脉病变包括胸主动脉夹层和胸主动脉瘤,目前,开放手术处理主动脉弓部病变多采用深低温停循环(DHCA)的主动脉弓部手术方式,常规过程为:深低温停循环,剪开主动脉弓,向降主动脉远端放置象鼻支架,而后进行四分支人工血管与象鼻支架吻合,在这整个过程中,下半身无血液供应,即降主动脉无向下半身灌注血流,如果时间较长可以增加下半身缺血、缺氧损伤的风险。
目前国际上有用气管插管进行气囊阻断、插管内灌注的操作方式,存在的主要问题是:1、各种原因导致气囊阻断不全,有反流血液;2、插管不易固定,影响手术野清晰,干扰手术操作。
因此,寻求新的方式,尽早恢复下半身血流,以显著降低术后风险,是目前亟待解决的问题。
发明内容
为克服现有深低温停循环(DHCA)的主动脉弓部手术方式易导致患者下半身缺血和缺氧损伤的问题,本发明的目的在于提供一种带吸引功能的气囊动脉插管,该插管用于行主动脉弓部手术低温停循环后进行降主动脉远端灌注,并显著缩短停循环时间。
为实现上述目的,本发明所采用的技术方案如下:
一种带吸引功能的气囊动脉插管,包括插管本体、蝶形气囊、气囊充气管、引流口、插管壁夹层和吸引出口;其中:
插管本体:用于降主动脉灌注,插管本体后端连接体外循环机动脉灌注管,插管本体前端穿入四分支人工血管后,放入降主动脉;
蝶形气囊:用于阻闭插管本体与降主动脉管腔之间的空隙,同时蝶形气囊能保证插管本体与蝶形气囊连接处是最低点,可以使可能存在的反流血液全部被收集到蝶形气囊与插管连接部位最低点的引流口处;所述蝶形气囊设置于插管本体的前段,包覆于插管本体外表面上,蝶形气囊采用橡胶制成,能够膨胀和收缩,当内部充入一定量气体或液体时,膨胀为蝶形(其由中侧(或外侧)向内侧逐渐收窄,气囊外表面与插管本体外表面的连接处夹角为锐角)。
气囊充气管:气囊充气管与蝶形气囊相连通,用于向蝶形气囊内充入气体(或液体),气囊充气管内设有单向阀门,气体(或液体)只能通过气囊充气管向气囊内注入,从而保证气囊充气后能够维持住气囊内足够的压力,进而阻止血液反流。需要气囊放气时,通过抽吸本接口,可以实现气囊放气(排水)。
引流口:用于吸引可能反流至引流口处的血液;所述引流口设置于插管本体外表面与蝶形气囊外表面连接处的环形线上(插管后端环形线上);所述引流口为多个,均匀分布于所述环线上,所述引流口与插管壁夹层相连通;
插管壁夹层:为反流血液引出的通道,设于插管本体内,插管壁夹层的末端设有与之相连通的吸引出口,反流血液经由吸收出口引出插管本体并回收。所述吸引出口与台下体外循环机心内吸引管路相连实现血液的回收。
所述带吸引功能的气囊动脉插管还包括定形金属条,其为铝制,设于插管本体内,可以随意弯曲,用于控制插管形状,以方便插管调整、固定。
本发明的优点和有益效果如下:
1、结合临床经验,本发明设计了带吸引功能的气囊动脉插管,完成下半身象鼻支架置入后,将本发明插管穿入四分支人工血管,并放入降主动脉,气囊充气,阻断插管与降主动脉间空隙,即可恢复下半身血流,此时可以进行四分支人工血管与象鼻支架的吻合,从而显著缩短下半身停循环,即无血液灌注的时间。
2、本发明的气囊动脉插管上设计了蝶形气囊及吸引口,可以保证反流血液全部收集至气囊于插管本体连接处,并被吸引口吸引。同时设计了定形用的金属条可以改变插管形状保证良好固定,同时最大限度的降低对手术视野的干扰。

具体实施方式
以下结合附图详述本发明。
本发明为行主动脉弓部手术低温停循环后进行降主动脉灌注用的带吸引功能的气囊动脉插管,其结构如图1-2所示。
该带吸引功能的气囊动脉插管包括插管本体1、蝶形气囊2、气囊充气管6、引流口3、插管壁夹层4和吸引出口5;其中:
插管本体:用于降主动脉灌注,插管本体后端连接体外循环机动脉灌注管,插管本体前端穿入四分支人工血管后,放入降主动脉;
蝶形气囊:用于阻闭插管本体与降主动脉管腔之间的空隙,同时蝶形气囊能保证插管本体与蝶形气囊连接处是最低点,可以使可能存在的反流血液全部被收集到蝶形气囊与插管连接部位最低点的引流口3处;所述蝶形气囊设置于插管本体的前段,包覆于插管本体外表面上,蝶形气囊采用(材质)制成,可收缩,当内部充入一定量气体或液体时,膨胀为蝶形(其由中侧(或外侧)向内侧逐渐收窄,气囊外表面与插管本体外表面的连接处夹角为锐角)。
气囊充气管:气囊充气管与蝶形气囊相连通,用于向蝶形气囊内充入气体(或液体),气囊充气管内设有单向阀门,气体(或液体)只能通过气囊充气管向气囊内注入,从而保证气囊充气后能够维持住气囊内足够的压力,进而阻止血液反流。需要气囊放气时,通过抽吸本接口,可以实现气囊放气(排水)。

插管壁夹层:为反流血液引出的通道,设于插管本体内,插管壁夹层的末端设有与之相连通的吸引出口,反流血液经由吸收出口引出插管本体并回收。所述吸引出口与台下体外循环机心内吸引管路相连实现血液的回收。
所述带吸引功能的气囊动脉插管还包括定形金属条7,其为铝制,设于插管本体内,可以随意弯曲,用于控制插管形状,以方便插管调整、固定。
本发明的带吸引功能的气囊动脉插管的使用过程为(图3):
进行主动脉弓部手术时,停循环后,先向降主动脉远端放置象鼻支架,而后将本发明所述带吸引功能的气囊动脉插管穿入四分支人造血管,将伸出的插管本体前端(带气囊端)放入降主动脉,给气囊充气以防止血液反流,插管本体后端连接动脉灌注管,即可实现降主动脉远端灌注。从而显著缩短停循环时间。
Claims (7)
1.一种带吸引功能的气囊动脉插管,其特征在于:该气囊动脉插管包括插管本体、蝶形气囊、气囊充气管、引流口、插管壁夹层和吸引出口;其中:
插管本体:用于降主动脉灌注,插管本体后端连接体外循环机动脉灌注管,插管本体前端穿入四分支人工血管后,放入降主动脉;
蝶形气囊:用于阻闭插管本体与降主动脉管腔之间的空隙,所述蝶形气囊设置于插管本体的前段,包覆于插管本体外侧表面上;
气囊充气管:气囊充气管与蝶形气囊相连通,用于向蝶形气囊内充入气体或液体;
引流口:用于吸引反流至引流口处的血液;所述引流口设置于插管本体外表面与蝶形气囊外表面连接处的环形线上;
插管壁夹层:为反流血液引出的通道,设于插管本体内,插管壁夹层的末端设有与之相连通的吸引出口,反流血液经由吸收出口引出插管本体并回收。
2.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述插管本体前端放入降主动脉后,蝶形气囊内充入气体后紧密贴合降主动脉内壁。
3.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述蝶形气囊采用橡胶制成,能够膨胀和收缩,当内部充入一定量气体或液体时,膨胀为蝶形,即膨胀后气囊由中侧(或外侧)向内侧逐渐收窄,气囊外表面与插管本体外表面的连接处夹角呈锐角。
4.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述气囊充气管内设有单向阀门,气体(或液体)只能通过气囊充气管向气囊内注入,从而保证气囊充气后能够维持住气囊内足够的压力,进而阻止血液反流;需要气囊放气时,通过抽吸气囊充气管,可以实现气囊放气(或排出液体)。
5.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述引流口为多个,均匀分布于所述环形线上,所述引流口与插管壁夹层相连通。
6.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述吸引出口与台下体外循环机心内吸引管路相连实现血液的回收。
7.根据权利要求1所述的带吸引功能的气囊动脉插管,其特征在于:所述带吸引功能的气囊动脉插管还包括定形金属条,其为铝制,设于插管本体内,可以随意弯曲,用于控制插管形状,以方便插管调整、固定。
