背景技术
现代医学将真菌病分为浅部真菌病和深部真菌病两大类。浅部真菌病是指皮肤角蛋白组织(包括角质层、甲板、毛发等)感染。深部真菌病也叫侵袭性真菌病(IFD),是指真菌侵犯人体皮下粘膜组织、血液和内脏器官等引起的真菌感染性疾病。最常见的真菌病原体是念珠菌和曲霉菌及其他如隐球菌、毛霉菌、肺孢子菌等。据全球真菌感染行动基金会(GAFFI)不完全统计,全球每年有3亿人患上严重的真菌感染疾病,超过2500万人有非常高的病死或失明风险。IFD死亡率为念珠菌30%至40%,曲霉菌为40%至90%。据统计在我国,侵袭性曲霉菌感染和念珠菌感染就有800多万例。最近流行性调查数据显示慢阻肺(COPD)患者高达1亿,其中慢性肺曲霉病(CPA)和过敏性支气管肺曲霉病(ABPA)患者超过1000万例,该类肺曲霉病如果不予治疗,5年的死亡率达到80%。但目前对真菌感染引发的疾病的研究非常有限,而且现在市面上用于治疗真菌深部感染的药物,往往副作用大,长期使用对身体器官造成伤害,而且疗效也不理想,同时产生许多不良反应。而申请人早先申请的专利《破解癌症的致病因素和中医治疗方法》(申请号:201310274489.7)和国际专利《一种治疗癌症的药物组合物及其制备方法》(申请号:PCT/CN2013/080936、进入中国申请号:201380075486.9进入美国专利号:US9498508B2)中涉及中医治疗霉菌感染和中医治疗癌症的方法,其中《半夏白术天麻汤》是治疗霉菌或癌症的基础方剂,所以本申请在此基础上继续申请。
本发明的创造思路是从金代名医张元素的《头痛案》的研究开始,从而破解了中国古代传统文化《阴阳五行学说》。对《阴阳五行学说》的破解内容具体见本人的另一申请201410299118.9《一种广谱抗厌氧菌的药物组合物》中详细地介绍了《阴阳五行》与现代医学的关系。《阴阳五行学说》也是古代中医理论的基础。因古人把生存在阴经里的病菌称为″阴邪″,″阴邪″又分为″湿邪″、″寒邪″或″风邪″等。本人受破解《阴阳学》的启发认为:中医所说的″阴经″为静脉。″阴经″里的″湿邪″便是生存在静脉里的对缺氧有耐受力的各种病菌,比如真菌、厌氧菌、病毒、杆菌等,″寒邪″则为真菌(霉菌),″风邪″则是通过空气传染的病菌,现代医学已证明空气中含有大量霉菌。近些年的医学研究也证明曲霉菌、隐球菌病、毛霉病等真菌主要传播途径为呼吸道和皮肤,可导致系统性隐球菌病,肺曲霉病、鼻脑毛霉病、肺毛霉病、胃肠毛霉病、播散性毛霉病等。
因此本人认为中医的″风寒感冒″就是曲霉菌、念珠菌或隐球菌等真菌感染上呼吸道引起的。现代医学研究也证明曲霉病的感染途径主要为吸入空气中的孢子,曲霉孢子随呼吸进入鼻窦和肺,萌发产生菌丝进入细胞质变,曲霉嗜好侵入血管。因申请人认为″寒邪″为真菌,″湿邪与风邪″中含有真菌,所以在现有中医理论的基础上得出″化湿药″、″祛寒药″、″祛风药″具有抗真菌的作用。受霉菌喜酸厌碱的特性,因此本人得出生物碱具有抗霉菌的作用,又受中医理论认为″芳香化湿、辛以化湿,温以化寒″的启发得出植物中的挥发油具有抗真菌的作用。
慢性肺曲霉病指慢性肺部曲霉菌感染。曲霉广泛分布于自然界,它存在于空气、食品、土壤、衣服、家具中,也可从正常人的皮肤、黏膜上分离到。曲霉菌也为条件致病菌,当组织损伤或机体抵抗力因慢性疾病使免疫功能受到抑制或损伤时,容易致病。肺曲霉病多为外源性感染,其感染途径主要为呼吸道吸入了曲霉孢子引起肺部曲霉病,或侵入血流播散至全身各器官。根据温海主编的《医学真菌学》临床上把肺曲霉病分为四型:
1型、肺曲霉球:一般无明显全身症状,但可有刺激性咳嗽和反复大咯血。
2型、变态反应性曲霉病:畏寒、发热、乏力、刺激性咳嗽,伴明显哮喘。咳棕黄色脓痰,有时带血。病变部位可闻干、湿啰音。
3型、支气管-肺炎型曲霉病:咳嗽、咳痰、低热、乏力、食欲减退等。病变侵袭肺实质则呈肺炎改变,并坏死化脓,形成肺脓肿。可有肺部实质病变体征及湿性啰音。影像学显示肺空洞,空洞周边肺实质破坏或纤维化以及明显的胸膜增厚,也可见曲霉菌脓胸。
4型、继发性肺曲霉病:有原发疾病的表现,并有发热、咳嗽、咳痰、略血、胸痛、呼吸困难等。双肺可有干、湿性啰音。
《半夏白术天麻汤》为治疗风痰眩晕、头痛的常用方。但一直未见《半夏白术天麻汤》用于治疗肺曲霉病的相关报道。
半夏为″温化寒痰药″。功效:燥湿化痰,降逆止呕,消痞散结,外用消肿止痛,主治湿痰、寒痰证,这说明半夏可以治疗″湿邪″和″寒邪″。现代研究半夏含有挥发油和生物碱,其生物碱具有抗肿瘤作用,半夏醇提取液具有广谱的抗菌活性,对真菌的抑制作用也有一定的效果。但一直未见半夏生物碱和挥发油具有抗肺曲霉菌病的作用。
白术为补气药,现代研究白术含挥发油,其挥发油有镇静作用。但一直未见白术生物碱和挥发油用于治疗肺曲霉病的相关报道。
天麻为《息风止痉药》是治疗头痛、肢体麻木、风湿痹痛等的药品,因此天麻具有治疗″湿邪和风邪″的作用。现代研究发现乌天麻挥发油对米曲霉、黄曲霉有较明显的抑制作用,乌天麻挥发油具有显著的镇咳和良好祛痰作用。但一直未见天麻生物碱用于治疗肺曲霉病的相关报道。
茯苓为利水消肿药,现代研究茯苓煎剂、醇提取物、乙醚提取物分别具有利尿、镇静、抗肿瘤、降血糖的作用。但一直未见茯苓生物碱和挥发油用于治疗肺曲霉病的相关报道。
鲜橘皮为理气药,现代研究鲜橘皮煎剂有扩张气管的作用,其挥发油有刺激性祛痰作用。但一直未见鲜橘皮的生物碱和挥发油用于治疗肺曲霉病的相关报道。
甘草为补气药,具有止咳祛痰的作用,但一直未见甘草的生物碱和挥发油用于治疗肺曲霉病的相关报道。
白前为化痰止咳药,归温化寒痰药,无论属寒属热,新咳久咳均可用之。现代研究其各种提取物均有明显的镇咳作用,水、醇提取物具有明显的祛痰作用,水提取物还有显著的抗炎作用。但一直未见白前的生物碱和挥发油用于治疗肺曲霉病的相关报道。
瓜蒌为清化热痰药,含挥发油和生物碱,有较好的祛痰作用,但一直未见瓜蒌的生物碱和挥发油用于治疗肺曲霉病的相关报道。
百部为止咳平喘药,现代研究百部含多种生物碱,其生物碱能降低呼吸中枢兴奋性,抑制咳嗽反射。体外试验对多种杆菌、球菌有抑制作用,对皮肤真菌也有抑制作用。但一直未见百部的生物碱和挥发油用于治疗肺曲霉病的相关报道。
侧柏叶为止血药,功效凉血止血,化痰止咳。现代研究侧柏叶含挥发油,侧柏叶煎剂能明显缩短出血时间及凝血时间,还有镇咳、祛痰、平喘、镇静等作用。体外实验表明,其对多种球菌、杆菌有抑制作用。但一直未见侧柏叶的生物碱和挥发油用于治疗肺曲霉病的相关报道。
发明内容
本发明的目的在于提供一种治疗慢性肺曲霉病的药品,具体在于提供植物药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶中含有的化学成分生物碱和挥发油在制备治疗慢性肺曲霉病的药物或保健品中的新用途。
进一步的,本发明还提供的药品其特征在于,所述药品的原料药的重量份是:半夏1~50份、白术1~50份、天麻1~50份、茯苓1~50份、鲜橘皮1~50份、甘草1~50份、白前1~50份、瓜蒌1~50份、百部1~50份、侧柏叶1~50份。
进一步的,所述治疗慢性肺曲霉病的药品,其特征在于,所述药品中各原料药的重量份进一步优选为:半夏20克、白术15克、天麻15克、茯苓15克、鲜橘皮15克、甘草10克、白前15克、瓜蒌20克、百部15克、侧柏叶15克。
进一步的,所述药品其特征在于,所述药品的原料药可根据临床症状选择性增加前胡、桔梗、紫菀、款冬花、枇杷叶、矮地茶、小蓟、大蓟,各原料药抗曲霉菌的有效成分为生物碱和挥发油。
进一步的,所述药品其特征在于,所述药品的制备方法为:从各原料药中提取生物碱和挥发油以及其他有效成分制备成药品,以常规的制备药品的方法制成片剂、粉针剂、软胶囊、外用搽剂、口服液、颗粒剂、丸剂或胶囊剂等药学上可接受的剂型。
所述用于治疗慢性肺曲霉病的药品或保健品,其适应范围包括:在医疗药品、保健食品、保健用品、动物药品、动物饲料方面的应用。
本发明药品的各种原料药的药理为:
半夏为″温化寒痰药″。药性辛,温,归脾、胃、肺经。功效:燥湿化痰,降逆止呕,消痞散结,外用消肿止痛,主治湿痰、寒痰证。这说明半夏可以治疗″湿邪″和″寒邪″。现代研究半夏含有挥发油和生物碱,半夏醇提取液具有广谱的抗菌活性,对真菌的抑制作用也有一定的效果。本发明提供的技术方案中半夏为君药,因肺曲霉病主要病变是肺部曲霉菌感染,引起炎症水肿,导致大量痰液积聚在肺部形成湿痰难以咳出,半夏为化痰止咳第一要药,配伍瓜蒌可治痰热结胸。瓜蒌为清化热痰药,为治疗黄痰阻肺,痰液粘稠难以咳出的要药。因曲霉菌为黄曲霉菌,其形成的痰液为黄脓色痰液。半夏与瓜蒌抗曲霉菌的主要有效成分为生物碱和挥发油。
白术为补气药,归脾、胃经。中医认为″脾虚生痰湿″也就是说霉菌侵犯肝脏血管引起播散性血液感染,肺部感染后形成肺部炎症反应,刺激性产生大量痰液阻塞呼吸道引起氧气与二氧化碳交换困难。″气虚″则为缺氧,补气药白术与理气药″鲜橘皮″同用可以促进氧气与二氧化碳的交换。白术在本发明提供的技术方案中为臣药。白术与鲜橘皮抗曲霉菌的主要有效成分为生物碱和挥发油。
天麻为息风止痉药,归肝经,现代研究发现乌天麻挥发油对米曲霉、黄曲霉有较明显的抑制作用,乌天麻挥发油具有显著的镇咳和良好祛痰作用。因曲霉菌通过播散性血液感染,肝经是最容易受到感染的静脉,而且脑部血管丰富,因此脑部也是曲霉菌容易滞留的部位,由于血-脑屏障的原因,许多药品无法进入脑部,而天麻是靶向作用于脑部的药品,因此天麻辅助半夏清除上呼吸道的曲霉菌感染。天麻抗曲霉菌的主要有效成分为生物碱和挥发油。
茯苓为利水消肿药,归心、脾、肾经,为健脾祛湿的要药,因曲霉菌感染肺部后,引起炎症水肿,产生大量痰液积聚在肺部,茯苓辅助白术增强氧气与二氧化碳的交换作用。茯苓抗曲霉菌的主要有效成分为生物碱和挥发油。
甘草与白术同为补气药,甘草归心、肺、脾、胃经。具有止咳祛痰的作用,还具有调和药性的作用。甘草协助白术增强氧气与二氧化碳的交换作用。甘草抗曲霉菌的主要化学成分为生物碱和挥发油、多糖。
白前与半夏同为温化寒痰药,白前归肺经。性微温而不燥烈,长于祛痰、平喘,无论属寒属热,新咳久咳均可用之。白前辅助半夏增强化痰止咳的作用。白前抗曲霉菌的主要化学成分为生物碱和挥发油。
百部为止咳平喘药,归肺经。具有润肺止咳的作用。因曲霉菌感染肺部引起脓痰阻塞肺部引起缺氧,容易出现气喘症状。现代研究百部含多种生物碱,其生物碱能降低呼吸中枢兴奋性,抑制咳嗽反射。体外试验对多种杆菌、球菌有抑制作用,对皮肤真菌也有抑制作用。百部抗曲霉菌的有效成分为生物碱和挥发油。
侧柏叶为止血药,归肺、肝、脾经。功效凉血止血,化痰止咳。因曲霉菌感染肺部后容易破坏肺毛细血管或肺静脉或肺动脉,引起咳血或咯血。现代研究侧柏叶含挥发油,侧柏叶煎剂能明显缩短出血时间及凝血时间,还有镇咳、祛痰、平喘、镇静等作用。体外实验表明,其对多种球菌、杆菌有抑制作用。侧柏叶抗曲霉菌的主要有效成分为生物碱和挥发油。
前胡和桔梗为清化热痰药,归肺经。前胡主要用于痰热咳喘,常与白前相须为用。当出现发热症状时,可增加前胡或桔梗,以协助瓜蒌增强清化热痰的作用。前胡和桔梗抗曲霉菌的主要有效成分为生物碱和挥发油。
紫菀、款冬花、枇杷叶、矮地茶为止咳平喘药,归肺经。当气喘咳嗽严重时可选择性增加紫菀、款冬花、枇杷叶、矮地茶,以协助百部增强止咳平喘的作用。紫菀、款冬花、枇杷叶、矮地茶抗曲霉菌的主要有效成分为生物碱和挥发油。
小蓟、大蓟为止血药,归心、肝经。当出现咳血或咯血症状严重时,可增加小蓟、大蓟以协助侧柏叶的止血作用。小蓟、大蓟抗曲霉菌的主要有效成分为生物碱和挥发油。
具体实施方式
1、一种治疗慢性肺曲霉病的药品,其特征在于,所述药品的原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶中抗曲霉菌的有效化学成分是生物碱和挥发油,各原料药的重量份为:半夏1~50份、白术1~50份、天麻1~50份、茯苓1~50份、鲜橘皮1~50份、甘草1~50份、白前1~50份、瓜蒌1~50份、百部1~50份、侧柏叶1~50份。
2、如上所述的治疗慢性肺曲霉病的药品,其特征在于,所述药品中各原料药的重量份进一步优选为:半夏20克、白术15克、天麻15克、茯苓15克、鲜橘皮15克、甘草10克、白前15克、瓜蒌20克、百部15克、侧柏叶15克。
3、如上所述药品其特征在于,所述药品的原料药可根据临床症状选择性增加前胡、桔梗、紫菀、款冬花、枇杷叶、矮地茶、小蓟、大蓟,各原料药抗曲霉菌的有效成分为生物碱和挥发油。
4、如上所述药品其特征在于,所述药品的制备方法为:从各原料药中提取生物碱和挥发油以及其他有效成分制备成药品,以常规的制备药品的方法制成片剂、粉针剂、软胶囊、外用搽剂、口服液、颗粒剂、丸剂或胶囊剂等药学上可接受的剂型。
5、如上所述药品的具体制备方法为:提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分生物碱和挥发油按比例混合制成药品或保健品,其制备方法采用现代制备药品或保健品的常规方法。

7、如上所述药品的具体制备方法为:提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分挥发油按比例混合制备成药品或保健品,其制备方法采用现代制备药品或保健品的常规方法。
8、如上所述的药品,其特征在于:所述药品在制备治疗慢性肺曲霉病时的适用范围包括:在医疗药品、保健食品、保健用品、动物药品、动物饲料方面的应用。

本发明的创造思路是从金代名医张元素的《头痛案》的研究开始,从而破解了中国古代传统文化《阴阳五行学说》。本人受破解《阴阳学》的启发认为:中医所说的″阴经″为静脉,″阴经″里的″湿邪″便是生存在静脉里的对缺氧有耐受力的各种病菌,例如真菌、厌氧菌、病毒、杆菌等,″寒邪″则为真菌(霉菌),″风邪″则是通过空气传染的病菌,现代医学证明空气中含有大量霉菌。
现代医学研究证明曲霉病的感染途径主要为吸入空气中的孢子,曲霉孢子随呼吸进入鼻窦和肺,萌发产生菌丝进入细胞质变,曲霉嗜好侵入血管。
本人受霉菌喜酸厌碱的特性启发,得出植物原料药中含有的生物碱是抗霉菌的主要化学成分,又受中医″芳香化湿、辛以化湿、温以化寒″的启发,得出植物原料药中含有的挥发油也是抗霉菌的化学成分。因曲霉菌归属霉菌,因此得出植物中的生物碱和挥发油是治疗肺曲霉病的主要有效成分。
所以本发明提供的药品的创造性和新颖性表现在:由原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分生物碱和挥发油的混合制成的药品的药理作用是治疗慢性肺曲霉病的药品,药品中各原料药治疗曲霉菌的主要有效化学成分是生物碱和挥发油。其药理机制是各原料药的生物碱和挥发油混合一起形成团队作用,它们相互合作,又各有分工。每种原料药的生物碱和挥发油的共性是均具有抗曲霉菌的作用,但每种原料药的生物碱和挥发油又具有特性,可以靶向作用于不同的器官、血管组织。
技术效果
传统中草药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟已被临床证明是安全有效的。申请人用原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟组方,采用传统的中药煎剂的方法治疗了1例肺曲霉病的老年患者,服药7天后,症状明显改善。该患者为慢性支气管炎,长期咳嗽,痰中带血,普通抗炎反复治疗无效后,怀疑是真菌感染,取痰液涂片发现曲霉菌丝。本发明提供的药品是在传统经方《半夏白术天麻汤》为基础的原料药的提取物,提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的挥发油和生物碱制备成治疗慢性肺曲霉病的药品或保健品,因本发明提供的药品或保健品的化学成分明确,相比传统的制药方法,其有效性是显而易见的。又因申请人没有条件提取半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中的生物碱和挥发油进行试验,但本领域技术人员基于说明书中记载的内容以及本领域普通的技术知识是能够实施本发明的技术方案,解决其技术问题,并且产生预期的技术效果。
Claims (8)
1.一种治疗慢性肺曲霉病的药品,其特征在于,所述药品的原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶中抗曲霉菌的有效化学成分是生物碱和挥发油,各原料药的重量份为:半夏1~50份、白术1~50份、天麻1~50份、茯苓1~50份、鲜橘皮1~50份、甘草1~50份、白前1~50份、瓜蒌1~50份、百部1~50份、侧柏叶1~50份。
2.如权利要求1所述的治疗慢性肺曲霉病的药品,其特征在于,所述药品中各原料药的重量份进一步优选为:半夏20克、白术15克、天麻15克、茯苓15克、鲜橘皮15克、甘草10克、白前15克、瓜蒌20克、百部15克、侧柏叶15克。
3.如权利要求1或2所述药品,其特征在于,所述药品的原料药可根据临床症状选择性增加原料药:前胡、桔梗、紫菀、款冬花、枇杷叶、矮地茶、小蓟、大蓟,各原料药抗曲霉菌的有效成分为生物碱和挥发油。
4.如权利要求1或2或3所述药品,其特征在于,所述药品的制备方法为:从各原料药中提取生物碱和挥发油以及其他有效成分制备成药品,以常规的制备药品的方法制成片剂、粉针剂、软胶囊、外用搽剂、口服液、颗粒剂、丸剂或胶囊剂等药学上可接受的剂型。
5.如权利要求1或2或3或4所述药品的具体制备方法为:提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分生物碱和挥发油按比例混合制成药品或保健品,其制备方法采用现代制备药品或保健品的常规方法。
6.如权利要求1或2或3或4所述药品的具体制备方法为:提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分生物碱按比例混合制备成药品或保健品,其制备方法采用现代制备药品或保健品的常规方法。
7.如权利要求1或2或3或4所述药品的具体制备方法为:提取原料药半夏、白术、天麻、茯苓、鲜橘皮、甘草、白前、瓜蒌、百部、侧柏叶或前胡或桔梗或紫菀或款冬花或枇杷叶或矮地茶或小蓟或大蓟中含有的化学成分挥发油按比例混合制备成药品或保健品,其制备方法采用现代制备药品或保健品的常规方法。
8.如权利要求1或2或3或4或5或6或7所述的药品,其特征在于:所述药品在制备治疗慢性肺曲霉病时的适用范围包括:在医疗药品、保健食品、保健用品、动物药品、动物饲料方面的应用。
