背景
脂肪酸结合蛋白(FABP)是充当疏水分子(特别是长链脂肪酸和视黄酸)的载体的小细胞质分子伴侣[1,2]。FABP家族的几个成员已被识别为多种代谢功能的关键调节物,并已与胰岛素抗性、脂质代谢异常和动脉粥样硬化的发展相关联。FABP将细胞内脂肪酸和其他疏水配体穿梭(shuttle)到细胞目的地诸如酶、膜和细胞核。FABP也参与调节细胞内脂质代谢和调控基因表达。
细胞内脂肪酸分子伴侣的家族成员以组织特异性方式表达。FABP4,也被称为A-FABP或aP2,是脂肪细胞和巨噬细胞中主要的脂肪酸结合蛋白。近期研究表明,FABP4是将肥胖症与胰岛素抗性联系起来的途径的核心,并在全身性葡萄糖和脂质代谢中发挥重要作用[3,4]。在巨噬细胞中FABP4具有促炎特性。在小鼠模型中,发现口服活性的小分子FABP4抑制剂是针对严重动脉粥样硬化和2型糖尿病的有效治疗剂[5]。此外,与携带纯合WT等位基因的受试者相比,携带导致降低的转录活性的FABP4启动子的T87C多态性的个体具有较低的血清甘油三酯和显著降低的动脉粥样硬化风险和2型糖尿病风险[6]。
转录因子过氧化物酶体增殖物活化受体γ(PPARγ)是与脂肪生成、胰岛素应答和免疫功能相关的基因的主要调节物,在其活化后具有良好的结果。PPAR已被认为可以降低心血管细胞,特别是内皮细胞的的炎症应答。已经观察到FABP4调控PPARγ表达[7],由此对FABP4的抑制增加了巨噬细胞和脂肪细胞中的PPARγ和水平,并且FABP4的过表达降低了PPARγ。还示出了FABP4通过触发PPARγ的泛素化和后续的蛋白酶体降解来降低PPARγ。在体内,与WT小鼠相比,无FABP4的小鼠前脂肪细胞(pre-adipocyte)表现出增加的PPARγ表达和显著增强的脂肪形成,表明FABP4调控分化过程。肥胖,主要是内脏肥胖,通过促进胰岛素抗性、糖尿病和动脉粥样硬化来增加发病率和死亡率。还发现,在小鼠和人类中,与它们在皮下脂肪中的水平相比,内脏脂肪中FABP4增加并且PPARγ减少。
皮肤作为与外界的机械屏障起作用,但它也利用免疫系统进行保护。然而,皮肤中的免疫应答并不总是保护性的,在自然界中也可以是有害的并且引起疾病[8]。许多皮肤病是由T淋巴细胞引起的,并且因此是免疫介导的。因此,许多皮肤病对全身性施用或局部施用的免疫抑制疗法良好地响应。许多皮肤病具有慢性过程,并因此在治疗上具有挑战性。
银屑病是一种影响约2%至4%的世界人口的慢性炎症性皮肤病。银屑病的发病机制涉及表皮和免疫功能缺陷。银屑病的特征是角质形成细胞的异常分化和过度增殖。在银屑病中,炎性细胞的浸润和T淋巴细胞的活化导致细胞因子的释放,导致角质形成细胞的增殖和异常分化[9]。
FABP5,也被称为表皮和银屑病相关的FABP的FABP家族成员,先前被发现在银屑病皮肤病变中增加[10]。然而,代谢调节物FABP4在一般皮肤疾病中的作用和特别是在银屑病中的作用尚未被观察到。
一般描述
本文描述的发明的发明人已经发现,FABP4是角质形成细胞和免疫细胞中用于皮肤病(例如炎症性皮肤病诸如银屑病)的发展的重要调节剂。这使得能够开发可用于抑制皮肤病的发展并且可用于在这些皮肤病发展后治疗这样的疾病的药剂(pharmaceuticalagent)和组合物。

FABP4抑制剂是指通过降低、限制或阻断FABP4蛋白的作用、功能或表达来影响FABP4活性的活性剂。FABP4蛋白活性的抑制应从广义上理解,包括FABP4抑制剂对正常(例如,未被抑制或对照)蛋白活性可能具有的抑制作用的范围。蛋白活性的抑制可以但不需要导致蛋白活性的指示物的水平或活性的增加(例如,这可以在感兴趣的蛋白作为下游指示物的抑制剂(inhibitor)或阻抑剂(suppressor)起作用时发生)。因此,当与相同指示物的对照测量值相比,蛋白的活性的任何直接或间接指示物的水平或活性改变(例如,增加或减少)至少10%、至少20%、至少30%、至少50%、至少80%、至少100%或至少250%或更多时,FABP4蛋白活性可以被抑制。蛋白活性的抑制还可以通过例如抑制编码蛋白的基因的表达或降低编码蛋白的mRNA的半衰期来实现。
根据一些实施方案,FABP4抑制剂可以选自肽、抗体、抗体片段、小分子、小干扰RNA(siRNA)、小发夹RNA(shRNA)及其混合物。
术语小分子是指具有分子量最高约1000Da(道尔顿),有时最高500Da的分子。
小分子可以是化学化合物,具有环作为其主要结构特征。环可以典型地是饱和或不饱和(即环烷基或芳基)的、完全含碳或包含一个或更多个杂原子的3至10元环(更典型地是5至6元环)。即,主环可以是环烷基、杂环基、芳基或杂芳基,其中所述一个或更多个杂原子(heteroatom(s))是氮、氧和硫中的一种或更多种。可选择地,小分子可以是两个或更多个稠环的系统或具有螺环共轭,每个环是环烷基、杂环基、芳基或杂芳基,每个环具有3至10个环原子(更典型地每个环具有5个或6个环原子)。

用于在治疗或预防炎症性皮肤病中使用的本公开内容的药物组合物可以适于诱导全身性或非全身性效应;即,组合物可以适于将活性剂,FABP4抑制剂,递送到受试者的循环系统中,或者将活性试剂递送到靶部位(例如皮肤的特定层)。
众所周知,人类皮肤由许多层组成,这些层可以被分为三个主要的组的层:角质层,其位于皮肤、表皮和真皮的外表面。虽然角质层是细胞外富含脂质的基质中的角蛋白填充的细胞层,这实际上是药物递送到皮肤的主要屏障,但表皮和真皮层是有活力的组织。表皮没有血管,但真皮含有可以引导治疗剂用于跨上皮的全身性分布的毛细血管环。

根据一些实施方案,皮肤病是淋巴瘤(CTCL)。
在其他实施方案中,本公开内容的药物组合物用于治疗或预防炎症性皮肤病或状况。即,所述组合物对涉及其中FABP4过表达的炎症性成分的皮肤疾病或状况产生至少一种治疗效果。

具体实施方式
提供以下实施例来说明某些特定特征和/或实施方案。这些实例不应被理解为将本公开内容限制于所描述的特定特征或实施方案。
实施例1:人类银屑病皮肤病变中FABP4、FABP5和PPARγ的组织表达分析
钻取活组织检查(punch biopsy)(直径4mm)从患有银屑病的患者的疾病影响的皮肤中获得(n=10)。还从患有慢性皮炎的患者的受影响皮肤获得活组织检查(n=5)。手术切除多余皮肤后,从患者获得正常的皮肤(n=10)。将组织固定在福尔马林中,并包埋在石蜡中。对于用光学显微镜进行的组织病理学检查,将切片用苏木精和伊红(H&E)染色,并由确认诊断的病理学家观察。此外,对于免疫组织化学分析,将切片用以下抗体染色:FABP4(兔多克隆抗FABP4抗体,PAB12276,Abnova)、FABP5(兔多克隆抗FABP5抗体,SC-50379,SantaCrus)和PPARγ(小鼠单克隆抗PPARγ抗体,E-8,Santa Crus),均以1:50稀释。石蜡包埋和冷冻保存的组织根据标准方案进行处理。
图1C示出了与正常皮肤(图1A)相比,在银屑病皮肤病变中检测到的表皮和真皮二者中FABP4的高表达水平。在所有测试的受银屑病影响的活组织检查中都观察到这一发现(100%患病率)。据本发明人所知,这是首次报道银屑病病变中角质形成细胞和真皮细胞中FABP4的表达。不希望受理论束缚,这一发现可以表明FABP4过表达与银屑病中角质形成细胞增殖和分化的失调有关。此外,该观察支持FABP4通过其在真皮免疫细胞中的作用来促进炎症的作用,这是银屑病的另一个显著特征。
如图1E所示,在来自患有慢性皮炎的患者的皮肤活组织检查中也观察到角质形成细胞和真皮中炎性细胞二者中FABP4的增加的水平,尽管低于银屑病(图1C)。
此外,在角质形成细胞和免疫细胞中观察到FABP4和PPARγ表达之间的负相关:而在正常皮肤中,PPARγ在整个表皮和真皮中的少数细胞中被检测到(如图1B中示出的),在银屑病病变中PPARγ的表达显著降低并且仅在表皮的最上层显示,且在真皮细胞中未被检测到(图1D)。如图1F中示出的,PPARγ表达在皮炎皮肤活组织检查中也降低。如之前报道的([16],未示出),FABP5仅在来自银屑病皮肤的角质形成细胞中表达。
银屑病皮肤以包括多层角质形成细胞的过度增生表皮,导致皮肤的厚度增加为特征。在更高放大倍数下,整个银屑病病变中的增生表皮中可见FABP4,其中染色主要在基底层中的细胞质和在上层中的细胞核,如图2B中示出的。如图2C中示出的,真皮中的FABP4表达主要在巨噬细胞、淋巴细胞和内皮细胞中以细胞质模式被观察到。银屑病皮肤中PPARγ的表达在表皮中显著降低,仅出现在最上层,且与正常皮肤相比,在真皮中未被检测到(见图3B对图3A)。
这表明可以通过评估真皮和表皮皮肤样品中FABP,且特别是FABP4的表达来检测发展具有炎症成分的皮肤病的趋势。这也表明FABP4过表达和PPARγ下调与银屑病的发病机制有关。
实施例2:人类皮肤T细胞淋巴瘤(CTCL)皮肤病变中FABP4的组织表达分析
CTCL是一组皮肤归巢性T细胞的赘生物。蕈样肉芽肿(mycosis fungoide,MF)代表最常见的CTCL类型,并占所有原发性皮肤淋巴瘤的约50%。虽然本质上是恶性的,但MF具有长期的临床过程,具有炎症性、皮炎样表现[8]。
通过对病变皮肤进行免疫组织化学分析,研究了患有MF的患者中FABP4的表达。来自5名患有MF的患者的样品被测试,并与正常人皮肤进行比较。钻取活组织检查(直径6mm)从患有MF的患者从受疾病影响的皮肤获得。手术切除多余皮肤后,从患者获得正常皮肤。组织切片如实施例1中描述地处理并用抗FABP4抗体染色。
如分别在图4B和4A中示出的,与所有被测试的患者的正常皮肤相比,在MF病变中观察到FABP4的高表达水平。FABP4的表达限于表皮和真皮中的浸润性T淋巴细胞。在真皮浸润性细胞以及穿透下表皮的恶性细胞中观察到强烈的细胞质染色,这是MF的标志。
因此,由于FABP4蛋白在恶性CTCL T淋巴细胞中的高表达,过表达的检测可以用于确定患者发展CTCL的倾向或允许其早期检测。
许多皮肤病学疾病(包括银屑病、皮炎和CTCL)由淋巴细胞介导,主要是T淋巴细胞[17]。FABP4表达已经在仅巨噬细胞、脂肪细胞和内皮细胞的有限的细胞类型的库(repertoire)中被报道。在本发明人进行的免疫组织化学分析中,发现FABP4在真皮中的炎性细胞(巨噬细胞和淋巴细胞二者中)表达。迄今为止,尚未报道淋巴细胞中的FABP4表达,也没有报道总体上的真皮细胞中的FABP4表达,因此这些发现与皮肤病学疾病高度相关。
实施例3:在原代鼠角质形成细胞中过表达FABP4
提出将FABP4引入到角质形成细胞中,以产生如银屑病中所见的具有受损的分化的过度增殖状态。通过测量两种角质形成细胞分化标志物K1和K5的表达来评估分化。K5是角蛋白,其水平在角质形成细胞分化过程中不改变,并因此用作上样对照,而K1在正常角质形成细胞分化期间被诱导。
为了评估FABP4过表达在小鼠原代角质形成细胞中的作用,将细胞用含有FABP4基因的慢病毒载体构建体感染。命名为FABP4-T2A-EGFP的FABP4慢病毒载体由CMV启动子下的FABP4基因构建,并含有绿色荧光蛋白(GFP)基因作为表达的报告物。T2A残基在FABP4蛋白的N末端的存在妨碍了使用抗FABP4商业抗体的FABP4检测,因为它们的结合受到干扰。因此,作为代替,使用抗GFP抗体来验证感染和FABP4过表达。
为了评估原代鼠角质形成细胞中在有或没有FABP4表达的情况下K1和K5分化标至物的表达,如前所述[18]由新生小鼠制备原代角质形成细胞,并用FABP4-T2A-EGFP载体感染。使用了两种内部对照:未被慢病毒感染的细胞和被缺乏FABP4基因的EGFP载体感染的细胞。细胞在含有三种不同的钙浓度的分化培养基中生长。细胞外钙从低浓度(0.05mM)升高至中浓度(0.12mM)或高浓度(1mM)诱导角质形成细胞分化。
角质形成细胞分化标志物K1和K5在被转染的和未被转染的细胞中的表达在图5中示出:与未被感染的细胞和被GFP感染的细胞相比,用表达FABP4-T2A-EGFP的慢病毒感染降低了中钙条件和高钙条件二者中K1的水平。如预期的那样,在不存在感染的情况下,K5的水平在角质形成细胞分化期间没有改变。FABP4过表达降低K1的观察结果表明FABP4干扰角质形成细胞的正常分化过程,并促进类似银屑病的过度增殖状态。
实施例4:咪喹莫特诱导的小鼠银屑病模型中FABP4的体内抑制
如已经阐述的,银屑病是一种慢性炎症性皮肤病。它在当免疫系统误把皮肤细胞作为病原体,并发出加速皮肤细胞增殖的错误信号时发生。咪喹莫特(IMQ)是一种有效的免疫活化剂,当局部应用时可以诱导和加剧银屑病。每日在小鼠背部皮肤上应用IMQ诱导类似斑块型银屑病的炎症性鳞状皮肤病变[19,20]。长期以来,IMQ诱导的小鼠银屑病已被用作人类银屑病的模型。
测试了口服施用FABP4抑制剂治疗IMQ诱导的小鼠模型中的银屑病的效果。
BMS309403(2-[[2’-(5-乙基-3,4-二苯基-1H-吡唑-1-基)[1,1’-联苯]-3-基]氧基]-乙酸,式I)被用作FABP4抑制剂:
测试使用Balb/c小鼠(Envigo RMS(Israel)Ltd)进行,研究开始时平均(±SD)体重为18.2±0.81g。动物自由地采食商业啮齿动物饮食(Teklad Certified Global 18%Protein Diet,Harlan cat#2018SC)。动物自由地获得灭菌的和酸化的饮用水(pH在2.5和3.5之间)。
该测试包括如下的六个组,每组3-10只小鼠:
-一个正常(无IMQ)组;
-一个媒介物对照组,媒介物制剂为在注射用水(WFI)中的10%1-甲基-2-吡咯烷酮和5%Cremophor EL。
-一个接受醋酸可的松作为阳性对照的治疗组-用研钵研碎一个片剂(每片剂25mg,Rekah Pharm)。将粉末溶解在2.5ml WFI中,得到10mg/ml。从IMQ应用的第一天(第1天)开始,将化合物以12.5mg化合物/kg小鼠BW(体重)每天一次地口服施用,持续六天;和
-接受3个剂量:5、15和30mg/kg的BMS(即测试项目)的治疗组。将BMS粉末(CaymanChemical)溶解在乙醇中,以产生30mg/ml溶液。将该储备溶液在媒介物中稀释,以产生三种不同的浓度0.5、1.5和3mg/ml。每天制备新的水溶液。从IMQ应用的第一天(第1天)开始,将BMS每天一次地口服施用,持续六天。
IMQ-诱导:将每日局部剂量为62.5mg商购可得的IMQ乳霜(5%)(Aldara;3MPharmaceuticals)应用到除了正常组之外的所有组动物的剃背上,持续连续六天,转化为3.125mg活性化合物的每日剂量。
实验条件:除正常组外,从第1天开始,所有动物均接受IMQ乳霜(Aldara TM 5%乳霜,#3Pharmaceuticals),每天应用到背部皮肤上,持续连续6天(约60mg/小鼠)。测试项目、媒介物和醋酸可的松每天口服施用,持续6天。在研究期间,进行发病率和死亡率、体重(BW)、临床体征、银屑病面积和严重程度指数评分(PASI),并拍摄代表性照片。在第9天将动物处死。
评分由经训练的观察者以“盲”,即不知晓治疗的方式进行。为了对背部皮肤的炎症的严重程度进行评分,采用了临床PASI评分。红斑、鳞化和增厚按从0至4的量表进行独立评分:0,无;1,轻微;2,中度;3,显著;4,非常显著。累积评分(红斑+鳞化+增厚),PASI评分,充当炎症的严重程度的衡量标准(0-12级)。
死亡率和发病率:在两项实验中,没有动物死亡或被发现处于病态状况。在研究期间中,没有在任何动物中观察到异常的临床征象。没有检测到治疗对体重的影响。
对皮肤炎症的严重程度进行评分:背部皮肤的炎症的严重程度的平均评分,即PASI,在图6中展示。单独地测量每个参数,即红斑严重程度、皮肤厚度和鳞化(分别见图7-图9)。根据PASI评分,与正常组相比,所有IMQ治疗组都发展了明显的皮肤反应,尤其在研究的第5天和第7天最。在第7天,观察到通过BMS治疗的皮肤炎症的显著改善。中等剂量的BMS(15mg/kg)表现出对炎症严重程度的影响最大。该剂量在被测试的参数—红斑、皮肤厚度和鳞化—上显示出最佳结果。在第7天拍摄的代表性照片在图10A-图10F中示出。
总之,与正常组相比,所有IMQ治疗组都发展了明显的皮肤反应,尤其在研究的第5天和第7天。在第7天,在BMS治疗组中观察到针对所有测试参数,即炎症严重程度、红斑、皮肤厚度和鳞化的显著改善的效果。将该实验重复三次,得到相似的结果。

9.根据权利要求8所述的药物组合物,其中所述皮肤病是CTCL。
10.根据权利要求1至7中任一项所述的药物组合物,其中所述皮肤病是炎症性皮肤病。
11.根据权利要求10所述的药物组合物,其中所述炎症性皮肤选自由以下组成的组:银屑病、皮炎(特应性、脂溢性、接触性)、湿疹、副银屑病、扁平苔藓、毛发扁平苔藓、急性痘疮样苔藓样糠疹、慢性苔藓样糠疹、毛发红糠疹、玫瑰糠疹、移植物抗宿主病、组织细胞增生症、药物诱导的皮疹、自身免疫性结缔组织疾病(例如狼疮)、红斑痤疮、毛囊炎、痤疮、疣、鱼鳞病、白癜风、瘢痕性脱发和CTCL。
12.根据权利要求11所述的药物组合物,其中所述炎症性皮肤病为银屑病。
13.根据权利要求1至12中任一项所述的药物组合物,还包含药学上可接受的载体。
14.根据权利要求1至13中任一项所述的药物组合物,所述药物组合物适于将所述至少一种FABP抑制剂局部地、口服地、吸入地、经鼻地、透皮地、眼内地或肠胃外地递送到受试者的循环系统中。
