技术领域
本发明涉及中医药工程技术领域,尤其涉及一种治疗细菌性肺炎的中药制剂及其制备方法。
背景技术
肺炎是指终末气道,肺泡和肺间质的炎症,可由疾病微生物、理化因素,免疫损伤、过敏及药物所致。细菌性肺炎是最常见的肺炎,也是最常见的感染性疾病之一。日常所讲的肺炎主要是指细菌性感染引起的肺炎,此肺炎也是最常见的一种,占成人各类病原体肺炎的80%,其致病菌最常见的是肺炎球菌,其次为葡萄球菌、肺炎克雷白菌、铜绿假单胞菌、流感嗜血杆菌等。当机体免疫力低下或病毒继发感染时,呼吸道防御机能受损,细菌侵入下呼吸道,引起肺泡毛细血管充血、水肿、肺泡内纤维蛋白渗出、细胞浸润等炎症表现。在抗生素应用以前,细菌性肺炎对儿童及老年人的健康威胁极大,抗生素的出现及发展曾一度使肺炎病死率明显下降,但近年来,尽管应用强有力的抗生素和有效的疫苗,肺炎总的病死率不再降低,甚至有所上升。目前细菌性肺炎出现一些新特点,包括病原谱变迁,特别是医院内肺炎G-杆菌比率显著上升,肺炎链球菌虽然在社区获得性肺炎病原体中仍占主导地位,但临床表现多趋于不典型。细菌耐药率增高,所谓“难治性”肺炎屡见不鲜,尤其在儿童、老年人和免疫抑制患者中病死率极高。提高病原学诊断水平,合理应用抗生素,避免耐药菌出现,以及改善支持治疗是肺炎临床处理方面迫切需要强调和解决的问题。
中医认为,细菌性肺炎根据不同的发病表现,可分属于“外感发热”、“咳嗽”、“喘症”等范畴,外邪侵袭肺卫而表现为咳嗽、咳痰或伴有发热等为主要症状的疾病,肺炎的病因多与气候突变、寒温失常有关,发生又与人体的正气强弱有关,若起居不慎,寒温不调,过度劳累等,均可使卫外功能减弱,易感受外邪而发病。外感时邪均由口鼻而入。肺主卫气,外邪入侵首先犯肺,卫气失于宣达,可见发热、头痛、恶寒等证。肺主呼吸,开窍于鼻,上系咽喉,外邪入内化热,肺气失宣,清窍不利,因而出现鼻塞流涕、咳嗽、喷嚏、咽痛、咳吐黄粘液等症。若继续传变则病入营血,则出现神志昏糊,抽搐,高热,斑疹症状等危候,继而出现气喘、气急,甚至威胁生命。
发明内容
本发明所要解决的技术问题在于,利用我国传统的中医理论,提供一种疗效确切且毒性作用小的治疗细菌性肺炎的中药制剂及其制备方法。
为了解决上述技术问题,本发明提供了一种治疗细菌性肺炎的中药制剂,所述中药制剂包括以下原料药材:吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子、小蜡树、双肾藤、卵叶橐吾、野鸦椿子、路边姜、梵天花根、猫儿屎和沙七。
所述中药制剂中的各种原料药材的重量份数比可以优选为:吹火筒15~25份、花叶九节木20~30份、鹿耳翎10~20份、榕树叶30~40份、湿鼠曲草20~30份、山飘儿草10~20份、牛奶子20~35份、小蜡树15~25份、双肾藤30~40份、卵叶橐吾5~15份、野鸦椿子20~30份、路边姜15~25份、梵天花根10~20份、猫儿屎20~30份和沙七25~35份。

当所述中药制剂的剂型为片剂时,其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子和小蜡树按比例混合,加相对于混合物3倍~5倍量醇浓度为55%~65%的乙醇回流提取2次~4次,每次2小时~3小时,过滤,滤液合并,回收乙醇,浓缩至70℃相对密度为1.30~1.32的膏体,备用;

第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶和湿鼠曲草按所述比例混合,加相对于混合物4~6倍的水提取挥发油,将获得的挥发油用β-环糊精包合,挥发油与β-环糊精的比例为1∶3~6,搅拌75分钟~90分钟,温度30℃~50℃,得挥发油包合物,备用;另将水提取液浓缩至60℃时相对密度为1.15~1.17的清膏;
第二步,将山飘儿草、牛奶子、小蜡树、双肾藤和卵叶橐吾按所述比例混合,加相对于混合物6~8倍量醇浓度为80%~90%的乙醇回流提取3~5次,每次2~3小时,过滤,滤液合并,回收乙醇,浓缩至60℃时相对密度为1.20~1.22的清膏,备用;
第三步,将剩余所述原料药材按所述比例混合,加相对于混合物8~10倍量水,煮沸6~8小时,过滤,浓缩至65℃时相对密度为1.28~1.30的清膏;
第四步,将第一步、第二步、第三步的清膏混合,60℃~70℃减压真空干燥,得干膏粉;
第五步,将第一步所述挥发油包合物和第四步所述干膏粉充分混合,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,5~8重量份羧甲基淀粉钠、10~50重量份淀粉混合均匀,加入150~200重量份醇浓度为80%~90%乙醇制粒,干燥,整粒,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,3~5重量份硬脂酸镁,然后装入明胶胶囊中。
本发明的有益效果:本发明的中药制剂,使用安全方便,联合西药传统方法治疗细菌性肺炎,治疗效果更好,见效快,无毒副作用,同时能消除或减少西药的不良反应,制备工艺简单。

以下采用实施例来详细说明本发明的实施方式,借此对本发明如何应用技术手段来解决技术问题,并达成技术效果的实现过程能充分理解并据以实施。
实施例1:本发明制备的片剂
取吹火筒160g、花叶九节木280g、鹿耳翎160g、榕树叶310g、湿鼠曲草260g、山飘儿草150g、牛奶子260g、小蜡树160g、双肾藤390g、卵叶橐吾120g、野鸦椿子290g、路边姜250g、梵天花根150g、猫儿屎220g和沙七290g。
其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子和小蜡树按比例混合,加相对于混合物5倍量醇浓度为65%的乙醇回流提取3次,每次3小时,过滤,滤液合并,回收乙醇,浓缩至70℃相对密度为1.32的膏体,备用;
第二步,将剩余原料药材按比例混合,加相对于混合物5倍量水,煮沸4小时,过滤,浓缩至70℃相对密度为1.29的膏体;
第三步,将第一步、第二步获得的膏体混合,60℃减压真空干燥,得干膏粉;
第四步,将所述的干膏粉与淀粉进行混匀处理,得到膏体混合物;所述的膏体和淀粉的重量比为1∶0.02;将所述膏体混合物进行制粒处理、干燥处理,得到的膏体混合物颗粒;
第五步,将所述的膏体混合物颗粒与滑石粉、硬脂酸镁依次进行混匀处理、压片处理、包衣处理,即得中药制剂片剂成品;所述的颗粒、滑石粉、硬脂酸镁的重量比为100∶2.5∶0.25。
实施例2:本发明制备的胶囊剂
取吹火筒190g、花叶九节木260g、鹿耳翎200g、榕树叶340g、湿鼠曲草280g、山飘儿草120g、牛奶子280g、小蜡树180g、双肾藤360g、卵叶橐吾150g、野鸦椿子260g、路边姜210g、梵天花根120g、猫儿屎250g和沙七260g。
其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶和湿鼠曲草按所述比例混合,加相对于混合物5倍的水提取挥发油,将获得的挥发油用β-环糊精包合,挥发油与β-环糊精的比例为1∶4,搅拌80分钟,温度40℃,得挥发油包合物,备用;另将水提取液浓缩至60℃时相对密度为1.16的清膏;
第二步,将山飘儿草、牛奶子、小蜡树、双肾藤和卵叶橐吾按所述比例混合,加相对于混合物6倍量醇浓度为85%的乙醇回流提取4次,每次3小时,过滤,滤液合并,回收乙醇,浓缩至60℃时相对密度为1.20的清膏,备用;
第三步,将剩余所述原料药材按所述比例混合,加相对于混合物9倍量水,煮沸8小时,过滤,浓缩至65℃时相对密度为1.28的清膏;
第四步,将第一步、第二步、第三步的清膏混合,65℃减压真空干燥,得干膏粉;
第五步,将第一步所述挥发油包合物和第四步所述干膏粉充分混合,随后相对于100重量份所述挥发油包合物和干膏粉的混合物,加入5重量份羧甲基淀粉钠、25重量份淀粉混合均匀,加入150重量份醇浓度为90%乙醇制粒,干燥,整粒,随后相对于100重量份所述挥发油包合物和干膏粉的混合物,加入5重量份硬脂酸镁,然后装入明胶胶囊中。
毒性实验:
急性毒性实验:应用小鼠60只,雌雄各半,体重25~35g,进行急性毒性试验。小鼠随机分为两组,即对照组和给药组,实验前禁食12小时,将本发明的实施例1制备的中药片剂溶解在水中,(浓度为8.83g生药/ml,最高浓度)灌胃,灌胃容积为5ml/kg(即单次给药剂量为44.15生药/kg),对照组给予等量生理盐水,一天给药2次,给药间隔时间6小时,给药后连续观察14天,并记录小鼠的的毒性反应及死亡数。实验结果表明:与对照组比较,给药后小鼠未见明显差异,实验连续观察14天,小鼠全身状况、饮食、饮水、体重增长均正常。小鼠口服灌胃本发明的片剂LD50>44.15生药/kg,每日最大给药量为88.3生药/kg/日。本发明的中药临床用药量为8.35g生药/日/人,成人体重以60KG计,平均用药剂量为0.139g生药/kg/日。按体重计:小鼠(平均体重以30g计)口服灌胃本发明的中药的耐受量为临床用量的635倍。因此本发明的中药急性毒性极低,临床用药安全。
长期毒性实验:本发明中药实施例1片剂对三组小鼠(每组20只)按13.27、25.43和42.35g生药/kg连续用药15周(1.0ml/100g体重,每天2次)及停药3周后,结果表明:本发明中药对小鼠的毛发、行为、大小便、体重、脏器重量、血象、肝肾功能、血糖、血脂等指标均无明显影响,脏器肉眼没有发现异样变化和组织学检查结果表明,用药15周及停药3周后,小鼠各脏器均无明显改变。说明本发明中药对小鼠长期用药后毒性小,停药后也没有异样反应,应用安全。
临床资料:
病例选择:所有126例病例均来源于本院门诊及住院的患者,随机分为两组,治疗组与对照组各63例患者。治疗组63例患者中,男36例,女27例;年龄20~65岁;病程1~7d;其中,咳嗽者63例,咳痰者57例,发热者59例,胸痛者27例,WBC>10×109/L者50例。对照组63例患者,男34例,女29例;年龄20~64岁;病程1~7d;其中,咳嗽者63例,咳痰者55例,发热者58例,胸痛者28例,WBC>10×109/L者49例。治疗组与对照组在年龄、性别、病程和临床症状上无明显差异(P≤0.01),具有可比性。
诊断标准:根据湖南省医院协会编著的《临床疾病诊断与疗效判断标准》制定。(一)新近出现的咳嗽、咳痰,或原有呼吸道疾病症状加重,并出现脓性痰,伴或不伴胸痛。(二)发热。(三)肺实变体征和(或)湿性啰音。(四)WBC>10×109/L或4×109/L,伴或不伴细胞核左移。(五)胸部X线检查显示片状、斑片状浸润性阴影或间质性改变,伴或不伴胸腔积液。以上1-4项中任何一项加第5项,并排除肺结核、肺部肿瘤、非感染性肺间质性疾病、肺水肿、肺不张、肺栓塞、肺嗜酸性粒细胞浸润症、肺血管炎等,可临床诊断为细菌性肺炎。
治疗方法:
对照组:1、常规治疗:卧床休息,进食易消化富含蛋白质、电解质、维生素的食物,注意水分的补充。高热者给予物理降温,必要时给予解热药物。胸痛剧烈者,可给予芬必得0.3g口服,每日2次。鼓励病人咳嗽、翻身,或拍背促进排痰;给予常规的止咳、化痰等对症处理,必要时生理盐水10ml加α-糜蛋白酶5mg、地塞米松5mg及少量合适抗生素,雾化吸入治疗,每日2次,或给予咳必清25mg,每日3次。2、抗生素治疗:头孢他啶2g加入100买了生理盐水中静脉滴注,每日2次。左氧氟沙星0.5g静滴,每日1次。7天一个疗程,治疗2个疗程后统计治疗结果。
治疗组:在对照组西医治疗基础上,同时口服本发明中药实施例1片剂,每次2片,每天3次;7天为一个疗程,治疗2个疗程后统计治疗结果。
疗效判断标准:(1)治愈:体温正常,症状和异常体征消失,血象恢复正常,胸部X线检查病变吸收,痰菌培养阴性。(2)好转:体温正常,症状明显减轻,胸部X线检查病变有吸收好转。(3)无效:体温、症状、体征和X线检查肺部病变无明显改变或加重。
治疗结果:两组分别治疗14天后,统计结果参见表1和表2。

从表2可以看出,治疗组采用本发明的中药,配合西医治疗细菌性肺炎,相对于对照组,不仅疗效更好,见效也更快。
不良反应:对照组63例患者,治疗过程中,5例患者出现恶心、呕吐、食欲缺乏、腹胀,1例患者出现头晕,对症治疗或停药后消失,所有患者均未见肝功能损害等其他不良反应。治疗组63例患者,治疗过程中,均未出现肝功能损害等任何不良反应。
具体病例:赵某,女,57岁,患者因外出受凉后出现发热,体温高达39℃,伴咳嗽、咳少量黄痰,痰液粘稠,不易咳出,自觉胸闷、胸痛、乏力,胸痛以右侧为主,持续性酸胀痛,前来就诊,检查:发热38.5℃,左肺呼吸音粗,右肺呼吸音减弱,血常规:白细胞17.9Ⅹ109/L,胸部X线检查显示斑片状浸润性阴影,伴少量胸腔积液,经临床症状及辅助检查确诊为细菌性肺炎。在上述对照组的常规治疗和抗生素治疗的西医治疗基础上,同时口服本发明中药实施例1片剂,每次2片,每天3次;治疗10天,体温正常,症状和异常体征消失,血象恢复正常,胸部X线检查病变吸收,痰菌培养阴性,治愈,随访3个月无复发。

Claims (8)
1.一种治疗细菌性肺炎的中药制剂,其特征在于,所述中药制剂包括以下原料药材:吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子、小蜡树、双肾藤、卵叶橐吾、野鸦椿子、路边姜、梵天花根、猫儿屎和沙七。
2.如权利要求1所述的中药制剂,其特征在于,所述原料药材的重量份数比为:吹火筒15~25份、花叶九节木20~30份、鹿耳翎10~20份、榕树叶30~40份、湿鼠曲草20~30份、山飘儿草10~20份、牛奶子20~35份、小蜡树15~25份、双肾藤30~40份、卵叶橐吾5~15份、野鸦椿子20~30份、路边姜15~25份、梵天花根10~20份、猫儿屎20~30份和沙七25~35份。
3.如权利要求1或2所述的中药制剂,其特征在于,所述原料药材的重量份数比为:吹火筒15~20份、花叶九节木25~30份、鹿耳翎15~20份、榕树叶30~35份、湿鼠曲草25~30份、山飘儿草10~15份、牛奶子25~30份、小蜡树15~20份、双肾藤35~40份、卵叶橐吾10~15份、野鸦椿子25~30份、路边姜20~25份、梵天花根10~15份、猫儿屎20~25份和沙七25~30份。
4.如权利要求1至3所述的中药制剂,其特征在于,所述原料药材的重量份数比为:吹火筒16份、花叶九节木28份、鹿耳翎16份、榕树叶31份、湿鼠曲草26份、山飘儿草15份、牛奶子26份、小蜡树16份、双肾藤39份、卵叶橐吾12份、野鸦椿子29份、路边姜25份、梵天花根15份、猫儿屎22份和沙七29份。
5.如权利要求1至4所述的中药制剂,其特征在于,所述中药制剂的剂型为片剂,其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子和小蜡树按比例混合,加相对于混合物3倍~5倍量醇浓度为55%~65%的乙醇回流提取2次~4次,每次2小时~3小时,过滤,滤液合并,回收乙醇,浓缩至70℃相对密度为1.30~1.32的膏体,备用;
第二步,将剩余原料药材按比例混合,加相对于混合物3~5倍量水,煮沸2~4小时,过滤,浓缩至70℃相对密度为1.27~1.29的膏体;
第三步,将第一步、第二步获得的膏体混合,55℃~65℃减压真空干燥,得干膏粉;
第四步,将所述的干膏粉与淀粉进行混匀处理,得到膏体混合物;所述的膏体和淀粉的重量比为1∶0.01~0.02;将所述膏体混合物进行制粒处理、干燥处理,得到的膏体混合物颗粒;
第五步,将所述的膏体混合物颗粒与滑石粉、硬脂酸镁依次进行混匀处理、压片处理、包衣处理,即得中药制剂片剂成品;所述的颗粒、滑石粉、硬脂酸镁的重量比为100∶1~3∶0.15~0.3。
6.如权利要求1至4所述的中药制剂,其特征在于,所述中药制剂的剂型为胶囊剂,其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶和湿鼠曲草按所述比例混合,加相对于混合物4~6倍的水提取挥发油,将获得的挥发油用β-环糊精包合,挥发油与β-环糊精的比例为1∶3~6,搅拌75分钟~90分钟,温度30℃~50℃,得挥发油包合物,备用;另将水提取液浓缩至60℃时相对密度为1.15~1.17的清膏;
第二步,将山飘儿草、牛奶子、小蜡树、双肾藤和卵叶橐吾按所述比例混合,加相对于混合物6~8倍量醇浓度为80%~90%的乙醇回流提取3~5次,每次2~3小时,过滤,滤液合并,回收乙醇,浓缩至60℃时相对密度为1.20~1.22的清膏,备用;
第三步,将剩余所述原料药材按所述比例混合,加相对于混合物8~10倍量水,煮沸6~8小时,过滤,浓缩至65℃时相对密度为1.28~1.30的清膏;
第四步,将第一步、第二步、第三步的清膏混合,60℃~70℃减压真空干燥,得干膏粉;
第五步,将第一步所述挥发油包合物和第四步所述干膏粉充分混合,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,5~8重量份羧甲基淀粉钠、10~50重量份淀粉混合均匀,加入150~200重量份醇浓度为80%~90%乙醇制粒,干燥,整粒,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,3~5重量份硬脂酸镁,然后装入明胶胶囊中。
7.一种权利要求1至4所述的中药制剂的制备方法,其特征在于,所述中药制剂的剂型为片剂,其包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶、湿鼠曲草、山飘儿草、牛奶子和小蜡树按比例混合,加相对于混合物3倍~5倍量醇浓度为55%~65%的乙醇回流提取2次~4次,每次2小时~3小时,过滤,滤液合并,回收乙醇,浓缩至70℃相对密度为1.30~1.32的膏体,备用;
第二步,将剩余原料药材按比例混合,加相对于混合物3~5倍量水,煮沸2~4小时,过滤,浓缩至70℃相对密度为1.27~1.29的膏体;
第三步,将第一步、第二步获得的膏体混合,55℃~65℃减压真空干燥,得干膏粉;
第四步,将所述的干膏粉与淀粉进行混匀处理,得到膏体混合物;所述的膏体和淀粉的重量比为1∶0.01~0.02;将所述膏体混合物进行制粒处理、干燥处理,得到的膏体混合物颗粒;
第五步,将所述的膏体混合物颗粒与滑石粉、硬脂酸镁依次进行混匀处理、压片处理、包衣处理,即得中药制剂片剂成品;所述的颗粒、滑石粉、硬脂酸镁的重量比为100∶1~3∶0.15~0.3。
8.一种权利要求1至4所述的中药制剂的制备方法,其特征在于,所述中药制剂的剂型为胶囊剂,其制备方法包括以下步骤:
第一步,将吹火筒、花叶九节木、鹿耳翎、榕树叶和湿鼠曲草按所述比例混合,加相对于混合物4~6倍的水提取挥发油,将获得的挥发油用β-环糊精包合,挥发油与β-环糊精的比例为1∶3~6,搅拌75分钟~90分钟,温度30℃~50℃,得挥发油包合物,备用;另将水提取液浓缩至60℃时相对密度为1.15~1.17的清膏;
第二步,将山飘儿草、牛奶子、小蜡树、双肾藤和卵叶橐吾按所述比例混合,加相对于混合物6~8倍量醇浓度为80%~90%的乙醇回流提取3~5次,每次2~3小时,过滤,滤液合并,回收乙醇,浓缩至60℃时相对密度为1.20~1.22的清膏,备用;
第三步,将剩余所述原料药材按所述比例混合,加相对于混合物8~10倍量水,煮沸6~8小时,过滤,浓缩至65℃时相对密度为1.28~1.30的清膏;
第四步,将第一步、第二步、第三步的清膏混合,60℃~70℃减压真空干燥,得干膏粉;
第五步,将第一步所述挥发油包合物和第四步所述干膏粉充分混合,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,5~8重量份羧甲基淀粉钠、10~50重量份淀粉混合均匀,加入150~200重量份醇浓度为80%~90%乙醇制粒,干燥,整粒,随后加入相对于100重量份所述挥发油包合物和干膏粉的混合物,3~5重量份硬脂酸镁,然后装入明胶胶囊中。
